ObjectiveTo assess the risk of vector-born infectious diseases by monitoring the major vectors in the relevant venues of the 19th Hangzhou Asian Games in Fuyang competition area, so as to provide references for the prevention and control of vector in similar large-scale events or activities in the future. MethodsFrom April to October 2023, monitoring and evaluation of rodent, mosquito, fly and cockroach density levels were carried out in various venues and reception hotels in Fuyang competition area (venues and hotels were the 1st circle area, and various environments and places in the surrounding 500 meters were the 2nd circle area). SPSS 25.0 software was used to analyze the data, and chi-square test or Fisher’s precision probability test were used to calculate the qualified rate and positive rate. Moreover, the path index, landing index and Bretrau index were analyzed by Kruskal-Wallis H test. ResultsThe control effect of vector density in the 1st circle was significantly better than that in the 2nd circle. Since September, the monitoring indices of the venues in the two circles had reached the national A-level and B-level standard, respectively. The positive rate of rodent traces generally showed a trend of first increasing and then decreasing, and the peak was shown to occur in July and August, with the highest rate of 5.38%. The qualified rate of rodent prevention facilities continued to rise, and facilities in the 1st and 2nd circles had reached national A-level and B-level standard respectively in late August. The landing index and path index of mosquitoes basically showed a changing trend from high to low, with the highest landing index of 4.56 individuals·person-time-1 and the highest path index of 2.44 places·km-1.The adult fly infestation rate remained at a low level, with the highest rate of 4.17%, and the minimum qualified rate of fly prevention facilities was 28.57%, which reached the national standard after July. The positive detection rates of adult flies and cockroach traces showed no significant pattern, with the highest rate of 7.00% and 8.33%, respectively. The detection rate of live ootheca was always at a low level. ConclusionThe relevant venues in the Fuyang competition area of the 19th Asian Games in Hangzhou meet national standards in terms of vector control indicators such as rodents, mosquitoes, flies, cockroaches and other vectors. By means of environmental improvement, facilities for preventing rodents and flies, and deepening biological and chemical prevention and control methods, it can be ensured that there will be no vector-borne diseases and infestation incidents during the event.

Objective:To investigate the characteristics and determinants of total scores of cerebral small vessel disease (CSVD) and to analyze the factors associated with enlarged perivascular space (EPVS) grading in pilots. Methods:The physical examination data of 72 pilots who were hospitalized and diagnosed with CSVD by MRI in the Air Force Medical Center (General Hospital of Air Force) between 2019 and 2022 was retrospectively analyzed. The pilots were grouped by the total CSVD score (0, 1, 2, 3, 4 points), and the distribution of CSVD imaging biomarkers was compared across groups. The severity of EPVS was classified into 3 levels: none or mild (0-10), moderate (11-20), and severe (>20). The impact of vascular risk factors on the total CSVD score and EPVS grading was analyzed. Results:The results of the total CSVD score showed that there were 19 cases (26.39%) with a score of 0, 43 cases (59.72%) with a score of 1, 10 cases (13.89%) with a score of 2, and 0 case with scores of 3 or 4. Among those who scored 1, there were 2 cases (4.65%) of lacunar infarction (LA), 1 case (2.33%) of moderate to severe white matter hyperintensity (WMH), 2 cases (4.65%) of cerebral microbleed (CMB), and 38 cases (88.37%) of moderate and severe EPVS. Among those who scored 2, there were 7 cases (70.00%) of LA combined with EPVS, 2 cases (20.00%) of CMB combined with EPVS, and 1 case (10.00%) of WMH combined with EPVS. According to the CSVD imaging classification of these pilots, there were 9 cases (12.50%) of LA, 52 cases (72.22%) of WMH, 4 cases (5.60%) of CMB and 61 cases (84.72%) of EPVS. Multiple ordered Logistic regression analysis showed that systolic blood pressure ( OR=1.068, 95% CI: 1.016-1.122) and high-density lipoprotein cholesterol ( OR=0.111, 95% CI: 0.015-0.843) made a difference in the total CSVD score. High-density lipoprotein cholesterol ( OR=0.166, 95% CI: 0.031-0.893) could affect the EPVS grading. Spearman′s correlation analysis showed that the systolic blood pressure level was positively correlated with the total CSVD score ( r=0.299, P=0.011), while the high-density lipoprotein cholesterol level was negatively correlated with the total CSVD score and EPVS grading ( r=-0.313, -0.263, P=0.041, 0.026). Conclusions:The total CSVD score of pilots is at a mild level with EPVS as the leading contributor. The systolic blood pressure and the high-density lipoprotein cholesterol level are determinants for the total CSVD score, while the high-density lipoprotein cholesterol level is a determinant for the EPVS grading of pilots. Blood pressure control and lipid regulation can go a long way towards preventing CSVD in pilots. The total CSVD score is of value for stratified evaluation and individual identification of pilots with CSVD.

4.Research progress on the efficacy and safety of dupilumab in the treatment of allergic diseases
Adverse Drug Reactions Journal 2022;24(10):540-545
Allergic diseases have serious influence on human health and quality of life. Cytokines secreted by T-helper cell type 2 (Th2), such as interleukin (IL)-4 and IL-13, play a pivotal role in the pathogenesis of allergic diseases. By recognizing and binding to IL-4 and IL-13 receptors, dupilumab can block the signal transduction pathways of IL-4 and IL-13, inhibit Th2 inflammatory reaction, and thus play a therapeutic role in allergic diseases. At present, dupilumab has been on the market in many countries and regions around the world, yet with different indications approved, mainly including atopic dermatitis, moderate-to-severe eosinophilic asthma, chronic rhinosinusitis with nasal polyps, eosinophilic esophagitis, etc. In addition, this drug may also have better curative effect on IgE-mediated allergic reactions and other dermal diseases. Meanwhile, the safety profile of dupilumab is generally good and the common adverse reactions mainly include injection site reactions (pain, ecchymosis and swelling at the injection site), ocular diseases (conjunctivitis, keratitis, itchiness of eyes and dry eyes), oral herpes, headache, and other herpes simplex virus infections. In China, dupilumab has been approved for the treatment of moderate-to-severe atopic dermatitis in patients aged ≥6 years, showing good efficacy and no obvious safety problems.

Objective:To intestigate the clinical efficacy between modified Overlap anastomosis and traditional auxiliary incision anastomosis in laparoscopic total gastrectomy. Methods:The retrospective cohort study was conducted. The clinicopathological data of 115 patients with gastric cancer who were admitted to the Second Affiliated Hospital of Fujian Medical University from January 2016 to December 2018 were collected. There were 62 males and 53 females, aged from 27 to 83 years, with a median age of 62 years. Of 115 patients, 51 patients undergoing totally laparoscopic total gastrectomy with modified Overlap anastomosis using linear stapler were divided into modified Overlap group and 64 patients undergoing laparoscopic assisted total gastrectomy with traditional auxiliary incision anastomosis using circular stapler were divided into traditional assisted group. Observation indicators: (1) surgical situations; (2) postoperative situations; (3) anastomotic complications; (4) follow-up. Follow-up using outpatient examination or telephone interview was conducted to detected tumor recurrence and survival of patients up to December 2019. Measurement data with normal distribution were represented as Mean± SD, and comparison between groups was analyzed using the t test. Count data were represented as absolute numbers or percentages, and comparison between groups was analyzed using the chi-square test or Fisher exact probability. Comparison of ranked data was analyzed using the rank sum test. Results:(1) Surgical situations: the operation time, time of esophagojejunostomy, volume of intraoperative blood loss, the number of lymph node dissected, length of proximal incisional margin and length of auxiliary incision of the modified Overlap group were (234.0±11.0)minutes, (29.4±2.1)minutes, (53±14)mL, 42±13, (2.0±0.3)cm and (5.1±0.4)cm, respectively. The above indicators of the traditional assisted group were (231.0±11.0)minutes, (29.2±2.2)minutes, (50±13)mL, 40±10, (2.2±0.4)cm and (8.2±0.4)cm, respectively. There was significant difference in the length of auxiliary incision between the two groups ( t=-43.098, P<0.05), and there was no significant difference in the operation time, time of esophagojejunostomy, volume of intraoperative blood loss, the number of lymph node dissected, length of proximal incisional margin between the two groups ( t=1.168, 0.460, 0.990, 1.127, -1.926, P>0.05). (2) Postoperative situations: cases with mild, moderate, severe pain (postoperative pain degree), time to first flatus, time to initial fluid diet intake, duration of postoperative hospital stay of the modified Overlap group were 40, 9, 2, (2.9±1.0)days, (4.8±2.2)days, (11.7±2.8)days, respectively. The above indicators of the traditional assisted group were 31, 27, 6, (3.9±1.4)days, (6.5±2.5)days, (13.0±3.1)days, respectively. There were significant differences in the above indicators between the two groups ( Z=-3.217, t= -4.344, -3.888, -2.261, P<0.05). (3) Anastomotic complications: cases with anastomotic leakage, cases with anastomotic bleeding, cases with anastomotic stenosis of the modified Overlap group were 1, 1, 0, respectively. The above indicators of the traditional assisted group were all 1. There was no significant difference in the above indicators between the two groups ( P>0.05). Cases with anastomotic leakage were cured after the treatment of enteral nutritional support through nasogastric catheterization, which were confirmed by gastroenterography. Cases with anastomotic bleeding were improved by active hemostatic therapy. Cases with anastomotic stenosis were improved after the symptomatic treatment of anti-inflammatory and anti-swelling. (4) Follow-up: 109 of the 115 patients were followed up. Forty-eight of 51 patients in the modified Overlap group were followed up for 15.0-45.0 months, with a median follow-up time of 33.5 months. Sixty-one of 64 patients in the traditional assisted group were followed up for 16.0-46.0 months, with a median follow-up time of 27.0 months. There was no tumor recurrence in the modified Overlap group. One patient in the traditional assisted group had tumor recurrence with liver metastasis and survived with tumor. There was no significant difference in tumor recurrence rate between the two groups ( P>0.05). There was no patient died during the follow-up. Conclusion:Compared with traditional auxiliary incision anastomosis, patients undergoing total laparoscopic total gastrectomy with modified Overlap anastomosis have small incision, good postoperative recovery.

Objective:To retrospect the clinical characters and medical evaluations of the aircrew with intracranial aneurysms (IAs) based on the hospitalized cases in Air Force Medical Center and published literatures. Methods:The cases and clinical reports on the subject of aircrew IAs were searched through the electrical case system in our center and medical literature database. We summarized the the clinical characters and medical evaluations of these aircrews. Results:Six cases with 7 IAs were found in this study, including 5 males and 1 female aged from 21 to 48 yrs. There were 3 pilots, 1 correspondent, 1 machinist and 1 attendant. Three cases were asymptomatic and 3 cases were symptomatic. Six IAs located in the cerebral anterior circulation system and one located in the cerebral posterior circulation system. All the IAs were the small aneurysms with diameter≤7 mm. Two asymptomatic cases were decided waiver by the aeromedical evaluation. Three symptomatic cases were treated by surgery clipping or endovascular coiling. One of them was given the waiver consideration. Conclusions:There are certain population with IAs in aircrew. The screening of asymptomatic IAs should be cared because of their potential risk in flight. Symptomatic IAs and the IAs with high risk to rupture are needed to be treated by surgery or endovascular coiling. The flight risk should be comprehensively assessed in conjunction with the clinical symptoms, aneurysms characteristics (morphology and hemodynamics), flight factors, flight duty therapy outcome and prognosis.

Critical ultrasonography(CUS) is different from the traditional diagnostic ultrasound, the examiner and interpreter of the image are critical care medicine physicians. The core content of CUS is to evaluate the pathophysiological changes of organs and systems and etiology changes. With the idea of critical care medicine as the soul,it can integrate the above information and clinical information, bedside real-time diagnosis and titration treatment, and evaluate the therapeutic effect so as to improve the outcome.CUS is a traditional technique which is applied as a new application method. The consensus of experts on critical ultrasonography in China released in 2016 put forward consensus suggestions on the concept, implementation and application of CUS.It should be further emphasized that the accurate and objective assessment and implementation of CUS requires the standardization of ultrasound image acquisition and the need to establish a CUS procedure. At the same time, the standardized training for CUS accepted by critical care medicine physicians requires the application of technical specifications, and the establishment of technical specifications is the basis for the quality control and continuous improvement of CUS.Chinese Critical Ultrasound Study Group and Critical Hemodynamic Therapy Collabration Group, based on the rich experience of clinical practice in critical care and research, combined with the essence of CUS,to learn the traditional ultrasonic essence, established the clinical application technical specifications of CUS,including in five parts:basic view and relevant indicators to obtain in CUS;basic norms for viscera organ assessment and special assessment;standardized processes and systematic inspection programs;examples of CUS applications;CUS training and the application of qualification certification. The establishment of applied technology standard is helpful for standardized training and clinical correct implementation. It is helpful for clinical evaluation and correct guidance treatment, and is also helpful for quality control and continuous improvement of CUS application.
