Journal of Traditional Chinese Medicine 2026;67(5):580-583
In response to the high risk of recurrence and metastasis after breast cancer surgery, this paper proposes a six-dimensional integrated preventive and therapeutic model, namely the "disease-syndrome-constitution-trend-strategy-efficacy" framework, including disease identification, syndrome differentiation, constitution differentiation, trend assessment, strategy implementation, and effect evaluation. This model builds upon the traditional three-dimensional approach of disease identification, syndrome differentiation and constitution differentiation, and introduces "trend" to dynamically capture the evolving tendencies of disease progression and the transformation of syndrome and constitution, thereby operationalizing the concept of "treating before disease onset". The "strategy" dimension enables the organic integration and individualized regulation of TCM and western medical interventions, while the "efficacy" dimension establishes a multidimensional evaluation system integrating TCM and western medicine to assess the efficacy of the "strategy", so as to promote the systematic, dynamic, and precise development of treating before disease onset in TCM.

ObjectiveTo investigate the synergistic inhibitory effect of chidamide (CHI) combined with compound Kushen injection (CKI) on breast cancer MCF-7 cells and to elucidate the potential molecular mechanisms based on transcriptomics analysis. MethodsCell proliferation was detected by the methyl thiazolyl tetrazolium (MTT) assay, and the "one belt and one line" method was adopted to screen the optimal mass concentration for synergistic effect. According to the combination index (CI) values, cells were allocated into the blank, CHI (1.6 mg·L-1), CKI (0.4 g·L-1), and CHI+CKI (1.6 mg·L-1+0.4 g·L-1) groups. Transwell assay was employed to examine cell migration, and flow cytometry to measure cell cycle distribution and apoptosis rate. Transcriptome sequencing (RNA-seq) and weighted gene co-expression network analysis (WGCNA) were performed to screen the key modules highly correlated with the cell inhibition rate. Candidate genes were identified by intersecting module genes with differentially expressed genes (DEGs), followed by functional enrichment analysis. Western blot was employed to quantify the protein levels of histone deacetylase 1 (HDAC1), protein kinase A (PKA), cyclin-dependent kinase inhibitor 1 (p21), cAMP response element-binding protein (CREB) and its phosphorylated form (p-CREB), B-cell lymphoma-2 (Bcl-2), and Bcl-2-associated X protein (Bax) in cells of each group. ResultsCompared with the blank group, both CHI and CKI monotherapies inhibited the proliferation of MCF-7 cells in a mass concentration-dependent manner. Compared with the blank and monotherapy groups, CHI+CKI inhibited cell migration, increased the proportion of cells arrested in the G0/G1 phase, and elevated the apoptosis rate (P<0.01). WGCNA identified the MEred module strongly correlated with the inhibition rate (r=0.90, P<0.01), yielding 97 candidate genes upon intersection with DEGs. Kyoto Encyclopedia of Genes and Genomes (KEGG) analysis indicated significant enrichment of the cAMP signaling pathway. Western blot results showed that compared with the blank group, all treatment groups down-regulated the expression of HDAC1, up-regulated the expression of PKA and p21, and increased the p-CREB/CREB and Bax/Bcl-2 ratios (P<0.01). Compared with the monotherapy groups, CHI+CKI further up-regulated the PKA and p21 expression and increased the p-CREB/CREB and Bax/Bcl-2 ratios (P<0.01). ConclusionThe combination of CHI and CKI exerts a significant synergistic anti-tumor effect. CHI exerts epigenetic regulation by inhibiting HDAC1, which, in combination with CKI, may further contribute to the activation of the cAMP/PKA/CREB signaling pathway, leading to up-regulation of p21 expression and the Bax/Bcl-2 ratio, thereby promoting cell cycle arrest and apoptosis.

Journal of Traditional Chinese Medicine 2025;66(3):256-261
ObjectiveTo investigate the association between the maintenance treatment of modified Yiqi Huoxue Jiedu Formula (益气活血解毒方) or poly ADP ribose polymerase (PARP) inhibitors and the recurrence and metastasis of advanced ovarian cancer. MethodsA case-control study design was employed, dividing patients with advanced ovarian cancer into two groups based on the occurrence of recurrence and metastasis following first-line maintenance treatment. Patients with recurrence and metastasis comprised the case group, while those without recurrence and metastasis served as the control group. The previous first-line maintenance treatment method was set as the exposure factor in the study (with the use of modified Yiqi Huoxue Jiedu Formula defined as exposed and PARP inhibitors defined as unexposed). Basic information was collected for both groups, including the achievement of satisfactory R0 surgery, age, stage, neoadjuvant chemotherapy, lymph node metastasis, germline BRCA1/2 mutations, homologous recombination deficiency positivity, first-line maintenance treatment method (modified Yiqi Huoxue Jiedu Formula or PARP inhibitors), and CA125 levels after the last chemotherapy. The baseline data of the two groups were assessed for differences. If there exists difference, a 1∶1 nearest neighbor matching method was used for propensity score matching. Univariate and multivariate logistic regression analyses were employed to evaluate the association between the modified Yiqi Huoxue Jiedu Formula or PARP inhibitors and the recurrence and metastasis of ovarian cancer. ResultsA total of 201 patients with advanced ovarian cancer were included, with 97 in the case group and 104 in the control group. Both groups showed statistically significant differences in R0 surgery, stage, neoadjuvant chemotherapy, and CA125 levels after the last chemotherapy (P<0.05), indicating baseline imbalance. After propensity score matching, there were 71 patients in both the case and control groups, achieving baseline balance (P>0.05). Univariate logistic regression analysis indicated that the achievement of satisfactory R0 surgery (P = 0.006), disease stage (P = 0.001), the use of neoadjuvant chemotherapy (P = 0.024), treatment modality (P = 0.006), and CA125 levels after the last chemotherapy (P = 0.013) were associated with the recurrence and metastasis of ovarian cancer. Multivariate logistic regression analysis revealed that disease stage was an independent influencing factor for the recurrence and metastasis of ovarian cancer (P = 0.030), whereas the P-value for the correlation between first-line maintenance treatment and ovarian cancer was 0.188. ConclusionFirst-line maintenance treatment of ovarian cancer patients with the use of modified Yiqi Huoxue Jiedu Formula or PARP inhibitors does not correlate with the recurrence and metastasis of ovarian cancer.

Cancer toxin is a specific pathogenesis leading to the heterogeneity of breast cancer. The nature and virulence of the cancer toxin determine the differences in the heterogeneity of breast cancer, which can dynamically evolve over time and space, resulting in varying invasion abilities and characteristics of the tumor.Cancer cells in the primary lesion possess"toxicity"that targets specific organs for metastasis, and cancer toxins can influence the metastatic propensity of different types of breast cancer. Therefore, breast cancer treatment strategies based on the theory of cancer toxins emphasize the continuous eradication of the cancer toxin, focusing on differentiating its strength and nature, protecting unaffected areas first, identifying the state based on symptoms, and targeting accordingly to combat resistance arising from tumor heterogeneity. This article aims to provide a new theoretical basis for the treatment strategies of different types of breast cancer.

Objective:To investigate the difference of radiofrequency ablation combined with ethanol ablation and radiofrequency ablation in the treatment of benign cystic solid thyroid nodule. Methods:A total of 80 patients who visited the Thyroid Surgery Department of the First Affiliated Hospital of Zhengzhou University from January 2015 to July 2018 were selected. All selected patients are required to meet the following criteria: (1)Color doppler ultrasonography of the neck revealed a cystic solid thyroid nodule greater than 20 mm in diameter. (2) The results of fine needle aspiration cytology of thyroid nodules were benign. (3)The patients is to undergo radiofrequency ablation of thyroid nodule. According to the condition and patients′ wishes, radiofrequency ablation (Group A, n=40) and combined ethanol and radiofrequency ablation(Group B, n=40) were performed respectively to observe the changes of nodule volume and maximum diameter at 3, 6 and 12 months after surgery. The difference of intraoperative radiofrequency ablation energy, postoperative complications and patient satisfaction at 12 months after operation were also observed. The respective clinical effects of the two groups and the difference of curative effects between the two groups were analyzed. Two-factor repeated measurement analysis of variance or independent sample t test was used to compare the measurement data in line with normal distribution between groups. Friedman′s rank sum test was used for comparison of measurement data groups that did not conform to normal distribution, and Bonferroni correction was used for pairwise comparison. Chi-square test was used to compare the counting data between groups. Results:On the 12th months after operation, the volume reduction of of nodules in group B was greater than that of group A, and the difference was statistically significant[(7.0±5.1) mL vs (5.5±4.9) mL, P<0.05]. The maximum diameter reduction of nodules in group B was greater than that of group A and the difference was statistically significant [(1.5±0.6) cm vs (1.4±0.8) cm, P<0.05]. During the period of 6 to 12 months after operation, the trend of nodular shrinkage in group B was more obvious than that in group A ( P<0.05). The radiofrequency ablation energy of group was lower than that of group A, and the difference was statistically significant [(2.37±1.18) kJ vs (3.89±1.17) kJ, P<0.05]. Voice reduction occurred in 2 cases and recovered within 2 weeks.Local bleeding occurred in 1 case during the operation, which was stopped after ablation. There was no statistical significance in the satisfaction of patients in group A and group B (87.5% vs 90%, P>0.05). Conclusion:Compared with radiofrequency ablation, radiofrequency ablation combined with ethanol ablation for benign cystic solid thyroid nodules can achieve better nodule reduction effect and reduce the ablation energy.
