1.Conditional Tnfaip6-Knockout in Inner Ear Hair Cells Does not Alter Auditory Function.

Noise-induced hearing loss is a worldwide public health issue that is characterized by temporary or permanent changes in hearing sensitivity. This condition is closely linked to inflammatory responses, and interventions targeting the inflammatory gene tumor necrosis factor-alpha (TNFα) are known to mitigate cochlear noise damage. TNFα-induced proteins (TNFAIPs) are a family of translucent acidic proteins, and TNFAIP6 has a notable association with inflammatory responses. To date, there have been few reports on TNFAIP6 levels in the inner ear. To elucidate the precise mechanism, we generated transgenic mouse models with conditional knockout of Tnfaip6 (Tnfaip6 cKO). Evaluation of hair cell morphology and function revealed no significant differences in hair cell numbers or ribbon synapses between Tnfaip6 cKO and wild-type mice. Moreover, there were no notable variations in hair cell numbers or hearing function in noisy environments. Our results indicate that Tnfaip6 does not have a substantial impact on the auditory system.

2.Analysis on the current situation and influencing factors of occupational burnout among bus drivers in Wuhu City

Objective:To investigate the current situation of occupational burnout among bus drivers in Wuhu City and analyze its influencing factors.Methods:A survey was conducted in 1 388 bus drivers in Wuhu City from Sep to Nov 2022 using the general information questionnaire,the Maslach Burnout Inventory-General Survey(MBI-GS),and the Pittsburgh Sleep Quality Index(PSQI).Multivariable logistic regression analysis was used to analyze the influencing factors of occupational burnout in bus drivers.Spearman correlation analysis was used to analyze the correlation between occupational burnout and sleep quality.Results:Among 1 388 bus drivers,642(46.3%)were positive for occupational burnout,and 139(10.0%)were found to have sleep disorders.Multivariable logistic regression analysis showed that longer bus driving experience(OR=1.873,95%CI:1.325-2.648),lower monthly income(OR=0.376,95%CI:0.158-0.774),higher smoking frequency(OR=1.313,95%CI:1.188-2.163),higher drinking frequency(OR=1.342,95%CI:1.018-1.769),lower weekly physical exercise frequency(OR=0.367,95%CI:0.243-0.555),and poor sleep quality(OR=13.110,95%CI:7.284-23.594)were the influencing factors of bus driver occupational burnout(P<0.05).Spearman correlation analysis showed that the total score of occupational burnout and its dimension scores were positively correlated with the total score of sleep quality and its dimension scores(rs=0.12-0.83,P<0.01).Conclusion:The problem of occupational burnout among bus drivers in Wuhu City is relatively serious,and a part of people have sleep problems,which should be paid great attention to and active measures should be taken in time.
3.Study on the construction of medical information technology curriculum system in higher medical schools based on the theory of competency

This article focuses on the issues of limited teaching content, single teaching methods, and backward course settings in the construction of medical information technology curriculum system in higher medical schools in China. We introduce the competency theory in human resource management on the basis of analyzing the dimensional composition of medical informatics competence of medical students, and we propose strategies for construction of a medical information technology curriculum system at the three levels of knowledge, skills, and practice and elaborate on the architecture of the curriculum system. This study provides reference and guidance for the construction of medical information technology curriculum system in higher medical schools under the background of new medicine.
4.A case report of late-onset MELAS with paroxysmal sympathetic hyperactivity syndrome

Paroxysmal sympathetic hyperactivity (PSH) is a syndrome characterized by paroxysmal tachycardia, increased blood pressure, tachypnea, hyperthermia, profuse sweating, abnormal posture or dystonia. It occurs in diseases such as moderate to severe brain injury, cerebral hypoxia, hydrocephalus, brain tumor and encephalitis. At present, the etiology and pathogenesis are still unclear, and it is easy to be misdiagnosed as epilepsy clinically. This article reports a 43-year-old male patient with late-onset mitochondrial encephalomyopathy, lactic acidosis and stroke-like episodes (MELAS) confirmed by genetic testing. During hospitalization, he suddenly developed episodic involuntary limb movements, profuse sweating, tachycardia, and arterial hypertension. He was initially diagnosed with symptomatic epilepsy, but long-term electroencephalogram monitoring showed no synchronized discharge, and he was given antiepileptic drugs. The treatment was also ineffective. Brain magnetic resonance imaging revealed a new lesion in the left insular and insular operculum. Dexmedetomidine, baclofen, and gabapentin were given to suppress sympathetic nerve excitability. Drugs were effective, so the diagnosis was corrected to PSH. There is no report of MELAS complicated with PSH in the previous literature. It is speculated that it may be related to the low clinical cognition of PSH. In this case, new lesions in the insula and insular operculum appeared during the onset of PSH, suggesting that may be related to the pathogenesis of PSH.
5.Effects of transcranial direct current stimulation on the picture naming and phonemic fluency of aphasics

Objective:To observe any effect of transcranial direct current stimulation (tDCS) on the speed and accuracy of picture naming and on the phonological fluency of aphasic stroke survivors.Methods:Twenty-four stroke survivors with aphasia were randomly divided into an observation group and a control group, each of 12. In addition to language training and picture naming training, the observation group received 20 minutes of tDCS over the left dorsolateral prefrontal cortex (DLPFC) daily, 5 days a week for 2 weeks. The control group was given sham stimulation. Before and after the 2 weeks of treatment, both groups were tested for picture naming and phonological fluency.Results:Significant improvement in the number of accurately pronounced high-frequency words and in reaction time was observed in both groups, but the observation group′s average reaction time for high-frequency words was significantly shorter than the control group′s average. The observation group′s average reaction time for low-frequency words had also improved significantly. After the 2 weeks of treatment, the phonological fluency of the observation group was significantly better than before the treatment and better than that of the control group.Conclusions:tDCS applied over the left DLPFC of stroke survivors with aphasia can promote lexical retrieval and strengthen their executive and control functioning.
6.Transcriptome analysis of mild hypothermia protection against radiation

OBJECTIVES:
To analyze the differentially expressed genes (DEGs) with radiation-induced rat lung injury, and to reveal the protective mechanism for mild hypothermia in the radiation-induced lung injury in rats at the transcriptome level.
METHODS:
A total of 10 male SD rats aged 6-8 weeks were randomly divided into 2 groups to establish a rat model of radiation-induced lung injury, and one group was treated with mild hypothermia. RNA was extracted from left lung tissue of each group, and sequenced by BGISEQ-500 platform. Significance analysis of DEGs was carried out by edgeR software. Gene ontology (GO) function enrichment analysis and Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway enrichment analysis were used to analyze the gene function. Then 5 key DEGs were verified by real-time reverse transcription PCR (real-time RT-PCR).

CONCLUSIONS
The DEGs and pathways related to mild hypothermia protection against radiation-induced lung injury in rats are obtained, which provides an experimental basis for the protection of mild hypothermia against radiation-induced lung injury.

7.Design and application of performance appraisal program at clinical departments based on the concept of value transfer

The authors introduce a comprehensive hospital performance appraisal scheme based on the concept of value transfer, including qualitative and quantitative index system, weight and appraisal method.In the pilot practice of a hospital, it is found that the appraisal scheme can effectively improve the operational efficiency, social and economic benefits, and has a strong incentive role for employees. This scheme weighs the operability and scientificity of performance appraisal, and provides a reference for clinical department performance appraisal.
8.Progress of CD73 in tumor immune microenvironment

Extracellular 5'-nucleotidase (CD73) is not only highly expressed in a variety of solid tumor tissues, but also closely related to tumor stage, treatment and tumor prognosis. CD73 can exert a wide range of immunosuppressive effects through multiple pathways such as A2A receptor, regulation of regulatory T cells, and inhibition of inflammation. Blocking CD73 molecules can not only effectively activate anti-tumor immune responses, but also significantly improve the clinical efficacy of programmed death 1 (PD-1)/programmed death ligand 1 (PD-L1) antibodies. Thus, CD73, as a new immunological checkpoint molecule in the tumor microenvironment, may become a promising new immunotherapy target following CTLA4, PD-1/PD-L1.
9.The association between asymmetrically hypointense veins on susceptibility-weighted imaging and collateral circulation

Objective To explore the relationship between asymmetrically hypointense veins (AHVs) on susceptibility-weighted imaging (SWI) and collateral circulation.Methods We retrospectively enrolled acute ischemic stroke patients with severe stenosis or occlusion of M1 segment of the middle cerebral artery ± intracranial internal carotid artery.All the patients underwent diffusion-weighted imaging (DWI),SWI,and computed tomography angiography (CTA) of intracranial and cervical arteries within 72 hours from symptom onset.We explored the association of the level of AHVs with the degree of the regional leptomeningeal score (rLMC) on baseline CTA and other clinical and image data.The factors that might influence the prognosis of stroke were also analyzed.Results Fifteen patients with mild AHVs and 15 with extensive AHVs were enrolled in our study.The level of AHVs was positively correlated with CTA rLMC (r =0.481,P =0.007) and the degree of collateral circulation (r =0.402,P =0.028).Patients with extensive AHVs had better collateral status,smaller DWI infarction lesion ((11.62 ± 9.07) ml vs (95.77 ± 91.12) ml,t =3.559,P =0.001),and lower NIHSS scores on admission (6.47 ± 4.34 vs 12.33 ± 7.60,t =2.595,P =0.015)and at discharge (4.80 ± 4.69 vs 9.60 ± 7.03,t =2.200,P =0.036).The high degree of rLMC,small DWI lesion,young age and lower NIHSS scores,but not extensive AHVs were related with favorable outcome at 3 months after stroke.Conclusion Extensive AHVs can reflect good collateral circulation to some extent,but cannot be equivalent to or replace the collateral status.
10.Quantitative evaluation of blood-brain barrier permeability in patients with ischemic white matter lesions with dynamic contrast-enhanced MRI

Objective To investigate the changes of blood-brain barrier (BBB) permeability and the correlation with cognitive function in patients with ischemic white matter disease with dynamic contrast-enhanced MRI (DCE-MRI).Methods Totally 71 subjects underwent routine MRI,DCE-MRI,mini-mental state examination (MMSE) and Fazekas scoring,and then were divided into patient group and control group according to the Fazekas scores.The leakage rate (K),area under the leakage curve (AUC) and fractional blood plasma volume (Vp) in cerebral white matter hyperintensities area and normal appearing white matter area were measured and compared between the two groups.The correlation between these parameters and MMSE scores were analyzed.Results K and AUC value of white matter hyperintensities area and normal appearing white matter area in patient group were significantly higher than those in control group (all P<0.001).Vp value of normal appearing white matter area in patient group was lower than that in control group (P=0.015).K and AUC value of white matter hyperintensities area in patient group had significant negative correlation with MMSE scores (r=-0.440,--0.540,both P<0.001).Conclusion BBB permeability increased in cerebral white matter hyperintensities and normal appearing white matter area in patients with ischemic white matter lesions,which correlated with cognitive function.
