A 35-year-old female patient with advanced lung cancer was treated with paclitaxel(albumin-bound)and carboplatin chemotherapy,combined with tislelizumab immunotherapy for one cycle.After 6 days of treatment,the patient developed sagging of the right eyelid compared to the leftside,mild morning and heavy evening,systemic muscle soreness,abnormal transaminases and myocardial enzymes,and abnormal electrocardiogram.It is suspected that there may be a correlation between tislelizumab and myasthenia gravis or related myositis.After admission,patients were treated with methylprednisolone,intravenous immunoglobulin,plasma exchange,mycophenolate mofetil,temporary pacemaker installation,and tracheal intubation.After more than a month of treatment,the patient's indicators decreased,but the patient became unconscious,the patient's family requested discharge.The association between the multisystem adverse reactions and tislelizumab was evaluated using the Naranjo's Assessment Scale,the correlation score was 7 and evaluated as probably relevant.It is suggested that when using tislelizumab in clinical practice,risk factor assessment and medication monitoring should be strengthened to ensure drug safety.

OBJECTIVE To conduct a survey of current status of surgical site infections(SSIs)among the orthope-dics department patients in the traditional Chinese hospitals of Guangdong Province so as to provide guidance for prevention and control of the SSIs.METHODS A retrospective questionnaire survey and field verification were car-ried out for the prevalence of SSIs among the orthopedics department patients of 61 secondary or above traditional Chinese hospitals(traditional Chinese medicine integrated with western medicine hospitals)in 2019.RESULTS To-tally 35 tertiary hospitals and 26 secondary hospitals were involved in the survey.A total of 167,051 patients who re-ceived orthopedic surgical procedures were investigated,355 of whom had SSIs,with the incidence 0.21％.The patients who had superficial surgical incision infections were dominant,accounting for 53.52％.The incidence rates of SSIs of grade 3 surgeries and SSIs of type Ⅰ incision were respectively 0.34％and 0.24％in the secondary hospitals,remarkably higher than those in the tertiary hospitals(all P＜0.05).There were no signficant differences in the incidence rate of SSIs of other grades of surgeries between the secondary hospitals and the tertiary hospitals.There were significant differences in the nutrition status,smoking history,complication with COPD,anemia,bath 1 night before surgery,preoperative pe-ripheral blood total white blood cell counts,type of surgery,title of surgeon,operating room,anesthesia mode,surgical site disinfectants,operation duration,American Society of Anesthesiologists(ASA)score and surgical risk index between the tertiary hospitals and the secondary hospitals(all P＜0.05).The etiological test was carried out for 331 patients with SSIs;totally 266 strains of pathogens were isolated,105(39.47％)of which were Staphylococcus aureus.CONCLUSIONS The incidence of SSIs is relatively low among the orthopedics department patients of the traditional Chi-nese hospitals(traditional Chinese medicine integrated with western medicine hospitals)in Guangdong Province.The inci-dence rates of SSIs of grade 3 surgeries and SSIs of type Ⅰ incision are remarkably higher in the secondary hospitals than in the tertiary hospitals.It is necessary to take comprehensive prevention and treatment measures according to the character-istics of the infections so as to reduce the incidence of SSIs among the orthopedics department patients.

Objective:Against the backdrop of population aging and air pollution,this study examined the re-gional distribution of depressive symptoms among middle-aged and elderly people in China and explores the influ-ence of patrticulate matter 2.5(PM2.5)concentration on these symptoms.This study examined the regional distri-bution of depressive symptoms in middle-aged and elderly people in China and explores the influence of PM2.5 concentration and related factors on these symptoms.Methods:Middle-aged and elderly people aged 45 years and above in CHARLS database in 2013,2015 and 2018 were selected as research objects,and the Center for Epidemio-logic Studies Depression Scale(CESD-10)to obtain the detection rate of depressive symptoms.The PM2.5 concen-tration data were derived from the annual data of the China Environmental Monitoring Station.t-test,x2 test and DID model were used to analyze the related factors of depressive symptoms.Stata17.0 and Arc GIS10.8 were used for spatial autocorrelation analysis to explore the regional distribution and relationship of depressive symptoms in mid-dle-aged and elderly people in China.Results:From east to west,the detection rate of depressive symptoms in mid-dle-aged and elderly people in China showed an increasing trend.The regional distribution of PM2.5 mainly showed that the central region was higher than the northern and southern regions and the western region(except Xinjiang)was higher than the eastern region.Conclusion:Both the PM2.5 concentration and depression symptoms exhibit spatial clustering across China.PM2.5 has a negative impact on depression symptoms in middle-aged and elderly people,with other related factors including gender,age,education level,marital status,living area,smoking status.

A 55-year-old male patient received immunotherapy combined with chemotherapy regimen(intravenous infusions of tislelizumab 200 mg on day 1, paclitaxel protein-bound 470 mg, and carboplatin 500 mg on day 1) after surgery for left upper lung squamous cell carcinoma, with 21 days as a treatment cycle. After more than half a month of medication in the 1st cycle, the patient developed multiple symmetrical purple red bruises on both lower limbs, which did not fade when pressed. The purple red bruising disappeared the next day after treatment with anti-allergic drugs. After 3 days of medication in the 3rd cycle, the patient developed a large number of scattered red papules on both lower limbs, protruding from the surface of the skin and accompanied by itching, a small number of red papules were scattered on both upper limbs and the face; after 15 days, the patient felt poor appetite; after 16 days, the patient experienced abdominal pain accompanied by vomiting and loose yellow stool. Laboratory tests showed a blood creatinine level of 204 μmol/L and occult blood in urine (+++). Pathological examination of skin tissue at the lesion site of the patient suggested IgA vasculitis (IgAV). After excluding other factors such as primary disease and infection, it was considered that the IgAV was probably related to tislelizumab. Tislelizumab was discontinued, anti-allergic and anti-inflammatory drugs, and glucocorticoid were given. After 2 weeks of treatments, the skin purpura gradually disappeared, and the abdominal pain and renal function indicators were improved. After fully aware of the risks, the patient was restarted tislelizumab immunotherapy combined with chemotherapy, and anti-allergic drugs were add at the same time. IgVA did not recur.

Oligodendrocyte lineage cells, including oligodendrocyte precursor cells (OPCs) and oligodendrocytes (OLs), are essential in establishing and maintaining brain circuits. Autophagy is a conserved process that keeps the quality of organelles and proteostasis. The role of autophagy in oligodendrocyte lineage cells remains unclear. The present study shows that autophagy is required to maintain the number of OPCs/OLs and myelin integrity during brain aging. Inactivation of autophagy in oligodendrocyte lineage cells increases the number of OPCs/OLs in the developing brain while exaggerating the loss of OPCs/OLs with brain aging. Inactivation of autophagy in oligodendrocyte lineage cells impairs the turnover of myelin basic protein (MBP). It causes MBP to accumulate in the cytoplasm as multimeric aggregates and fails to be incorporated into integral myelin, which is associated with attenuated endocytic recycling. Inactivation of autophagy in oligodendrocyte lineage cells impairs myelin integrity and causes demyelination. Thus, this study shows autophagy is required to maintain myelin quality during aging by controlling the turnover of myelin components.

Objective:To establish a quality control method for Aiye standard decoction.Methods:The ultra performance liquid chromatogrphy (UPLC) column Waters ACQUITY HSS T3 C18 (2.1 mm×150 mm,1.8 μm) was used to gradient elution by acetonitrile and 0.1% formic acid in water. 16 batches of Aiye standard decoction fingerprints were established by UPLC and the common peaks were determined in the fingerprints. The contents of 12 components were determined. The 16 batches of Aiye standard decoction were analyzed by similarity calculation, hierarchical cluster analysis (HCA), principal component analysis (PCA) and orthogonal partial least squares discriminant analysis (OPLS-DA) for analysis of differential components of Artemisiae Argyi Folium from different origins.Results:A total of 13 common peaks were marked in the fingerprints of 16 batches of Aiye standard decoction, 12 of which were identified by comparison with reference substance, including chlorogenic acid, sochlorogenic acid A, neochlorogenic acid, cryptochlorogenic acid, caffeic acid,1,3-O-Dicaffeoylquinic acid, schaftoside, isochlorogenic acid B,1,5-O-Dicaffeoylquinic acid, isochlorogenic acid C, jaceosidin and eupatilin. Similarity evaluation, PCA and HCA all classified the 16 batches of Aiye standard decoction into 2 categories. Orthogonal partial least squares discriminant analysis screened 5 differential biomarkers from 13 common peaks. The content determination results showed that the phenolic compounds and flavonoids in samples from Hubei were significantly higher than that in samples from other areas.Conclusion:This method can effectively analyze the differences in the quality of Aiye standard decoction from different origins, and provide reference for the formulation of quality standards for Aiye standard decoction and related preparations.
