Pulmonary sarcoidosis is an immune system disease with an unclear etiology. Guided by the yang-reinforcing method, it is believed that the fundamental pathogenesis of pulmonary sarcoidosis lies in the disharmony between water and fire and the reckless movement of the ministerial fire. The failure of the spleen and stomach to maintain warmth, leading to the production of phlegm and blood stasis, is an important pathogenesis. The invasion of external pathogenic toxins, deeply penetrating into the interior, is considered a triggering factor for the disease. The treatment focuses on supplementing the yang, consolidating the kidney, drawing fire back to its source, warming the yang, benefiting the kidney, and nourishing the spleen to generate metal. It also emphasizes unblocking the yang, transforming turbidity, and eliminating phlegm and blood stasis.

Background The decline of physical activity in the elderly due to aging may increase the risk of sarcopenia. Currently, there is a lack of evidence from large natural populations on the relationship between PA and sarcopenia. Objective To explore the relationship between PA and sarcopenia in the elderly aged 60 years and above in 10 provinces (autonomous regions) of China. Methods Data were retrieved from the 2022—2023 round of the China Development and Nutrition Health Impact Cohort. Personal basic information and PA data were collected by questionnaire survey. Skeletal muscle mass was measured by bio-electrical impedance analysis, muscle strength was measured using a grip dynamometer, and physical performance was reflected by 6-meter walk speed. Data analysis models may assist the transmission of traditional Chinese medicine (TCM) experience and clinical diagnosis and treatment, and the possibility of constructing a “data-knowledge” dual-drive model was explored by taking gastric precancerous state as an example. Data-driven is to make clinical decisions around data analysis, and its syndrome-differentiation decision-making research relies on hidden structural models and partially observable Markov decision-making processes to identify the etiology of diseases, syndrome elements, evolution of pathogenesis, and syndrome differentiation protocols; knowledge-driven is to make use of data and information to promote decision-making and action processes, and its syndrome-differentiation decision-making research relies on convolutional neural networks to improve the accuracy of local disease identification and syndrome differentiation. The “data-knowledge” dual-driven model can make up for the shortcomings of single-drive numerical simulation accuracy, and achieve a balance between local disease identification and macroscopic syndrome differentiation. On the basis of previous research, we explored the construction method of diagnostic assisted decision-making platform for gastric precancerous state, and believed that the diagnostic and decision-making ability of doctors can be extended through the assistance of machines and algorithms. Meanwhile, the related research methods were integrated and the core features of gastric precancerous state based on TCM syndrome differentiation and endoscopic pathology diagnosis and prediction were obtained, and the elements of endoscopic pathology recognition based on TCM syndrome differentiation were explored, so as to provide ideas for the in-depth research and innovative application of cutting-edge data analysis technology in the field of intelligent TCM syndrome differentiation.
5.Discussion on the Treatment of Acute Exacerbation of Rheumatoid Arthritis-Associated Interstitial Lung Disease Based on ZHANG Jingyue's View of Yin-Yang Holism

This article is based on ZHANG Jingyue's theory of yin-yang holism to explore the etiology, pathogenesis, and treatment of acute exacerbation of rheumatoid arthritis-associated interstitial lung disease (RA-ILD). It is believed that the foundation of acute exacerbation of RA-ILD lies in real yin insufficiency and yin fail to control yang; external pathogen attacking is a common cause of acute exacerbation of RA-ILD. For treatment, it is important to first suppress ministerial fire by prescribing modified Yinhuo Decoction (引火汤); if ministerial fire submerged, focus should be on nourishing both yin and yang; if real yin deficiency is the main issue, modified Zuogui Pill (左归丸) should be used; if real yang deficiency is prominent as well, modified Yougui Pill (右归丸) can be chosen. When yin and yang balanced, the disease could be solved.

BACKGROUND:Exosomes play a role in all stages of wound repair,and there is currently a large body of research on exosomes in skin wound repair,which has been shown to have great potential for clinical applications. OBJECTIVE:To summarize and discuss the main mechanisms and clinical applications of exosomes in the treatment of skin wounds,in order to promote the clinical translation of exosomes. METHODS:PubMed,clinicaltrials.gov,China National Knowledge Infrastructure,Food and Drug Administration database,and Chinese Clinical Trial Register were searched from inception to March 2023.The English search terms were"exosomes,wound healing,stem cells,chronic wound,immunoregulation,inflammation,skin,therapeutic use,isolation,characterization,infections".The Chinese search terms were"exosomes,wound healing,stem cells,immunomodulation,clinical applications".A total of 79 articles were included for the summary. RESULTS AND CONCLUSION:(1)Exosomes can improve and accelerate wound healing through inflammation regulation,immune protection,angiogenesis,cell proliferation and migration,and collagen remodeling.(2)Exosomes derived from stem cells have mature preparation techniques and related mechanism research,which is currently the mainstream research direction.Non-stem cell-derived exosomes have the advantages of convenience,economy,and easy production,and can be used as a supplement for clinical applications.(3)The clinical application of exosomes is still in its infancy,but has great potential for application.Various exosome modification techniques have laid the foundation for the future development of clinically personalized services and require further research.(4)The clinical translation of exosomes faces many challenges,such as low yield,high heterogeneity,lack of unified standards for isolation,purification,and quality control,and difficulties in storage.
7.Risk factors analysis and early prediction model construction for necrosis in interstitial oedematous pancreatitis

Objective To investigate the risk factors associated with necrosis in interstitial oedematous pancreatitis(IOP)and to develop a nomogram model for the early prediction of necrosis in IOP.Methods A retrospective analysis was conducted on 306 patients diagnosed with IOP.Patients were stratified into necrosis and edema groups based on the presence or absence of pancreatic necrosis through follow-up CT-enhanced examinations.Logistic regression analysis was employed to identify independent predictive factors for necrosis in IOP.Subsequently,a nomogram model was developed,and its discriminative ability,accuracy,and practicality were assessed through receiver operating characteristic(ROC)curve,calibration curve,and decision curve analysis(DCA).Results Balthazar computed tomography severity index(CTSI),gender,lactate dehydrogenase(LDH),and triglyceride(TG)were finally identified as four independent predictors for constructing the nomogram model.The area under the curve(AUC)of the nomogram model was 0.800[95%confidence interval(CI)0.731-0.869].The calibration curve indicated good consistency between the predicted probabil-ity and the actual probability of necrosis in IOP(P=0.737).DCA suggested high practicality of the nomogram model within the threshold probability range of 3%to 66%and 75%to 96%.Conclusion The nomogram model based on Balthazar CTSI,gender,LDH,and TG demonstrates good efficacy in early prediction of necrosis in IOP.
8.Effectiveness of multi-layer silicon foam dressing in preventing sacrococcygeal and heel pressure injury in patients: a Meta-analysis

Objective:To analyze the effectiveness of multi-layer silicone foam dressing in preventing pressure injury of sacrococcygeal and heel by systematic evaluation.Methods:Randomized controlled studies on the effectiveness of multi-layer silicone foam dressings in preventing stress injury of sacrococcygeal and heel of hospitalized patients in CNKI, Wanfang, VIP, CBM, PubMed, Embase, The Cochrane Library and Web of Science were systematically reviewed. The retrieval time was from January 2012 to January 2023, and the Meta were screened and extracted by two researchers respectively. After evaluating the bias risk of the included studies, they were conducted by RevMan 5.4 software.Results:Eight randomized controlled studies involving 4 725 patients were included. Meta-analysis showed that multilayer silicone foam dressing could reduce the incidence of sacrococcygeal stage I pressure injury ( RR=0.18, 95% CI 0.09-0.35, P<0.01), and reduce the incidence of the first-stage heel pressure injury ( RR=0.29, 95% CI 0.13-0.66, P<0.05). It could reduce the incidence of sacrococcygeal stage Ⅱ and more serious pressure injury ( RR=0.42, 95% CI 0.31-0.58, P<0.01), but the influence on the incidence of the second-stage heel pressure injury and more serious heel pressure injury was not clear ( RR=0.52, 95% CI 0.27-0.99, P=0.05) Conclusions:Multi-layer silicone foam dressing can effectively reduce the incidence of sacrococcygeal pressure injury and reduce the incidence of first-stage pressure injury in heel, but the impact on the incidence of second-stage and more serious pressure injury in heel is still unclear.
9.Clinical effect of ginkgo leaf extract and armillariella mellea powders oral solution on the treatment of coronary heart disease combined with depression and its impact on immune function

Objective To investigate the clinical effect of ginkgo leaf extract and armillariella mellea powders oral solution on the treatment of coronary heart disease complicated with depression and its impact on immune function.Methods A total of 124 patients with coronary heart disease combined with depression admitted to Jinhua Hospital of Traditional Chinese Medicine and Cixi Seventh People's Hospital from July 2019 to July 2023 were randomly divided into control group and observation group,with 62 cases in each group.The control group received conventional treatment,while the observation group received additional treatment with ginkgo leaf extract and armillariella mellea powders oral solution on this basis.After 8 weeks of treatment,inflammatory factors,immune function,quality of life and depression scores were compared between two groups.Results After treatment,the levels of C-reactive protein(CRP),tumor necrosis factor-α(TNF-α),interleukin-8(IL-8)and CD8+in two groups were significantly lower than before treatment,while the levels of CD3+,CD4+and CD4+/CD8+were significantly higher than before treatment(P<0.05).The levels of CRP,TNF-α,IL-8 and CD8+in observation group were significantly lower than those in control group,while the levels of CD3+,CD4+and CD4+/CD8+were significantly higher than those in control group(P<0.05).After treatment,the scores of Seattle angina questionnair(SAQ)in all dimensions in both groups were significantly higher than before treatment(P<0.05),and the scores of SAQ in all dimensions in observation group were significantly higher than those in control group(P<0.05).After treatment,the Hamilton depression scale score of observation group was significantly lower than that of control group[(9.26±1.58)points vs.(12.87±1.89)points,t=11.538,P<0.001].Conclusion Ginkgo leaf extract and armillariella mellea powders oral solution can inhibit the inflammatory response and improve the symptoms of depression in patients with coronary heart disease and improve the immune function and quality of life.
10.Changes of Intestinal Microbiota in Type 2 Diabetic Diarrhea Patients with Different Syndromes Based on High-throughput Sequencing

ObjectiveTo investigate the changes of intestinal microbiota in type 2 diabetic diarrhea (T2DD) patients with dampness-heat syndrome and spleen-kidney deficiency syndrome. MethodT2DD patients who were admitted to the Department of Endocrinology Ⅰ of the Affiliated Hospital of Shaanxi University of Chinese Medicine from March 2020 to May 2021 were selected, including 14 patients with type 2 diabetes mellitus (T2DM), 12 T2DD patients with dampness-heat syndrome, and 13 T2DD patients with spleen-kidney deficiency syndrome. Twelve healthy subjects receiving medical examination were selected as control group. Their body mass index (BMI), fasting plasma glucose (FPG), 2 h postprandial blood glucose (2 h PBG) and glycosylated hemoglobin (HbA1c) were compared. Fecal samples were collected for DNA extraction to build a database. High-throughput 16S rDNA sequencing was used to compare the composition of intestinal microbiota and the differential bacteria among the four groups. ResultCompared with the conditions in control group, the levels of FPG, 2 h PBG and HbA1c in the other groups were increased (P<0.05). Alpha diversity showed no significant difference in species richness, evenness and diversity of intestinal microbiota among the groups. Beta diversity indicated that intestinal microbiota tended to be consistent in each group, and there was no marked difference between groups. The top 5 phylum by relative abundance were Bacteroidetes,Proteobacteria, Firmicutes, Actinobacteria, and Fusobacteria, among which,Bacteroidetes, Proteobacteria and Firmicutes were dominant. Compared with the control group, the three diabetic groups had elevated relative abundance of Bacteroidetes while decreased relative abundance of Firmicutes. The relative abundance of Actinomycetes in spleen-kidney deficiency T2DD group was significantly higher than that in the other groups, and the relative abundance of Firmicutes and Fusobacteria in the dampness-heat T2DD group was significantly lower than that in the other groups. At the genus level, the top 10 bacteria by relative abundance were Phocaeicola, Bacteroides, Pseudescherichia, Prevotella, Bifidobacterium, Faecalibacterium, Fusobacterium, Roseburia, Citrobacter, and Cetobacterium. LEfSe analysis revealed that the relative abundance of Prevotella, Mediterraneibacter, Parabacteroides, and Fusicatenibacter in diabetic patients was remarkably higher than that in healthy patients. Bacteroides and Sutterella might be the characteristic microbiota of T2DD patients with dampness-heat syndrome, while Faecalibacterium, Limosilactobacillus, Eubacterium, Gemmiger, Enterocloster, Alistipes, Parasutterella and Oscillibacter might be the characteristic microbiota of T2DD patients with spleen-kidney deficiency syndrome. ConclusionBacteroides and Parasutterella might be the characteristic microbiota of T2DD patients with dampness-heat syndrome and spleen-kidney deficiency syndrome, respectively. This paper provided reference for studying the mechanism, diagnosis and treatment of modern traditional Chinese medicine for T2DD of dampness-heat type and spleen-kidney deficiency type.
