1.Clinical analysis of assisted reproductive technology assisted pregnancy outcome in female patients with thyroid cancer after surgery

ObjectiveTo evaluate the pregnancy outcomes of assisted reproductive technology (ART) in women with a history of thyroid cancer who retained fertility intentions after completing cancer treatment. MethodsA retrospective analysis was performed on 61 patients with a history of thyroid cancer who underwent in vitro fertilization/intracytoplasmic sperm microinjection and embryo transfer (IVF/ICSI-ET). These patients were included as the case group. A total of 122 non-cancer patients who received ART during the same period were selected as the control group using 1∶2 matching based on age and oocyte retrieval time. Baseline characteristics, outcomes of the first ART cycle, and cumulative pregnancy outcomes were compared between the two groups. ResultsThere was no significant difference in the basic data, the total amount of gonadotropin (Gn) and the days of use between the case group and the control group (P>0.05). However, the case group had significantly fewer retrieved oocytes, mature oocytes (MII), lower fertilization and cleavage rates, and fewer transferable and high-quality embryos, as well as fewer embryos transferred during the first cycle (P < 0.05). However, there was no significant difference in the rate of first embryo implantation and first clinical pregnancy between the two groups (P>0.05). In the analysis of cumulative outcomes, the two groups did not show statistically significant differences in the cumulative pregnancy rate, clinical pregnancy rate per transfer cycle, the number of oocyte retrieval cycles required per live birth, the number of embryo transfer cycles required per live birth, and the number of embryos used for each live birth (P>0.05). However, the cumulative live birth rate was significantly lower in the case group compared to the control group (P=0.005). ConclusionAfter treatment for thyroid cancer, when ART is used to help pregnant women, the pregnancy outcome is comparable to that of women without tumors. Individualized reproductive management and timely fertility preservation strategies are recommended to optimize reproductive outcomes in this population.
2.Association between Mediterranean diet scores and dental caries among children and adolescents with neurodevelopmental disorders

Objective:
To explore the association between mediterranean diet (MD) patterns and dental caries among children and adolescents with neurodevelopmental disorders (NDD), so as to provide a basis for developing scientific anti caries strategies related to diet.
Methods:
From December 2021 to June 2024, a questionnaire survey, a three day 24 hour dietary review survey, oral health examination, physical development measurement and Childhood Autism Rating Scale (CARS) evaluation were conducted involving 147 children and adolescents aged 2-22 years with NDD from nine special education schools and rehabilitation institutions in Tianjin. Group comparisons were carried out using the Mann-Whitney U test, Chi-square test, or Fisher s exact probability method. The correlation between dietary quality and dental caries was analyzed by adopting multiple linear regression analysis and restricted cubic spline.
Results:
There were 46 children and adolescents (31.3%) in the non dental caries group and 101 children and adolescents (68.7%) in the dental caries group. The number of decayed missing and filled teeth (dmft) was 2.0 (4.0), and the MD score was 4.0 (2.0) points. There were 62 children and adolescents (42.2%) in the low MD scores group and 85 children and adolescents (57.8%) in the high MD scores group. There was no significant difference in MD scores between NDD children in the non dental caries group and those in the dental caries group [nondental caries group:4.0(2.0), dental caries group:4.0(2.0), Z= -0.14, P >0.05]. The MD scores and dmft exhibited increasing and then decreasing trend ( P total =0.02, P non lineary = 0.04 ). Children and adolescents with NDD in the MD high scores group had a lower number of dmft than those in the MD low scores group ( β= -2.00 , 95%CI =-3.39 to -0.62, P <0.05). However, in children and adolescents with NDD and CARS scores ≥30, the above association was insignificant ( β=-0.63, 95%CI=-0.29-0.15, P >0.05).
Conclusion
Children and adolescents with NDD who have dietary patterns similar to the Mediterranean diet, are found to have fewer dental caries, and this is observed among those with no or mild symptoms of autism spectrum disorder.
3.Value of prealbumin-to-total bilirubin ratio in predicting the short-term prognosis of patients with HBV-related acute-on-chronic liver failure

ObjectiveTo investigate the value of prealbumin-to-total bilirubin (PA/TBil) ratio on admission in predicting 90-day mortality or liver transplantation in patients with HBV-related acute-on-chronic liver failure (HBV-ACLF), as well as the effect of its combination with Model for End-Stage Liver Disease (MELD) score on the predictive performance for short-term prognosis. MethodsA retrospective analysis was performed for the clinical data of 216 HBV-ACLF patients who were admitted to Department of Infectious Diseases in the First Affiliated Hospital of Soochow University from April 2020 to May 2025, and the patients were followed up for 3 months. According to the outcome, the patients were divided into survival group with 104 patients and death/transplantation group with 112 patients. The Kolmogorov-Smirnov test was used to check whether the continuous data was in accordance with the normal distribution; the two-independent-samples t test was used for comparison of normally distributed continuous data between two groups, while the Mann-Whitney U test was used for comparison of continuous data with skewed distribution between two groups. The chi-square test was used for comparison of categorical data between two groups. Univariate and multivariate binary Logistic regression analyses were used to investigate the influencing factors for prognosis, and the receiver operating characteristic (ROC) curve was used to analyze the performance of each indicator in predicting the prognosis of ACLF. The area under the ROC curve (AUC) was calculated, and the Delong test was used for comparison of AUC. ResultsA total of 216 patients were enrolled in this study, with a 90-day survival rate of 48.15% (104/216). Compared with the death/transplantation group, the survival group had significantly higher platelet count, lymphocyte count, albumin, and PA/TBil ratio (all P<0.05) and significantly lower age, white blood cell count, neutrophil count, prothrombin time, international normalized ratio, aspartate aminotransferase, total bilirubin, creatinine, and MELD score (all P<0.05). The multivariate Logistic regression analysis showed that age (odds ratio [OR]=1.05, 95% confidence interval [CI]: 1.02 — 1.09, P<0.001), PA/TBil ratio (OR=0.16, 95%CI: 0.05 — 0.46, P<0.001), and MELD score (OR=1.09, 95%CI: 1.01 — 1.17, P=0.024) were independent influencing factors for 90-day prognosis in HBV-ACLF patients. PA/TBil ratio and MELD score used alone or in combination had an AUC of 0.760, 0.779, and 0.811, respectively, in predicting the prognosis of HBV-ACLF patients, and PA/TBil ratio combined with MELD score had a better AUC than PA/TBil ratio or MELD score used alone (Z=-2.058 and 2.017, both P<0.05). ConclusionBoth serum PA/TBil ratio and MELD score can effectively predict the prognosis of patients with HBV-ACLF, and a combination of the two indicators had a better predictive performance than MELD score alone, which provides an important reference for clinical risk stratification management and timely intervention.
4.Improvement effect and mechanism of desloratadine citrate disodium in hypersensitivity pneumonitis model mice

OBJECTIVE To investigate the improvement effect and mechanism of desloratadine citrate disodium in mice with hypersensitivity pneumonitis (HP). METHODS Sixty mice were randomly divided into blank control group (normal saline), model group (normal saline), prednisone group (positive control, 20 mg/kg) and desloratadine citrate disodium low-, medium- and high-dose groups (0.5, 1, 2 mg/kg), with 10 mice in each group. Except for the blank control group, mice in other groups were intraperitoneally injected with ovalbumin (OVA) and exposed to OVA inhalation to establish the HP model. On day 22 post- modeling, mice in each group were administered the corresponding drugs or normal saline, once a day, for 11 consecutive days. After the last administration, lung function and airway hyperreactivity were assessed. The levels of interleukin-1β (IL-1β), IL-4 and IL-6 in serum as well as the levels of IL-8, IL-13 and IL-17A in bronchoalveolar lavage fluid were determined. Pathological changes in lung tissue of mice were evaluated using Masson staining. Furthermore, the expressions of fibrosis-related proteins, including transforming growth factor β1 (TGF-β1), type Ⅲ collagen (Col-Ⅲ) and fibronectin (FN) were determined in lung tissues. RESULTS Compared with the blank control group, the model group showed significant deterioration in lung function (P< 0.01), while airway resistance and serum levels of IL-1β, IL-4, IL-6 and the levels of IL-8, IL-13 and IL-17A in the bronchoalveolar lavage fluid were increased significantly (P<0.01). The lung tissues exhibited alveolar collapse, atrophy, and structural disarray, along with the formation of extensive deposits of blue collagen fibers, the percentage of positive staining increased significantly (P<0.01). Additionally, the expression levels of TGF-β1, Col-Ⅲ, and FN proteins in the lung tissues were also increased significantly (P<0.01). After intervention with desloratadine citrate disodium, the pathological changes in the lung tissues of mice in each dosage group of desloratadine citrate disodium showed varying degrees of improvement, and most of the aforementioned indicator levels were significantly reversed (P<0.05 or P<0.01). CONCLUSIONS Desloratadine citrate disodium can improve the lung function and airway hyperreactivity of HP mice, inhibit the release of inflammatory factors in serum and bronchoalveolar lavage fluid, and reduce the deposition of collagen fibers. Its mechanism of action may be related to anti-inflammatory, immunomodulatory, and antifibrotic effects.
5.Observation on safety and effectiveness of modified hydrodissection fluid in microwave ablation of benign thyroid nodules

Objective To investigate the safety and effectiveness of normal saline as a hydrodissection band versus sodium hyaluronate and biopolysaccharide mixture as a modified hydrodissection band in microwave ablation (MWA) for benign thyroid nodules. Methods A total of 196 patients with 245 benign thyroid nodules(maximum diameter ≥20 mm) treated at Shanghai Ruijin Rehabilitation Hospital from August 2018 to February 2022 were enrolled. Patients were divided into control group (n=114): MWA using normal saline as the hydrodissection fluid; modified group (n=131): MWA using sodium hyaluronate and biopolysaccharide mixture as the hydrodissection fluid. A hydrodissection band (about 10 mm wide) was established before MWA. Intraoperative hydrodissection fluid absorption time was recorded. At the 12-month follow-up endpoint, complications were assessed, and changes in nodule volume (volume reduction rate, VRR) were evaluated. Results The complication rate was significantly lower in the modified group (1.9% vs.14.0%, P<0.05). 92.3% of complications in the control group were thermal injury-related, while no thermal injuries occurred in the modified group. Hydrodissection fluid absorption time was significantly prolonged in the modified group [(15.2±1.6) min vs. (11.0±2.3) min, P<0.05].No significant difference was observed in overall treatment efficacy (nodule VVR) between two groups (95.4% vs. 96.5%, P>0.05). Conclusions The sodium hyaluronate and biopolysaccharide mixture demonstrates prolonged retention time and superior thermal insulation as an hydrodissection fluid in MWA. It reduces adhesion risks and complications such as hoarseness, dysphagia, and thermal injury, thereby enhancing procedural safety and postoperative recovery. MWA is an effective treatment for benign thyroid nodules.
6.Research progress of construction and application of artificial intelligence predictive models in rectal cancer radiotherapy

In recent years, the application of artificial intelligence technology in rectal cancer radiotherapy has become increasingly significant. By constructing models from patient clinical information, accurate prediction of dose distribution, treatment effect, and toxic side effects of rectal cancer can be achieved. This allows optimizing the radiotherapy plan, ensuring the dose is focused on the tumor target area while reducing the radiation damage to the bladder, rectum, and other surrounding tissues. Thus, it can achieve precision and personalization in radiotherapy. In this review, the construction method of artificial intelligence predictive models was described, and the value of different predictive factors to the model was systematically analyzed, including patient clinical data, radiomics, and dosimetry. Moreover, the application and limitations of artificial intelligence predictive models in radiotherapy were summarized. This information can serve as a reference for the clinical application of artificial intelligence predictive models in rectal cancer radiotherapy.
7.Effectiveness of different specific exercise therapies in treatment of adolescent idiopathic scoliosis:a network meta-analysis

OBJECTIVE:At present,there are a variety of treatment methods for scoliosis using specific exercise therapy,but there is a lack of comparison of efficacy between different specific exercise therapy.This article compared the effectiveness of different specific exercise therapies to treat adolescent idiopathic scoliosis through a network meta-analysis. METHODS:Domestic and foreign electronic databases of relevant studies were searched for randomized controlled trials of specific exercise therapy for adolescent idiopathic scoliosis.Search time was from January 2000 to July 2023.The literature was screened by two reviewers using RevMan 5.4 and Stata 16.0 software to extract data and assess the bias risk of of inclusion studies. RESULTS:(1)This article includes 20 randomized controlled trials with 1 377 patients.Of them,12 studies involved Schroth therapy;2 studies involved BSPTS therapy,and 6 studies involved SEAS therapy.(2)The network meta-analysis indicated that in terms of improving Cobb angle and reducing trunk rotation angle in scoliosis patients,the BSPTS therapy group and Schroth therapy group were better than the conventional control group[WMD=-4.60,95%CI(-8.37,-0.82),P<0.05;WMD=-3.37,95%CI(-4.98,-1.75),P<0.05;WMD=-3.20,95%CI(-5.50,-0.90),P<0.05;WMD=-2.13,95%CI(-3.16,-1.09),P<0.05].The Schroth therapy group performed better than the conventional control group effective in improving the International Society for Scoliosis Research-22 Questionnaire quality of life score[WMD=1.41,95%CI(0.07,2.75),P<0.05]. CONCLUSION:Given the current evidence,BSPTS therapy group and Schroth therapy group were better than the conventional control group in improving Cobb angle and reducing trunk rotation angle.In the comparison of different specific exercise therapies,BSPTS therapy can be preferred to improve Cobb angle and reduce trunk rotation angle in adolescent idiopathic scoliosis patients.In addition,Schroth therapy may be the best treatment to improve the quality of life of adolescent idiopathic scoliosis patients.Limited by the quantity and quality of the included studies,the above conclusions should be interpreted with caution and need more high-quality studies to further validation.
8.Four cases of COVID-19 associated Guillain-Barré syndrome

COVID-19 associated Guillain-Barré syndrome (GBS) caused by peripheral nerve damage after SARS-CoV-2 infection is one of the most common COVID-19 related nervous system inflammatory diseases, with high incidence of respiratory failure and mortality. Positive SARS-CoV-2 RNA in cerebrospinal fluid of COVID-19 associated GBS patients has been rarely reported. This paper reports 4 patients with COVID-19 associated GBS in China who developed neurological symptoms 4-15 days after fever and were confirmed SARS-CoV-2 infection. All patients presented with progressive weakness of both lower limbs, 3 patients with autonomic dysfunction such as defecation and urination disorders, and 1 patient with polycranial neuritis and Miller-Fisher syndrome such as bilateral facial palsy, dysphagia, diplopia and ataxia. Nerve conduction velocity and F wave were abnormal in 3 patients, and motor conduction pathway was abnormal in 1 patient. Anti-ganglioside antibodies were tested in 3 patients, and GD1a-IgG was positive in 1 patient. All 4 patients underwent metagenomic next-generation sequencing examination in blood and cerebrospinal fluid. SARS-CoV-2 RNA was positive in blood and cerebrospinal fluid of 3 patients, and SARS-CoV-2 RNA was positive in cerebrospinal fluid of 1 patient.
9.Clinical Comprehensive Evaluation of Chinese Patent Medicines in Treatment of Stable Angina Pectoris with Qi Stagnation and Blood Stasis

ObjectiveTen Chinese patent medicines for stable angina pectoris with qi stagnation and blood stasis were comprehensively evaluated, with the aim of providing reference for optimizing the drug list of medical institutions and promoting rational drug utilization. MethodAccording to the Expert Consensus on the Selection of Chinese Patent Medicines in Medical Institutions 2022, an evaluation framework of "6+1" dimensions was established for comprehensive evaluation of the drugs based on the indicators in three levels. ResultThe evaluation results showed that Compound Danshen dripping pills, Shexiang Baoxin pills, Di'ao Xinxuekang capsules, and Xuefu Zhuyu capsules were strongly recommended for stable angina pectoris with Qi stagnation and blood stasis, with the scores of 78.5, 76, 72, and 70.8, respectively. Suxiao Jiuxin pills, Xinkeshu Tablets, Guanxin Danshen Dripping pills, and Yindan Xinnaotong soft capsules were weakly recommended, with the scores of 68.5, 65.5, 60.5, and 60.5 respectively. Lemai pills and Yangxin Dawayimixike migao were not recommended for the time being due to their low scores (50.5 and 48.8, respectively). There is a lack of research on special populations, pharmacokinetic parameters, post-marketing re-evaluation, and economics of the medicines. ConclusionWith the updating of Chinese patent medicines, real-world research data are needed to enhance the evidence-based support. This evaluation can only reflect the comprehensive situation at a certain stage, and dynamic evaluation remains to be carried out to provide support for decision makers.
10.Research progress in animal models of delirium and their evaluation methods

Delirium is an acute brain dysfunction syndrome characterized by confusion and difficulty concentrating,which mainly affects intensive care unit patients and elderly inpatients.Treatment is expensive and may also lead to increased risks of serious complications and death.The complex etiology and unknown pathological mechanisms of delirium mean that clinical drug treatment is largely ineffective.Animal models therefore provide a powerful tool to help understand the mechanism of delirium,screen new drugs,and study potential intervention measures.We review experimental research related to delirium animal models worldwide,and summarize the latest progress in the construction and evaluation of these models from the aspects of animal selection,model construction method,and model evaluation,to provide a reference for further experimental research based on delirium animal models.
