Objective To investigate the therapeutic mechanism of Shen Wu Yizhi Capsule in the treatment of post-stroke cognitive impairment(PSCI)by using network pharmacology methods and clinical trial validation.Methods A prospective trial was carried out in 90 cases of patients with PSCI admitted to Taihe Traditional Chinese Medicine Hospital Affiliated to Anhui University of Chinese Medicine from August 2022 to February 2024.The patients were randomly divided into the control group and the trial group by random number table method,with 45 cases in each group.The control group was treated with conventional treatment for PSCI,and the trial group was treated with Shen Wu Yizhi Capsule orally on the basis of treatment for the control group.The treatment course for the two groups covered 28 days.The changes of Mini-Mental State Examination(MMSE)score,Montreal Cognitive Assessment(MoCA)score,and the serum levels of inflammatory factors such as tumor necrosis factor α(TNF-α)and interleukin 6(IL-6)in the patients of the two groups were observed before and after treatment.Moreover,the incidences of adverse events in the two groups were recorded,thus to evaluate the safety of the treatment regimens in the two groups.And then the network pharmacological research was performed.TCMSP and literature review were used to obtain the active ingredients of Shen Wu Yizhi Capsule,GeneCards and other databases were used to obtain the PSCI disease targets,and the common targets were inputted into the STRING database to construct the PPI network.Cytoscape 3.9.0 was used to construct the network diagram of Shen Wu Yizhi Capsule-PSCI-targets,DAVID was used to perform GO and KEGG pathway enrichment analysis,and then molecular docking was used to verify the binding activity.Results(1)The results of clinical trial showed that after 28 days of treatment,the MMSE and MoCA scores of patients in the two groups were increased compared with those before treatment(P<0.05),and the increase of the scores in the trial group was significantly superior to that in the control group(P<0.05).The serum levels of TNF-α and IL-6 were decreased compared with those before treatment(P<0.05),and the decrease in the trial group was significantly superior to that in the control group(P<0.05).During the trial,both groups of patients did not show obvious adverse reactions,with high safety.(2)The network pharmacological research of Shen Wu Yizhi Capsule yielded 92 active ingredients,803 targets,5 209 disease targets and 556 intersection targets.The core targets were AKT1,TNF,IL-6,TP53 and IL-1B,and the key compounds were deoxyharringtonine,senkyunone and genkwanin.The GO enrichment analysis obtained 1 812 GO entries,of which 154 entries were related with cellular component(CC),1 332 entries were related with biological process(BP),and 326 entries were related with molecular function(MF).The KEGG pathway enrichment analysis yielded 195 signaling pathways.The molecular docking results showed that the key compounds of Shen Wu Yizhi Capsule had good binding activities with the core targets.Conclusion The clinical efficacy of Shen Wu Yizhi Capsule in the treatment of PSCI is remarkable,and its therapeutic mechanism is probably related with multiple components through the signaling pathways such as AKT1,TNF,and IL-6.The results will provide reference for the in-depth study of Shen Wu Yizhi Capsule.

Objective To observe the clinical advantages of acupuncture combined with Lokohelp robot rehabilitation in the treatment of lower limb hemiplegia following acute ischemic stroke. Methods From June 2017 to May 2018,117 patients with acute ischemic stroke hemiplegia were selected in the Traditional Chinese Medicine Hospital of Taihe County. The patients were divided into control group and observation group by random digital table method. There were 58 cases in the control group and 59 cases in the observation group. The patients of the two groups were all treated with rehabilitation and essential drugs. The control group received routine rehabilitation training,and the observation group received acupuncture combined with Lokohelp robot gait rehabilitation on the basis of routine rehabilitation training. Thirty days was set as the end time of the test. Results After treatment,the FMA - B score of the control group was (9. 15 ± 2. 58)points,which of the observation group was (12. 85 ± 2. 69)points. The FMA - L score of the control group was (23. 78 ± 5. 33)points,which of the observation group was (29. 79 ± 6. 45)points. The levels of FMA - B and FMA - L in the two groups were higher than those before treatment,and the levels in the observation group were higher than those in the control group,the differences were statistically significant(t = 3. 731,11. 441, 7. 213,11. 318,7. 591,5. 489,all P < 0. 01). After treatment,the BaRthel index score of the control group was (56. 11 ± 17. 12),which of the observation group was (79. 59 ± 21. 31). The BaRthel index score of the two groups was higher than those before treatment,and the score in the observation group was higher than that in the control group,the difference was statistically significant(t = 12. 355,18. 279,6. 563,all P < 0. 01). Conclusion Acupunc-ture combined with Lokohelp robot rehabilitation for hemiplegic patients with acute ischemic stroke can significantly improve the degree of neurological impairment,reduce the score of neurological impairment,and synergetically improve the patients' walking ability,balance function,motor function and daily life activity ability.

Objective To observe the clinical effect of Shenwuyizhi capsule in the treatment of vascular dementia.Methods According to the digital table method,80 patients with vascular dementia were divided into the treatment group and control group,each group with 40 cases.On the basis of routine treatment,the control group was treated with piracetam tablets,and the treatment group was treated by the Shenwuyizhi capsule.60 d for 1 course.Before and after treatment,the simple mental state examination(MMSE) scale scores,Blessed behavior scale scores were compared between the two groups.Results Before and after treatment,the MMSE score,Bcessed behavior scale scores of the two groups were significantly improved (all P ＜ 0.05).After treatment,the MMSE score,Bcessed behavior scale scores of the treatment group were (19.83 ± 5.24) points,(7.18 ± 4.21) points,respectively,which were better than those of the control group [(20.01 ± 4.89) points,(7.23 ± 3.99) points] (t =4.148,2.141,all P ＜0.05).Conclusion Shenwuyizhi capsule can improve the light,moderate cognitive function in patients with vascular dementia,and improve memory and computing power,directional force,and enhance patient behavior ability,and reduce dementia symptoms,with good curative effect in the near future.

Objective To explore the effect of Xiongbiyihao mixture in the treatment of coronary heart disease with angina pectoris.Methods 90 coronary heart disease patients with angina pectoris were divided into the treatment group( 60 cases) and control group( 30 cases).The control group was given routine westerm medicine therapy,and the treatment group was administered with Xiongbiyihao mixture at the base of the control group.Ater treatment,the heart disease attack frequency,duration,and degree,clinical symptoms and electrocardiogram changes of the two groups were observed.Results The total efficiency symptoms of and electrocardiogram of the treatment group was 95.0％,90.0％,respectively,and was higher than that of the control group(76.7％,73.3％ ) ( x2 =9.7,x2 =7.8,all P ＜ 0.05).Conclusion Xiongbiyihao mixture could effectively improve the coronary blood,reduce myocardial ischemia,and alleviate the symptoms of angina pectoris,chest tightness,chest pain.It is safe and effective in the treatment of angina pectoris,and worthy of clinical application.
