2.Pathway analysis of self-disclosure to posttraumatic growth in patients after cervical cancer surgery

Objective:To investigate the pathway of self-disclosure to posttraumatic growth in patients after cervical cancer surgery, and to provide reference for improving the level of posttraumatic growth in patients.Methods:A convenient sampling method was used to investigate 300 patients with cervical cancer after surgery in Shandong Provincial Hospital Affiliated to Shandong First Medical University from June to November 2022 by using general data questionnaire, Distress Disclosure Index (DDI), Connor-Davidson Resilience Scale (CD-RISC), the Shortened Chinese Version of the Family Resilience Assessment Scale (FRAS-C) and the Posttraumatic Growth Inventory (PTGI) in a cross-section study.Results:A total of 290 valid questionnaires were collected, with an effective recovery rate of 96.7%. The patients were aged 23-70(48.13 ± 10.39) years. The scores of self-disclosure, resilience, family resilience and posttraumatic growth were (46.41 ± 9.82), (67.06 ± 14.63), (108.18 ± 11.06) and (58.24 ± 17.86) respectively. The results of pathway analysis showed that self-disclosure could not only directly predict posttraumatic growth, but also indirectly predict posttraumatic growth through the mediating role of resilience and family resilience, and the chain mediating role of resilience and family resilience, respectively. The direct effect of self-disclosure on posttraumatic growth was 0.236(95% CI 0.138-0.335), and the chain mediating effect of family resilience and resilience between self-disclosure and posttraumatic growth was 0.036(95% CI 0.018-0.060). Conclusions:Medical staff should not only consider the direct influence of self-disclosure on posttraumatic growth, but also pay attention to improve the resilience and family flexibility of patients after cervical cancer surgery, so as to promote their posttraumatic growth.
3.Correlation analysis of baseline data,early treatment response and prognosis in children with acute lymphoblastic leukemia

Objective To investigate the correlation between baseline data,early treatment response and prognosis in children with acute lymphoblastic leukemia(ALL).Methods Ninety-two children with ALL were divided into the endpoint event group(19 cases)and the event-free survival group(73 cases)according to whether there was an endpoint event(recurrence or death).The age and gender at initial diagnosis were recorded.Initial white blood cell count(WBC),platelet count(PLT),immunophenotype,chromosome karyotype,fusion gene,prednisone test,bone marrow remission status on the 15th day of induction chemotherapy and minimal residual disease(MRD)on the 15th,33rd and 55th day of induction chemotherapy were detected.The correlation between the above baseline data and early treatment response and the occurrence of endpoint event in children with ALL was analyzed.Logistic regression was used to analyze influencing factors of endpoint events in children with ALL.Receiver operating characteristic(ROC)curve was plotted to evaluate the predictive value of baseline data and early treatment response to endpoint events in children with ALL.Results The proportion of WBC≥100×109/L at first diagnosis,prednisone poor reaction and positive MRD on the 33rd day of induction chemotherapy were higher in the endpoint event group than those in the event-free survival group(P<0.05),and there were no significance differences in remaining indicators(P>0.05).Logistic regression analysis showed that prednisone poor reaction and positive MRD on the 33rd day of induction chemotherapy were risk factors for endpoint event in children with ALL(P<0.05),and the combined value of the two indicators was better than that of a single indicator in predicting endpoint events in children with ALL.Conclusion Prednisone poor reaction and positive MRD on the 33rd day of induction chemotherapy are associated with recurrence and death in children with ALL.
4.Epidemiological and clinical characteristics of respiratory syncytial virus infections in children in Jiangsu Province, 2014-2023

Objective:To analyze the epidemiological and clinical characteristics of respiratory syncytial virus (RSV) infection in children in Jiangsu Province from 2014 to 2023.Methods:The acute respiratory infection cases in children aged 0-14 years were selected from outpatient/emergency or inpatient departments in 2 surveillance sentinel hospitals, respectively, in Nanjing, Suzhou and Taizhou of Jiangsu from 1 July 2014 to 31 December 2023, and RSV nucleic acid test was conducted and the intensity of the RSV infection was accessed by WHO influenza epidemiological threshold method, and case information and clinical data were collected. χ2 test was used to compare the differences between groups, and the Bonferroni method was used for pairwise comparisons between groups. Results:In 4 946 cases of acute respiratory infections, the RSV positive rate was 8.21% (406/4 946), and the age M( Q1, Q3) of the cases was 1 (0, 3) years. The RSV positive rate was 10.92% (258/2 362) during 2014-2019 and 6.06% (118/1 948) during 2019-2023, the difference was significant ( χ2=31.74, P<0.001). RSV infection mainly occurred from October to March during 2014-2019, with the incidence peak in December and moderate or higher intensity. The seasonality of RSV infection was not obvious during 2019-2023, with low intensity. The RSV positive rate was highest in children in age group 0- years (17.85%, 151/846), and the positive rate declined gradually with age ( χ2=184.51, P<0.001). The RSV positive rate was higher in inpatient cases (9.84%, 244/2 480) than in outpatient/emergency cases (6.57%, 162/2 466) ( χ2=17.54, P<0.001). In the 155 RSV infection cases with complete clinical data, the clinical symptoms mainly included cough (99.35%, 154/155), fever (55.48%, 86/155), and shortness of breath (45.16%, 70/155). In the cases aged <6 months, the proportion of those with fever was low, but the proportion of those with shortness of breath, transferred to intensive care units, and receiving oxygen therapy were higher (all P<0.05). Children aged <6 months and those with underlying diseases were more likely to have severe RSV infection (all P<0.05). Conclusions:RSV infection in children in Jiangsu Province showed seasonal prevalence in winter from 2014 to 2019. Since 2020, the seasonal characteristics of the epidemic have changed, the epidemic period has been dispersed and the epidemic intensity has decreased. Infants <1 year old were at high risk for RSV infection, and those <6 months old and with underlying diseases might have severe infection.
5.Effect of sluggish cognitive tempo on the efficacy of atomoxetine in children with attention-deficit/hyperactivity disorder

Objective To explore the impact of sluggish cognitive tempo(SCT)on the efficacy of atomoxetine in the treatment of attention-deficit/hyperactivity disorder(ADHD).Methods A prospective study was conducted.Pediatric patients aged 6-12 years with a diagnosis of ADHD were selected from the Department of Psychiatry,Xijing Hospital,Air Force Military Medical University from May 2020 to May 2023.All the patients were treated with atomoxetine for 8 weeks.ADHD symptoms and SCT symptoms were assessed by Swanson,Nolan,and Pelham-Ⅳrating scale(SNAP-Ⅳ)and SCT rating scale at baseline(before treatment),4 weeks and 8 weeks after treatment.Pearson correlation analysis was used to analyze the correlation between SCT and SNAP-Ⅳscores.Results A total of 61 pediatric patients were enrolled,including 10 cases of inattentive type and 51 cases of mixed type.Baseline SCT score was positively correlated with baseline SNAP-Ⅳscore(r=0.490,P<0.001)and reduction rate of SNAP-Ⅳscore at week 8(r=0.670,P<0.001).The total SCT score at week 8 was positively correlated with total SNAP-Ⅳscore at baseline(r=0.320,P=0.012).The baseline SCT score of patients with mixed type was positively correlated with reduction rate of SNAP-Ⅳscore at week 8(r=0.578,P<0.001).Conclusions The severity of SCT symptoms is positively correlated with the SNAP-Ⅳreduction rate 8 weeks after atomoxetine treatment,which could be a predictor of the efficacy of atomoxetine.
6.Clinical characteristics of psoriasis and current status of medical care for patients in county areas of China

Objective:To investigate the clinical characteristics of psoriasis and status quo of medical care for patients in county areas of China.Methods:This study was a cross-sectional investigation. Based on the “Qianxian Wuyin” Project (a national project for upgrating ability for psoriasis care at county level), an online questionnaire survey was conducted in the dermatology departments of 459 county hospitals in 404 pilot administrative counties across China from February to June 2023. The questionnaire included demographic information of patients (gender, ethnicity, age, place of residence, education, marital status), and clinical characteristics of psoriasis (disease course, type, comorbidities, body surface area (BSA) and previous treatment. The Dermatology Life Quality Index (DLQI) and Psoriasis Area and Severity Index (PASI) were applied for assessing the quality of life and disease severity, and completed by patients or guardian and doctors, respectively.Results:A total of 16 935 patients completed the questionnaire. The age of patients was 1-102(44.17±11.58)years, and 71.0% (12 036/16 935) were 30-59 years old. The ratio of male to female was 2.21∶1; 24.3%(4 117/16 935) of patients had high school education; there were 9 940 patients(58.7%) with previous or current smoking and/or alcohol use; 42.8%(7 218/16 855) of patients had a disease course of 1-5 years. There were 15 630 patients(92.3%) with DLQI≥10, 8 346 patients(49.7%) with PASI≥10, 15 017 patients(89.2%) with BSA≥10%. The plaque type was the most common disease type ( n=14 965, 88.7%), and spotting type ranked the second ( n=1 141, 6.8%). The most common initial site was the trunk ( n=12 309, 72.9%). Among the comorbidities, hypertension was the most common one ( n=1 681, 10.0%). There were 7 650 reports of treatment response to conventional topical drug therapy and 3 112 reports of treatment response to systemic drug therapy, with 6 269 (81.9%) and 2 493 (80.1%) reporting poor or no response, respectively. Conclusions:The survey shows that in the county areas of China, the majority of psoriasis patients are severe patients with short course of disease, plaque type is the most common type, and hypertension is the most common comorbidity; and the conventional treatment is less effective for most patients.
7.Expression of influenza A H1N1 and H3N2 viruses Mosaic-HA1 antigens and evaluation of its immunogenicity in mice.

Vaccination is the most effective measure for reducing and preventing influenza and related complications. In this study, we analyzed the mutation trend and the antigen dominant site changes of the amino acid sequence of hemagglutinin subunit 1 (HA1) of human influenza A virus (IAV) in the northern hemisphere from 2012 to 2022. According to the HA1 sequences of A/Darwin/6/2021 (H3N2) and A/Wisconsin/588/2019 (H1N1) recommended by the World Health Organization in the 2022 influenza season in northern hemisphere, we employed the mosaic algorithm to design three Mosaic-HA1 antigens through stepwise substitution. Mosaic-HA1 was expressed and purified in 293F cells and then mixed with the alum adjuvant at a volume ratio of 1:1. The mixture was used to immunize BALB/c mice, and the immunogenicity was evaluated. Enzyme-linked immunosorbent assay showed that Mosaic-HA1 induced the production of IgG targeting two types of HA1, the specific IgG titers for binding to H3 protein and H1 protein reached 105 and 103 respectively. The challenge test showed that Mosaic-HA1 protected mice from H3N2 or H1N1. This study designs the vaccines by recombination of major antigenic sites in different subtypes of IAV, giving new insights into the development of multivalent subunit vaccines against influenza.

8.Clinical efficacy and safety analysis of cetuximab combined with mFOLFOX6 chemotherapy in the treatment of advanced colorectal cancer patients

Objective:To analyze the clinical efficacy of cetuximab combined with mFOLFOX6 chemotherapy regimen in the treatment of advanced colorectal cancer, to explore its effects on the distribution of T lymphocyte subsets, patients' physical status and tumor marker levels, and to analyze its safety.Methods:A total of 90 patients with advanced colorectal cancer admitted to the Hainan Cancer Hospital from April 2020 to April 2022 were selected and randomly divided into observation group ( n=45) and control group ( n=45) according to random number table method. The control group was treated with the mFOLFOX6 chemotherapy regimen, while the observation group was treated with cetuximab combined with the mFOLFOX6 chemotherapy regimen. Objective response rate (ORR), disease control rate (DCR), T lymphocyte subsets levels (percentages of CD3 +, CD4 +, CD8 +), Karnofsky performance status (KPS) score, tumor marker vascular endothelial growth factor (VEGF), endothelial cell-specific molecule 1 (ESM1), carcino-embryonic antigen (CEA) levels and the incidence of adverse reactions were compared between the two groups after treatment. Results:After 4 cycles of treatment, the ORR was 66.67% (30/45) in the observation group and 42.22% (19/45) in the control group, and there was a statistically significant difference ( χ2=5.42, P=0.020). The DCR of the observation group was 86.67% (39/45) and that of the control group was 66.67% (30/45), and there was a statistically significant difference ( χ2=5.03, P=0.025). The percentages of T lymphocyte subsets CD3 +, CD4 + and CD8 + in the observation group were (63.35±6.71) %, (35.67±3.96) % and (17.03±2.11) %, respectively, while those in the control group were (52.23±5.92) %, (30.55±3.51) % and (20.64±2.83) %, respectively. The percentages of CD3 + and CD4 + in the observation group were significantly higher than those in the control group ( t=8.34, P<0.001; t=6.49, P<0.001), while the percentage of CD8 + ratio was significantly lower ( t=6.86, P<0.001). The KPS score of the observation group was (95.55±9.74) points, and that of the control group was (85.03±8.92) points, and there was a statistically significant difference ( t=5.34, P<0.001). VEGF, ESM1 and CEA levels in the observation group were (303.45±33.21) ng/L, (75.66±8.36) pg/ml and (7.73±0.98) ng/ml, respectively, while those in the control group were (364.53±39.07) ng/L, (92.53±9.91) pg/ml and (9.95±1.13) ng/ml, respectively. VEGF, ESM1 and CEA levels in observation group were significantly lower compared with control group ( t=7.99, P<0.001; t=8.73, P<0.001; t=9.96, P<0.001). The total incidence of adverse reactions was 75.56% (34/45) in the observation group and 66.67% (30/45) in the control group, and there was no statistically significant difference ( χ2=0.87, P=0.352) . Conclusion:Cetuximab combined with mFOLFOX6 chemotherapy regimen in the treatment of advanced colorectal cancer patients has significant clinical efficacy, can improve the immune function of patients, alleviate clinical symptoms, regulate the level of serum tumor markers in patients without increasing adverse reactions, and has good safety.
9.Adult dental epithelial stem cell-derived organoids deposit hydroxylapatite biomineral.

Ameloblasts are specialized cells derived from the dental epithelium that produce enamel, a hierarchically structured tissue comprised of highly elongated hydroxylapatite (OHAp) crystallites. The unique function of the epithelial cells synthesizing crystallites and assembling them in a mechanically robust structure is not fully elucidated yet, partly due to limitations with in vitro experimental models. Herein, we demonstrate the ability to generate mineralizing dental epithelial organoids (DEOs) from adult dental epithelial stem cells (aDESCs) isolated from mouse incisor tissues. DEOs expressed ameloblast markers, could be maintained for more than five months (11 passages) in vitro in media containing modulators of Wnt, Egf, Bmp, Fgf and Notch signaling pathways, and were amenable to cryostorage. When transplanted underneath murine kidney capsules, organoids produced OHAp crystallites similar in composition, size, and shape to mineralized dental tissues, including some enamel-like elongated crystals. DEOs are thus a powerful in vitro model to study mineralization process by dental epithelium, which can pave the way to understanding amelogenesis and developing regenerative therapy of enamel.

10.Efficacy and safety of tenecteplase in the treatment of acute ischemic stroke:a meta-analysis

OBJECTIVE To systematically evaluate the efficacy and safety of intravenous bolus of tenecteplase in the treatment of acute ischemic stroke (AIS), in order to provide evidence-based support for the clinic’s choice of intravenous thrombolytic drugs. METHODS Randomized controlled trials (RCTs) about the efficacy and safety of tenecteplase versus alteplase (control) in the treatment of AIS were collected from PubMed, Embase, the Cochrane Library, Web of Science, Sinomed, CNKI, Wanfang Data, and VIP during the inception to June 2022. Two evaluators independently screened the literature, extracted data from the literature, assessed the bias risk of included study, and then conduct meta-analysis by using Stata 15 software. RESULTS A total of 8 literature were included, involving 2 129 patients. Meta-analysis results showed that the early improvement rate of neurological function [OR(95%CI)=2.44(1.09,5.46),P=0.030] and the good rate of neurological function recovery (modified Rankin scale score 0-2 after 90 days of intravenous thrombolysis treatment) [OR(95%CI)=1.54(1.00,2.36),P=0.048] were higher in 0.25 mg/kg tenecteplase group (medium dose) than alteplase group. According to meta-analysis of other outcome indicators (including recanalization rate, percentage of reperfusion lesions, excellent rate of neurological function recovery, the incidence rate of bleeding, the incidence rate of symptomatic intraventricular hemorrhage and all-cause mortality rate within 90 d), the tenecteplase group had no statistically significant difference with alteplase group (P>0.05). CONCLUSIONS Compared with alteplase, medium dose of tenecteplase has some advantages in terms of early neurological function improvement and neurological function recovery, and it does not increase the risk of adverse events.
