1.A real world study on the effectiveness and safety of deu-cravacitinib in the treatment of moderate-to-severe plaque psoriasis in China.

OBJECTIVES:
To evaluate the effectiveness and safety of deucravacitinib in Chinese patients with moderate-to-severe plaque psoriasis.
METHODS:
This retrospective study included 41 patients with moderate-to-severe plaque psoriasis treated with deu-cravacitinib 6 mg once daily for 16 weeks at the First Affiliated Hospital of Wenzhou Medical University between January and September 2024. Effectiveness was assessed by the psoriasis area and severity index (PASI), static physician's global assessment (sPGA), palmoplantar psoriasis area and severity index (PPASI), modified nail psoriasis severity index (mNAPSI), and dermatology life quality index (DLQI) at baseline, 4, 8, 12 and 16 weeks after treatment. Adverse events during treatment were recorded. Laboratory parameters, including complete blood count, liver and kidney function, electrolytes, and lipids, were assessed at baseline, 8, 16 weeks after treatment to evaluate safety. Univariate and multivariate logistic regression analyses were performed to explore factors associated with achieving PASI75 at week 16, using baseline characteristics as independent variables.
RESULTS:
Significant reductions from baseline in PASI and DLQI scores were observed from week 4 through week 16 (all P<0.01). Overall response rates for PASI75, PASI90, PASI100, sPGA 0 or 1 grade, and DLQI 0 or 1 point increased progressively over the treatment period. 75, 90, and 100 refer to a score reduction of at least 75%, at least 90%, and 100%, respectively, from baseline. Response rates of PASI75, PASI90, PASI100 for the scalp, limbs, and trunk, PPASI75, PPASI90, PPASI100 for palmoplantar lesions, and mNAPSI75, mNAPSI90 for nail lesions increased progressively over time but with different trends. Scalp lesions improved most markedly from week 4, followed by the limbs, whereas improvements in trunk and palmoplantar lesions were relatively slower. Nail lesions responded more slowly, with only 20% of patients achieving marked improve-ment at week 16. Deucravacitinib demonstrated good tolerability and compatibility with concomitant medications. No severe adverse events were reported, indicating a favorable safety profile. Multivariate logistic regression analysis revealed no significant association between the achievement of PASI75 response at week 16 and patient age, body mass index, disease duration, or baseline PASI, sPGA, or DLQI scores (all P>0.05).
CONCLUSIONS
In this real world study of Chinese patients with moderate-to-severe plaque psoriasis, deucravacitinib demonstrated favorable effectiveness and safety over 16 weeks of treatment.

The continuous emergence of SARS-CoV-2 variants as well as other potential future coronavirus has challenged the effectiveness of current COVID-19 vaccines. Therefore, there remains a need for alternative antivirals that target processes less susceptible to mutations, such as the formation of six-helix bundle (6-HB) during the viral fusion step of host cell entry. In this study, a novel high-throughput screening (HTS) assay employing a yeast-two-hybrid (Y2H) system was established to identify inhibitors of HR1/HR2 interaction. The compound IMB-9C, which achieved single-digit micromolar inhibition of SARS-CoV-2 and its Omicron variants with low cytotoxicity, was selected. IMB-9C effectively blocks the HR1/HR2 interaction in vitro and inhibits SARS-CoV-2-S-mediated cell-cell fusion. It binds to both HR1 and HR2 through non-covalent interaction and influences the secondary structure of HR1/HR2 complex. In addition, virtual docking and site-mutagenesis results suggest that amino acid residues A930, I931, K933, T941, and L945 are critical for IMB-9C binding to HR1. Collectively, in this study, we have developed a novel screening method for HR1/HR2 interaction inhibitors and identified IMB-9C as a potential antiviral small molecule against COVID-19 and its variants.
3.Transrectal ultrasonographic features of rectal lymphoma

Objective:To summarize endorectal ultrasound(ERUS)findings of rectal lymphoma(RL)and evaluate its clinical value for preoperative diagnosis.Methods:Nine RL patients between Jan 2015 and March 2025 in the Sixth Affiliated Hospital of Sun Yat-Sen University were included in this retrospective study. ERUS findings including lesion location,size,echogenicity,mucosal integrity,depth of infiltration,and blood flow characteristics were reviewed. The diagnostic performance of ERUS was evaluated using pathology as the gold standard.Results:ERUS features of RL included extensive bowel wall involvement(≥1/2 circumference in 8/9 cases),intact mucosa(7/9),homogeneous markedly hypoechoic echogenicity(6/9),and Adler grade 3 blood flow(9/9). No case presented bowel obstruction;three cases involved multi- segmental bowel. All four cases who received ultrasound-guided biopsy reached conclusive diagnosis,while 2 of them were infeasible for endoscopic biopsy.Conclusions:Typical ERUS findings of RL include extensive infiltration,preserved mucosa,and homogeneous markedly hypoechoic echogenicity. ERUS could clearly show the submucosal infiltration pattern and bowel wall layers,and significantly improve preoperative diagnostic accuracy in combination with ultrasound-guided biopsy.
4.Controversy and consensus on the role of tumor infiltrating lymphocytes in the diagnosis and treatment of breast cancer

The incidence of breast cancer(BC)is on the rise.The effective and precise management of BC continues to be topical.Tumor in-filtrating lymphocytes(TILs),significant biomarkers indicative of the biological behavior of BC,have not been well recognized in clinical prac-tice.There are some impediments,including the lack of a standard protocol for the quantification of TILs,an optimized threshold for TILs,and a repeatable noninvasive method for the assessment of TILs.These issues are the major controversies preventing the use of TILs as bio-markers in the clinical management of BC.Nevertheless,the value of TILs as biomarkers in the assessment of the prognosis and prediction of the response to treatment cannot be neglected,as they show promise in promoting precise and individualized management of BC.We aimed to offer a comprehensive outline of the current controversies and consensus regarding TILs as biomarkers in the management of BC,with the aim of providing a basis for the rational application of TILs as biomarkers.
5.Correlation analysis of serum human epididymis protein 4 and LIPS score with sepsis-associated acute respiratory distress syndrome and 28 d mortality

Objective To investigate the correlation of serum human epididymis protein 4(HE4)and lung injury Prediction(LIPS)score with sepsis-associated acute respiratory distress syndrome(ARDS)and 28 d mortality.Methods A total of 207 patients with sepsis admitted to the hospital from January 2021 to January 2024 were selected.According to the presence or absence of ARDS,the patients were divided into ARDS group(73 cases)and non-ARDS group(134 cases).According to the 28 d survival,the ARDS group was further di-vided into death group(26 cases)and survival group(47 cases).HE4 level was detected by electrochemilumi-nescence,and LIPS score was calculated.Multivariate Logistic regression model was used to analyze the influ-encing factors of death in ARDS patients.Point biserial correlation was used to analyze the correlation between serum HE4,LIPS score and 28 d death in ARDS patients.The receiver operating characteristic(ROC)curve was used to evaluate the value of serum HE4 and LIPS score in the diagnosis of sepsis-associated ARDS and predicting 28 d mortality.Results Compared with non-ARDS group,the serum HE4 level and LIPS score were significantly increased in ARDS group(P<0.05).The independent risk factors for sepsis-associated ARDS were increased SOFA score,increased blood lactic acid,increased HE4 and increased LIPS score(P<0.05).The 28 d mortality of 73 patients with sepsis-associated ARDS was 35.62%(26/73).Compared with the survival group,the serum HE4 and LIPS scores were increased in the death group(P<0.05).Serum HE4 and LIPS scores were positively correlated with 28 d mortality in patients with sepsis-associated ARDS(r=0.605,0.579,P<0.05).The area under the curve(AUC)of serum HE4 combined with LIPS score in the di-agnosis of sepsis-associated ARDS was 0.909,which was larger than 0.828 and 0.806 of serum HE4 and LIPS score alone(Z=3.360,3.387,all P<0.05).The AUC of serum HE4 combined with LIPS score in predicting 28 d mortality of patients with sepsis-associated ARDS was 0.891,which was larger than 0.808 and 0.779 of by serum HE4 and LIPS score alone(Z=2.028,2.017,all P<0.05).Conclusion The elevated serum HE4 level and LIPS score are independent risk factors for sepsis-associated ARDS,and are closely related to 28 d mortality.The combination of serum HE4 and LIPS score has a high value in the diagnosis of sepsis-associated ARDS and the prediction of 28 d mortality.
6.Clinical characteristics and outcomes of Coronavirus Disease 2019 in immunocompromised hosts

Objective To investigate the clinical characteristics and outcomes of Coronavirus Dis-ease 2019 in immunocompromised hosts.Methods A retrospective analysis was conducted on the clinical data of 230 hospitalized patients diagnosed with Coronavirus Disease 2019 at Nanjing Yimin Hospital from December 2022 to November 2023.The patients were divided into three groups based on their immune status:immunocompromised group(n=59),relatively immunocompromised group(n=129),and immunocompetent group(n=42).The clinical characteristics(such as clinical manifesta-tions,imaging features,and laboratory examinations)and outcomes(such as length of hospital stay and in-hospital mortality)were compared among three groups.Results Compared with there latively immunocompromised and immunocompetent groups,the immunocompromised group showed no obvious specific clinical manifestations.However,the proportions of patients with symptoms such as cough and expectoration were lower,and the occurrences of symptoms such as myalgia and fatigue were less fre-quent in the immunocompromised group(P<0.05).The chest CT findings in the immunocompro-mised group also lacked specific changes,mainly presenting as subpleural ground-glass opacities and consolidations with multilobar distribution,but fibrotic changes were more common(P<0.05).The proportion of patients with decreased absolute lymphocyte counts in the immunocompromised group was higher than that in the immunocompetent group,and the proportion of patients with elevated procalcitonin levels was higher than that in the other two groups(P<0.05).The proportion of severe case sand the length of hospital stay in the immunocompromised group were higher and longer than those in the relatively immunocompromised and immunocompetent groups(P<0.05).The in-hospital mortality rates in the immunocompromised,relatively immunocompromised,and immunocompetent groups were 10.17%,6.98%,and 2.38%,respectively,with no statistically significant difference(P>0.05).Conclusion After Coronavirus Disease 2019,immunocompromised hosts do not show obvi-ous clinical and imaging features.However,they have a prolonged length of hospital stay,a signifi-cantly higher proportion of severe cases,and a tendency towards increased in-hospital mortality,which should be given high clinical attention.
7.Clinical efficacy and mechanism of Xuanbi Decoction as an adjuvant therapy for rheumatoid arthritis with dampness-heat obstruction syndrome

Objective To investigate the efficacy of Xuanbi Decoction as an adjuvant therapy for rheumatoid arthritis(RA)with dampness-heat obstruction syndrome,and its impacts on the interleu-kin-23(IL-23)/helper T cell 17(Th 17)inflammatory axis,as well as the levels of matrix metallo-proteinase-3(MMP-3)and tissue inhibitor of metalloproteinase-1(TIMP-1).Methods A total of 95 RA patients with dampness-heat obstruction syndrome were selected as the study subjects,and randomly divided into control group(48 cases)and observation group(47 cases).The control group received western medicine treatment,while the observation group received Xuanbi Decoction treat-ment based on the control group.The clinical efficacy,traditional Chinese medicine(TCM)syn-drome scores,imaging assessment results,disease activity,indicators of the IL-23/Th17 inflammatory axis,MMP-3,TIMP-1 and adverse reactions were compared between the two groups.Results The total effective rate in the observation group was 95.74%,which was significantly higher than 81.25%in the control group(P<0.05).After treatment,the TCM syndrome scores(joint pain,joint swell-ing,yellow and greasy tongue coating as well as slippery and rapid pulse)in the observation group were significantly lower than those in the control group(P<0.05).After treatment,the DAS28 and Sharp scores in the observation group were significantly lower than those in the control group(P<0.05).After treatment,the levels of IL-23,Th17,IL-17 and MMP-3 in the observation group were significantly lower than those in the control group,while the TIMP-1 level was significantly higher(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Xuanbi Decoction as an adjuvant therapy for RA patients with dampness-heat obstruction syndrome demonstrates significant efficacy and high safety.

In view of the issues of the incomplete disease names,different names for the same disease,and different diseases with the same name in modern traditional Chinese medicine(TCM),Professor Liu Minru,a master of TCM,believes that we ought to propose accurate and standardized new disease names of TCM under the guidance of traditional Chinese medicine theory,clarify the scope of diseases,and maintain the integrity of academic system of TCM.Based on the current situation and dilemma of the new disease names in modern Chinese medicine of gynecology,and according to Professor Liu Minru's academic views on the research of new disease names in TCM,this article proposes a research system for the naming and argumentation of new traditional Chinese medicine diseases,which includes"textual research on the origin and development of disease names-summary of academic views from famous TCM scholars-data mining of literature-experts argumentation by Delphi method".Taking Professor Liu Minru's research on the disease name of"the Syndromes Inducing Premature Cessation of Menstrual Fluid"as an example,this article demonstrates the disease name and connotation of"the Syndromes Inducing Premature Cessation of Menstrual Fluid"in multiple dimensions,verifies the accuracy,scientificity and generalizability of the new disease name,and forms a consensus about the diagnosis and treatment of disease in TCM,hoping to be helpful to provide ideas for further exploring the standardization and normalization of research on TCM disease name.

Objective This study aims to develop a guideline for assessing and maintaining intrinsic capacity in older adults,offer recommendations to professionals regarding these assessments,and encourage the implementation of evidence-based clinical practices across various settings,including communities,hospitals,nursing homes,and other geriatric care environments.Methods An evidence-based approach guided the collection of questions through a lit-erature review.Preliminary recommendations were developed through a systematic search of domestic and interna-tional guideline networks,professional association websites,and comprehensive databases.Subsequently,the recom-mendations were revised,and the consensus was achieved through a round of expert consensus meetings and 3 rounds of expert correspondence,culminating in the formation of the guidelines.Results The developed guideline encompasses 2 aspects and 5 dimensions of assessment and maintenance,comprising a total of 28 questions and 39 recommendations.Specifically,6 questions and 9 recommendations were formulated for the cognitive dimension,5 questions and 7 recommendations for the locomotion dimension,6 questions and 7 recommendations for the vitality dimension,6 questions and 9 recommendations for the psychological dimension,and 5 questions and 7 recommenda-tions for the sensory dimension.Among these,34 are classified as strong recommendations,while 5 are categorized as weak recommendations.Conclusion The guideline offers scientifically robust,acceptable,and comprehensible rec-ommendations that equip the professionals with a foundation for decision-making aiming at preserving the intrinsic capacity of older adults.
