1.Analysis of HBV resistance mutations in treatment of chronic hepatitis B with entecavir and lamivudine

Objective:To analyze Hepatitis B virus(HBV)drug resistance mutations in patients with chronic hepatitis B(CHB)infection who have undergone long-term monotherapy with Entecavir(ETV)and those receiving combination therapy with ETV and Lamivudine(LAM), and to explore the related factors affecting HBV drug resistance mutations.Methods:The study retrospectively analyzed patients with CHB, compensated cirrhosis, decompensated cirrhosis, and liver cancer who received long-term nucleotide analogue antiviral therapy at the Fifth Medical Center of PLA General Hospital from August 2012 to August 2019.The patients were divided into an ETV monotherapy group and a combined LAM+ETV therapy group.Chi-square tests, independent sample t-tests, and Wilcoxon rank-sum tests were used to compare the clinical baseline characteristics and HBV drug resistance mutation features between the two therapy groups.A multivariate logistic regression model was used to analyze the factors related to HBV drug resistance mutations. Results:A total of 533 patients were enrolled in this study, 357 in the ETV monotherapy group and 176 in the LAM+ETV group. The ETV monotherapy group had 122 (34.17%) patients with resistance mutations, while the LAM+ETV group had 126 (71.59%).In general, the difference in gene mutation rate between the two therapy groups was statistically significant( χ2=66.337, P<0.001). The median age and alanine aminotransferase levels of patients with drug resistance mutations in the two therapy groups were higher than those in the non-mutation group[( t=-4.743, P<0.001)/( Z=-4.809, P<0.001), ( Z=-2.667, P=0.007)/( Z=-2.001, P=0.045)].Age( OR=1.044, 95% CI:1.023-1.066), compensated cirrhosis( OR=2.163, 95% CI:1.193-3.922), liver cancer( OR=4.017, 95% CI:2.170-7.436) and the treatment regimen( OR=6.075, 95% CI:3.889-9.489) were associated with drug resistance gene mutations( P<0.001).The mutation rates in different stages of chronic liver disease(CHB, cirrhosis, and liver cancer)showed statistically significant( χ2=41.038, P<0.001; χ2=15.894, P<0.001).The overall mutation rates of ETV-related genes in the two therapy groups were 25.49% and 32.39%, respectively.Additionally, 10 mutation sites and 38 variant combinations were identified, containing five common combinations being rtL180M, rtM204V, rtS202G;rtL180M, rtM204V, rtT184A; rtL180M, rtM204V, rtT184L;rtM204I and rtL180M, rtM204V. Conclusion:In CHB patients undergoing long-term therapy, the rate of HBV resistance mutations is higher in those receiving ETV and LAM combination therapy than in those receiving ETV monotherapy.Monitoring older patients and those with cirrhosis or liver cancer is especially important for preventing resistance mutations.
2.Efficacy analysis of an improved radiofrequency ablation method for primary great saphenous vein varicose

Objective:To investigate the safety and efficacy of a modified radiofrequency ablation(RFA)treatment method for primary great saphenous vein varicose.Methods:Clinical data of 90 patients with primary great saphenous vein varicose treated with ultrasound-guided RFA from January 2021 to April 2024 in the Ultrasound Department of the First Affiliated Hospital of Zhengzhou University were retrospectively analyzed. Among them,45 patients were treated with traditional RFA treatment method(traditional group)and 45 patients were treated with improved RFA treatment method(improved group). Number of punctures,operation time,foam hardener dosage,intraoperative and postoperative complications were recorded in the two groups. The preoperative and postoperative venous clinical severity score(VCSS)and chronic venous insufficiency questionnaire(CIVIQ-14)were compared. The closure rate and recurrence rate of great saphenous vein varicose were followed up and the efficacy of the two methods were analyzed.Results:The success rate of the improved group and the traditional group was 100%.The number of punctures in the improved group was less than those of the traditional group[1(1,1) vs. 2(2,3), Z = -7.431, P<0.001],and the operation time of the modified group was shorter than that of the traditional group[(15.89 ± 3.63)min vs.(30.91 ± 5.58)min, t=-15.145, P<0.001],the average volume of lauryl foam was lower than that of the traditional RFA group[(7.96 ± 2.36)ml vs.(15.69 ± 2.89)ml, t=-13.892, P<0.001]. The incidence of complications was similar between the two groups,with no statistical significance(all P>0.05). Postoperative VCSS and CIVIQ-14 scores were significantly improved compared with before(all P<0.001),with no statistical significance between the two groups(all P>0.05). At 12 months after the operation,there was no significant difference in the closure rate of the saphenous vein between the improved group and the traditional group( P>0.05),and the recurrence rate of varicose veins in both groups was 0. Conclusions:This modified RFA treatment method for the treatment of lower extremity varicose veins is minimally invasive,safe,and has the same efficacy as the traditional RFA treatment method. Compared with the traditional RFA treatment method,the modified RFA treatment method has the advantages of convenient operation,less puncture times and shorter operation time,and is worthy of clinical promotion.
3.Chinese Medicine Regulates JAK2/STAT3 Signaling Pathway to Treat Ovarian Cancer: A Review

Ovarian cancer (OC) is one of the most common malignant tumors in women, with the mortality rate being the highest among gynaecological malignant tumors. As the atypical symptoms of OC are difficult to be detected in the early stage, most patients are already in the advanced stage when being diagnosed. As a result, the clinical treatment has limited effects. Currently, the main therapies for OC are surgery and chemotherapy, while their drug resistance and adverse reactions seriously reduce the quality of life of patients. In recent years, traditional Chinese medicine (TCM) has attracted the attention of clinicians and researchers because of its high efficacy, low toxicity, and mild side effects. According to the TCM philosophy of treatment based on syndrome differentiation, the Chinese medicines with multiple targets, wide range, and mild side effects can be screened based on the molecular targets involved in the occurrence and development of OC, which can bring out the unique advantages of TCM in the treatment of OC. Modern studies have shown that the occurrence and development of OC are closely related to the abnormal expression of multiple signaling pathways. The continued abnormal activation of the signal transducer and activator of transcription 3 (STAT3) signaling pathway can lead to abnormal proliferation and malignancy of OC. cause abnormal proliferation and malignant transformation of OC, which is closely related to the development of OC. In addition, studies have shown that Chinese medicine can inhibit the proliferation, angiogenesis, invasion, and metastasis and promote the autophagy and apoptosis of OC cells by regulating the Janus kinase 2 (JAK2)/STAT3 signaling pathway, providing new therapeutic strategies and ideas for the prevention and treatment of OC. This paper summarizes the role of JAK2/STAT3 signaling pathway in OC development by reviewing the relevant articles and reviews the mechanism and research progress of active components and compound prescriptions of Chinese medicine intervening in OC development by regulating the JAK2/STAT3 signaling pathway. This review is expected to provide a systematic reference for clinical research and drug development of OC.
4.Scoping review of financial toxicity assessment tools

Objective:To conduct a scoping review of financial toxicity assessment tools both domestically and internationally,and to summarize their scoring methods,evaluation content,development approaches,reliability/validity,and application status,aiming to provide references for selecting or developing such tools.Methods:Following scoping review methodology,databases including CNKI,VIP,Wanfang,PubMed,Web of Science,Embase,and CINAHL were searched for studies on financial toxicity assessment tools,with the search period spanning from database inception to Oct 8,2024.Results:A total of 34 eligible studies were included,covering 14 financial toxicity assessment tools.These tools primarily focused on economic status,psychosocial responses,or coping behaviors.Tools developed by domestic scholars exhibited strong specificity but lacked generalizability.Conclusions:Existing financial toxicity assessment tools are diverse,yet their reliability and validity in Chinese patient populations require further validation.Most scales lack comprehensive dimensions,and domestically developed tools are overly specific.A universal assessment tool suitable for China's cultural and policy environment needs to be developed.
5.Application of BOPPPS teaching model combined with case-based learning in clinical practice ability of orthopedic nursing interns

Objective:To evaluate the effect of case-based learning(CBL)based on BOPPPS teaching model on the clinical practice ability of orthopedic nursing interns.Methods:A total of 100 nursing interns practiced in the Department of Orthopedics in our hospital from Mar 2023 to Nov 2023 were enrolled and divided into the control group and the observation group according to random number table method,with 50 nursing interns in each group.The control group received traditional teaching method,and the observation group received the CBL based on BOPPPS teaching model.The scores of theoretical exam,clinical comprehensive practice ability,self-learning ability,critical thinking ability,empathy ability,professional identity,and teaching satisfaction were evaluated through closed book exam,Mini-CEX scale,nursing student self-learning ability scale,critical thinking disposition inventory(CTDI-CV),Jefferson Scale of Empathy for Healthcare Providers(JSE-HP),professional identity scale(PIS),and self-designed teaching satisfaction questionnaire.Results:There was no statistically significant difference in the scores of theoretical exam,critical thinking ability,empathy ability,professional identity,and self-learning ability between the two groups before teaching(P>0.05).The scores of theoretical exam,clinical comprehensive practice ability,critical thinking ability,empathy ability,professional identity,and self-learning ability in the observation group were higher than those in the control group after teaching(P<0.05).The scores of stimulating professional learning interest,deepening theoretical knowledge understanding,cultivating teamwork ability,improving the ability to analyze problems,and improving the ability to review literature in the observation group were higher than those of the control group(P<0.05).Conclusion:CBL based on BOPPPS teaching model can improve the clinical comprehensive practice ability,critical thinking ability,empathy ability and self-learning ability of orthopedic nursing interns,enhance professional identity,and thus enhance teaching satisfaction.

Lung cancer patients frequently experience multiple co-occurring symptoms that manifest as symptom clusters during treatment,severely compromising patients'quality of life.Sentinel symptoms,coexisting with symptom clusters,serve as predictive indicators for the emergence of these clusters.This study focuses on sentinel symptoms within symptom clusters in lung cancer patients,reviewing the current research status,identification methods,related intervention measures,and exist-ing research shortcomings.It summarizes the problems in current studies,with the goal of providing clinical nursing staff to as-sess and manage symptom clusters,as well as to conduct related nursing research.
