ObjectiveTo observe the clinical efficacy and safety of Yanghe Xiaonang Decoction (阳和消囊汤, YXD) in the treatment of polycystic ovarian syndrome (PCOS)-related infertility with healthy qi deficiency and phlegm stasis syndrome. MethodsA total of 120 PCOS-related infertility participants with healthy qi deficiency and phlegm stasis syndrome were randomly divided into a treatment group and a control group in a 1∶1 ratio, with 60 cases in each group. The treatment group started taking YXD on the first day of the menstrual cycle, one dose daily, until the day of sexual intercourse when follicles matured and were released. Those without mature follicles or ovulation could take it until the 14th to 28th day. The control group was given oral letrozole at 2.5 mg daily starting on the 5th day of the menstrual cycle for 5 consecutive days. If the basal body temperature curve remained monophasic on the 28th day of the menstrual cycle, and no dominant follicle was detected by color Doppler ultrasound, estrogen plus progesterone or progesterone alone was administered for withdrawal bleeding, followed by a new treatment cycle. The treatment lasted for three consecutive menstrual cycles. Patients were followed up for 3 months upon confirmed pregnancy or after the completion of the 3rd treatment cycle. The clinical pregnancy rate, total effective rate, pregnancy rate, biochemical pregnancy rate, embryonic arrest rate, and ectopic pregnancy rate between the two groups were compared. Transvaginal color Doppler ultrasound was used for monitoring endometrial thickness, cyclic ovulation status, number of follicles between 2-9 mm, and the difference in transverse and longitudinal diameters of dominant follicles. The levels of endocrine hormone indicators including serum testosterone (T), prolactin (PRL), luteinizing hormone (LH), follicle-stimulating hormone (FSH), progesterone (P), estradiol (E2), free testosterone (F-T), androstenedione (ASD), inhibin B (INH-B), and anti-Müllerian hormone (AMH), as well as the total traditional Chinese medicine (TCM) symptom scores before and after treatment were compared between the two groups. ResultsA total of 58 patients in each group were included in the statistical analysis. The clinical pregnancy rate in the treatment group was 39.66% (23/58), higher than 22.41% (13/58) in the control group (P<0.05). The total effective rate in the treatment group was 96.55% (56/58), higher than 79.31% (46/58) in the control group (P<0.05). The biochemical pregnancy rate and embryonic arrest rate in the treatment group were lower than those in the control group (P<0.05). The treatment group was superior to the control group in increasing endometrial thickness and reducing the number of 2-9 mm follicles at the 2nd and 3rd cycles, as well as in improving the cyclic ovulation status and the difference in transverse and longitudinal diameters of dominant follicles at the 3rd cycle (P<0.05). After treatment, the treatment group exhibited significant reductions in serum T, LH, F-T, ASD, INH-B, and AMH levels (P<0.05). The control group also showed significant decreases in LH, F-T, INH-B, and AMH levels (P<0.05). The levels of F-T, INH-B, and AMH in the treatment group after treatment were lower than those in the control group (P<0.05). The improvement of the total TCM symptom score in the treatment group was superior to that in the control group (P<0.05). ConclusionYXD can significantly improve the clinical pregnancy rate, reduce the rates of biochemical pregnancy and embryonic arrest, alleviate clinical symptoms, increase endometrial thickness, enhance dominant follicles, and improve endocrine hormone levels in PCOS-related infertility with healthy qi deficiency and phlegm stasis syndrome, showing good safety.

Objective:To explore the chain mediating role of general self-efficacy and humanistic care competence between oncology nurses' empathy and medical narrative competence, with a view to informing the development of targeted medical narrative competence enhancement strategies. Methods:Convenience sampling was used to select 1 104 oncology nurses from 17 ClassⅢ Grade A hospitals in Yunnan Province from May to August 2023 for the study. The General Information Questionnaire, Narrative Competence Scale, Chinese version of the Jefferson Scale of Empathy, Caring Ability Inventory, and General Self-Efficacy Scale were used to survey oncology nurses. Pearson correlation was used to analyze the correlation among oncology nurses' general self-efficacy, empathy, humanistic care competence, and medical narrative competence. Mediating effects were analyzed using Model 6 in the SPSS macro program PROCESS 4.0 plug-in. Bootstrap method was used to repeat the sampling 5 000 times for the chained mediating effect test. Results:A total of 1 104 questionnaires were distributed and the number of valid questionnaires recovered was 931, with a valid recovery rate of 84.33% (931/1 104). Among 931 oncology nurses, the scores of Narrative Competence Scale, Caring Ability Inventory, Chinese version of Jefferson Scale of Empathy, and General Self-Efficacy Scale were (151.52±17.61), (191.68±18.80), (95.31±13.96), and (27.59±6.02), respectively. There was a positive two-by-two correlation among oncology nurses' medical narrative competence, empathy, humanistic care competence, and general self-efficacy ( P<0.01). General self-efficacy and humanistic care competence acted as chain mediators between oncology nurses' empathy and medical narrative competence, with the mediating effect accounting for 43.13% of the total effect. Conclusions:General self-efficacy and humanistic care are mediating variables between oncology nurses' empathy and medical narrative competence. Nursing managers should focus on the cultivation of nurses' empathy, stimulate nurses' self-efficacy, and promote the practice of nursing humanistic care, which in turn improves oncology nurses' medical narrative competence and the quality of nursing services.

Objective To detect the expression of Hedgehog signaling pathway receptor Smoothened (Smo) protein in chronic myeloid leukemia (CML) CD34-positive (CD34+) cells, and to explore the role of Hedgehog signaling pathway in CML stem/progenitor cells. Methods Seventeen chronic phase (CP) and 6 advanced phase (AP) CML patients who were diagnosed in West China Hospital of Sichuan University from September 2010 to March 2011 and 10 People (healthy people and patients without hematologic malignances) as control were included in this study. The expression levels of Smo protein were detected by the protocol of indirect immunofluorescence labeling for cytometry and analyzed statistically. Results The mean of relative fluorescence intensity of Smo protein was 282.5±102.4, 188.8±55.4 and 354.0±297.9 in the CML-CP, CML-AD and control groups, respectively. The difference between CML-CP and CML-AD groups was statistically significant (P= 0.032). However, there were no significant differences between the CML-CP, CML-AD groups and control group (both P>0.05). The percentage of CD34+Smo+cells was (58.9±24.2)%, (42.6±17.6)%and (55.9±29.7) % in the CML-CP, CML-AD and control groups, respectively. There were still no significant differences between the CML-CP, CML-AD groups and control group (F= 0.950, P= 0.398). The mean of relative fluorescence intensity of Smo protein in the CML-CP patients with tyrosine kinase inhibitor (TKI) (9 cases) administered and without TKI (8 cases) were 282.3 ±122.6 and 282.6 ±82.4, respectively. There were no significant differences between the two groups(P=0.157). Conclusions Flow cytometry can qualitatively and quantitatively detect the expression level of Smo protein in CML CD34+cells. Smo expression is associated with stage of CML;TKI could not inhibit the activation of the Hedgehog signaling pathway in CML CD34+cells.

Objective To explore the best vibration frequency for activating human muscles. Methods Nineteen healthy college students accepted vibration stimulation at frequencies between 10 and 50 Hz.Surface electromyograms (sEMG) were recorded. The subjects were sitting, standing, squatting (knee flexion 30°) and recumbent.Their left anterior tibial muscles and the medial heads of the gastrocnemius were targeted as test muscles. The vibration stimulation point was on the surface of the left distal tibia. The sEMG characteristics of the calf muscles were analyzed under vibration stimulation at different frequencies. ResultsThe leg muscles were activated significantly at all vibration frequencies, but the sEMG values of the anterior tibialis were significantly different at different frequencies, except for among 30 Hz,40 Hz and 50 Hz in any position. The gastrocnemius sEMG values were not significantly different at different frequencies. ConclusionVibration at 30 to 50Hz may be the normal human muscle activation frequency.

Chinese Journal of Hospital Administration 1996;0(03):-
Objective To control medical expenses in a rational and effective way by analyzing the main factors influencing hospitalization expenses in TCM hospitals. Methods Based on ICD 10 codes, the top 50 disease entities causing hospitalization, as had been accumulated over the years, were extracted from the hospitalization record data warehouses of the national TCM supervision networks using nationally representative medical record databases, the main factors influencing hospitalization expenses were identified using multi layer BP neural network, and further analyses were made by disease entities and hospital grades. Results When disease entity mix analyses were conducted, it was found that the variables most influencing hospitalization expenses/days were hospital code and disease entity while variables specific to TCM had little influence on the expenses. The results were also true of analyses made by hospital grades and disease entities. Conclusion Under the current circumstances in China, the influence on hospitalization expenses resulting from the difference between hospitals is far greater than any other factor; it is even greater than that resulting from the difference between disease entities. It is therefore imperative to pay attention to the extensiveness and coverage of data sources when conducting research on the control of medical expenses.
