1.Quantitative Molecular Detection of Angelicae Sinensis Radix and Its Processed Products Based on Herb-Q Method

ObjectiveAngelicae Sinensis Radix, a commonly used medicinal herb with both medicinal and edible properties, is frequently adulterated in the market, severely affecting the clinical efficacy of preparations. While qualitative identification techniques for adulterants and counterfeits are now relatively mature, quantitative detection methods for adulterated processed products remain unexplored. Quantitative detection research of Angelicae Sinensis Radix and its primary closely related adulterant, "Tu Danggui" (Angelica gigas), was conducted to establish a herbal quantitative molecular detection (Herb-Q) method for Angelicae Sinensis Radix and its processed products, providing a model for the establishment of quantitative detection technologies for Angelicae Sinensis Radix and related health products. MethodsThe specific single-nucleotide polymorphism (SNP) loci of Angelicae Sinensis Radix and Angelica gigas Nakai were screened based on the complete chloroplast genome sequence. The specific SNP loci of Angelicae Sinensis Radix were selected for quantitative methodological investigations (linearity, limit of quantification, limit of detection, and reproducibility) by mixing the powder of the herbs with different adulteration ratios. Huoxue Zhitong powder with three distinct adulteration ratios (15%, 25%, and 35%) was utilized to ascertain the precision of the Herb-Q method for the quantitative detection of Chinese patent medicines containing Angelicae Sinensis Radix. ResultsBy comparing the 123 chloroplast genome sequences of Angelicae Sinensis Radix, based on the principles of intraspecies conservation, interspecies specificity, and meeting the requirements of pyrophosphate high-throughput sequencing, it was determined that 9 674th locus (A/G) in the chloroplast genome sequence NC_042826.1 and 38 592nd locus (T/C) in the chloroplast genome sequence NC_029393.1 could be the exclusive molecular identification loci of Angelicae Sinensis Radix and Angelica gigas Nakai, respectively. The linear relationship R2 of the Herb-Q method established by selecting the specific 9 674th locus (A/G) of Angelicae Sinensis Radix was 0.997 4 (R2>0.99), indicating an excellent linear relationship. The limits of quantification and detection were established at 2.0%, exhibiting excellent reproducibility [relative standard deviation(RSD)<2.0%]. The established quantitative system based on the Herb-Q method detected the adulteration amount of counterfeit A. gigas in the Huoxue Zhitong powder, with an average deviation of 1.3% for three molecular quantitative replicates. ConclusionThis research demonstrates that the Herb-Q quantitative detection method established based on the 9 674th locus (A/G) in the chloroplast genome sequence NC_042826.1 of Angelicae Sinensis Radix has good applicability, objectivity, and accuracy for Angelicae Sinensis Radix and A. gigas, and its processed products. This method has the capacity to provide technical support for the quantitative detection of commercially available Angelicae Sinensis Radix derivatives, including traditional Chinese medicinal preparations, dietary supplements, and nutraceuticals.

Background Under the background of global warming, research on association between ambient temperature and risk of injury is needed. Objective To examine the effect of temperature on injury in Bao'an district, Shenzhen and identify the sensitive population, thereby providing a scientific basis for formulating prevention and control strategies and measures of injury. Methods The injury reports from the Injury Surveillance System and the meteorological data of Bao'an District between 2018 to 2022 were collected. The meteorological data were sourced from the fifth generation of the European Centre for Medium-Range Weather Forecasts (ECMWF) land reanalysis data. 4.Application and efficacy of Bioabsorbable steroid-eluting sinus stent in endoscopic treatment of chronic rhinosinusitis with nasal polyps

OBJECTIVE To observe the efficacy of Bioabsorbable steroid-eluting sinus stent implantation in endoscopic treatment of chronic rhinosinusitis with nasal polyps(CRSwNP).METHODS In a self-controlled study,25 patients with CRSwNP who underwent nasal endoscopic surgery were randomly assigned to implant a Bioabsorbable steroid-eluting sinus stent into one side of the ethmoid sinus after nasal endoscopic surgery,leaving one side of the ethmoid sinus without stent implantation.Follow-up and endoscopic evaluation were performed before surgery,2 weeks,1 month,2 months,and 3 months after surgery.Main outcome measures:visual analogue scale(VAS)score,Lund-Kennedy endoscopic score,eosinophils(Eos)count in mucosal tissue,nasal discharge smear,postoperative intervention.RESULTS The Lund-Kennedy endoscopy score on the stent side was significantly lower than that on the control side at 1 and 3 months after surgery.There was no significant difference between the VAS scores of the stent side and the control side.One month after surgery,the Eos count of nasal mucosa pathology and Eos level of nasal secretions smear in patients with eosinophilic CRSwNP(eCRSwNP)and non-eosinophilic CRSwNP(neCRSwNP)on the stent side were significantly lower than those in the control side.However,there was no significant difference between eCRSwNP patients and neCRSwNP patients in the improvement degree of Eos count in nasal mucosa pathology and nasal secretions smear 1 month after stent operation.The ratio of requiring surgical intervention 1 month after surgery was lower in the neCRSwNP group than in the control group,while there was no significant difference between the two sides in the eCRSwNP group.The polypoid tissue score in neCRSwNP group were significantly lower than those in eCRSwNP group one month after surgery,the ratio of bilateral intervention in neCRSwNP group were lower than those in eCRSwNP group one month after surgery,with a significant difference on the stent side.CONCLUSION The application of Bioabsorbable steroid-eluting sinus stent in nasal endoscopy can improve the nasal endoscopy score,reduce the Eos inflammatory load,reduce surgical intervention,and thus improve the early postoperative outcome.Stent implantation can benefit both eCRSwNP patients and neCRSwNP patients.eCRSwNP patients with high inflammation burden may require more effective stents and release higher doses of corticosteroids to achieve higher benefits.
5.Clinical Efficacy of Shuxin Jieyu Decoction in Treating Coronary Heart Disease with Angina Pectoris Complicated by Anxiety Disorder

Objective To observe the clinical efficacy of Shuxin Jieyu Decoction(composed of Chuangxiong Rhizoma,Angelicae Sinensis Radix,Bupleuri Radix,Paeoniae Radix Alba,Paeoniae Radix Rubra,Curcumae Radix,Ziziphi Spinosae Semen,Glycyrrhizae Radix et Rhizoma Praeparata cum Melle,Platycladi Semen,Polygoni Multiflori Caulis,Polygalae Radix,Toosendan Fructus,Corydalis Rhizoma,and Albiziae Cortex)in treating coronary heart disease(CHD)with angina pectoris complicated by anxiety disorder in patients with qi stagnation and blood stasis syndrome.Methods Sixty patients diagnosed with qi stagnation and blood stasis syndrome type of CHD with angina pectoris and anxiety disorder were enrolled from the outpatient and inpatient departments of Taihe Hospital of Traditional Chinese Medicine Affiliated to Anhui University of Chinese Medicine between January 2024 and January 2025.Patients were randomly divided into an observation group(n=30)and a control group(n=30)using a random number table method.The control group received conventional CHD treatment and anxiolytic therapy(Flupentixol and Melitracen Tablets),while the observation group additionally received Shuxin Jieyu Decoction.Both groups were treated for 4 weeks.Changes in traditional Chinese medicine(TCM)syndrome scores and Hamilton Anxiety Rating Scale(HAMA)scores were evaluated before and after treatment.The therapeutic effects of angina pectoris,nitroglycerin reduction/cessation rates,and total adverse event rates were compared between the two groups.Results(1)After 4 weeks of treatment,the total effective rate in the observation group was 93.33%(28/30),compared to 73.33%(22/30)in the control group.The intergroup comparison(by Fisher's precision probability test)showed that the observation group's efficacy was significantly superior to that of the control group(P<0.05).(2)After treatment,both groups showed significant reductions in TCM syndrome scores(P<0.05)for symptoms including chest tightness and pain,costal distension,anxiety,palpitations,excessive sighing,irritability,and insomnia.The observation group's reduction was significantly greater than that of the control group(P<0.05).(3)After 4 weeks,the total reduction/discontinuation rate of nitroglycerin was 90.00%(27/30)in the observation group versus 66.67%(20/30)in the control group.The intergroup comparison(by Fisher's precision probability test)showed that the observation group's reduction rate was significantly higher than that of the control group(P<0.05,Fisher's precision probability test).(4)After treatment,both groups exhibited reduced Hamilton Anxiety Rating Scale(HAMA)scores(P<0.05),with the observation group showing a significantly greater improvement(P<0.05).(5)The total incidence of adverse reactions was 20.00%(6/30)in the observation group and 23.33%(7/30)in the control group,with no statistically significant difference between groups(P>0.05).Conclusion The combination of Shuxin Jieyu Decoction with conventional western medicine treatment demonstrates superior clinical efficacy compared to conventional western medicine alone in patients with qi stagnation and blood stasis syndrome type of CHD with angina pectoris and anxiety disorder.It effectively alleviates angina symptoms and anxiety disorder,reduces nitroglycerin usage,without increasing adverse effects.

Objective To investigate the efficacy and safety of camrelizumab monoclonal antibody combined with molecular-targeted therapy in elderly patients with unresectable or advanced hepatocellular carcinoma(HCC).Methods A retrospective analysis was performed for the patients with unresectable/advanced HCC who attended six hospitals from January 1,2019 to March 31,2021,and all patients received camrelizumab monoclonal antibody treatment,among whom 84.8%also received targeted therapy.According to the age of the patients,they were divided into elderly group(≥65 years)and non-elderly group(<65 years).The two groups were assessed in terms of overall survival(OS),progression-free survival(PFS),objective response rate(ORR),disease control rate(DCR),and immune-related adverse events(irAE).The chi-square test or the Fisher's exact test was used for comparison of categorical data between groups;the independent samples t-test was used for comparison of normally distributed continuous data,and the Mann-Whitney U test was used for comparison of non-normally distributed continuous data between two groups.The Kaplan-Meier method was used for survival analysis,and the log-rank test was used for comparison of survival curves.Univariate and multivariate Cox proportional hazards regression analyses were used to determine the independent influencing factors for PFS and DCR at 6 months.Results A total of 99 HCC patients were enrolled,with 27 in the elderly group and 72 in the non-elderly group.The elderly group had an OS rate of 67.8%,an ORR of 44.4%,and a DCR of 74.1%at 12 months and a median PFS of 6.4(95%confidence interval[CI]:3.0-12.4)months,with no significant differences compared with the non-elderly group(all P>0.05).The median OS was unavailable for the elderly group,while the non-elderly group had an OS of 18.9(95%CI:13.0-24.8)months;there was no significant difference between the two groups(P=0.485).The univariate and multivariate Cox regression analyses showed that major vascular invasion(MVI)was an independent risk factor for PFS(hazard ratio[HR]=2.603,95%CI:1.136-5.964,P=0.024)and DCR(HR=3.963,95%CI:1.671-9.397,P=0.002)at 6 months,while age,sex,etiology of HBV infection,presence of extrahepatic metastasis,Child-Pugh class B,and alpha-fetoprotein>400 ng/mL were not associated with PFS or DCR at 6 months.For the elderly group,the incidence rates of any irAE and grade 3/4 irAE were 51.9%and 25.9%,respectively,with no significant differences compared with the non-elderly group(P>0.05),and skin disease was the most common irAE in both groups(39.4%).Conclusion Camrelizumab monoclonal antibody combined with molecular-targeted therapy has similar efficacy and safety in patients with unresectable/advanced HCC aged≥65 years and those aged<65 years.MVI is associated with suboptimal response to immunotherapy and poor prognosis.

Recombinase polymerase amplification (RPA) is a newly developed isothermal amplification technology with high sensitivity and specificity. The combination of RPA and lateral flow strips (LFS) enables rapid identification of target genes. This technique has been widely used in medicine, food, botany, and other fields. This review generalizes the use of RPA-LFS technology for the diagnosing pathogenic microorganisms, providing a reference for point-of-care diagnosis of pathogenic microorganisms.

Molecular knowledge of human gastric corpus epithelium remains incomplete. Here, by integrated analyses using single-cell RNA sequencing (scRNA-seq), spatial transcriptomics, and single-cell assay for transposase accessible chromatin sequencing (scATAC-seq) techniques, we uncovered the spatially resolved expression landscape and gene-regulatory network of human gastric corpus epithelium. Specifically, we identified a stem/progenitor cell population in the isthmus of human gastric corpus, where EGF and WNT signaling pathways were activated. Meanwhile, LGR4, but not LGR5, was responsible for the activation of WNT signaling pathway. Importantly, FABP5 and NME1 were identified and validated as crucial for both normal gastric stem/progenitor cells and gastric cancer cells. Finally, we explored the epigenetic regulation of critical genes for gastric corpus epithelium at chromatin state level, and identified several important cell-type-specific transcription factors. In summary, our work provides novel insights to systematically understand the cellular diversity and homeostasis of human gastric corpus epithelium in vivo.

9. 3-bromopyruvate cholesterol ester enhances the sensitivity of breast cancer cells to tamoxifen

AIM: To investigate the effect of 3-bro-mopyruvate cholesterol ester (3-BP-Cl) on the sensitivity of breast cancer to tamoxifen (TAM). METHODS: MTT assay and Calcein AM/PI staining were used to detect the effect of drugs on the viability of breast cancer cells. Kim's formula was used to detect synergistic anti-breast cancer effect of cholesterol 3-bromopyruvate and tamoxifen. The inhibitory effect of drugs on proliferation of breast cancer cells was detected by colony-forming assay. Flow cytometry was used to detect the apoptosis of breast cancer cells. Western blot assay was used to detect the expression of hexokinase 2, Bcl-2 and Bax proteins. RESULTS: MTT results showed that combination 3-BP-Cl and TAM could significantly inhibit the activity of MCF-7 cells (P<0.05). The results of King's formula showed that 3-BP-Cl and TAM had synergistic inhibitory effect on the proliferation of MCF-7 cells (q>1.15). Calcein AM / PI staining showed that the number of dead cells was the highest in the combination group. Colony-forming assay showed that the combination group had stronger inhibitory effect on the proliferation of MCF-7 cells than that of single drug groups. AnnexinV flow cytometry results showed that, the cell apoptosis in the combination group was significantly increased (P<0.01). Western blot results showed that 3-BP-Cl inhibited the expressions of hexoktokinase 2 and Bcl-2, and enhanced the expression of Bax in MCF-7 cells. CONCLUSION: 3-BP-Cl could increase the sensitivity of breast cancer cells to tamoxifen, and synergically inhibit the proliferation of breast cancer cells. The mechanism is possibly related to its effects of inhibiting the expression of HK2/Bcl-2, and enhancing the expression of Bax.
10.Lingguizhugan Decoction, a Chinese herbal formula, improves insulin resistance in overweight/obese subjects with non-alcoholic fatty liver disease: a translational approach.

Lingguizhugan Decoction (LGZG) has been investigated in basic studies, with satisfactory effects on insulin resistance in non-alcoholic fatty liver disease (NAFLD). This translational approach aimed to explore the effect and underlying mechanism of LGZG in clinical setting. A randomized, double-blinded, placebo-controlled trial was performed. A total of 243 eligible participants with NAFLD were equally allocated to receive LGZG (two groups: standard dose and low dose) or placebo for 12 weeks on the basis of lifestyle modifications. The primary efficacy variable was homeostasis model assessment of insulin resistance (HOMA-IR). Analyses were performed in two populations in accordance with body mass index (BMI; overweight/obese, BMI ⩾ 24 kg/m2; lean, BMI < 24 kg/m2). For overweight/obese participants, low-dose LGZG significantly decreased their HOMA-IR level compared with placebo (-0.19 (1.47) versus 0.08 (1.99), P = 0.038). For lean subjects, neither dose of LGZG showed a superior effect compared with placebo. Methylated DNA immunoprecipitation sequencing and real-time qPCR found that the DNA N6-methyladenine modification levels of protein phosphatase 1 regulatory subunit 3A (PPP1R3A) and autophagy related 3 (ATG3) significantly increased after LGZG intervention in overweight/obese population. Low-dose LGZG effectively improved insulin resistance in overweight/obese subjects with NAFLD. The underlying mechanism may be related to the regulation of DNA N6-methyladenine modification of PPP1R3A and ATG3. Lean subjects may not be a targeted population for LGZG.
