1.Risk factor analysis and nomogram model construction of pulmonary hemorrhage complicating lung nodule localization with a new type of 4-hook localization needle

Objective To construct a nomogram model for predicting pulmonary hemorrhage associated with the positioning of pulmonary nodules with the new four-hook positioning needle based on clinical-CT imaging features and evaluate its predictive efficacy.Methods We made a retrospective analysis of the clinical,imaging and pathological data of 449 patients with pulmonary nodules positioned by the new four-hook positioning needle.According to the random number table method(7∶3),they were divided into a training set of 314 cases and a validation set of 135 cases.Each data set was further divided into positive group and negative group for pulmonary hemorrhage according to the presence or absence of pulmonary hemorrhage.We evaluated the CT imaging features of pulmonary nodules,including nodule nature(pure ground-glass density,mixed ground-glass density,solid nodule),nodule diameter,distance from the nodule to the pleural surface(hereinafter referred to as length),nodule positioning time,and association with pulmonary hemorrhage.Independent sample t-test,Mann-Whitney U test and x2 test were used to compare the correlations of clinical and CT features of pulmonary nodules with pulmonary hemorrhage.LASSO regression and multivariate Logistic regression were employed to screen the independent risk factors related to pulmonary hemorrhage and construct a nomogram model.The receiver operating characteristic(ROC)curve was used to evaluate the predictive efficacy of the model,and the calibration curve and decision curve were respectively used for the verification of the nomogram model and evaluation of the clinical net benefit.Results The results of LASSO regression showed that the nature of pulmonary nodules,underlying diseases,smoking and length were the characteristic variables related to pulmonary hemorrhage.Based on the minimum akaike information criterion(AIC),the screened characteristic variables were included in the multivariate Logistic backward stepwise regression analysis.The results showed that the nature of pulmonary nodules,underlying diseases,smoking and length were all independent risk factors related to pulmonary hemorrhage.A nomogram was established according to the above independent risk factors and the ROC curve was drawn.The AUC of the training set was 0.86(95%CI:0.80-0.91),and the AUC of the validation set was 0.88(95%CI:0.80-0.96),with no statistically significant difference(P>0.05).The calibration curve suggested that the predicted values of the nomogram were close to the actual values,and the decision curve analysis showed that the net benefit of the model was good.Conclusion The nomogram model established by combining clinical-CT features such as the nature of pulmonary nodules,underlying diseases,smoking and length can effectively predict pulmonary hemorrhage associated with the positioning of pulmonary nodules with the new four-hook positioning needle.
2.The role and mechanism of calcium-binding protein S100A9 in acute lung injury induced by hepatic ischemia-reperfusion in mice

Objective:To investigate the role of calcium-binding protein S100A9 in acute lung injury induced by hepatic ischemia-reperfusion (HIR) in mice, and to explore its relationship with nuclear factor erythroid 2-related factor 2 (Nrf2).Methods:A total of 12 specific pathogen-free (SPF) male wild-type (WT) and 12 S100A9 knockout (S100A9 KO) C57BL/6J mice aged 6~8 weeks and weighing 20-25 g were randomly divided into four groups using a random number table: WT+Sham group, S100A9 KO+Sham group, WT+HIR group, and S100A9 KO+HIR group ( n=6 per group). The HIR model was established by clamping the portal vein and hepatic artery of the left and median liver lobes for 60 minutes followed by reperfusion. At 6 hours post-reperfusion, mice were anesthetized again, and blood samples were collected from the inferior vena cava. Both lungs were harvested. The lung wet-to-dry (W/D) weight ratio was measured. Hematoxylin and eosin (HE) staining was used to assess histopathological changes and calculate lung injury scores. The levels of inflammatory markers—S100A9, tumor necrosis factor-α (TNF-α), and interleukin-6 (IL-6) —as well as oxidative stress indicators including myeloperoxidase (MPO), reactive oxygen species (ROS), malondialdehyde (MDA), and superoxide dismutase (SOD) in serum and lung tissue were measured. Western blotting was used to assess the expression levels of nuclear and cytoplasmic Nrf2, and cytoplasmic HO-1. Results:Compared with the WT+Sham group, both the WT+HIR and S100A9 KO+HIR groups showed significantly increased lung injury scores, W/D ratio, TNF-α, IL-6, ROS, MPO, and MDA levels (all P<0.05). Compared with the WT+HIR group, the S100A9 KO+HIR group exhibited significantly reduced levels of these indicators (all P<0.05). Moreover, the S100A9 KO+HIR group showed elevated nuclear Nrf2 expression and decreased cytoplasmic Nrf2 expression, accompanied by increased expression of HO-1, Gclm, Gclc, and Nqo1 (all P<0.05). Conclusion:Upregulation of S100A9 is involved in the development of HIR-induced acute lung injury, possibly through inhibition of Nrf2 nuclear translocation.

7.Risk factors and construction of prediction model of perineural invasion of gallbladder carcinoma based on enhanced CT-image features

Objective To construct the prediction model of perineural invasion(PNI)in gallbladder carcinoma(GBC)based on preoperative enhanced CT image features and evaluate its prediction efficiency.Methods The clinical,imaging and pathological data of 180 GBC patients undergoing radical operation were retrospectively analyzed.They were divided into positive and negative groups according to the presence or absence of PNI.Preoperative enhanced CT imaging features(including presence of gallstones,imaging hepatic invasion,vascular invasion,T-stage,and hilar or retroperitoneal lymph node metastases)were evaluated by two radiologists.Independent sample t-test,Mann Whitney U test,and X2 test were used to compare the correlation between CT signs and PNI.Logistics regression analysis was used to screen independent risk factors and establish the prediction model formula.ROC curve was used to evaluate the prediction efficiency of the prediction model and the corresponding area under the curve(AUC)was calculated.Hosmer-Lemeshow goodness of fit test was used to verify the prediction model.Results Unifactorial analysis showed that CA199,CA125,imaging hepatic invasion,vascular invasion(hepatic artery or portal vein),T-stage,and hilar or retroperitoneal lymph node metastasis were correlated with nerve invasion(P<0.05).Logistics multi-factor analysis results showed that CA199,imaging vascular invasion(hepatic artery or portal vein),and imaging T stage were independent risk factors for PNI.Based on the above independent risk factors,a prediction model formula was established and ROC curve was drawn,with an AUC of 0.807(95%CI:0.734~0.879),sensitivity of 0.792,specificity of 0.697,and the chi-square value of Hosmer-Lemeshow goodness of fit test of 0.594(P=0.997),indicating that the predicted value of the model was close to the actual value.Conclusion Combining CA199,imaging vascular invasion,T-stage,and other preoperative clinically-enhanced CT features to establish a prediction model can effectively predict postoperative PNI of GBC.
8.Research advances of esketamine in patients undergoing cardiovascular surgery

Esketamine is a spin isomer of ketamine,which has the triple effect of sedation,analge-sia and amnesia,and is superior to ablative ketamine in terms of efficacy,controllability.It has been widely used in anesthesia,emergency and critical care in Europe and America,and is mostly used for sedation,analgesia and antidepressant in China.Esketamine is used in cardiac surgery to maintain stable hemodynam-ics,reduce the secretion of inflammatory factors and relieve postoperative pain.Its sympathomimetic effect allows it to be used for the induction of anesthesia in patients with hemodynamic instability and acute heart attack.This paper reviews the recent advance in the clinical value and limitations of esketamine in the perio-perative period of cardiovascular surgery and provides a reference for clinicians to use esketamine in the perioperative period of cardiovascular surgery.
9.Correlation of peripheral blood eosinophils with left ventricular hypertrophy in elderly patients with hypertension

Objective To investigate the correlation between peripheral blood eosinophil(EOS)and left ventricular hypertrophy(LVH)in elderly patients with hypertension.Methods A retrospec-tive cross-section analysis was conducted on 711 elderly hypertensive patients admitted to our de-partment from January to December 2023.According to their EOS count,they were divided into Group 1(<100×106/L),Group 2[(100-190)× 106/L]and Group 3(>190 × 106/L).The gen-eral clinical data,biochemical indicators,and echocardiographic parameters were compared among the three groups.The correlation between EOS and each indicator was analyzed by using Spearman correlation analysis.Logistic regression analysis was employed to identify the influential factors for hypertension complicated with LVH.Results Smoking history,TG and UA levels,and lymphocyte counts were gradually increased,while HDL-C level was gradually decreased in the three groups with the increment of EOS count(P<0.05,P<0.01).The proportions of usingβ-blockers,calcium channel blockers and statins were gradually increased in the three groups(P<0.05,P<0.01),so were the values of LVM,LVMI,IVST,LVEDD and LVPWT in them(P<0.01).EOS count was positively correlated with the values of LVM,LVMI,IVST,LVEDD and LVPWT,smoking history,TG and UA levels,WBC,lymphocyte and monocyte counts,and ratios of using β-blockers,calcium channel blockers and statins,but negatively with HDL-C level(P<0.05,P<0.01).Binary logistic regression analysis showed that female and HDL-C were protective factors for LVH in hypertensive patients,while age and EOS count were risk factors(OR=0.149,95%CI:0.096-0.230;OR=0.632,95%CI:0.399-0.999;OR=1.034,95%CI:1.007-1.062;OR=1.956,95%CI:1.446-2.645).Conclusion In elderly hypertensive patients with LVH,female and HDL-C are the protective factors,while age and EOS are the risk factors.EOS has a certain correlation with the occurrence of LVH in elderly hypertensive patients.
10.Effect of s-ketamine on perioperative myocardial injury in patients undergoing liver transplantation

Objective:To evaluate the effect of s-ketamine on perioperative myocardial injury in patients undergoing liver transplantation.Methods:This was a prospective randomized controlled study. Sixty American Society of Anesthesiologists Physical Status classification Ⅲ or Ⅳ patients, aged 18-64 yr, with New York Heart Association classⅠ-Ⅲ, undergoing elective liver transplantation with general anesthesia in our hospital from May to October 2023, were divided into 2 groups ( n=30 each) using a random number table method: s-ketamine group (group S) and control group (group C). In group S, s-ketamine was intravenously injected at a dose of 0.5 mg/kg after induction of anesthesia, followed by an infusion of 0.5 mg·kg -1·h -1 until the end of surgery. The equal volume of normal saline was given instead in group C. Central venous blood samples were collected after induction of anesthesia (T 0), at 30 min of anhepatic phase (T 1), 30 min of neopepatic phase (T 2), abdominal closure (T 3), 24 h after operation (T 4) and 72 h after operation (T 5) for determination of the concentrations of serum high-sensitivity cardiac troponin I, creatine kinase-MB isoenzyme, N-terminal pro-B-type natriuretic peptide, tumor necrosis factor-α, interleukin-6 (IL-6), IL-10 and high-mobility group protein B1 by enzyme-linked immunosorbent assay. The occurrence of adverse cardiac events during surgery and within 24 h after surgery, postoperative mechanical ventilation time, time of intensive care unit stay, and postoperative length of hospital stay were recorded. Results:Compared with group C, the concentrations of serum high-sensitivity cardiac troponin I, creatine kinase-MB isoenzyme, N-terminal pro-B-type natriuretic peptide, tumor necrosis factor-α and IL-6 at T 2-5 and high-mobility group protein B1 at T 2-4 were significantly decreased, the concentrations of serum IL-10 were increased at T 2-5, the incidence of myocardial ischemia was decreased, the mechanical ventilation time was shortened ( P<0.05), and no significant change was found in the time of intensive care unit stay and postoperative length of hospital stay in S group ( P>0.05). Conclusions:Intraoperative usage of s-ketamine can inhibit the inflammatory responses and reduce perioperative myocardial injury in the patients undergoing liver transplantation.
