1.Quality Evaluation of Gegen Qinlian Tablets Based on HPLC Multi-component Quantification Combined with Chemical Pattern Recognition and TOPSIS Analysis

ObjectiveTo establish a high-performance liquid chromatography(HPLC) for the quantitative analysis of multiple components in Gegen Qinlian tablets, and to comprehensively evaluate the quality of samples from different manufacturers by integrating chemical pattern recognition and technique for order preference by similarity to ideal solution(TOPSIS), in order to provide a reference basis for quality evaluation and control of Gegen Qinlian tablets. MethodsHPLC was employed to determine the contents of 10 components in 28 batches of Gegen Qinlian tablets collected from 6 manufacturers, and taking the detection results as variables, SIMCA 14.1 and SPSS 26.0 were employed for cluster analysis(CA), principal component analysis(PCA), and orthogonal partial least squares-discriminant analysis(OPLS-DA) to identify key components affecting the quality. Then, TOPSIS analysis was employed to rank the quality of Gegen Qinlian tablets from the 6 manufacturers and establish a comprehensive quality evaluation method. ResultsA quantitative method for Gegen Qinlian tablets was established. After methodological validation, the method was found to be stable and reliable, and could be used for the quantitative analysis of this preparation. The contents of 3′-hydroxy puerarin, puerarin, 3′-methoxy puerarin, daidzein, coptisine hydrochloride, epiberberine, jatrorrhizine hydrochloride, berberine hydrochloride, palmatine hydrochloride and baicalin in 28 batches of samples were 3.58-7.35, 24.88-42.32, 4.20-9.36, 4.33-7.60, 2.52-6.44, 0.93-4.10, 0.58-3.05, 10.68-22.92, 0.82-4.82, 11.73-60.16 mg·g-1, respectively. Among them, puerarin, berberine hydrochloride and baicalin all met the limit requirements for this preparation specified in the 2025 edition of the Pharmacopoeia of the People's Republic of China. CA and PCA clustered the 28 batches of samples into 5 categories, PCA extracted 2 principal components with a cumulative variance contribution rate of 90.588%, and OPLS-DA screened out 4 differential markers with variable importance in the projection(VIP) values>1.0, namely baicalin, 3′-hydroxy puerarin, coptisine hydrochloride and palmatine hydrochloride, which might be the main components affecting the quality of Gegen Qinlian tablets. TOPSIS analysis showed that the comprehensive score of each evaluation index(Ci) values of different manufacturers were different. Among them, the Ci of manufacturer B was ranked higher, indicating potentially superior quality, while the Ci of manufacturer A was ranked lower, suggesting potentially inferior quality. ConclusionThis study establishes a quantitative method for Gegen Qinlian tablets, and the content uniformity of the same manufacturer is good, while there are differences in the contents of active components among different manufacturers. Through the chemical pattern recognition analysis, it is found that the content differences of Gegen Qinlian tablets may be related to baicalin, 3′-hydroxy puerarin, coptisine hydrochloride and palmatine hydrochloride.
2.Anesthesia effect of remazolam or ciprofol combined with remifentanil for painless gastroscopy in elderly overweight patients

Objective To compare and analyze the application effects of ciprofol+remifentanil and remazolam+remifentanil in painless gastrointestinal gastroscopy in elderly overweight patients.Methods The subjects of this study,122 elderly overweight patients for painless gastrointestinal endoscopy from October 2023 to October 2024,were randomly divided into H group and R group,each with 61 cases.At the same time,122 elderly patients with normal body weight who planned to undergo painless gastrointestinal endoscopy during the same period were randomly divided into H1 group and R1 group,61 cases in each group.All subjects received a slow intravenous injection of remifentanil at a dose of 0.5 μg/kg,30 s later Group H and H1 group were administered with a slow intravenous injection of ciprofol at a dose of 0.4 mg/kg,while Group R and R1 group were given a slow intravenous injection of remazolam at a dose of 0.2 mg/kg.Anesthesia effect and perioperative indicators(examination time,induction time,etc.),and the hemodynamic parameters[mean arterial pressure(MAP),heart rate(HR)and percutaneous arterial oxygen saturation(SpO2)]were compared of the four groups immediately after the establishment of electrocardiogram monitoring(T0),upon completion of anesthesia induction(T1),when gastroscopy passed through the throat(T2),when enteroscopy passed through the hepatic flexure(T3),and immediately after completion of surgery(T4),as well as adverse reactions were compared of the four groups.Results Excellent anesthesia rate:group H and H1 group were higher than group R(96.72%,98.36%vs.86.89%,P<0.05).Induction time:no difference was found of the four groups(P>0.05).Examination time:group H and group R were significantly longer than that in group H1 and group R1(P<0.05),but there was no significant difference between group H and group R,and between group H1 and group R1(P>0.05).The time of awakening and moving out of postanesthesia care unit(PACU):group H,group R,group H1 and group R1 gradually decreased,and the differences between each two groups were statistically significant(P<0.05).There were statistically significant differences in MAP and HR of the four groups,time and interaction effects(P<0.05).The difference of SpO2 of the four groups was statistically significant(P<0.05),and the difference of time and interaction effect was not statistically significant(P>0.05).Incidence of injection pain:group H and group H1 were lower than group R and group R1(P<0.05).Incidence of other adverse reactions:no significant difference of four groups(P>0.05).Conclusion During painless gastrointestinal endoscopy of elderly overweight patients the application of ciprofol+remifentanil can achieve better anesthetic effect and lower incidence of injection pain as compared with remazolam+remifentanil.The anesthetic effect of the two anesthesia regimens is similar in elderly normal weight patients,but the elderly normal weight patients have a faster recovery from anesthesia than the elderly overweight patients.
3.Evaluation of vaccine's effectiveness based on school varicella outbreaks

Objective To evaluate the effectiveness of vaccination on the onset of varicella and clinical symptoms of varicella based on the survey data of varicella outbreaks in schools.Methods Six schools with chickenpox outbreaks in Shaanxi Province were selected as the sites,and students in the classes or dormitories where the cases were located were chosen as the research subjects.A 1∶2 matching case control was used to conduct a questionnaire survey to analyze the relationship between the history of varicella vaccine immunization and the incidence of chickenpox so as to evaluate the effectiveness of varicella vaccine.Results The total prevalence of the six varicella outbreaks involved in this study was 1.39%.The proportion of chickenpox vaccinations was 44.53%,of which 15.78% were vaccinations with two doses.The overall effectiveness of varicella vaccine was 63.52% (95% CI:40.690%-77.560%),and the effectiveness of one and two doses was 58.72% and 72.90%,respectively.The effectiveness of varicella vaccine against moderate to high fever and moderate to severe rash was 82.77% and 76.27%,respectively.The risk of chickenpox in people with vaccination intervals of 6 to 8 years and ≥ 9 years was 13.05 times and 3.93 times higher than that of those vaccinated before the age of 3 years,respectively.Conclusion Varicella vaccine has a good effectiveness on high-exposure people,and the effectiveness of two doses is better than that of one dose.Varicella vaccination can lessen the clinical symptoms of chickenpox.It is recommended to further strengthen the publicities of varicella vaccination and increase the number of children to complete two doses of varicella vaccination before entering nurseries,so as to actively and effectively control varicella outbreaks.

6.Application of Artificial Intelligence in Sperm Quality Analysis and Sperm Screening

Infertility is a global health issue,and more and more people are hoping to have babies by means of assisted reproductive technology.However,there are still many challenges in fertilization and pregnancy outcomes.Sperm quality is a key factor affecting the success rate of assisted reproduction.Therefore,sperm quality screening is crucial for achieving breakthroughs in assisted reproduction technology.At present,with its capabilities in the field of image recognition,artificial intelligence(AI)is providing new ideas and methods for sperm screening.Various attempts have been made with AI-based models to evaluate indicators such as sperm morphology,DNA quality,and motility level,and some results have been achieved.Herein,we reviewed the application of AI in sperm quality analysis and selection,providing support for the future development of AI and the improvement in the fertilization rate and outcomes of assisted reproductive technology.
7.Clinical study on thermal moxibustion combined with Duhuo Jisheng Decoction for the patients with knee osteoarthritis and liver and kidney deficiency syndrome

Objective:To explore the clinical efficacy of thermal moxibustion combined with Duhuo Jisheng Decoction in the treatment of liver and kidney deficiency syndrome of knee osteoarthritis (KOA).Methods:From January 2020 to January 2021, 124 KOA patients with liver and kidney deficiency syndrome, who met the inclusion criteria, were divided into 2 groups according to the random number table method, with 62 in each group. The control group was treated with Duhuo Jisheng Decoction, and the observation group was treated with thermal moxibustion on the basis of the control group. Both groups were treated for 28 days. TCM symptom scores were performed before and after treatment, the Osteoarthritis Index of Western Ontario and McMaster University (WOMAC) was used to evaluate joint function. ELISA was used to detect serum insulin-like growth factor-1 (IGF-1), fibroblast growth factor-2 (FGF-2), transforming growth factor-β1 (TGF-β1), IL-1β, IL-6, TNF-α levels, and the clinical efficacy was evaluated.Results:The total effective rate was 91.9% (57/62) in the observation group and 77.4% (48/62) in the control group, and there was significant difference between two groups ( χ2=5.04, P=0.025). After treatment, the TCM symptom score and WOMAC score of the observation group were significantly lower than those of the control group ( t values were 11.33 and 12.23, respectively, all Ps<0.01). After treatment, the serum levels of IGF-1 [(15.63±2.03) ng/L vs. (12.78±1.57) ng/L, t=8.75], FGF-2 [(30.26±5.37) ng/L vs. (26.31±1.94) ng/L, t=5.45] and TGF-β1[(30.39±6.71)μg/L vs. (24.31±5.12) μg/L, t=5.67] in the observation group were significantly higher than those in the control group ( P<0.01), while the levels of IL-1β [(12.50±3.36) ng/L vs. (16.09±4.90) ng/L, t=4.76], IL-6 [(10.59±3.28) ng/L vs. (21.75 ± 4.09) ng/L, t=16.76] and TNF-α [(4.04±1.92) ng/L vs. (6.48±1.43) ng/L, t=8.03] in the observation group were significantly lower than those in the control group ( P<0.01). There was no adverse events from both groups during treatment. Conclusion:Thermal moxibustion combined with Duhuo Jisheng Ddecoction can alleviate the joint pain of KOA patients with liver and kidney deficiency syndrome, promote cartilage repair and improve the clinical curative effect.
8.Correlation analysis among morning blood pressure surge,ambulatory arterial stiffness index and ca- rotid artery intima-media thickness

Objective:To explore the correlation among morning blood pressure surge (MBPS) ,ambulatory arterial stiffness index (AASI) and carotid artery intima‐media thickness (IMT) .Methods :A total of 212 hypertension pa‐tients received 24h ambulatory blood pressure monitoring and carotid artery ultrasonography to measure carotid IMT .According to IMT thickened or not ,they were divided into normal IMT group (n=84) and thickened IMT group (n = 128) ,then MBPS and AASI were calculated .According to MBPS existed or not ,patients were divided into MBPS group (n = 120) and no MBPS group (n = 92) ,then the correlation among MBPS ,AASI and carotid IMT were observed .Results : Compared with normal IMT group ,there were significant rise in AASI [ (0.42 ± 0.13) vs .(0.68 ± 0.14)] and MBPS value [ (25.94 ± 4.57) mmHg vs .(36.57 ± 8.41) mmHg] in thickened IMT group , P < 0.05 both ;compared with no MBPS group ,there were significant rise in AASI [ (0.43 ± 0.13) vs .(0.71 ± 0.14)] and IMT [(1.01 ± 0.20) mm vs .(1.25 ± 0.17) mm] in MBPS group ,P < 0.05 both ;Linear correlation analysis indicated that MBPS and AASI were positively correlated with carotid IMT ( r = 1.22 ,0.51 , P < 0.05 both) .Conclusion : Morning blood pressure surge and ambulatory arterial stiffness index are independent risk factors for carotid artery intima‐media thickness ,and they possess predictive value .
