1.Syndrome Differentiation and Treatment of Rosacea Using the Method of Venting Heat and Resolving Stagnation

This paper summarizes the clinical experience in the syndrome differentiation and treatment of rosacea using the method of venting heat and resolving stagnation. It is considered that the key pathogenesis of rosacea is the accumulation of heat with stagnation. Accordingly, the method of venting heat and resolving stagnation is proposed, which vents and disperses constraint heat by applying approaches such as dredging defensive qi, clearing qi, venting ying (营) level, and cooling blood, while eliminating stagnation and masses through regulating qi, resolving dampness, dispelling phlegm and removing stasis. In clinical practice, a core prescription for venting heat and resolving stagnation is formulated, with flexible modifications made according to the clinical characteristics of different rosacea subtypes, including erythematotelangiectatic, papulopustular, phymatous, and ocular types, thereby providing therapeutic insights for the treatment of rosacea with traditional Chinese medicine.
2.Identification and analysisof drug resistance in Gordonia strains isolated from sputum samples in Henan Province

This study was aimed at exploring the prevalence and drug sensitivity of Gordonia strains isolated from sputum samples in Henan Province,to provide data to aid in the prevention and treatment of Gordonia infection.A combination of 16S rDNA and sec A1 gene sequencing was used to identify the isolated strains,and susceptibility to16 drugs was determined with the broth microdilution method.A total of 21 strains were identified through 16S rDNA gene and sec A1 gene sequencing,including five strains of Gordonia broncians,eight strains of Gordonia paraphernivans,seven strains of Gordonia sputi,and one strain of Gordonia aichiensis.Drug sensi-tivity testing showed high Gordonia sensitivity to drugs such as ceftriaxone,linezolid,doxycycline,amoxicillin/clavulanic acid,mino-cycline,cefotaxime,trimethoprim/sulfamethoxazole,imipenem,tobramycin,and clarithromycin.The sensitivity rates of the isolated strains were 90.48%(19/21),100%(21/21),90.48%(19/21),90.48%(19/21),95.24%(20/21),90.48%(19/21),90.48%(19/21),90.48%(19/21),and 95.24%(20/21),respectively.Gordonia showed high resistance to rifampicin and cefepime,with rates of 28.57%(6/21)and 19.05%(4/21),respectively.Meanwhile,the resistance varied among bacterial strains.The resistance rate of G.sputi to rifampicin reached 71.43%(5/7),whereas that of G.parapffinivoras to cefepime was 37.5%(3/8).The main species of Gordo-nia isolated from sputum samples of patients in Henan Province were G.bronchialis,G.paraffinivoras,G.sputi,and G.aichiensis.Drug sensitivity tests indicated that drugs including amoxicillin/clavulanic acid,ceftriaxone,cefotaxime,tobramycin,clarithromycin,mi-nocycline,trimethoprim/sulfamethoxazole,linezolid,and doxycycline had good antibacterial effects against Gordonia.
3.Genetic evolution analysis of chicken-origin H3N8 subtype avian influen-za virus and study on its pathogenicity to hamsters

AIM:A strain was isolated and identified as the H3N8 subtype of the avian influenza virus from a sick chicken at a farm in Yangjiang,Guangdong Province,named A/chicken/Yangjiang/552/2023(abbreviated as YJ/552).The aim of this research is to determine its genetic evolution,biological properties and pathogenicity in hamsters.This study may provide a theoretical strategy for preventing and treating the H3N8 subtype avian influenza virus-induced epidemic.METHODS:A strain of H3N8 avian influenza virus from chickens was characterised by phylogenetic analy-sis,antigenic diversity,receptor-binding specificity,neuraminidase activity,replication,and transmission in hamsters and a systematic pathological analysis was conducted.RESULTS:This novel avian influenza virus was generated through complex recombination of Eurasian avian H3 genes,North American avian N8 genes and six internal genes of H9N2 sub-type AIV.The cleavage site of the outer protein,HA,was PEKQTR↓GLF,which is characteristic of the low pathogenic avian influenza virus.The HA gene of YJ/552 exhibited the highest nucleotide homology with A/China/ZMD-22-2/2022(H3N8)at 99.09%,while the NA gene showed the highest homology with A/chicken/Dongguan/879/2022(H3N8)at 99.01%.This strain preferentially binds to avian-type receptors and could bind to human-type receptors.This virus could effectively replicate in the trachea and lungs of inoculated and contact hamsters.CONCLUSION:YJ/552 is a recombi-nant H3N8 avian influenza virus replicated in the upper respiratory system and transmitted in hamsters.This study pro-vides data support for the early warning and prevention of H3 subtype avian influenza viruses.
4.Effects of radiofrequency technology combined with electrical stimulation biofeedback training on stress urinary incontinence in female patients

Objective:To investigate the effects of radiofrequency technology combined with electrical stimulation biofeedback training on stress urinary incontinence in female patients.Methods:This is a prospective study that included 360 female patients with stress urinary incontinence who visited the Department of Gynecology and Obstetrics at Lianyungang Maternal and Child Health Hospital from June 2021 to June 2023. The patients were divided into three groups using a random number table method: a radiofrequency treatment group ( n = 120, treated with radiofrequency technology), an electrical stimulation treatment group ( n = 120, treated with electrical stimulation biofeedback training), and a combined treatment group ( n = 120, treated with a combination of radiofrequency technology and electrical stimulation biofeedback training). The clinical efficacy of the three groups was evaluated. Before and after treatment, a 1-hour pad test and urine test were conducted. The Incontinence Questionnaire-Urinary Incontinence Short Form was used to assess the surface electromyography values of the pelvic floor muscles in patients across the three groups. Results:The effective treatment rate in the combined treatment group was 87.50% (105/120), which was significantly higher than the rates in the radiofrequency treatment group (69.17%, 83/120) and the electrical stimulation treatment group (71.67%, 86/120) ( χ2 = 13.05, P < 0.05). After treatment, the 1-hour pad test showed that the urine leakage amounts and the Incontinence Questionnaire-Urinary Incontinence Short Form scores for the combined treatment group were (1.14 ± 0.16) g and (4.15 ± 0.48), respectively. In comparison, the values in the radiofrequency treatment group were (3.04 ± 0.42) g and (8.66 ± 0.89), while in the electrical stimulation treatment group they were (3.01 ± 0.39) g and (8.78 ± 0.91). Differences among the three groups were statistically significant ( F = 1 024.37, 1 354.96, all P < 0.05). After treatment, the surface electromyography values during the rapid contraction, sustained contraction, and endurance contraction phases for the combined treatment group were (31.97 ± 3.24) μV, (27.01 ± 3.02) μV, and (20.05 ± 2.11) μV, respectively. For the radiofrequency treatment group, the values were (27.85 ± 2.72) μV, (21.63 ± 2.39) μV, and (15.14 ± 1.63) μV, while the electrical stimulation treatment group showed values of (27.93 ± 2.75) μV, (22.04 ± 2.41) μV, and (15.39 ± 1.67) μV. Differences among the three groups were also statistically significant ( F = 78.49, 156.43, 278.16, all P < 0.05). Conclusions:Radiofrequency technology combined with electrical stimulation biofeedback training can substantially improve pelvic floor muscle strength and reduce urinary incontinence symptoms in female patients with stress urinary incontinence.
5.Construction of Nomogram prediction model for pulmonary infection in patients after aortic dissection surgery

Objective:To construct Nomogram prediction model for pulmonary infection in patients after aortic dissection surgery, so as to provide reference for early screening of high-risk groups and carrying out preventive nursing measures.Methods:This was a retrospective case-control study. The case data of patients after aortic dissection surgery in the Second Affiliated Hospital of Anhui Medical University from January 2020 to October 2023 were selected by convenient sampling method and divided into pulmonary infection group and non-pulmonary infection group according to whether pulmonary infection occurred within one week after surgery. The risk factors of pulmonary infection after aortic dissection surgery were analyzed by Logistic regression and the Nomogram prediction model was constructed by R4.3.3.The model was evaluated by area under the receiver operating characteristic curve, calibration curve and decision curve analysis.Results:A total of 324 patients with aortic dissection were collected, and the incidence of postoperative pulmonary infection was 26.9%(87/324). There were 87 cases in pulmonary infection group, including 65 males and 22 females, with a median age of 58.0 years. There were 237 cases in non-pulmonary infection group, including 180 males and 57 females, with a median age of 60.0 years. Finally, operation time ( OR=1.015, 95% CI 1.007-1.022), intraoperative blood transfusion ( OR=1.001, 95% CI 1.000-1.022), mechanical ventilation time ( OR=7.624, 95% CI 2.679-21.692), postoperative invasive operation ( OR=6.310, 95% CI 1.545-25.778) and postoperative renal insufficiency ( OR=6.723, 95% CI 1.219-37.063) were independent risk factors for pulmonary infection after aortic dissection surgery. The area under the receiver operating characteristic curve of the model was 0.978, sensitivity of 93.7%, and specificity of 90.8%. The calibration curve showed good consistency, and the decision curve analysis curve showed good net benefit. Conclusions:Operation time, intraoperative blood transfusion, mechanical ventilation time, postoperative invasive operation and postoperative renal insufficiency are high-risk factors of pulmonary infection after aortic dissection surgery and the constructed predictive model has predictive value.
6.The inhibitory mechanism of rhodiosin targeting the urease active centre of Helicobacter pylori and its protective effect on gastric mucosa

Objective To screen natural small-molecule compounds with anti-Helicobacter pylori urease(HPU)activity based on traditional Chinese medicine active ingredients and to systematically investigate their inhibitory mechanisms against HPU,as well as their regulatory effects on cellular inflammation and oxidative stress following Helicobacter pylori(Hp)infection.Methods A multi-dimensional screening strategy was adopted.Firstly,virtual screening was performed on the traditional Chinese medicine monomer compound database based on the crystal structure of HPU,and the candidate molecules were selected in combination with bibliometric analysis.Subsequently,the modified Berthelot method was applied to verify urease inhibitory activity in vitro.Inhibition kinetics were analyzed with Lineweaver-Burk plots.The inhibitory sites were explored through sulfhydryl blocking agents and Ni2+competitive inhibitors,followed by molecular docking simulations with AutoDock Vina(version 1.2.3).A Hp-infected human gastric mucosal epithelial cells(GES-1)model was established.The compound's cytotoxicity was assessed with the CCK-8 assay and lactate dehydrogenase release assay.The mRNA expression levels of interleulain(IL)-6,IL-8,and IL-1β were quantified with quantitative real-time PCR(qRT-PCR).Intracellular reactive oxygen species(ROS)levels were measured with a DCFH-DA fluorescent probe.Results According to the screening results,the natural small-molecule compound rhodiosin(RHO)significantly inhibited HPU activity with a half-maximal inhibitory concentration(IC50)of(82.38±5.45)μmol/L.Enzyme kinetics analysis revealed that RHO acted as an anti-competitive inhibitor,showing an inhibition constant of(146.40±2.19)μmol/L;RHO-sulfhydryl/Ni2+-HPU interaction experiments confirmed that its target was located in the sulfhydryl group in the active center of HPU.Molecular docking simulations suggested that RHO is bound exactly to the Flap domain of the urease active pocket,with a binding energy of-8.678 kcal/mol.No significant cytotoxicity towards GES-1 cells was observed with RHO at 80 μmol/L in cellular experiments.Furthermore,RHO significantly down-regulates the mRNA overexpression of IL-6,IL-8,and IL-1β induced by Hp and reduces the production of ROS by 95%.Conclusion The monomer RHO of traditional Chinese medicine inhibits HPU through anti-competitive binding to the sulfhydryl site.It can effectively alleviate the inflammatory response and oxidative stress injury of GES-1 caused by Hp infection,providing a theoretical foundation for developing novel anti-Hp treatment strategies.
7.Guideline for assessment and maintenance of intrinsic capacity in older adults

Objective This study aims to develop a guideline for assessing and maintaining intrinsic capacity in older adults,offer recommendations to professionals regarding these assessments,and encourage the implementation of evidence-based clinical practices across various settings,including communities,hospitals,nursing homes,and other geriatric care environments.Methods An evidence-based approach guided the collection of questions through a lit-erature review.Preliminary recommendations were developed through a systematic search of domestic and interna-tional guideline networks,professional association websites,and comprehensive databases.Subsequently,the recom-mendations were revised,and the consensus was achieved through a round of expert consensus meetings and 3 rounds of expert correspondence,culminating in the formation of the guidelines.Results The developed guideline encompasses 2 aspects and 5 dimensions of assessment and maintenance,comprising a total of 28 questions and 39 recommendations.Specifically,6 questions and 9 recommendations were formulated for the cognitive dimension,5 questions and 7 recommendations for the locomotion dimension,6 questions and 7 recommendations for the vitality dimension,6 questions and 9 recommendations for the psychological dimension,and 5 questions and 7 recommenda-tions for the sensory dimension.Among these,34 are classified as strong recommendations,while 5 are categorized as weak recommendations.Conclusion The guideline offers scientifically robust,acceptable,and comprehensible rec-ommendations that equip the professionals with a foundation for decision-making aiming at preserving the intrinsic capacity of older adults.
8.Related factors of hyperuricemia in adolescents with bipolar disorder

Objective:To investigate the related factors of hyperuricemia(HUA)in adolescents with bipolar disorder.Methods:Fifty-nine adolescent patients with bipolar disorder and 60 normal controls were select-ed.General demographic and clinical data were collected.Serum uric acid(UA),triglyceride(TG),total cholesterol(TC),high density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C),fasting blood glucose(FPG),fructosamine(FA)and other physiological indices were detected.The Hamilton Depression Scale 17(HAMD-17)and Bech-Rafaelsen Mania Scale(BRMS)were used to evaluate the emotional symptoms of the patients.Results:The detection rate of HUA was higher in the adolescent patients with bipolar disorder than in the normal control group(30.5%vs.18.3%,P<0.05),and was higher in manic episode than in depressive episode(48.2%vs.20.8%,P<0.05).Unconditioned logistic regression analysis showed that high level of TG and high BMI were risk factors for hyperuricemia in adolescents with bipolar disorder(OR=2.51,1.34).Conclusion:This study shows that adolescents with bipolar disorder may have a higher risk of HUA than normal controls,and its oc-currence is associated with high TG level and BMI.
9.Effect of foot position and chair height on lower limb kinematic and kinetic parameters during sit-to-stand tasks in children with cerebral palsy

BACKGROUND:Foot position and seat height are important factors affecting "Sit-to-Stand",but most of the current research on "Sit-to-Stand" focuses on healthy people and Parkinson's disease patients. The kinematic and kinetic characteristics of the lower limbs of children with spastic cerebral palsy during the "Sit-to-Stand" task under different foot positions and seat heights are not known.OBJECTIVE:To investigate the effects of different foot positions and different seat height on lower limb kinematic and kinetic parameters during the "Sit-to-Stand" task in children with cerebral palsy. METHODS:Seven children with spastic cerebral palsy were selected as the research subjects. All subjects received the "Sit-to-Stand" test of six tasks,namely three seat heights (high,medium,and low stools) × two foot positions (front and back foot positions). The kinematic and dynamic data of children with cerebral palsy were collected under different foot positions and seat heights.RESULTS AND CONCLUSION:(1) The time characteristics results showed that the total time required for the children with cerebral palsy to perform the sit-to-stand transfer task was significantly smaller in the high stool condition compared to the low stool condition (P=0.046). (2) The kinetic results showed that at the moment of lifting,the knee flexion moment was significantly larger in the bipedal posterior condition than the bipedal anterior condition (P=0.049). The knee flexion moment was significantly smaller in the high stool condition compared to the medium stool condition (P<0.001). (3) It is concluded that raising the seat height and changing the foot position had an effect on the sit-to-stand transfer in children with spastic cerebral palsy. The children were able to perform the sit-to-stand maneuver with less motor compensation in the high-stool bipedal-rear position condition. Meanwhile,the high chair can be used as an aid to enhance the performance of sit-to-stand transfer in children with spastic cerebral palsy. The high stool bipedal hindfoot condition was the most effective in improving the sit-to-stand transfer in children with spastic cerebral palsy.
10.Regulatory mechanism of electroacupuncture on hypothalamic-pituitary-testicular axis in oligospermic rats

BACKGROUND:Electroacupuncture can treat oligoasthenospermia by stimulating hormone synthesis and release in the hypothalamus,pituitary gland and testis,but its mechanism is complex,and the efficacy of electroacupuncture is closely related to the idea of acupoint compatibility.OBJECTIVE:To explore whether electroacupuncture therapy can improve the function status of hypothalamic-pituitary-testicular axis by adjusting the level of serum related sex hormones in rats,so as to treat adenine-induced oligoasthenospermia in rats.METHODS:Fifty adult male Sprague-Dawley rats were randomly divided into blank,model,electroacupuncture,L-carnitine,and non-meridian and non-acupuncture point groups,with 10 rats in each group.Except for the blank group,the rats in the other four groups were given 200 mg/(kg·d)adenine via intragastric administration for 28 days to establish oligoasthenospermia models.After modeling,the rats in the electroacupuncture group were given electroacupuncture treatment at Zhongji,Guanyuan,Zusanli and Sanyinjiao acupoints,the non-acupoint group was selected for electroacupuncture treatment at three non-acupoint points,and the rats in the L-carnitine group were given oral infusion of L-carnitine,once daily,for 28 continuous days.After treatment,the kidney and testicular organ coefficients were calculated.Sperm count,survival rate and motility rate were determined.Pathological morphological changes of kidney and testicular tissues were observed.Serum sex hormones testosterone,gonadotropin releasing hormone,follicle stimulating hormone,luteinizing hormone,estradiol,statin B and prolactin levels were detected in rats of each group.RESULTS AND CONCLUSION:(1)The renal organ coefficient in the model group was higher than that in the blank,electroacupuncture and L-carnitine groups(P<0.05),the renal organ coefficient in the non-acupoint group was higher than that in the electroacupuncture group(P<0.05),and there was no significant difference in the testicular organ coefficient among all groups(P>0.05).(2)Compared with the blank group,sperm count,survival rate and motility rate were decreased in the model group(P<0.05);compared with the model group,sperm count,survival rate and motility rate were increased in the electroacupuncture group and L-carnitine group(P<0.05);compared with the non-acupoint group,sperm count,survival rate and motility rate were higher in the electroacupuncture group(P<0.05).(3)Hematoxylin-eosin staining results indicated that the kidney and testicular tissues of rats in the model group were severely damaged,while damage to kidney and testicular tissues was less in the electroacupuncture group and L-carnitine group compared with the model group,but severer in the non-acupoint group than the electroacupuncture group.(4)Compared with the blank group,the levels of testosterone,gonadotropin releasing hormone,and inhibin B in the serum of rats in the model group were decreased(P<0.05),and the levels of follicle-stimulating hormone,luteinizing hormone,estradiol,and prolactin were increased(P<0.05).Compared with the model group,the levels of testosterone,gonadotropin-releasing hormone,and inhibin B in the serum of rats in the electroacupuncture group and L-carnitine group were increased(P<0.05),and the levels of follicle-stimulating hormone,luteinizing hormone,estradiol,and prolactin were decreased(P<0.05).Compared with the non-acupoint group,the levels of testosterone,gonadotropin-releasing hormone,and inhibin Bin the serum of the rats in electroacupuncture group were increased(P<0.05),and the levels of follicle-stimulating hormone,luteinizing hormone,estradiol,and prolactin were decreased(P<0.05).To conclude,electroacupuncture at Zhongji,Guanyuan,Zusanli,and Sanyinjiao acupoints can effectively treat oligoasthenospermia by regulating the functional state of the hypothalamic-pituitary-testicular axis in rats.
