ObjectiveTo observe the clinical efficacy of Anshen Buxin Gao in patients with coronary artery disease (CAD) complicated with somatization disorder after percutaneous coronary intervention (PCI), as well as its effect on heart rate variability (HRV). MethodsA total of 96 patients with somatization disorder after PCI were selected and randomized into control and treatment groups (48 cases). On the basis of standardized Western medical treatment for CAD, the control group received Dailixin, while the treatment group received Dailixin combined with Anshen Buxin Gao. The somatic symptom scale (SSS), generalized anxiety disorder 7-item scale (GAD-7), patient health questionnaire-9 (PHQ-9), and self-rating scale of sleep (SRSS) scores in both groups were recorded before and after treatment. The traditional Chinese medicine symptom efficacy, HRV, and incidence of adverse drug reactions were observed. ResultsA total of 90 patients completed the trial, encompassing 45 patients in the control group and 45 patients in the treatment group. Baseline characteristics between the two groups showed no statistically significant differences, indicating comparability. After treatment, both groups exhibited reductions in SSS, GAD-7, and PHQ-9 scores (P<0.05), and the treatment group outperformed the control group in alleviating somatic symptoms and anxiety-depression symptoms (P<0.05). The control group did not achieve a significant reduction in SRSS score, whereas the treatment group effectively lowered the SRSS score (P<0.05). Regarding traditional Chinese medicine symptom efficacy, the total response rate in the treatment group was 91.1% (41/45), which was higher than that (71.1%, 32/45) in the control group (Z=-2.663, P<0.05). Both groups improved HRV in patients with somatization disorder, and the treatment group showed greater improvement (P<0.05). There were no serious clinical adverse events during the study period. The incidence of adverse reactions in the treatment group was 6.7% (3/45), which was lower than that (14/45, 31.10%) in the control group (χ2=7.252, P<0.05). ConclusionThe addition of Anshen Buxin Gao to Dailixin therapy significantly alleviates the clinical symptoms and improves the sleep quality, treatment efficacy, and HRV in patients with CAD complicated with somatization disorder after PCI, while reducing the adverse effects associated with Dailixin alone. This approach demonstrates considerable clinical value and warrants further promotion.

ObjectiveTo systematically collate the randomized controlled trials (RCTs) on the intervention of traditional Chinese medicine (TCM) in chronic kidney disease (CKD) and analyze the characteristics of clinical trials and the current situation of outcome indicators, thus providing references for constructing a core outcome set (COS) for CKD. MethodsThe RCTs on the intervention of TCM in CKD were retrieved from CNKI, Wanfang Data, VIP, SinoMed, PubMed, Cochrane Library, EMbase, and Web of Science with the time interval from January 2022 to December 2024. The risk of bias was evaluated, and the outcome indicators were qualitatively analyzed. ResultsA total of 242 RCTs were included, which reported 238 outcome indicators, with the cumulative frequency of 2 724 times. These outcome indicators can be classified into seven domains: TCM syndromes and diseases (270 times, accounting for 9.90%), symptoms and signs (218 times, 8.00%), physical and chemical examinations (1 609 times, 59.07%), quality of life (29 times, 1.06%), long-term prognosis (6 times, 0.22%), economic evaluation (0 times), and safety events (535 times, 19.64%). The results demonstrated that the RCTs of TCM intervention for CKD had several shortcomings such as the lack of a basis for sample size estimation, lack of standardization in TCM syndrome differentiation and classification, room for improvement in methodological quality, flaws in the design of the timing of efficacy measurement, absence of long-term prognosis evaluation, imbalance in the selection of surrogate and endpoint indicators, disconnection between overall evaluation and individualized assessment, lack of TCM characteristic indicators, and insufficient reporting of safety indicators. ConclusionAt present, the design of outcome indicators in the RCTs for TCM intervention in CKD is still not standardized. It is recommended that future research endeavors to enhance methodological quality, establish a standardized COS for TCM intervention in CKD, and thereby propel the high-quality development of TCM.

Objective:This study sought to examine the clinicopathological features of monomorphic epitheliotropic intestinal T-cell lymphoma (MEITL) and to discuss its differential diagnosis. Methods:A total of 36 MEITL cases, collected between June 2015 and January 2024 from the Fourth Affiliated Hospital of Soochow University and the First Affiliated Hospital, College of Medicine, Zhejiang University, were analyzed. Patients underwent immunohistochemistry, in situ hybridization for Epstein-Barr virus-encoded small RNA (EBER), and T-cell receptor (TCR) gene rearrangement testing. Clinical data, laboratory results, and follow-up information were collected for correlation analysis. Results:The cohort included 36 patients (20 males and 16 females) aged 17-76 years (median: 57 years). Tumors outside the intestine were observed in 22 cases (61%). A total of 32 patients (89%) underwent surgical intervention and/or chemotherapy, and one patient received auto-HSCT. The median follow-up duration was 11.5 months (range: 8-73 months), with a median overall survival of 6 months (range: 1-67 months) ; 34 patients died during the follow-up period. Morphologically, nine cases (25%) exhibited significant pleomorphism. Immunohistochemical analysis revealed that high expression levels of both P53 and c-Myc were correlated with atypical morphology ( P=0.003 and P=0.016, respectively). Notably, patients with high P53 expression had significantly shorter survival times than those with low P53 expression ( χ2=4.922, P=0.027), whereas survival did not differ significantly based on c-Myc expression levels ( χ2=0.034, P=0.854). Furthermore, a PD-L1 CPS score ≥10 was observed in 22 cases (68.8%). Scattered EBER positivity in background cells was identified in four cases. All tested cases (17/17, 100.0%) showed clonal TCR gene rearrangements. Conclusions:MEITL is a rare but highly aggressive lymphoma with distinct clinical and pathological features. A subset of cases may exhibit atypical morphological patterns, complicating the diagnostic process. Improving awareness of this neoplasm is helpful for early and precise diagnosis as well as the estabolishment of novel therapy regimen.
