1.Effectiveness and safety of belumosudil in 20 patients with chronic graft-versus-host disease

Objective:To evaluate the effectiveness and safety of belumosudil for the treatment of chronic graft-versus-host disease (cGVHD) .Methods:We retrospectively collected data on patients with cGVHD who received belumosudil at Ruijin Hospital Affiliated to Shanghai Jiaotong University School of Medicine from May 2023 to March 2024. The study endpoints were overall response rate (ORR), organ-specific response rates, time to response (TTR), changes in Lee Symptom Scale (LSS) scores, tapering or discontinuation of corticosteroid treatment, failure-free survival (FFS), and adverse events.Results:The study included 20 patients with cGVHD who received belumosudil, of whom 15 were men and 5 women. The median age was 34.5 (12-67) years, and three patients were under 18 years old. The median follow-up duration was 5.0 (1.4 - 9.8) months. All patients had severe cGVHD, and 18 (90.0%) showed involvement of at least four organs. The median number of prior treatment lines was 4, and 15 patients (75%) had previously received ruxolitinib. All patients received 200 mg of belumosudil once daily in combination with other cGVHD systemic therapies. The ORR was 90.0% (95% CI: 68.3%-98.8%), and all responses were partial responses. The median TTR was 1.6 (0.9 - 8.4) months. The LSS scores improved in a clinically meaningful way in 80.0% (16/20) of the patients within 3 months. The corticosteroid dose was reduced in 42.6% (6/14) of the patients. The 3-month FFS was 79.6% (95% CI: 61.4%-100.0%). Most adverse events were grade 1 or grade 2, and two patients (10.0%) experienced grade 3 or higher-grade adverse events. Conclusions:In the real-world setting, belumosudil demonstrated good effectiveness and safety in patients with cGVHD with a history of severe disease and multiorgan involvement.

Objective:
To investigate the potential efficacy and safety of Lutai Danshen Baishao granules (LDBG) for treating female melasma associated with kidney deficiency and blood stasis patterns.
Methods:
A randomized, double-blind, placebo-controlled trial was conducted at the Third Central Hospital of Tianjin, China from March to December 2023. A total of 110 female patients with melasma linked to kidney deficiency and blood stasis were enrolled and treated with either LDBG or a placebo twice daily for 60 days. Efficacy was assessed through measures such as the total melasma area, reduced melasma area, reduction rate of melasma area, melasma color score, Melasma Area and Severity Index (MASI) score, and traditional Chinese medicine (TCM) symptom score scale. Safety assessments included routine blood and biochemical tests.
Results:
Participants in both groups were aged 52–63 years, with no significant differences. After the 2-month intervention, the total melasma area decreased in both groups; however, a greater reduction was observed in the test group [462.50 mm2 (12.81%) vs. 100.00 mm2 (3.11%), P < .001]. Moreover, LDBG treatment significantly reduced the MASI and melasma color scores in the test group (P < .05). The total TCM symptom evaluation score significantly decreased (test group: 6.00 vs. placebo group: 7.00, P = .001), with significant relief in symptoms such as improvement in dark lips, nails, and waist soreness in the test group, compared with that in the placebo group (P < .05). Within-group comparisons revealed that TCM syndrome was significantly alleviated in the test group (P < .05).
Conclusion
LDBG intervention shows promising effectiveness in reducing female melasma and alleviating TCM syndromes.
3.Pneumonia caused by Rhizopus microsporus:a case report and literature review

Objective To review the characteristics of Rhizopus microsporus infection for better awareness of the disease.Methods One case of pneumonia caused by R.microsporus was reported in a 66-year-old male patient.Similar reports on infections caused by R.microsporus were retrieved in PubMed and CNKI databases since 2013.The characteristics of patients with R.microsporus infection were reviewed.Results This case involves a 66-year-old male patient presenting with a 10-day history of cough,sputum production,and hemoptysis.The patient had a previous history of diabetes mellitus.Pulmonary CT scan revealed an irregular soft tissue density mass in the right lower lobe and pneumonia.The clinical presentation and laboratory findings were consistent with pulmonary mucormycosis caused by R.microsporus.R.microsporus was isolated from bronchoalveolar lavage fluid culture.The patient was treated with intravenous liposomal amphotericin B followed by oral posaconazole for antifungal therapy,and achieved a favorable prognosis.A total of 24 cases(18 males,5 females,1 unknown)of R.microsporus infection were reviewed(including this one).The specific site of infection included pulmonary infection(n=10),ocular infection(n=1),skin tuberculosis(n=1),splenic abscess(n=1),oral mucositis(n=1),gouty arthritis(n=1),esophageal ulceration(n=1),abdominal infection(n=1),and others(n=7).The clinical symptoms varied with the organs involved.Majority of the patients(n=11)were cured by surgery combined with antimicrobial therapy.Overall,13 patients died.Conclusions Clinicians should be aware of the possibility of R.microsporus infection in case of pulmonary infection,especially those with diabetes mellitus.
4.Correlation between IL-6,IL-8,antimicrobial peptide LL-37,and microbial abundance in vaginal secretions and the degree of mucosal injury in patients with trichomonal vaginitis

Objective To explore the correlations between interleukin-6(IL-6),interleukin-8(IL-8),antimicrobial peptide LL-37 and the microbial abundance and the degree of mucosal injury in patients with trichomonal vaginitis(TV).Methods A total of 120 TV patients admitted from January 2022 to December 2024 were selected and divided into the occurrence group(n=63)and the non-occurrence group(n=57)according to presence and absence of mucosal injury.The general data,IL-6,IL-8,LL-37 and microbial abundance were compared between the two groups.According to the degree of mucosal injury,the occurrence group was divided into the mild subgroup(n=34),the moderate subgroup(n=20),and the severe subgroup(n=9).Pearson correlation analysis was applied to investigate the correlations between IL-6,IL-10,antimicrobial peptide LL-37,microbial abundance and the degree of mucosal injury.Multivariate logistic regression was used to analyze the influencing factors of mucosal injury in TV patients.The receiver operating characteristic curve(ROC)was used to evaluate the predictive value of relevant indicators for mucosal injury in TV patients.Result The levels of IL-6,IL-8,antimicrobial peptide LL-37,and the abundance of Actinobacteria and Gardnerella in the occurrence group were higher than those in the non-occurrence group,while the abundance of Firmicutes and Lactobacillus was lower than that in the non-occurrence group(P<0.05,P<0.01).Pearson correlation analysis showed that IL-6,IL-8,antimicrobial peptide LL-37,and abundance of Actinobacteria and Gardnerella were positively correlated with the degree of mucosal injury(r=0.543,0.713,0.352,0.409,0.659,P<0.01),while the abundance of Firmicutes and Lactobacillus was negatively correlated with the degree of mucosal injury(r=-0.540,-0.504,P<0.01).Multivariate logistic regression analysis showed that IL-6,IL-8,antimicrobial peptide LL-37,and abundance of Actinobacteria were risk factors for mucosal injury in TV patients,while the abundance of Firmicutes and the abundance of Lactobacillus were protective factors(P<0.05).The ROC curve showed that the area under the ROC curve(AUC)of the combined detection of IL-6,IL-8,antibacterial peptide 37,and the abundance of Actinobacteria,Firmicutes,and Lactobacillus in evaluating mucosal injury in TV patients was higher than that of the single detection(P<0.01).Conclusion IL-6,IL-8,antimicrobial peptide LL-37 and microbial abundance are closely related to the degree of mucosal injury in TV patients.The combined detection has a high value in evaluating the occurrence of mucosal injury in TV patients.
5.Efficacy of nucleos(t)ide analogues combined with peginterferon α-2b on antiviral therapy in children with chronic hepatitis B

Objective:To analyze the efficacy of combination of peginterferon α-2b(Peg-IFN α-2b)with nucleos(t)ide analogues(NAs)on antiviral therapy in children with chronic hepatitis B(CHB)and to provide an optimized clinical treatment strategies for CHB children.Methods:A retrospective analysis was conducted on 30 CHB children treated in The First Affiliated Hospital of the Army Medical University(Southwest Hospital)from January 2022 to January 2025 with treatment duration at least 48 weeks. The enrolled children were aged between 2 and 17 years and divided into the NAs combined with Peg-IFN α-2b(NPI)group(n=13)and NAs group(n=17)by their therapy regimens. The characteristics of baseline,week 12,week 24,week 48 and week 96 were compared between groups,as well as the differences in response to biochemical,immune and viral indicators at each observation point. Logistic regression analysis and receiver operating characteristic(ROC)curve analysis were performed to identify factors influencing the HBsAg seroclearance. Results:At baseline of treatment,the proportion of HBeAg positivity in the NPI group and the NAs group was high(76.9% vs 86.6%, χ2=0.679, P=0.628),and the alanine aminotransferase(ALT)and aspartate aminotransferase(AST)in the NPI group were significantly lower than those in the NAs group( P<0.001). At 24 weeks,the decrease in HBsAg in the NPI group was also significantly higher than that in the NAs group( Z=-3.161, P=0.002). Finally,the cumulative seroclearance rate of HBsAg at 96 weeks in the NPI group was significantly higher than that in the NAs group(46.15% vs 5.88%, χ2=0.679, P=0.025). Mulitivariate Logistic regression analysis showed that treatment regimen and gender were risk factors affecting the outcome of HBsAg( P<0.05). ROC curve analysis showed that the increase in ALT at 12 weeks compared with baseline(AUC=0.857,Cutoff value=3.615 IU/L),the decrease in ALT at 24 weeks(AUC=0.870,Cutoff value=47.85 IU/L),and the decrease in HBsAg at 12 weeks and especially at 24 weeks(AUC=0.885,Cutoff value=0.97log IU/ml)were effective predictors of HBsAg prognosis at 96 weeks. Conclusion:In CHB children,antiviral regimen Peg-IFN α-2b combined with NAs was more effective than NAs alone in improving the HBsAg seroclearance rate of CHB,and the effects in female were better than in male. The decline of HBsAg and the fluctuation of ALT in the early treatment period are valid predictors of HBsAg clearance.

10.Focusing on the Benefits of Patient, Promoting the Sustainable Development of Medical Consortium

As a practical carrier for promoting the tiered diagnosis and treatment model, the medical consor-tium is of great significance for balancing medical resources and boosting medical service efficiency. The construction of medical consortiums not only improves the accessibility of high-quality medical resources for patients, but also enhances the diagnostic and treatment level of member units. Meanwhile, it provides space for the leading hospital to adjust the structure of diseases and improve the level of discipline construction. As the core of medical insurance payment reform, DRG, through indicators such as the case mix index(CMI) and the number of diagnosis related group (DRG), provides objective and quantified data support for case management and disease structure optimization, thus effectively guiding the rational allocation of medical resources and the adjustmentof diseases and surgical types within the medical consortium. Comprehensive use of DRG evaluation indicators can construct a multidimensional medical consortium construction evaluation system, provides a clear direction for medical consortium cooperation, thereby promoting the overall healthy and sustainable development of medical consortiums and achieving a win-win situation for all parties involved. This paper, based on the "1+5+1" medical consortium cooperation model centered around Peking Union Medical College Hospital, utilizes DRG indicators to analyze the benefits for patients, member hospitals, and the leading hospital during the medical consortium construction process, with the hope of providing reference for the construction of a medical consortium evaluation system.
