Objective:To investigate the diagnostic value of different conventional ultrasound-based radiomics models and their combination with clinical ultrasound features in the pathological subtyping of renal cell carcinoma. Methods:Retrospective data from 286 patients diagnosed with renal cell carcinoma by pathology at the Tenth Affiliated Hospital of Southern Medical University between May 1,2017 and June 7,2024 were collected. Among the 286 patients,203 were clear cell carcinoma,44 were papillary renal cell carcinoma, and 39 were chromophobe renal cell carcinoma. The patients were randomly divided into a training group(201 cases)and a validation group(85 cases)in a ratio of 7 to 3. Regions of interest(ROI)were delineated on conventional ultrasound images, and the radiomics features were extracted. Feature selection was performed using Student's t-test,Pearson correlation, and the least absolute shrinkage and selection operator(LASSO). Six different machine learning methods included category gradient boosting(CatBoost),light gradient boosting machine(LightGBM),Logistic regression(LR),random forest(RF),support vector machine(SVM)and extreme gradient boosting(XGBoost)were used to establish radiomics models. Weight balancing was applied to correct for sample imbalance, and an imaging genomics model was constructed after balancing the samples. Independent predictors of renal cell carcinoma subtyping were selected from clinical ultrasound features using univariate and multivariate logistic regression analyses, and a clinical imaging model was constructed. The best-performing radiomics model was combined with the clinical independent predictors to construct a combined model. Receiver operating characteristic curves and the obuchowski index were plotted to evaluate model performance. Results:Among the radiomics models, the model constructed using Random Forest(RS RF)after balancing the samples exhibited the best predictive performance, with area under the curve(AUCs)of 0.918(micro-average ROC)and 0.903(macro-average ROC),and the obuchowski index was 0.885 in the validation group. The long and short axes of ultrasound image tumor masses were used as imaging independent predictors to construct a clinical imaging model. In the validation group, the AUCs of the clinical model were 0.886(micro-average ROC)and 0.606(macro-average ROC),and the obuchowski index was 0.569. The combined model achieved AUCs of 0.888(micro-average ROC)and 0.967(macro-average ROC),with an obuchowski index of 0.933,outperforming any single model. Conclusions:The combination of conventional ultrasound-based radiomics models with clinical ultrasound features demonstrates high diagnostic value in differentiating clear cell carcinoma, papillary renal cell carcinoma, and chromophobe renal cell carcinoma. It may serve as an auxiliary tool for providing timely and effective clinical guidance.

Case 1 A 68-year-old male patient presented with poor appetite persisting for over 2 months and had been diagnosed with liver cancer 7 days prior. He was admitted to Liaocheng Second People′s Hospital on October 17, 2017. On October 18, 2017, at 8:00, the patient underwent transcatheter arterial chemoembolization under local anesthesia. At 17:30, the patient developed a fever. At 20:00, "port wine-colored" urine was noted during catheterization, prompting transfer to the intensive care unit (ICU) for enhanced treatment. Case 2 A 67-year-old male patient with a previous diagnosis of hilar cholangiocarcinoma presented to the Emergency Department of Liaocheng Second People′s Hospital at 17:40 on March 25, 2025, due to "unconsciousness for one hour". Routine blood testing revealed significantly decreased white blood cells, red blood cells, and platelets. Considering the critical condition, the patient was transferred to the ICU. At 19:30, "port wine-colored" urine was observed in the urinary catheter. Both cases showed evident signs of hemolysis in blood samples, rapid disease progression, with fatal outcomes occurring within a short period. Blood cultures from both patients signaled positive in anaerobic bottles. Identification using the bioMérieux VITEK 2 COMPACT ANC card confirmed Clostridium perfringens. Bloodstream infections caused by Clostridium perfringens leading to such severe intravascular hemolysis are rare. The disease course is notoriously aggressive. In clinical practice, early recognition of this infection is paramount.

Objective To explore the role of the PPARα/HOXA10 signaling pathway in mediating the effect of adiponectin(APN)for improving endometrial receptivity in a rat model of polycystic ovary syndrome(PCOS).Methods Forty female SD rat models with letrozole-induced PCOS were randomized, with 10 normal rats as the control, into 4 equal groups for treatment with APN alone,APN combined with GW6471(a specific PPARα inhibitor)or the vehicle for 20 days,or no further treatment(PCOS model group).GW6471 treatment(daily dose of 1 mg/kg)and vehicle treatment were initiated on the 11th day following the start of APN treatment, all administered via intraperitoneal injection. The rats were observed for changes in estrous cycle, body weight, ovarian index and morphology, uterine index and morphology, serum hormone levels and lipid metabolism parameters.Endometrial expressions of PPARα and HOXA10 were detected with immunohistochemistry and Western blotting. The development of endometrial pinopodes was observed under electron microscope, and pregnancies of the rats were recorded. Results The rat models of PCOS exhibited obvious estrous cycle disorders with significantly prolonged estrous interval, increased body weight and ovarian index, decreased uterine index, disordered serum hormones and lipid metabolism(P<0.05),and polycystic ovarian changes, and these changes were significantly improved by APN treatment.Endometrial expressions of PPARα and HOXA10 were significantly lowered in PCOS rats and effectively up-regulated after APN treatment, but GW6471 treatment obviously blocked the effect of APN(P<0.05).APN showed strong protective effect against PCOS-induced impairment of endometrial pinopode development, and this effect was obviously attenuated by GW6471.APN also significantly increased the pregnancy rate and embryo number in PCOS rats, while GW6471 obviously reduced the embryo number and caused developmental retardation of the embryos. Conclusion APN can improve endometrial receptivity in PCOS rats by upregulating the PARα/HOXA10 pathway.
