OBJECTIVE To establish a method for simultaneous determination of plasma concentration of lamotrigine(LTG), levetiracetam(LEV) and perampanel(PER) in children with epilepsy and apply this method in clinical practice. METHODS Plasma proteins were precipitated with acetonitrile. Using PER-D 5 as internal standard, ultra-high performance liquid chromatography-tandem mass spectrometry (UPLC-MS/MS) method was adopted. The determination was performed on ACQUITY UPLC HSS T3 C 18 column with mobile phase consisted of 0.1% formic acid with 5 mmol/L ammonium acetate-acetonitrile (gradient elution) at the flow rate of 0.3 mL/min. The column temperature was 40 ℃, and sample size was 5 μL. The analysis time was 5 min. The electrospray ionization source and multiple reaction monitoring mode were used for positive ion scanning. The ion pairs used for quantitative analysis of LTG, LEV, PER and internal standard were m / z 255.9→144.9, m / z 171.1→126.1, m / z 350.1→219.0 and m / z 354.9→220.2, respectively. The steady-state trough concentrations of the aforementioned drugs in the plasma of 14 pediatric epilepsy patients receiving combination therapy were determined using the same UPLC-MS/MS method as above. RESULTS The linear ranges of LTG, LEV and PER were 0.15-24 μg/mL ( R 2 >0.993), 0.312 5-50 μg/mL ( R 2 >0.997) and 6.25-1 000 ng/mL ( R 2 >0.997), respectively. The lower limits of quantification were 0.15 μg/mL, 0.312 5 μg/mL and 6.25 ng/mL, respectively. RSDs of intraday and interday precision tests of the three drugs were no more than 9.83%, and the accuracies (relative errors) were between -9.33% and 13.72%( n =6 or n =18); the average extraction recovery rates were 86.4%-97.9%, and the average matrix effects were 86.9%-110.0% ( n =6). The absolute values of the relative errors in the stability tests were all below 15%. The steady-state trough concentrations of LTG, LEV and PER were (5.64±4.03)μg/mL, (10.67±8.78)μg/mL and(450.20±251.27)ng/mL, respectively; the rates of achieving target trough concentrations were 71.4%, 37.5% and 84.6%, respectively. CONCLUSIONS The established UPLC-MS/MS method is specific, rapid and suitable for the plasma concentration monitoring in epileptic children receiving combination therapy.

Objective To develop a nomogram-based predictive model for assessing the risk of respiratory distress syndrome(RDS)in premature infants in the high-altitude region of Dali. The predictive performance and clinical applicability of the model will be systematically evaluated to provide evidence-based guidance for the early diagnosis and clinical management of respiratory distress in premature infants. Methods A total of 680 preterm infants admitted to the Dali Maternal and Child Health Hospital between January 2020 and December 2024 were enrolled in the study and randomly divided into a training set(n=476)and a validation set(n=204)at a ratio of 7∶3.Independent predictors were identified through univariate logistic regression and multivariate stepwise regression analyses, and a nomogram model was subsequently developed using R software. The performance of the model, including its discrimination, calibration, stability, and clinical applicability, was evaluated using the receiver operating characteristic curve(ROC),Hosmer-Lemeshow goodness-of-fit test, bootstrap resampling method, and decision curve analysis(DCA).Results The final model incorporated seven independent variables:gestational age, birth weight,Apgar score, blood oxygen saturation, gestational hyperglycemia, prenatal glucocor-ticoid therapy, and maternal history of infection. The areas under the curve(AUCs)for the training and validation sets were 0.88(95%CI:0.84~0.92)and 0.83(95%CI:0.76~0.89),respectively,with all Hosmer-Lemeshow test p-values exceeding 0.05.The bootstrap-corrected AUC was 0.85(95%CI:0.81~0.89).DCA indicated that the model achieved the highest net benefit at a risk threshold range of 10%to 35%.Conclusions This model integrates multiple risk factors associated with the occurrence of RDS in plateau environments, demonstrating robust predictive performance for RDS in preterm infants residing in high-altitude areas such as Dali.It can serve as a valuable tool for risk stratification and clinical decision-making,and may also provide a reference for future multicenter prospective studies.

From August 2019 to July 2024, the Institute of Chest Wall Surgery at the Affiliated Guangdong Second Provincial General Hospital of Jinan University admitted multiple patients with severe thoracic deformities. Authors summarized the characteristics and treatment measures of three cases with typical acute-angled thoracic deformities. Three patients aged from 15 to 21 year-old were suffered from severe pectus excavatum, pectus carinatum, and complex thoracic deformities, respectively. Visually, the local bones were severely distorted, and CT scans showed that the local bones formed acute-angled intersections. Three patients underwent Wung surgery, Wenlin surgery, and Wang+ Wenlin surgery. During the operation, based on pre-shaping, the principle of template shaping was applied to implement correction, and the process was uneventful. All deformities were satisfactorily corrected. There were no postoperative complications such as bleeding, pneumothorax, or poor wound healing during 8 months to 4 years of follow-up. Acute angle deformity is a special type of thoracic deformity, and its occurrence often indicates that the local deformity is severe and complex. Therefore, in treatment, surgeons are required to first understand the basic principle of thoracic deformity correction, have a strong comprehension of the overall deformity, and be skillful and experienced in applying various thoracic deformity correction techniques.

BACKGROUND:
The effect of the interval between previous Helicobacter pylori (H. pylori) eradication and rescue treatment on therapeutic outcomes remains unknown. The aim of this study was to investigate the association between eradication rates and treatment interval durations in H. pylori infections.
METHODS:
This prospective observational study was conducted from December 2021 to February 2023 at six tertiary hospitals in Shandong, China. We recruited patients who were positive for H. pylori infection and required rescue treatment. Demographic information, previous times of eradication therapy, last eradication therapy date, and history of antibiotic use data were collected. The patients were divided into four groups based on the rescue treatment interval length: Group A, ≥4 weeks and ≤3 months; Group B, >3 and ≤6 months; Group C, >6 and ≤12 months; and Group D, >12 months. The primary outcome was the eradication rate of H. pylori . Drug compliance and adverse events (AEs) were also assessed. Pearson's χ2 test or Fisher's exact test was used to compare eradication rates between groups.
RESULTS:
A total of 670 patients were enrolled in this study. The intention-to-treat (ITT) eradication rates were 88.3% (158/179) in Group A, 89.6% (120/134) in Group B, 89.1% (123/138) in Group C, and 87.7% (192/219) in Group D. The per-protocol (PP) eradication rates were 92.9% (156/168) in Group A, 94.5% (120/127) in Group B, 94.5% (121/128) in Group C, and 93.6% (190/203) in Group D. There was no statistically significant difference in the eradication rates between groups in either the ITT ( P = 0.949) or PP analysis ( P = 0.921). No significant differences were observed in the incidence of AEs ( P = 0.934) or drug compliance ( P = 0.849) between groups.
CONCLUSION:
The interval duration of rescue treatment had no significant effect on H. pylori eradication rates or the incidence of AEs.

With the extension of the global population's lifespan and the increasingly severe aging problem,cardio-vascular diseases have become the leading cause of death among the elderly population. Most cardiovascular diseases orig-inate from the formation of atherosclerotic plaques. In addition to common risk factors such as dyslipidemia,diabetes,hy-pertension,smoking,and obesity, circadian rhythm disruption is also regarded as an important but often overlooked risk factor for atherosclerosis. The circadian rhythm is involved in regulating key physiological processes such as inflammation and metabolism, which in turn affect the pathological processes of arteriosclerosis and thrombosis. In this process, the key genes that maintain the stability of the circadian rhythm, namely clock gene, play a crucial role.Clock gene have an important role in the pathological mechanism of atherosclerosis, and they may become potential new targets for the preven-tion and treatment of atherosclerosis. This paper reviews the latest research progress on the mechanism of action of clock gene in the occurrence and development of atherosclerosis. These findings may provide new ideas for the diagnosis and treatment of atherosclerosis.
