1.Characteristics of peripheral blood leukocyte morphology and scatterplot in severe fever with thrombocytopenia syndrome

OBJECTIVE To analyze the characteristics of peripheral blood leukocyte morphology,scatterplot and leukocyte cell population data(CPD)in patients with severe fever and thrombocytopenia syndrome(SFTS),infec-tious mononucleosis(IM)and hemorrhagic fever with renal syndrome(HFRS),and to explore their differences.METHODS Blood routine examination data were collected,including microscopic cell images,scatter-plots and leukocyte CPD study parameters from 44 patients diagnosed with SFTS,17 with IM and 11 with HFRS at the First Affiliated Hospital with Nanjing Medical University from Mar.2023 to May 2024.These data were compared with those of healthy controls to explore specific differences in the characteristics of peripheral blood leu-kocyte.RESULTS All 3 viral infections exhibited a left shift in the nucleus under the microscope,yet the blood routine scatterplot and microscopic leukocyte characteristics differed.SFTS commonly presented with a plasmacy-toid reactive lymphocyte with scattered points appearing in the upper reactive lymphocyte area of the scatterplot.IM showed a predominant irregular fried-egg-like reactive lymphocyte under the microscope,with the scatterplot typically presenting a rocket-like shape,in patients with HFRS,there was a significant increase in im-mature granulocytes in peripheral blood and plasmacytoid reactive lymphocyte scatter points with blue immature granulocyte scatter points visible.The fluorescence distribution width(MO-WY)of monocytes in the SFTS group was lower than those in the IM,HFRS and healthy control groups(P<0.05).Compared to SFTS,IM and healthy control groups,the lateral scattered light distribution width(NE-WX)and fluorescence distribution width(NE-WY)of neutrophils in the HFRS group were both increased(P<0.05).CONCLUSIONS There are signifi-cant differences in the characteristics of peripheral blood leukocyte morphology,scatterplot and CPD study param-eters among three viral infection diseases SFTS,IM and HFRS.These differences can provide early clinical advice and assist in clinical differentiation.
2.Latent profile analysis of emotional intelligence among undergraduate nursing interns and its impact on patients' privacy protection ability

Objective:To identify latent profiles of emotional intelligence (EI) among undergraduate nursing interns and explore its impact on their ability to protect patient privacy, with the aim of providing a basis for targeted interventions.Methods:A convenience sampling method was used to recruit 470 undergraduate nursing interns from four nursing colleges in Shandong Province between June and September 2024. Data were collected using a general demographic questionnaire, the Wong Law Emotional Intelligence Scale-Chinese Version (WLEIS-C), the Patient Privacy Scale (PPS), the Moral Sensitivity Questionnaire for Nursing Students (MSQ-ST), and the Jefferson Scale of Empathy for Health Professionals (JSE-HP). Mplus 8.7 software was used to conduct latent profile analysis of emotional intelligence, and Logistic regression analysis was employed to identify influencing factors for the different EI profiles.Results:A total of 470 questionnaires were distributed in this study and 444 valid questionnaires were recovered, yielding a response rate of 94.47% (444/470). The EI of nursing interns was categorized into three latent profiles: low EI group ( n=81), moderate EI group ( n=170), and high EI group ( n=193). Logistic regression analysis indicated that liking for the nursing profession, moral sensitivity, and empathy ability were significant predictors of EI profile membership ( P<0.05). Statistically significant differences were observed in total PPS scores and subscale scores among interns across the three EI profiles (all P<0.01) . Conclusions:Emotional intelligence among undergraduate nursing interns exhibits heterogeneity, and interns with different EI profiles show distinct levels of competence in protecting patient privacy. Nursing educators and administrators are advised to establish risk screening mechanisms to identify subgroups with low emotional intelligence. Targeted interventions should be developed based on influencing factors specific to each profile, aiming to enhance both emotional intelligence and privacy protection capabilities in a coordinated manner.

5.Clinical laboratory features of Meigs' syndrome: a retrospective study from 2009 to 2018.

Meigs' syndrome (MS), a rare complication of benign ovarian tumors, is easily misdiagnosed as ovarian cancer (OC). We retrospectively reviewed the clinical laboratory data of patients diagnosed with MS from 2009 to 2018. Serum carbohydrate antigen 125 and HE4 levels were higher in the MS group than in the ovarian thecoma-fibroma (OTF) and healthy control groups (all P < 0.05). However, the serum HE4 levels were lower in the MS group than in the OC group (P < 0.001). A routine blood test showed that the absolute counts and percentages of lymphocytes were significantly lower in the MS group than in the OTF and control groups (all P < 0.05). However, these variables were higher in the MS group than in the OC group (both P < 0.05). The neutrophil-to-lymphocyte ratio (NLR) was also significantly lower, whereas the lymphocyte-to-monocyte ratio was higher in the MS group than in the OC group (both P < 0.05). The NLR, platelet-to-lymphocyte ratio, and systemic immune index were significantly higher in the MS group than in the OTF and control groups (all P < 0.05). The hypoxia-inducible factor-1 mRNA levels were also significantly higher, whereas the glucose transporter 1, lactate dehydrogenase, and enolase 1 mRNA levels were lower in peripheral CD4

6.Ovarian cancer cells promote glycolysis metabolism of peripheral CD4 + T cells

Objective:
To investigate the effects of ovarian cancer cells on the glycolysis metabolism and functions of peripheral CD4 + T cells.
Methods:
We established a coculture system of human CD4 + T cells with ovarian cancer cell SKOV3, and the mRNA expression levels of 8 glycolysis-related genes (mTORC1, HIF1α, GLUT1, GPI, ENO1, PKM2, LDHα and PDK1) in cocultured CD4 + T cells were determined by real-time fluorescence quantitative PCR. The levels of glucose uptake and glycolysis were detected by colorimetry. The proportion of CD4 + CD25 + Foxp3 + T cells was tested by flow cytometry. The proliferation levels of CD4 + T cells were measured by tritium incorporation method. The expression levels of 8 glycolysis-related genes in peripheral CD4 + T cells were determined by real-time fluorescence quantitative PCR in 10 ovarian cancer (OC) patients, 10 benign ovarian tumor (BOT) patients and 10 healthy control (HC).
Results:
Results: Compared with the CD4 + T cells in single culture without SKOV3, the expression levels of 8 glycolysis-related genes in CD4 + T cells cocultured with SKOV3 were elevated (all P<0.01). The levels of glucose uptake and glycolysis were increased (P<0.05). The proportion of CD4 + CD25 + Foxp3 + T cells was up-regulated (P<0.05) and the proliferation levels of CD4 + T cells were decreased (P<0.05). Moreover, the mRNA expression levels of all the glycolysis-related genes were increased (all P<0.05) in the peripheral CD4 + T cells of OC patients compared with BOT patients and HC.
Conclusion
Ovarian cancer cells could promote the glycolysis metabolism of CD4 + T cells, which may be relevant to its differentiation and functions.
7.Diagnosis and treatment of congenital pulmonary airway malformation

Objective To summarize the diagnosis and treatment of congenital pulmonary airway malformation in children.Methods The clinical data of 68 children with congenital pulmonary airway malformations from June 2014 to June 2017 were analyzed retrospectively,including 39 males and 29 females with mean age(1.1 ±:0.5) years old(from 2 months 12 days to 15 years),7 cases of pulmonary cyst resection in 68 cases,3 cases with lung segment resection,1 cases with lingulectomy,57 cases with lobectomy.Results 68 cases were cured and discharged without death.Postoperative short-term complications included subcutaneous emphysema in 5 cases,pleural effusion in 2 cases,pneumothorax in 2 cases.All cases were followed up well between 3 months and 1 year.Conclusion The diagnosis of congenital pulmonary airway malformation in children mainly depends on imaging examination.Because it is easy to repeat infection,and can not rule out malignant transformation,early diagnosis and early treatment should be done.Surgical resection is the only way to treat the disease.
8.Clinical outcomes of surgery-first approach correcting adult skeletal class Ⅲ malocclusion

Objective To explore the clinical effect of surgery-first approach (SFA) for correcting adult skeletal class Ⅲ malocclusion deformity patients.Methods A total of 28 adult patients diagnosed with skeletal class Ⅲ malocclusion were treated.Bilateral sagittal split ramus osteotomy (BSSRO) and geniplasty were performed without presurgical orthodontics treatment;postoperative orthodontics treatment was carried out after a healing period of 2-4 weeks.Lateral cephalometric radiographs were taken preoperatively (T1),within a week posoperatively (T2) and six months posoperatively (T3);cephalometric measurements were carried out by the software.Results All the patients were satisfied with the effect,no complications occured.The mean postoperative orthodontics treatment duration was 13.2 months.The mean setback of mandible at Po and B point was (7.74 ± 3.93) mm (P<0.01) and (8.13±3.84) mm (P<0.01),and superior movement lengths were (2.73±1.83) mm and (2.76±1.67) mm,respectively.Compared to T2,Po and B point moved forwardly with (2.36±1.23) mm and (2.66±1.65) mm,and inferior movment were (2.16±1.37) mm and (1.21±0.87) mm,respectively.The mean decrease of SNB and GA was (3.74± 1.61)° (P< 0.01),(3.41± 1.87)° (P<0.01),respectively.During postoperative period,both of them increased,although these were no statistical differences.Conclusions SFA combined postoperative orthodontics therapy is feasible for the correction of adult skeletal class Ⅲ malocclusion,which has shorter treatment duration than traditional joint orthognathic-orthodontic.With the advantages of earlier improvements in patient's facial aesthetics and dental function,the reduction in difficulty and treatment duration of orthodontic management,and increasing patient acceptance.

10.Design and clinical application of the instrument for percutaneous posterolateral lumbar foraminoplasty

ObjectiveTo introduce the design of the instrument for percutaneous posterolateral foraminoplasty and analyze long-term outcomes of percutaneous foraminoplasty and transforaminal endoscopic discectomy in the treatment of non-contained lumbar disc herniation.MethodsFifty-six patients with noncontained lumbar disc herniation confirmed by symptom,physical sign and concordant imaging underwent percutaneous foraminoplasty and transforaminal endoscopic discectomy,including 7 cases of L3-4,30 of L4-5 and 19 of L5S1.The visual analogue scales(VAS) of pre- and post-operative low back pain and sciatica were compared,and the Macnab scores were also evaluated.ResultsAll of the procedures were performed successfully,with mean operation time of 60 min(range,40-120 min),and a mean blood loss of 30 ml (range,20-50 ml).The follow-up time was more than 36 months.Postoperative VAS of low back pain and sciatica were significantly decreased compared with preoperative VAS (P<0.01).There were 44 cases of excellent,10of good,and 2 of fair according to Macnab score system,with total successful rate (excellent and good) up to 96.4%.Only 5 cases with L5S1 disc herniation were found complicated with sun-burn syndrome,which were relieved by pulsed electro-stimulant therapy for 1 week.ConclusionPercutaneous foraminoplasty and transforaminal endoscopic discectomy is an effective and safe minimally invasive treatment alternative for non-contained lumbar disc herniation.
