1.Traditional Chinese Medicine Prevents and Treats Ulcerative Colitis via TLR4/NF-κB Signaling Pathway: A Review

Ulcerative colitis (UC) is a chronic inflammatory bowel disease primarily affecting the colon and rectum. With a rising incidence, UC has aroused an increasingly serious public health concern. The etiology and pathogenesis of UC are complex, involving genetic susceptibility, intestinal barrier dysfunction, immune dysregulation, and environmental factors such as diet, smoking, and infection. Current UC therapies face limitations including persistent drug resistance, multiple side effects, and compromised immune function, necessitating the exploration of novel intervention strategies. In recent years, with the continuous advancement of research into UC mechanisms, the Toll-like receptor 4 (TLR4)/nuclear factor-κB (NF-κB) signaling pathway has been revealved as one of the classical pathways involved in inflammation. Being crucial for maintaining intestinal homeostasis, this pathway regulates inflammatory and immune responses by controlling gene transcription, thereby playing a significant role in the occurrence and progression of UC. However, systematic summarization within this field remains lacking. Traditional Chinese medicine (TCM) possesses unique advantages through its multi-target, multi-pathway, and multi-mechanism approaches, being capable of curbing the pathological progression of UC by targeting key molecules in the TLR4/NF-κB signaling pathway. This paper systematically summarizes the mechanisms by which TLR4/NF-κB-related pathways regulate UC, primarily encompassing inflammatory responses, oxidative stress, immune balance, and gut microbiota. It reviews single herbs (e.g., Rosa roxburghii root and Saposhnikoviae Radix), active components (e.g., resveratrol, Tripterygium glycosides, and gypenosides), herbal extracts (e.g., Zingiberis Rhizoma extract and Taraxaci Herba extract), and compound formulas for clearing heat and removing dampness, invigorating the spleen and warming the kidneys, clearing heat and detoxifying, harmonizing the liver and spleen, and treating cold and heat in complexity (e.g., Huaihua Powder, Shenling Baizhu Powder, and Jiedu Huayu Formula). The aim is to fully leverage the advantages of TCM in UC treatment and provide novel insights for clinical and pharmacological research on UC.

At present,the various levels of safety evaluations of traditional Chinese medical (TCM) injections were in force in post-marketing clinical researches,while the process quality is guaranteed with difficulty.Therefore,it is requisite to call for technical guidance.By drawing lessons from the international experience of drug marketing safety evaluation and the related methods,we proposed the key techniques of quality control for clinical safety evaluation of TCM injections from the three aspects:preparation,implementation and summarization of researches,combining with the characteristics of TCM injections.The key techniques contained some scientific problems,ethics,publicity,data management,statistical analysis,specification report,etc.This paper comprehensively introduced the requisite technologies and methods over research quality,laying a foundation for the studies of post-marketing clinical safety evaluation of TCM injections.
4.Comparison of Different Surgical Treatments for Solitary Adrenal Metastasis Following Nephrectomy in Patients with Renal Cell Carcinoma:18 Cases Report

Objective to investigate the feasibility and outcomes of different surgical treatments for adrenal metastasis after previous radical ne-phrectomy for patients with renal cell carcinoma. Methods A total of 18 adrenal solitary metastasis of renal cell carcinoma were identified from da-tabase of two institutions between 2003 and 2013. Clinical and pathologic data were collected and analyzed. Results Of 9 patients who had ipsilater-al metastasis of the renal tumor,the estimated blood loss were obviously fewer in the transperitoneal LA group. Of 9 cases who had contralateral me-tastasis of the renal tumor,the fasting time[(1.4±0.8)h vs(4.0±1.4)h,P = 0.036]and length of hospital stay[(4.5±1.2)d vs(7.0±4.5)d,P =0.041]were obviously fewer in the retroperitoneal LA group. the averages of the fasting time and length of hospital stay in the retroperitoneal LA group were obviously smaller than in the transperitoneal LA group. Conclusion LA for treatment of renal cell carcinoma metastasis is technically feasible in selected patients. transperitoneal and retroperitoneal LA can be respectively recommended as an appropriate approach for isolated adrenal metastases of ipsilateral and contralateral renal cell carcinoma after nephrectomy.
