Neurocritical care involves complex pathophysiological mechanisms, and its incidence is higher, injuries are more severe, and treatment is more challenging in high-altitude environments. This consensus, based on the latest domestic and international evidence-based medical data, establishes a standardized, goal-oriented framework for neurocritical care management applicable in high-altitude regions and nationwide. The consensus was developed following international standards for evidence quality assessment and underwent two rounds of Delphi expert consultation, resulting in 32 recommendation statements covering three parts: management systems, monitoring and assessment, and core strategies. Key updates include: advocating for the establishment of independent neurocritical care units and implementing precise tiered diagnosis and treatment based on the "Five Differences in Critical Care" concept; constructing a "trinity" multimodal brain monitoring system centered on cerebral blood flow, cerebral oxygenation, and brain function, emphasizing routine bedside transcranial Doppler ultrasound, cerebral oximetry, and continuous electroencephalography monitoring; shifting management strategies from mild hypothermia therapy to targeted temperature management, and defining the "446" target management pathway for the supercritical stage; emphasizing the assessment of static and dynamic cerebrovascular autoregulation functions through multimodal methods to achieve individualized optimal mean arterial pressure management; elevating cerebrospinal fluid management goals to the level of "glymphatic system" function maintenance; implementing a multidisciplinary collaborative, whole-process management model focusing on patients' long-term neurological functional outcomes; de-escalation criteria include multidimensional indicators such as recovery of brain structure, restoration of cerebrovascular autoregulation, improvement in cerebrospinal fluid dynamics, and reduction in biomarker levels; and integrating cutting-edge technologies like artificial intelligence into post-critical care management and rehabilitation planning. This consensus systematically integrates the entire process of neurocritical care management, reflecting the modern connotation of goal-oriented, dynamic, and multimodal integration in neurocritical care medicine. It aims to adapt to new trends such as deepening understanding of pathophysiological mechanisms, the integration of medicine and engineering, and the empowerment of artificial intelligence, thereby further advancing the discipline of critical care medicine.
2.Acellular dermal matrix hydrogel promotes skin wound healing in rats

BACKGROUND:Promoting skin wound healing is a huge challenge facing global public health.To promote faster and higher-quality wound healing,it is necessary to explore more advantageous dressings to address this problem.OBJECTIVE:To investigate the hemostatic properties of acellular dermal matrix hydrogel and its effect on skin wound healing.METHODS:(1)Acellular dermal matrix hydrogel was prepared,and the differences in microscopic morphology and main components between it and acellular dermal matrix were analyzed.(2)Acellular dermal matrix hydrogel and chitosan hydrogel were used to cover the femoral artery puncture site of rats,and the bleeding quality and coagulation time were recorded.Acellular dermal matrix hydrogel and chitosan hydrogel were mixed with rat anticoagulated blood,and the coagulation index within 30 minutes was detected.(3)A full-thickness skin defect model with a diameter of 12 mm was made on the back of 18 SD rats,and they were randomly divided into 3 groups,with 6 rats in each group:the model group used PBS to clean the wound,and the control group and the experimental group used chitosan hydrogel and acellular dermal matrix hydrogel to cover the wound,respectively.The hydrogel dressing was changed every day,and the treatment was continued for 14 days,and the wound healing was observed.On day 3 after modeling,immunofluorescence staining of inducible nitric oxide synthase(M1 macrophages)and CD206(M2 macrophages)was performed on the wound surface.On day 14 after modeling,hematoxylin-eosin staining,Masson staining,and CD31 immunohistochemical staining were performed on the wound surface.RESULTS AND CONCLUSION:(1)Scanning electron microscopy revealed that the acellular dermal matrix hydrogel had a porous structure,and the Fourier transform infrared spectrum showed that it had the same main components as the acellular dermal matrix.(2)Both acellular dermal matrix hydrogel and chitosan hydrogel had obvious hemostatic ability in vivo.In the in vitro coagulation experiments,the coagulation index of acellular dermal matrix hydrogel was significantly higher than that of chitosan hydrogel.(3)In the rat skin full-thickness defect model,both acellular dermal matrix hydrogel and chitosan hydrogel could improve the wound healing rate.Hematoxylin-eosin and Masson staining results showed that acellular dermal matrix hydrogel could reduce the infiltration of inflammatory cells in the center of the wound.Both acellular dermal matrix hydrogel and chitosan hydrogel could decrease scar width and increase collagen deposition rate.CD31 immunohistochemical staining results showed that both hydrogels could promote angiogenesis in the wound site.Immunofluorescence staining results showed that both hydrogels could reduce the proportion of M1 macrophages and increase the proportion of M2 macrophages,and the effect of acellular dermal matrix hydrogel was stronger than that of chitosan hydrogel.(4)The results show that the acellular dermal matrix hydrogel has good hemostatic properties and the ability to promote wound healing.

4.Relationship between non-high-density lipoprotein cholesterol and cardiovascular disease in maintenance hemodialysis patients

Objective To investigate the relationship between non-high-density lipoprotein cho-lesterol(non-HDL-C)and cardiovascular disease(CVD)in maintenance hemodialysis(MHD)pa-tients.Methods A total of 124 MHD patients were enrolled and divided into CVD group(53 pa-tients)and non-CVD group(71 patients)based on the presence or absence of CVD.Clinical data be-tween the two groups were compared.Additionally,patients were divided into severe calcification group[coronary artery calcification score(C ACS)≥ 400,40 patients]and non-severe calcification group(CACS<400,84 patients)based on CACS,and clinical data between these two groups were also compared.Multivariate Logistic regression analysis was used to explore the independent risk fac-tors for CVD in MHD patients,and the predictive performance of related indicators for CVD in MHD patients was assessed using receiver operating characteristic(ROC)curves.Results The levels of serum total cholesterol,low-density lipoprotein cholesterol(LDL-C),and non-HDL-C were higher,while the level of high-density lipoprotein cholesterol(HDL-C)was lower in the CVD group compared with the non-CVD group(P<0.05).The levels of serum total cholesterol,LDL-C,and non-HDL-C were higher,and the level of HDL-C was lower in the severe calcification group compared with the non-severe calcification group(P<0.05).Multivariate Logistic regression analysis showed that high levels of LDL-C and non-HDL-C were both independent risk factors for CVD in MHD patients(P<0.05).ROC curve analysis showed that the areas under the curve for predicting CVD in MHD patients were 0.858 and 0.723 for non-HDL-C and LDL-C,respectively,and non-HDL-C had high-er specificity and Youden index than LDL-C.Conclusion Elevated non-HDL-C level is an inde-pendent risk factor and has high predictive performance for CVD in MHD patients.
5.Characteristics and determinants of total cerebral small vascular disease scores in pilots

Objective:To investigate the characteristics and determinants of total scores of cerebral small vessel disease (CSVD) and to analyze the factors associated with enlarged perivascular space (EPVS) grading in pilots.Methods:The physical examination data of 72 pilots who were hospitalized and diagnosed with CSVD by MRI in the Air Force Medical Center (General Hospital of Air Force) between 2019 and 2022 was retrospectively analyzed. The pilots were grouped by the total CSVD score (0, 1, 2, 3, 4 points), and the distribution of CSVD imaging biomarkers was compared across groups. The severity of EPVS was classified into 3 levels: none or mild (0-10), moderate (11-20), and severe (>20). The impact of vascular risk factors on the total CSVD score and EPVS grading was analyzed.Results:The results of the total CSVD score showed that there were 19 cases (26.39%) with a score of 0, 43 cases (59.72%) with a score of 1, 10 cases (13.89%) with a score of 2, and 0 case with scores of 3 or 4. Among those who scored 1, there were 2 cases (4.65%) of lacunar infarction (LA), 1 case (2.33%) of moderate to severe white matter hyperintensity (WMH), 2 cases (4.65%) of cerebral microbleed (CMB), and 38 cases (88.37%) of moderate and severe EPVS. Among those who scored 2, there were 7 cases (70.00%) of LA combined with EPVS, 2 cases (20.00%) of CMB combined with EPVS, and 1 case (10.00%) of WMH combined with EPVS. According to the CSVD imaging classification of these pilots, there were 9 cases (12.50%) of LA, 52 cases (72.22%) of WMH, 4 cases (5.60%) of CMB and 61 cases (84.72%) of EPVS. Multiple ordered Logistic regression analysis showed that systolic blood pressure ( OR=1.068, 95% CI: 1.016-1.122) and high-density lipoprotein cholesterol ( OR=0.111, 95% CI: 0.015-0.843) made a difference in the total CSVD score. High-density lipoprotein cholesterol ( OR=0.166, 95% CI: 0.031-0.893) could affect the EPVS grading. Spearman′s correlation analysis showed that the systolic blood pressure level was positively correlated with the total CSVD score ( r=0.299, P=0.011), while the high-density lipoprotein cholesterol level was negatively correlated with the total CSVD score and EPVS grading ( r=-0.313, -0.263, P=0.041, 0.026). Conclusions:The total CSVD score of pilots is at a mild level with EPVS as the leading contributor. The systolic blood pressure and the high-density lipoprotein cholesterol level are determinants for the total CSVD score, while the high-density lipoprotein cholesterol level is a determinant for the EPVS grading of pilots. Blood pressure control and lipid regulation can go a long way towards preventing CSVD in pilots. The total CSVD score is of value for stratified evaluation and individual identification of pilots with CSVD.
6.Research on the rapid diagnosis of three common Gram-negative bacilli in bloodstream infections based on the CNN-Dinov2 hybrid model

Objective:To develop CNN-Dinov2, a deep learning-based automatic classification model for Gram-stained images, enabling rapid diagnosis of three prevalent Gram-negative bacilli in bloodstream infections: Escherichia coli ( E.coli), Klebsiella pneumoniae ( K.pneumoniae), and Pseudomonas aeruginosa ( P.aeruginosa). Methods:This evaluation study analyzed 1 425 Gram-stained microscopic images from patients with bloodstream infections at Houjie Hospital, in Dongguan City, collected between January 2023 and January 2024. The images, all positive for blood culture and identified as target strains, were categorized into Escherichia coli (419 images), Klebsiella pneumoniae (411 images), Pseudomonas aeruginosa (413 images), and other Gram-negative bacilli (182 images). They were randomly split into a training set (1 141 images), a validation set (141 images), and a test set (143 images) in an 8∶1∶1 ratio. A hybrid CNN-Dinov2 model was developed by integrating ResNet′s local feature extraction with Dinov2′s global pre-trained features, followed by a fully connected layer. The model was optimized by inputting the preprocessed images and adjusting parameters through loss calculation and backpropagation. AlexNet, Dinov2, and ResNet18 served as control models. The models′ classification performance was assessed using accuracy, precision, weighted F1 score, and recall rate, derived from the confusion matrix. The PR curve and AP value further evaluated each model′s classification capability across the four image categories. Results:The CNN-Dinov2 model achieved a training accuracy of 99.74%, a validation accuracy of 98.12%, and a validation loss of 0.070 6, demonstrating robust generalization without overfitting. Validation metrics revealed superior performance with an accuracy of 98.60%, precision of 98.65%, a weighted F1 score of 98.60%, and a recall rate of 98.60%, outperforming other models. The confusion matrix confirmed its strong classification capability, with the highest sum of diagonal values for identifying four types of bacteria. The macro average precision (AP) values under the precision-recall (PR) curves were all 1, indicating excellent discrimination across all categories. Overall, the CNN-Dinov2 model exhibited the best performance among the four models evaluated.Conclusion:This study successfully developed CNN-Dinov2, an automated classification model for Gram staining images. It offers valuable support for the rapid diagnosis of bloodstream infections caused by Escherichia coli, Klebsiella pneumoniae, and Pseudomonas aeruginosa, demonstrating practical utility.

Turning to critical illness is a common stage of various diseases and injuries before death. Patients usually have complex health conditions, while the treatment process involves a wide range of content, along with high requirements for doctor′s professionalism and multi-specialty teamwork, as well as a great demand for time-sensitive treatments. However, this is not matched with critical care professionals and the current state of medical care in China. Telemedicine, which shortens the distance of medical professionals and the gap of disease diagnosis and treatments in various regions through electronic information, can effectively solve the current problem. Therefore, there is an urgent need to develop a standardized, high-quality visualization telemedicine round system .Therefore, experts have been organized to search domestic and foreign literature on telemedicine round for critically ill patients and to form this consensus based on clinical experiences so as to further improve the level of critical care treatments in regions.

The preferred treatment for adrenal functional tumors is laparoscopic surgical resection. However, in recent years, various ablation techniques have gradually been applied in the treatment of adrenal tumors. This report presents the successful diagnosis and treatment experience of 4 aldosterone-producing adenomas and 1 cortisol-secreting adenoma treated with a combined cold and hot ablation system at Xi’an Daxing Hospital from Dec. 2023 to Dec. 2024, providing a new treatment approach for clinical exploration.
9.Research progress of live-attenuated Salmonella-based carrier vaccine

Salmonella has demonstrated considerable potential as a vaccine vector,exhibiting robust immunogenicity,ease of oral administration,and cost-effective production.Live attenuated Salmo-nella carrying heterologous antigens can induce both localized mucosal immunity and systemic a-daptive immune responses in hosts after successfully reaching the intestinal tract via oral delivery.Recent advances such as permanent deletion of virulence genes,regulated-delayed attenuation and lysis systems,have initially achieved a balance between the safety and immunogenicity of these vaccine platforms.Nevertheless,practical applications of such vaccine vectors remain constrained by challenges related to gastrointestinal barrier obstruction,inefficient antigen delivery,and im-mune tolerance.With the rapid advancement of multidisciplinary technologies,these limitations are anticipated to be progressively addressed.This review presented a comprehensive summary and dis-cussion of the immune mechanisms,development strategies,current applications,advantages,and challenges associated with oral live attenuated Salmonella vaccine vectors.It also delineated the fu-ture direction of research and development,with a view to providing theoretical references for re-lated research.
10.The correlation between sarcopenia and anastomotic leakage after rectal cancer surgery

Objective To quantitatively assess the correlation between the skeletal muscle index(SMI)of patients and the occur-rence of anastomotic leakage(AL)in rectal cancer patients after surgery,and to analyze the risk factors for AL in rectal cancer patients and the influencing factors of sarcopenia.Methods The clinical,pathological,and related imaging data of 362 patients who under-went radical surgery for rectal cancer were retrospectively analyzed.All patients underwent pelvic MRI and abdominal CT scans(plain/enhanced)within one month before surgery,and the third lumbar vertebra skeletal muscle area(L3-SMA)was measured from the images.All patients were divided into AL group(56 cases)and control group(306 cases)based on the presence or absence of postoperative complications.The differences in clinical characteristics and imaging parameters between the two groups were analyzed.A logistic risk prediction model was established.Results Significant differences were observed between the two groups in sarcopenia,type of surgery,surgical approach,serum albumin level,operation duration,stoma type,and extramural vascular invasion(EMVI)(P<0.05).These factors were incorporated in a multivariate logistic regression analysis model,the area under the curve(AUC)of receiver operating characteristic(ROC)curve of the model was 0.810[95%confidence interval(CI)0.743-0.876,P<0.001],with a sensitivity of 0.865 and specificity of 0.669.Conclusion Sar-copenia is a significant risk factor for AL after rectal cancer surgery.It enhances the predictive efficacy for postoperative AL and serves as a basis for identifying high-risk populations for AL in clinical practice.
