1.Analysis of pathogen species and clinical characteristics in patients with acute upper respiratory tract infection

Objective To analyze the pathogen species and clinical characteristics in patients with acute up-per respiratory tract infection.Methods A total of 275 patients with acute upper respiratory tract infection admitted to Jinling Hospital,Affiliated Hospital of Medical School,Nanjing University from December 1,2023 to May 15,2024 were selected as research objects.Real-time fluorescence quantitative polymerase chain reac-tion was used to detect pathogen species in throat swab samples,and clinical data and peripheral blood test in-dexes were collected in the medical record system.Results The positive detection rate of influenza A virus was 9.45%,influenza B virus 17.82%,respiratory syncytial virus 9.45%,adenovirus 9.21%,severe acute re-spiratory syndrome coronavirus 2 18.37%,mycoplasma pneumoniae 10.04%,and chlamydia pneumoniae 1.67%.Compared with other age groups,the positive detection rates of influenza A virus,influenza B virus,respiratory syncytial virus,adenovirus,mycoplasma pneumoniae and chlamydia pneumoniae in patients<18 years old were higher,and the difference was statistically significant(P<0.05).The proportion of monocytes in patients infected with influenza B virus was higher than that in patients infected with other pathogens,and the difference was statistically significant(P<0.05).Conclusion Identification the pathogen types of acute upper respiratory tract infection patients is helpful for developing appropriate treatment plans and providing a basis for clinical diagnosis and treatment for the patients.
2.Clinical Experience of Qin Zhenhua in Treating Cutaneous Pruritus

This study summarizes Professor Qin Zhenhua's clinical experience in treating cutaneous pruritus using a modified self-prescribed formula,Yinqiao Xingzi Decoction.Following the viewpoints from the classical Chinese medical literature and after years of clinical practice,Professor Qin proposes that the onset of cutaneous pruritus is often associated with the attack of exogenous pathogens,disharmony of qi and blood,or debility due to prolonged illness.The disease primarily involves the skin,with the fundamental pathogenesis of pathogens stagnating in the muscular superficies.Clinically,the treatment is frequently conducted from the perspective of fire-heat syndrome,and the therapeutic methods of clearing heat and removing toxins,releasing muscles and dispersing pathogens from the superficies,and dispelling wind to relieve itching are utilized.The approaches aim to cool heat in the blood,expel pathogens from the muscular layer,eliminate heat for stopping itching,and extinguish wind for calming the skin.The basic prescription,Yinqiao Xingzi Decoction(composed of Lonicerae Japonicae Flos,Forsythiae Fructus,Armeniacae Semen Amarum,Arnebiae Radix,Tribuli Fructus,Dictamni Cortex,Kochiae Fructus,and Glycyrrhizae Radix et Rhizoma),is used by modification depending on the predominance of pathogenic factors of wind,heat,dryness,or dampness-toxins,and according to the various syndromes like blood-heat,yin deficiency,or qi-blood deficiency.Yinqiao Xingzi Decoction is applicable to cutaneous pruritus with the most of clinical common syndrome types,but it is unsuitable for cases caused by yang deficiency or qi stagnation with blood stasis.
3.The influence of the minimal invasive Nuss procedure on the postoperative pulmonary function of pectus excavatum patients

Objective To study the influence of the minimal invasive Nuss procedure on the pulmonary function of post-operative pectus excavatum patients. Methods Conduct retrospective analysis on the data from 676 pectus excavatum patients who were treated by the minimal invasive Nuss procedure from August 2006 to November 2014. Wherein 182 cases have com-plete preoperative and postoperative pulmonary-function data of one year, three years. These cases were divided into 3 groups according to the age, namely, children group( from 6 to 12 years old, 34 cases) , adolescents group( from 13 to 18 years old, 80 cases) , adults group( above 18 years old, 68 cases) , among which there were 71 cases with pulmonary function data of 1 year after removal of steel plate, they were divided into 3 groups in the same way,namely, children group(20 cases), adoles-cents group(22 cases), adults group(29 cases) . To compare and analyze the pulmonary function indicatrix of patients with dif-ferent ages in preoperative stage, 1 year, 3 years postoperative stages and 1 year after dismantling the steel plate stage, and to investigate the influence of the minimal invasive Nuss procedure on the postoperative lung function. Results The pulmonary function indicatrix in preoperative stage, 1 year, 3 years postoperative stages of the children group did not have significant difference(P>0. 05); the FVC, FEV1 indicatrix of adolescents and adults groups declined after operation in 1 year and 3 years compared with the preoperative stage(P<0. 05), FEF 25% -75%, FEF 50%, FEF75% were improved after operation in 1 year and 3 years compared with the preoperative stage(P<0. 05);the pulmonary function indicatrix of three age groups in the 1 year after dismantling the steel plate stage had all improved, in which the pulmonary function indicatrix of the children group improves most significantly(P<0. 05). Conclusion After the minimal invasive Nuss procedure before the plate dis-mantling process, the pulmonary function of children patients remains to be similar. Partial ventilatory function was damaged in the adolescents and adults patients. After the plate dismantling process, the pulmonary function indicatrix of each age group hasimproved in different degrees. Improvement effect is the most significant in patients below the age of 12.
4.Study on relationship between Hcy,hs-CRP,Cys-C and Fib with acute cerebral infarction

Objective To study the level changes and correlation of homocysteine (Hcy) ,high-sensitivity C-reactive protein(hs-CRP) ,cystatin C(Cys-C) and plasma fibrinogen(Fib) in the patients with acute cerebral infarction (ACI) .Methods The fasting blood samples were collected from 178 cases of ACI(ACI group) and 93 healthy individuals blood samples (control group) .The lev-els of serum Hcy ,hs-CRP and Cys-C were detected by the BECKMAN AU-680 fully automatic biochemical analyzer and plasma Fib was determined by the RAC-100 fully automatic coagulometer .Results The levels of serum Hcy ,hs-CRP ,Cys-C and plasma Fib in the ACI group were significantly increased compared with the control group (P<0 .05) .There was significantly positive correlation between Hcy with hs-CRP and Cys-C in the ACI group(r=0 .326 ,0 .361 ,P<0 .05) ,but there was no significant correlation be-tween Hcy and Fib ;there was significantly positive correlation between hs-CRP with Cys-C and Fib(r=0 .365 ,0 .421 ,P<0 .05);the same significant positive correlation also existed between Cys-C and Fib (r=0 .447 ,P<0 .05) .The positive rate of the joint de-tection of Hcy ,hs-CRP ,Cys-C and Fib was 93 .8 % ,which was obviously higher than that of the single indicator detection (P<0 .05) .Conclusion Cys-C ,hs-CRP ,Fib and Hcy participate in the occurrence and development process of ACI ,their joint detection has the important clinical significance for the prevention ,early diagnosis and treatment of ACI .
5.Quality of life analysis of 96 patients who underwent a Nuss procedure

Objective The Nuss procedure is a minimally invasive pectus repair.The aim of this study was to explore the changes in quality of life in patients who underwent a Nuss procedure.Methods 96 patients,who underwent a Nuss procedure in our institution,were interviewed at preoperation,1 year after operation with the bar in place,and before the planned bar removal.The Nuss Questionnaire modified by George Krasopoulos et al,which evaluates psychosocial and physical well-being,was independently used by patients.Results Most of the scoring of the individual questions and the total score of individual patients revealed a significant improvement,both in preoperation vs.1 year after operation and 1 year after operation vs.before the planned bar removal.Conclusion The Nuss procedure has been shown to improve the quality of life in patients with pectus excavatum deformity in the short term.
