Objective:To analyze the genetic characteristics of hemagglutinin(HA)and neuraminidase(NA)of influenza A(H1N1)pdm09 viruses in Kunming during the 2022-2023 influenza season.Methods:A total of 15 strains of A(H1N1)pdm09 influenza virus isolated from sentinel hospital surveillance and from outbreaks from April 2022 to March 2023 in Kunming were chosen for sequencing. The genetic analysis,which included sequence alignment,homology analysis,construction of phylogenetic tree and amino-acid mutations analysis,was carried out using MAFFT version 7,MegAlign and MEGA 11.Results:Fourteen strains isolated during the 2022-2023 influenza season in Kunming belong to clade 6B.1A.5a.2a,and A/Kunming/284/2023 belonged to clade 6B.1A.5a.2a.1. They all diverged from the northern hemisphere vaccine strain A/Wisconsin/588/2019 in clade 6B.1A.5a.2 recommended by WHO during the 2022-2023 influenza season. Comparing with the HA of the vaccine strain:A186T and Q189E,which might cause the reduction of vaccine protection,were identified in Sb of 14 strains;P137S,K142R in Ca and A186T,Q189E in Sb were identified in A/Kunming/284/2023 and two strains isolated from Thailand. The four mutations at tow antigenic sites identified immune escape at the molecular level. Q189E in the 190-helix and E224A in the 220-loop,which might change the pathogenicity of A(H1N1)pdm09 viruses,were identified in 15 strains. Comparing with NA of the vaccine strain:S200N in 14 strains and S339L in 1 strain were identified in antigenic sites. The two mutations might reduce the protection of antibodies induced by NA.Conclusion:Strengthening influenza surveillance and timely detecting new variants in Kunming contributes to preventing the importation of foreign strains and issuing early warnings for influenza outbreaks.

4.Endovascular recanalization for symptomatic non-acute internal carotid artery occlusion:analysis of its short-term efficacy

Objective To discuss the short-term efficacy of endovascular recanalization for symptomatic non-acute internal carotid artery occlusion.Methods A total of 90 patients with symptomatic non-acute internal carotid artery occlusion,who were admitted to the Department of Neurology of Tianjin Municipal Teda Hospital of China from August 2017 to December 2021,were selected as the research objects.The patients were divided into study group(n=45)and control group(n=45).Percutaneous endovascular recanalization of internal carotid artery occlusion was performed for the patients of the study group,and standardized antiplatelet aggregation and anti-lipid therapy(including oral aspirin,clopidogrel bisulphate and atorvastatin)was adopted for the patients of the control group.The symptom recurrence rate at one year after treatment in both groups was analyzed.Results Of the 45 patients in the control group,4 were lost in touch,and 41 completed the one-year follow-up.Of the 45 patients in the study group,2 patients had failure of surgery,one patient had lost contact visit,and 42 completed the one-year follow-up.Postoperative intracranial hemorrhage occurred in 2 patients.After one year of follow-up,in the control group 26 patients(63.41%)developed recurrence of symptoms,presenting as transient ischemic attack(TIA,n=13,31.7%)and cerebral infarction(n=13,31.7%),and in the study group 8 patients(4.76%)developed recurrence of symptoms,presenting as TIA(n=6,14.3%)and cerebral infarction(n=2,4.8%);the incidence of cerebral infarction in the study group was strikingly lower than that in the control group,and the difference between the two groups was statistically significant(P<0.05).In the patients with grade Ⅲ compensation,the recurrence rate of symptoms was remarkably decreased after endovascular recanalization of internal carotid artery occlusion,and the difference between the two groups was statistically significant(P<0.05).However,in the patients with grade Ⅰ or grade Ⅱ compensation,although the recurrence rate of symptoms was decreased after endovascular recanalization of internal carotid artery occlusion,the difference between the two groups was not statistically significant(P>0.05).Conclusion For the treatment of patients with symptomatic non-acute internal carotid artery occlusion,percutaneous endovascular recanalization of internal carotid artery occlusion is clinically safe,it can significantly decrease the recurrence rate of symptoms.

Schizophrenia is a common, severe, and complex psychiatric disorder worldwide. Genetic factors account for around 80% of the etiology of schizophrenia, yet objective diagnostic biomarkers remain lacking. This article reports two cases of female monozygotic twins diagnosed with schizophrenia, exhibiting a balanced translocation between 22q11.2 and 4p15.3. Reviewing the literature, we analyze and discuss the correlation between chromosomal balanced translocation regions and the pathogenesis of mental disorders. This aims to encourage psychiatrists to consider new perspectives on the diagnosis of schizophrenia.

7.Meta-analysis of the Effectiveness and Safety of the Sedative Effect of Remimazolam in Endoscopy

OBJECTIVE
To systematically evaluate the efficacy and safety of the sedative effect of remimazolam in endoscopy and to compare it with propofol and midazolam.
METHODS
Search PubMed, Embase, Cochrane Library, Wanfang database, CNKI and other databases to collect the literature of randomized controlled trials of remimazolam for sedation in endoscopy. The search period was from 2018 onwards when remimazolam was approved for clinical trials until April 2022. The search strategy included the following variable keywords: remimazolam, gastroscopy, bronchoscopy, and colonoscopy. The quality of the included literature was assessed and the collected data were subjected to meta-analysis by RevMan 5.4 software.
RESULTS
Ten relevant RCTs involving midazolam and propofol, involving a total of 2 076 patients were included in the analysis. The results showed that the sedative effect of remimazolam was significantly higher than that of midazolam [OR=0.03, 95%CI(0.02, 0.05), I2=0%, P<0.000 01]; but lower than that of propofol [OR=11.32, 95%CI(2.12, 60.56), I2=0%, P=0.005]. The onset time of remimazolam was longer than that of propofol, but shorter than that of midazolam; the recovery time was faster than that of propofol and midazolam. Compared with midazolam, there was no significant difference in the incidence of adverse reactions. Compared with propofol, remimazolam was associated with lower rates of hypotension, slowed heart rate, hypoxemia, and injection pain, but higher risk ratio of nausea, with no difference invomiting.
CONCLUSION
The sedative effect and onset of action of remimazolam are better than midazolam but less than propofol when used for endoscopy. Wake-up time is faster than that of propofol and midazolam. The incidence of respiratory and circulatory depression is lower with remimazolam than with propofol, and there are no significant differences in adverse effects compared with midazolam.
8.Guided endodontics for the treatment and vital pulp concervation of type Ⅲ Dens Invaginatus in an immature maxillary lateral incisor with periapical periodontitis

A young maxillary lateral incisor of Oehlers type Ⅲ Dens invaginatus with peri-invagination periodontitis was reconstructed by CBCT,with the help of guided endodontics,the pathway to invagination was successfully established.The invaginated pseudo-root canal was treated with Vitapex mediation while preserving the pulp.After 6-month follow-up,the tooth was clinically asymptomatic.Radiological ex-amination indicated complete healing of the peri-invagination lesion with narrowed open apex,and the thickened root canal wall.
9.Correlation between stereopsis and cognitive function in first-episode drug-na?ve patients with schizophrenia

Objective:To explore stereoscopic vision and its correlation with cognitive function in first-episode drug-na?ve patients diagnosed with schizophrenia (FNPS).Methods:A study was conducted from January 2019 to September 2020. A total of 146 FNPS, 124 patients with chronic schizophrenia (PCS) and 101 healthy controls (HCs) were recruited. Stereoscopic vision was evaluated by Titumus stereotests. Their clinical symptoms were assessed by positive and negative syndrome scale. Cognitive function was assessed by the repeatable battery for the assessment of neuropsychological status (RBANS). The differences in stereoscopic vision and cognitive function among the three groups were analyzed by analysis of covariance. The Spearman correlation and multiple regression analysis were used to identify the correlation between stereoscopic vision and clinical symptoms or cognitive function.Results:(1) There were significant differences in stereoscopic vision and cognitive function among the three groups ( P<0.05). After pairwise comparison, it was found that the stereoscopic vision and cognitive functions of FNPS group and PCS group were significantly different from HCs group. However, there were no significant differences in stereoscopic vision and cognitive function between FNPS and PCS groups. (2) Spearman correlation analysis showed that stereoscopic vision was not correlated with the severity of clinical symptoms in FNPS, but it was correlated with visuospatial score ( r=-0.193, P=0.019), language score ( r=-0.261, P=0.001), attention score ( r=-0.168, P=0.042), and RBANS total scores ( r=-0.236, P=0.004). Moreover, there was no correlation of stereoscopic vision with the severity of clinical symptoms or cognitive function in HCs and PCS. Further multiple linear regression showed the significant effect of stereoscopic vision on visuospatial score ( β=-0.213, P=0.011), language score ( β=-0.252, P=0.003), attention score ( β=-0.189, P=0.019), RBANS total score ( β=-0.235, P=0.003) in FNPS. Conclusions:FNPS and PCS show significant impairments in stereoscopic vision and cognitive function. Stereoscopic vision is closely correlated with cognitive function rather than the severity of clinical symptoms in FNPS.
