1.Value of lung ultrasound monitoring in the clinical diagnosis and treatment of mycoplasma pneumoniae pneumonia in children

Objective:To evaluate the lung ultrasound characteristics of mycoplasma pneumoniae pneumonia in children and to investigate the value of lung ultrasound monitoring in clinical diagnosis and treatment.Methods:A retrospective analysis of 62 children with mycoplasma pneumoniae pneumonia admitted to Xi'an Chest Hospital from 7 November to 30 November 2023 was performed,and the characteristic parameters of bedside lung ultrasound and their related clinical data were collected. Pathological lung ultrasound features such as interrupted pleural line,well-spaced B-lines,coalescent B-lines,small subpleural patchy pulmonary consolidation,large pulmonary consolidation and pleural effusion in 12 scan areas of both lungs were observed. The maximum upper and lower diameters,right and left diameters,and anterior and posterior diameters of the large pulmonary consolidations were measured,and the changes in the above signs before and after treatment were measured and compared.Results:In sixty-two children with mycoplasma pneumoniae pneumonia,including 32 males and 30 females,with a mean age of(8.18 ± 2.05)years old and a mean hospital stay of(8.79 ± 2.93)days,lung ultrasound showed interrupted pleural line,well-spaced B-lines,coalescent B-lines,small subpleural patchy pulmonary consolidation,large pulmonary consolidation and pleural effusion,with the incidence of 93.5%(58 /62),33.9%(21/62),32.3%(20/62),59.7%(37/62),66.1%(41/62)and 17.7%(11/62),respectively,in which the large pulmonary consolidations presented rich blood supply were more common in the L6 and L4 areas,while the pleural effusions were more common in the L6 area.The signs of interrupted pleural line,coalescent B-lines,large pulmonary consolidation and pleural effusion were significantly improved after treatment compared with before treatment(all P<0.05). The upper and lower diameters,left and right diameters,and anterior and posterior diameters of large pulmonary consolidations were significantly reduced after treatment compared with before treatment[(4.19 ± 2.42)cm vs.(2.84 ± 2.31)cm, t=2.613, P=0.011;(2.80 ± 1.82)cm vs.(1.96 ± 1.62)cm, t=2.226, P=0.029;(3.41 ± 2.11)cm vs.(2.12 ± 1.82)cm, t=2.972, P=0.004].With the process of treatment,the dynamic observation of lung ultrasound showed that the well-spaced B-lines/coalescent B-lines gradually decreased until they completely disappeared or a small number of B-lines remained,and the area of the large pulmonary consolidation showed a dynamic downward trend(all P<0.001),and the area of large pulmonary consolidations gradually decreased until they completely disappeared or only small subpleural patchy pulmonary consolidations and well-spaced/coalescent B-lines remained,and at the same time,the pleural effusion gradually absorbed until it disappeared. Conclusions:Lung ultrasound can detect the distribution area of lung lesions,morphology and blood supply characteristics of children with mycoplasma pneumoniae pneumonia,as well as the dynamic changes after treatment,and lung ultrasound can dynamically monitor and evaluate the progression and regression of the disease in real time,providing a reliable imaging evidence for clinical practice.
2.Establishment and optimization of an autoverification system for thyroid function reports

Objective:To establish and optimize an autoverification system for thyroid function test reports of 5 items using historical test data.Methods:Based on the docoment' Autoverification of Clinical Laboratory Quantitative Test Results′, CLSI AUTO 10-A and AUTO 15 guidelines, an autoverification system for thyroid function test reports of 5 items was established combining with manual verification experience. A total of 193 860 thyroid function test reports of 5 items in 2021 were collected for the assessment of the original system. Totally 210 097 thyroid function test reports of 5 items in 2022 and 299 198 reports in 2023 were collected for the optimization of the autoverification system. There were 160 666 thyroid function test reports of 5 items from the first half of 2024 for the manual and autoverification comparison after optimization.Results:The pass rate of the autoverification system based on original thyroid function report in 2021 was 69.56%(134 849/193 860). The optimized system utilizing historical data from 2022 and 2023 covered 21 pattern rules and established verification for different patterns including range rules, delta check rules, and review rules. Taking manual verification as the standard for the data from the first half of 2024, the sensitivity and specificity of the optimized system were 100% (499/499) and 81.57% (130 646/160 167), respectively, with a false-negative rate of 0. The concordance rate between autoverification and manual verification was 81.63% (131 145/160 666), and the pass rate was 81.32% (130 646/160 666).Conclusion:Establishing and optimizing the autoverification system for thyroid function tests of 5 items using historical test data, and formulating verification rules for different patterns can be applied to clinical practise, which not only ensures the accuracy of test reports but also improves work efficiency, allowing continuously optimized and perfected of the system.
3.Guided bone regeneration therapy based on plaque control of peri-implantitis with follow-up at 7 years.

Peri-implantitis is a pathologic condition associated with dental plaque that occurs in the implant tissue and is characterized by inflammation of the mucous membrane surrounding the implant, followed by the progressive loss of supporting bone. In this study, a case of guided bone regeneration therapy based on plaque control of peri-implant inflammation was reported. Four years after surgery for the left second premolar implant, the patient presented with "left lower posterior tooth swelling and discomfort for more than 2 years". The X-ray periapical film showed a decrease in distal bone mineral density of implant, and the clinical diagnosis was peri-implantitis of the left second premolar. Implants underwent guided bone regeneration and regular periodontal maintenance treatment. Re-examination at 3.5 months, 11 months, 18 months, and 7 years showed that the alveolar bone height and bone mineral density were stable, and the periodontal depth became shallow. However, the gingival recession was mild. In the present case, follow-up at 7 years demonstrated that the clinical periodontal indexes could be remarkably improved after complete periodontal treatment for peri-implantitis, and the alveolar bone could be well restored and regenerated.

4.Benchmarking China's Top-Tier Medical Institutions Against Global Standards:A Quantitative Analysis of the Gap Using Internationally Recognized Metrics

Objective To evaluate the gap between China's top-tier medical institutions and top-tier international medical institutions,and to provide references for the construction of first-class medical science discipline in China.Methods Using globally recognized rankings of medical institutions,we selected 24 top-tier international medical institutions and 11 top-tier Chinese medical institutions.Publicly available,general,and comparable data on indicators were collected to analyze the performance and gaps between top-tier international and Chinese medical institutions in human resources development,talent cultivation,scientific research,social services,and discipline construction.Results In the field of medicine,the largest gap between top-tier international and Chinese medical institutions was in high-level talent.Specifically,the average numbers of individuals who are Clarivate Analytics'Highly Cited Researchers,who are Nobel Prize laureates in Physiology or Medicine,and who serve on advisory boards or editorial boards of top medical journals,and who rank among the Top 2 000 Medicine Scientists were 1.00,0.09,0.45 and 4.00,respectively,among top-tier Chinese medical institutions,while those of the top-tier international medical institutions were 131.46,118.25,9.72,and 6.76 times,respectively,those of the Chinese medical institutions.The second largest gap was in social services and medical innovation.The average proportion of industrial collaboration papers and the number of clinical trials of China's top-tier medical institutions were 1.51%and 1 851,respectively,while those of international top-tier medical institutions were 3.62 and 1.87,times,respectively,those of top-tier Chinese medical institutions.However,the average number of(untranslated)patents held by top-tier international medical institutions was only 15% of that of China's top-tier medical institutions.The third largest gap was in scientific research.The average number of papers published in New England Journal of Medicine,Journal of the American Medical Association,The Lancet,and British Medical Journal,the percentage of hot papers in papers included in Web of Science,the percentage of highly cited papers,the percentage of international collaboration papers,the total number of citations per paper,category normalized citation impact(CNCI),and the number of publications of top-tier Chinese medical institutions were 78,0.03%,1.39%,22.55%,19.61,1.26,30 706,while those of the top-tier international medical institutions were 6.96,2.66,2.57,2.15,1.83,1.58 and 1.54 times those of the Chinese medical institutions,respectively.However the average percentage of zero-citation papers of top-tier international medical institutions was only 71%of that of China's top-tier medical institutions.Furthermore,in discipline development,the average overall scores of the Times Higher Education(THE)and QS rankings for medicine-related disciplines of top-tier Chinese medical institutions were 72.84 and 69.30,respectively,while those of top-tier international medical institutions were 1.38 and 1.21 times those of the Chinese medical institutions.However,in terms of talent cultivation,the average number of students of China's top-tier medical institutions was 10724,which is roughly double that of international institutions.Conclusion Currently,China's top-tier medical institutions are still in a basic stage that emphasizes the quality of talent cultivation and medical services.There is considerable room for development and potential for catching up in multiple aspects,especially in high-level talent,medical research,and innovation.It is recommended that the construction experience of top-tier international medical institutions should be fully utilized to build China's first-class medical science discipline.

7.Ivabradine Prevents Remifentanil Induced Hyperalgesia in Mice

[Objective]To investigate the effect of ivabradine,an inhibitor of peripheral HCN channel,on remifentanil-induced hyperalgesia in mice.[Methods]The model of remifentanil-induced hyperalgesia was established by intravenously infusing remifentanil 2 μg/(kg·min)for 1 h through tail vein of adult male C57/BL6 mice.To observe the effect of ivabradine on remifentanil induced hyperalgesia,ivabradine(5 mg/kg)was injected subcutaneously 30 minutes before remifentanil infusion.Forty mice were equally and randomly divided into 4 groups:saline group,remifentanil group,remifentanil+vehicle group and remifentanil+ivabradine group.In each group,six mice were used to test mechanical and thermal pain thresholds at 24 h before(baseline)and on 1 d,3 d,5 d after remifentanil or saline infusion.Four mice of each group were used to detected c-Fos positive cell in spinal dorsal horn by immunofluorescence on 1 d after remifentanil or saline infusion.[Results]Compared with the saline group,a significant decrease in mechanical or thermal threshold was observed on 1 d and 3 d after remifentanil infusion(P<0.001),and the number of c-Fos positive neurons in the lumbar dorsal horn increased significantly(P<0.001).Compared with vehicle group,subcutaneous injection of ivabradine effectively inhibited remifentanil induced hyperalgesia(P<0.001)and blocked the increase of c-Fos positive neurons in the lumbar dorsal horn on 1 d following remifentanil treatment(P<0.001).[Conclusions]Ivabradine could effectively prevent remifentanil-induced hyperalgesia in mice.The possible mechanism underlying this effect is that ivabradine suppresses the enhanced peripheral nociceptive input onto spinal cord neurons.
8.Application of A Tiered Progressive Training Model in the Standardized Residency Training on Laboratory Medicine

Objective To cultivate the ability of laboratory resident physicians in multiple aspects and enhance their post-competence for laboratory medicine.Methods The residents recruited into the Cancer Hospital of China Academy of Medical Sciences Laboratory Base were divided into junior residents and senior residents.According to the different training contents and objectives,the exploration of the hierarchically progressive training model was carried out,which mainly included three aspects:training plan,process training and process assessment.Results After the implementation of the hierarchical progressive training model,the average theoretical score and the average score in the skill operation examination of the residents increased to over 90 and 95,respectively.Meanwhile,the comprehensive clinical ability was also improved.Breakthroughs of teaching,scientific research and honor were achieved from"nothing"before the implementation to"something"after the implementation,and it actively promoted the improvement of the post-competency of the residents in laboratory medicine.Conclusion The application of the hierarchically progressive training mode in standardized training of residents in laboratory medicine could play a good role in promoting the training of post-competence for residents.
9.Influencing factors and outcomes of acute rejection within one year after pediatric kidney transplantation

Objective:To explore the influencing factors of acute rejection (AR) within one year after pediatric kidney transplantation (KT) and the effect of AR onset time on prognosis.Methods:From January 2011 to October 2021, a total of 112 patients aged under 18 years at the time of transplantation were selected.After excluding 6 of them with early renal non-function caused by non-rejection, 106 cases were examined.There were 63 males and 43 females with the age of 15(12, 16) years.The donors were living related (n=26) and deceased (n=80).According to the presence/absence and onset time of AR, they were assigned into three groups of AR within one year, AR after one year and non-AR.The relevant clinical data of donor/recipient, influencing factors of AR and therapeutic outcomes of AR were retrospectively compared.One-way ANOVA or Kruskal-Wallis test was utilized for comparing 1-year renal function after the occurrence of AR among three groups.With graft-function loss as an end-point event of follow-up, the effects of AR within one year and AR after one year on survival rate and function of graft-kidney were analyzed by Kaplan-Meier survival curve.Results:The median follow-up period of 106 pediatric KT recipients was 35 months.During follow-ups, 19 episodes of AR occurred in 17(16.0%) patients and 89 recipients exhibited no AR episode by the end of follow-up (non-AR group).As for initial AR, 9 episodes of AR occurred within one year (AR within one year group) and 8 episodes of AR after one year (AR after one year group).After anti-rejection treatment, 8 patients (47.1%) achieved full recovery and 6 patients (35.3%) failed to completely normalize and 3 patients (17.6%) developed graft failure.Univariate analysis indicated that, as compared with non-AR group, female recipients, donors aged under 8 years and early postoperative infection with parvovirus B19 were risk factors of AR within one year ( P=0.032, P=0.039, P=0.047).Kaplan-Meier survival analysis revealed that the incidence rates of AR within one year in patients with donors aged under 8 years and early postoperative parvoviral infection were 14.5%(8/55) and 30.0%(3/10) respectively.They were significantly higher than 2.0%(1/51) and 6.3%(6/96) of patients with donors aged above 8 years and those without parvoviral infection ( P=0.012, P=0.004).With graft-function loss as an end-point event of follow-up, Kaplan-Meier survival analysis showed that 10-year kidney graft survival rate in AR within one year and AR after one year groups were 88.9% and 65.6%.Both were significantly lower than that in non-AR group (98.9%).And the inter-group differences were statistically significant ( χ2=4.286, P=0.038; χ2=7.787, P=0.005).However, no significant difference existed in survival rate between AR within one year and AR after one year groups ( P=0.689).One-way ANOVA and Kruskal-Wallis test indicated that estimated glomerular filtration rates at 3/6/12 months after an onset of AR in AR within one year group were (76.8±51.6), (80.6±56.6) and (85.6±40.2) ml·min -1·1.73 m -2.The values of 3/6 months were lower than (125.3±39.2) and (124.7±38.2) ml·min -1·1.73 m -2 in AR after one year group.And the inter-group differences were statistically significant ( P=0.021, P=0.039).The values of 3/6/12 months were lower than (112.2±34.2), (115.3±33.2) and (117.4±30.2) ml·min -1·1.73 m -2 in non-AR group.And the inter-group differences were also statistically significant ( P=0.019, P=0.020, P=0.020). Conclusions:Female recipients, donors aged under 8 years and early postoperative infection with parvovirus B19 may elevate the risks of AR in children within one year of KT.AR within one year affects the survival rate of graft-kidney and renal function.
10.In vitro research of oral microscope-assisted implant surface decontamination.

OBJECTIVES:
To investigate the effect of oral microscope-assisted surface decontamination on implants in vitro.
METHODS:
Twelve implants that fell off because of severe peri-implantitis were collected, and decontamination was carried out on the surfaces of implants through curetting, ultrasound, titanium brushing, and sandblasting at 1×, 8×, or 12.8× magnifications. The number and sizes of residues on the implants' surfaces after decontamination were determined, and the decontamination effect was analyzed according to the thread spacing in the different parts of the thread.
RESULTS:
1) The 8× and 12.8× groups scored lower for implant surface residues than the 1× group (P<0.000 1), and the 12.8× group scored lower than the 8× group (P<0.001); 2) no difference in residue score was found between the wide and narrow thread pitch (P>0.05), and the 8× and 12.8× groups had lower scores than the 1× group (P<0.001); 3) the lowest number of contaminants was observed at the tip of the thread, whereas the highest was observed below the thread, and the difference was significant (P<0.001). However, the thread pitch had no effect on the number of contaminants in different areas (P>0.05); 4) the residue scores of the 8× and 12.8× groups were lower than those of the 1× group at the thread tip and above, sag, and below the thread of the implants (P<0.05).
CONCLUSIONS
Residues on the surfaces of contaminated implants can be effectively removed by using an oral microscope. After decontamination, the residues of pollutants were mainly concentrated below the thread of the implants, and the thread pitch of the implants had no significant effect on the residues.
