1.Clinical efficacy of intensive conservative treatment for acute aortic syndrome

Objective:To evaluate the outcomes of intensive conservative treatment compared to conventional conservative treatment in patients with acute aortic syndrome(AAS).Methods:The study prospectively enrolled consecutive patients with AAS who were admitted to Beijing Anzhen Hospital, affiliated with Capital Medical University, and Beijing Dawanglu Emergency Rescue Hospital from January 2024 to December 2024. These patients with surgical contraindications or refused surgery for various reasons opted for conservative treatment. A total of 282 patients were included, and 15 patients with missing data or those who died without any treatment were excluded. Finally, 267 patients were enrolled, of whom 94 received intensive conservative treatment, and 173 received conventional conservative treatment, the inverse probability of treatment weighting (IPTW) was used to reduce the influence of confoundings. After adjusting of baseline datas via IPTW, the survival outcomes of the two groups were compared at 14 days, 30 days, and at the end of follow-up.Results:The results showed significant differences in acute phase survival rates between the enhanced conservative treatment group and the conventional conservative treatment group at 14 days(82.40%vs.53.20%, P<0.0001). Significant survival differences were also observed at 30 days and at 276-day mid-term follow-up (96.29% vs.51.60%, P<0.0001; 78.50% vs.48.50%, P<0.0001). In the subgroup analysis, for type A aortic dissection, the enhanced conservative treatment group had higher survival rates compared to the conventional conservative treatment group at 14, 30 and 276 days (63.46% vs.41.35%, P<0.05; 52.17% vs.37.90%, P<0.05; 50.00% vs. 31.97%, P<0.05). However, for type B aortic dissection, although the enhanced conservative treatment group had higher survival rates than the conventional conservative treatment group, no statistically significant differences were observed (96.29% vs. 80.00%, P=0.054; 95.65% vs.78.37%, P=0.067; 94.12% vs.74.20%, P=0.088). Conclusion:For patients diagnosed with AAS are forced to choose conservative treatment if emergency surgery is not possible in the first place, intensive conservative treatment strategies can significantly reduce the mortality in the acute phase compared with conventional conservative treatment. Mid-term follow-up, intensive conservative treatment still has a significant survival advantage.
2.Establishment of Fingerprints of Danggui Buxue Granules with Different Drying Processes and Study of the Spectrum-Effect Relationship of Antioxidant Activity

OBJECTIVE To establish HPLC fingerprints of three drying processes(Atmospheric pressure drying,Decompression drying,Freeze-drying)of Danggui Buxue granules,and combine them with antioxidant tests to determine the optimal drying process and main active components of Danggui Buxue granules.METHODS The fingerprints of multiple batches of Danggui Buxue granules were established by HPLC;the"Fingerprint Similarity Evaluation System of Traditional Chinese Medicine Chromatogram"was used to evaluate the similarity;Hierarchical Cluster Analysis(HCA)and Principal Component Analysis(PCA)were used to characterize the different drying processes of Danggui Buxue granules;evaluate the antioxidant activity of Danggui Buxue granules in different drying processes using 1,1-diphenyl-2-picrylhydrazy(DPPH)and 2,2'-azino-bis(3-ethylbenzothiazoline-6-sulfonic acid)diammonium salt(ABTS)free radical scavenging method;the spectrum-effect relationship between Danggui Buxue granules fingerprint and antioxi-dant activity was analyzed by grey correlation degree and Pearson,Spearman,Kendall's tau-b correlation analysis methods.RE-SULTS The results of fingerprint showed that there were 16 common peaks in 30 batches of Danggui Buxue granules,and 6 of them were identified by comparison.The results of Cluster Analysis and Principal Component Analysis showed that 30 batches of Danggui buxue granules were divided into 3 categories,and the difference between the groups of atmospheric pressure drying was the least.The results of oxidation test showed that different drying processes of Danggui Buxue granules had good antioxidant activity,and the atmos-pheric pressure drying had the lowest IC50 and the strongest antioxidant activity.Finally,combining the results of gray correlation anal-ysis and correlation analysis,the compounds F2,F10,F13(calycosin),F15(formononetin),F16 might be important characteristic peaks reflecting the antioxidant activity of Danggui Buxue granules.CONCLUSION Compared with other drying processes,atmos-pheric pressure drying has higher batch consistency and stronger antioxidant activity,and can be used as the preferred drying process for Danggui Buxue granules,and components 2,10,13,15 and 16 are the main active ingredients for Danggui Buxue granules to exert antioxidant effects.

5.Effects of robot-assisted intelligent graded pain management in acute pain management of patients undergoing hip and knee joint replacement surgery

Objective:To investigate the effect of robot-assisted intelligent graded pain management in managing acute postoperative pain for patients undergoing hip and knee joint replacement.Methods:Totally patients who underwent hip and knee joint replacement surgery between February and December 2022 at the First Affiliated Hospital of Zhengzhou University were selected by convenience sampling. Patients were randomly assigned into two groups: a control group ( n=78), receiving standard pain management, and an observation group ( n=78), receiving robot-assisted intelligent graded pain management. Postoperative data collected and compared between groups included use of patient-controlled analgesia (PCA), analgesic consumption, time to first ambulation, length of hospital stay, and pain management quality. Results:The observation group showed statistically significant reductions in PCA duration, fentanyl dosage, and lappaconitine hydrobromide dosage compared to the control group ( P<0.05). In addition, the time to first ambulation was earlier, and the postoperative hospital stay was shorter in the observation group ( P<0.05). In terms of pain management quality, the observation group reported lower pain scores and higher satisfaction scores than the control group ( P<0.05) . Conclusions:Robot-assisted intelligent graded pain management effectively reduces the time of PCA and the use of analgesics for patients undergoing hip and knee joint replacement surgery. It facilitates earlier ambulation, shortens hospital stays, and improves the quality of acute postoperative pain control.

7.Epidemic characteristics of pulmonary tuberculosis in Zibo in 2012 - 2020

Objective To analyze the epidemic characteristics of tuberculosis in Zibo City in recent years, and to provide a scientific basis for the formulation of tuberculosis prevention and control strategies and measures during the 14th Five-Year Plan period. Methods Descriptive epidemiological analysis was used to analyze the information of tuberculosis patients registered in “China Disease Control and Prevention Information System-Tuberculosis Information System” from 2012 to 2020 in Zibo City. Results From 2012 to 2020, a total of 12,706 pulmonary tuberculosis patients were registered in Zibo City, and the registered incidence rate decreased year by year (F=53.348, P<0.001), with an average annual rate of 30.45/10 million. There was no significant change in mortality. In regional distribution, the incidence was higher in northern region of Zibo than that in the southern region, with a cross distribution in the central region. In terms of time distribution, there were cases in each month over the years, and the overall trend increased first, then decreased and once again increased (F=8.856, P=0.006<0.01), with two low points occurring between January to February and also in October, and two peaks occurring between May to June and also in December. In terms of population distribution, most patients were local cases (82.38%). The incidence rate of males was about 2.09 times that of females. The incidence rate of children aged 0-14 was the lowest, while elderly people aged 65 years old and over had the highest incidence (61.02/10 million). Farmers were the main occupation among tuberculosis cases, accounting for 60.59% of the total. The main source of patients was from tracking, accounting for more than 50% since 2016. Conclusion The epidemic of tuberculosis in Zibo is stable and the incidence rate is decreasing year by year, showing seasonal variations. The majority of the patients are male. Special attention should be paid to farmers and the elderly, and further research on risk factors in high-risk areas and counties should be strengthened.
8.Epidemiology, prevention and control strategy of Group A Streptococcus infection

As a pathogen of public health significance, group A Streptococcus (GAS) infects 18.1 million people and leads to 500 000 deaths all over the world every year.GAS is often transmitted through respiratory droplets and contact with damaged skin.Children, the elderly and immunocompromised people are highly susceptible to GAS infection.Schools, kindergartens, hospitals, nursing homes and other densely populated areas are high-risk areas for GAS transmission.Prevention and control measures should highlight the improvement of living conditions and hand hygiene.Adherence to infection prevention and control measures should be emphasized in high-risk environments.In this paper, the risk factors, prevention and control strategies of GAS infection and transmission were summarized, so as to provide basis for the prevention and control of GAS.
9.Analysis of emm typing and genomic polymorphism of Group A Streptococcus from children with scarlet fever in Shenzhen 2016-2018

Objective:To identify the epidemic types of Group A Streptococcus (GAS)causing scarlet fever, to compare the gene structure and variability of GAS with different emm types, and to elucidate the epidemiological pattern of scarlet fever pathogens in Shenzhen. Methods:Pharyngeal swab samples were collected and analyzed retrospectively from children diagnosed with scarlet fever in Shenzhen Children′s Hospital from January 1, 2016 to May 31, 2018.The GAS strains were preserved for emm genotyping analysis.The strains of representative emm types were selected for whole-genome sequencing.The genomic polymorphism of GAS strains was described by comparative genomic analysis.Moreover, a phylogenetic tree was constructed based on the whole genome core-gene single nucleotide polymorphisms(SNPs) to clarify the evolutionary relationship between strains.Data between groups were compared by Rank sum test. Results:Among 176 GAS isolates that caused scarlet fever in children, 8 emm types were detected.The most common genotype was emm12.0 and its subtype(108/176 strains, 61.4%), followed by emm1.0 and its subtype(53/176 strains, 30.1%). These two genotypes accounted for 91.5% of all isolates collected.Comparative genome analysis was made taking GCA-900984775 as a reference sequence, and the results showed that the genomes of GAS strains had high levels of SNP and insertion or deletion (InDel) polymorphisms.There were more SNPs in emm12.0 strains[183(163, 213)] than those in emm1.0 strains[63 (54, 75)], and the difference was statistically significant ( P<0.05). As for InDel, more insertions and deletions [4(3, 6), 8(6, 10)] were observed in emm12.0 strains than those [1(0, 2), 5(3, 7)] in emm1.0 strains.According to the phylogenetic tree built by taking MGAS5005 as the reference sequence based on the whole genome core-gene SNPs, 18 strains and reference strains formed two clades. Conclusions:The emm12.0 and emm1.0 types are the most common GAS strains leading to scarlet fever in children.There are differences in the genome composition of different GAS strains.The emm12.0 strains have higher genetic diversity.
10.Application of the emm cluster typing system in related studies of Group A Streptococcus

Group A Streptococcus (GAS) is one of the most important pathogens leading to children deaths all over the world.Bacterial typing is the commonest approach to analyzing the epidemiological characteristics of pathogenic microorganisms.The emm typing is frequently adopted to study GAS.The emm cluster typing is a recently proposed typing method based on the amino acid sequence homology of M proteins and the ability to bind to the host serum protein.This typing system has been extensively used for epidemiological investigation, strain selection and vaccine deve-lopment in foreign countries.However, it has not been applied in China yet. emm typing is based on a small variable region of emm genes, while the emm cluster typing system defines GAS types according to nearly intact sequences of emm genes.Besides, the emm cluster grouping system is acquired directly by emm typing comparison, so it is simple and feasible.Furthermore, the emm cluster typing can provide more information regarding the functional and structural properties of M proteins in different emm types of GAS.In this review, the methods, principles and applications of the emm cluster typing system in GAS research were summarized, in order to promote its application in China.
