1.Expression and prognostic value of triggering receptor expressed on myeloid cells-1 in patients with cirrhotic ascites and intra-abdominal infection

Objective To analyze the expression level of triggering receptor expressed on myeloid cells-1(TREM-1)in serum and ascites of patients with cirrhotic ascites,and to investigate its correlation with clinical features and inflammatory markers and its role in the diagnosis of infection and prognostic evaluation.Methods A total of 110 patients with cirrhotic ascites who were hospitalized in The Fifth Hospital of Shijiazhuang from January 2019 to December 2020 were enrolled,and according to the presence or absence of intra-abdominal infection,they were divided into infection group with 72 patients and non-infection group with 38 patients.The patients with infection were further divided into improvement group with 38 patients and non-improvement group with 34 patients.Clinical data and laboratory markers were collected from all patients.Serum and ascites samples were collected,and ELISA was used to measure the level of TREM-1.The independent-samples t test was used for comparison of normally distributed continuous data between two groups;the Mann-Whitney U test was used for comparison of non-normally distributed continuous data between two groups,and the Kruskal-Wallis H test was used for comparison between multiple groups;the chi-square test was used for comparison of categorical data between two groups.A Spearman correlation analysis was used to investigate the correlation between indicators.A multivariate Logistic regression analysis was used to identify the influencing factors for the prognosis of patients with cirrhotic ascites and infection.The receiver operating characteristic(ROC)curve was used to evaluate the diagnostic and prognostic efficacy of each indicator,and the Delong test was used for comparison of the area under the ROC curve(AUC).Results The level of TREM-1 in ascites was significantly positively correlated with that in serum(r=0.50,P<0.001).Compared with the improvement group,the non-improvement group had a significantly higher level of TREM-1 in ascites(Z=-2.391,P=0.017)and serum(Z=-2.544,P=0.011),and compared with the non-infection group,the infection group had a significantly higher level of TREM-1 in ascites(Z=-3.420,P<0.001),while there was no significant difference in the level of TREM-1 in serum between the two groups(P>0.05).The level of TREM-1 in serum and ascites were significantly positively correlated with C-reactive protein(CRP),procalcitonin(PCT),white blood cell count,and neutrophil-lymphocyte ratio(r=0.288,0.344,0.530,0.510,0.534,0.454,0.330,and 0.404,all P<0.05).The ROC curve analysis showed that when PCT,CRP,and serum or ascitic TREM-1 were used in combination for the diagnosis of cirrhotic ascites with infection,the AUCs were 0.715 and 0.740,respectively.The multivariate Logistic regression analysis showed that CRP(odds ratio[OR]=1.019,95%confidence interval[CI]:1.001-1.038,P=0.043)and serum TREM-1(OR=1.002,95%CI:1.000-1.003,P=0.016)were independent risk factors for the prognosis of patients with cirrhotic ascites and infection,and the combination of these two indicators had an AUC of 0.728 in predicting poor prognosis.Conclusion The level of TREM-1 is closely associated with the severity of infection and prognosis in patients with cirrhotic ascites,and combined measurement of TREM-1 and CRP/PCT can improve the diagnostic accuracy of infection and provide support for prognostic evaluation.
2.Effects of different pretreatment methods on the culture results of Mycobacterium marinus in tissue specimens

Objective To explore the effects of different methods on the culture results of Mycobacterium marinum,including direct inoculation,grinding followed by digestion,and digestion followed by grinding.Methods From June 2022 to March 2024,122 patients with suspected Mycobacterium marinum infection were collected from the Department of Dermatology in this hospital,including 44 cases with superficial skin type and 78 cases with subcutaneous tissue type.Direct inoculation,grinding followed by and digestion followed by grinding were used respectively to analyze the effects of these three methods on the positive detection rate and contamination rate of Mycobacterium marinum culture.Results The culture positive detection rate of Myco-bacterium marinum in superficial skin type and in subcutaneous tissue type were 47.7%(21/44)and 67.9%(53/78),respectively.The use of grinding combined with digestion method for processing skin or subcutane-ous tissue specimens results in a higher positive detection rate and a lower contamination rate compared with the direct inoculation method,with statistically significant differences(P<0.05).For superficial skin type specimens,the grinding followed by digestion method had a higher positive detection and a lower contamina-tion rate than the digestion followed by grinding method,with statistically significant differences(P<0.05).For subcutaneous tissue samples,the grinding followed by digestion method had a higher positive detection rate compared to the digestion followed by grinding method,but the difference was not statistically significant(P>0.05).There were no significant differences in the primary culture time of Mycobacterium marinum a-mong the three pretreatment methods(P>0.05).Conclusion The pretreatment of skin tissue samples with grinding followed by digestion,and the pretreatment of subcutaneous tissue samples with grinding followed by digestion or digestion followed by grinding can significantly improve the positive rate of Mycobacterium mari-num culture and reduce the contamination rate of miscellaneous bacteria.

4.Antibacterial mechanism of allicin against carbapenem-resistant Klebsiella pneumoniae based on gene cluster analysis

OBJECTIVE To investigate the in vitro inhibitory mechanism of allicin against carbapenem-resistant Klebsiella pneumoniae(CRKP).METHODS The standard microbroth dilution method was employed to determine the minimum inhibitory concentration(MIC)of allicin against the experimental CRKP.Growth curve experiments were conducted to verify the relationship between allicin concentration and antibacterial action.RNA sequencing(RNA-seq)was performed on allicin-treated CRKP and a normal control group to explore the molecular mecha-nism of allicin inhibiting CRKP growth.RESULTS The MIC of allicin against CRKP was 140 mg/ml.A total of 1 775 differentially expressed genes were screened from the normal control group and the allicin-treated group,with 845 genes upregulated and 930 genes downregulated.The expression of bacterial phosphotransferase system(PTS)genes showed significant differences between the control group and the treated group(P<0.05),and the expression levels of EIIA-EIID genes decreased(P<0.05).CONCLUSION Based on the analysis of RNA-seq re-sults,it is found that allicin may inhibits the growth of CRKP by disrupting its sugar transport system,thereby affecting its biofilm formation.

6.A Case of Premature Infant with Hyperthyroidism

Objective To improve clinicians'understanding of the diagnosis and treatment of neonatal hyperthyroidism which is rare in clinical practice.Methods The clinical data of a neonatal hyperthyroidism case diagnosed and treated was retrospectively analyzed,and the relevant literature was reviewed.Results A female infant,borned at 32 weeks and 6 days of gestation,exhibited dyspnea,a rapid heart rate,hepatosplenomegaly,thrombocytopenia,cholestasis,and abnormal liver function that continued to deteriorate after birth.She was later diagnosed with neonatal hyperthyroidism after undergoing thyroid function and antibody tests.After treated with methimazole,propranolol and other symptomatic supportive therapies,the infant's symptoms improved and she was discharged in good health.Conclusions The clinical manifestations of neonatal hyperthyroidism are non-specific and the symptoms are atypical.Therefore,when encountering symptoms such as growth retardation,tachycardia,cardiac insufficiency,tachypnea,thrombocytopenia,liver injury,hepatosplenomegaly,and a small head circumference,it is essential to consider the possibility of congenital hyperthyroidism.Timely thyroid function and antibody tests should be conducted to confirm the diagnosis,followed by early drug intervention to enhance the prognosis.
7.Safety Evaluation of Tianzhi Granules in Treating Mild-to-moderate Vascular Dementia

ObjectiveTo systematically evaluate the safety of Tianzhi granules used in the treatment of mild-to-moderate vascular dementia. MethodA randomized, double-blind, double-simulated, positive drug/placebo parallel controlled multi-center phase Ⅳ clinical trial and an open multi-center phase Ⅳ clinical trial of Tianzhi granules in the treatment of mild-to-moderate vascular dementia were conducted. Safety data of 1 492 patients were included and analyzed according to inclusion and exclusion criteria. The main evaluation measures were the incidence rate of adverse events/adverse reactions, laboratory indicators, vital signs, and electrocardiogram (ECG) results. ResultA total of six adverse events possibly related to the test drug occurred in 520 patients of the double-blind trial, and the symptoms were all mild and recovered. The incidence of adverse events was not statistically different among Tianzhi granules, donepezil, and placebo groups. Nine adverse events possibly related to the test drug were observed in 972 patients of the open trial, and the symptoms were mild and recovered. Laboratory tests (blood routine, urine routine, liver function, kidney function, and coagulation) and vital signs were compared before treatment (baseline) and after treatment of 12 and 24 weeks, respectively. There was no statistical significance in the main indicators before and after treatment. In the double-blinded trial, there was no significant difference in safety indicators between different groups before and after treatment. The most frequent adverse reaction was gastrointestinal discomfort, with an incidence rate of 6.64‰. ConclusionAdverse reactions occasionally occur in patients using Tianzhi granules, and it is safe to use Tianzhi granules to treat mild-to-moderate vascular dementia clinically.
8.Fasting bioequivalence and safety assessment of domestic versus original pyrazinamide tablets

Objective:To evaluate the bioequivalence and safety of domestic and original pyrazinamide tablets under fasting condition.Methods:A single center, randomized, open-label, two period self-crossover trial was conducted in healthy adult volunteers. The test preparation (T) of pyrazinamide tablets was produced by Taicang Pharmaceutical Factory and the reference preparation (R) was produced by A&Z Pharmaceutical Inc. (Pyrazinamide ?). The healthy subjects were randomly divided into 2 groups and took the drugs for 2 times with different order in each group, which were R-T group (subjects took R on day 1 and then T on day 8) and T-R group (subjects took T on day 1 and then R on day 8). Pharmacokinetic parameters were used as endpoints to assess the bioequivalence. Peripheral venous blood samples (3 ml) were collected from subjects within 1 hour before taking the drug and at 15, 30, 45, 60, 75, 90, 105, 120, 150, 180, and 210 minutes and 4, 5, 6, 8, 12, 24, 36, and 48 hours after taking the drug. Plasma was frozen after centrifugation of the blood samples. The plasma pyrazinamide concentrations were determined by a tandem liquid chromatography-mass spectrometry method and the main pharmacokinetic parameters such as peak concentration ( Cmax) and the area under the concentration-time curve (AUC), including AUC from time zero (pre-dose) to the time of the last quantifiable concentration (AUC 0-t) and AUC from time zero to infinity (AUC 0-∞), were calculated. The test and reference preparations were judged as bioequivalence when the 90% confidence intervals ( CI) of geometric mean ratios for AUC 0-t, AUC 0-∞, and Cmax all ranged from 0.80 to 1.25. Adverse events occurred in subjects during the trial were observed and recorded. Results:A total of 24 healthy volunteers were enrolled in the trial, including 16 males and 8 females aged 18-50 years, with 12 in the R-T and T-R groups, respectively. All subjects completed the trial. After taking medicine under fasting condition, the 90% CI of the geometric mean ratios of Cmax, AUC 0-t, and AUC 0-∞ for the test and reference preparations were 0.93-1.09, 0.98-1.03, and 0.98-1.03, respectively. During the trial, the incidence of adverse events was 25.0% (6/24) and 3 cases occurred after taking the test and reference preparations respectively, which were grade 1 in severity. Conclusion:The domestic and original pyrazinamide tablets were bioequivalence and with good safety when taken under fasting condition.
9.Effects of multilevel nutritional support based on Nutrition Risk Screening on nutritional status and prognosis of patients undergoing craniocerebral tumor surgery

Objective:To study effects of multilevel nutritional support based on Nutrition Risk Screening (NRS 2002) on nutritional status and prognosis of patients with craniocerebral tumor surgery.Methods:A total of 40 patients undergoing craniocerebral tumor surgery who were admitted to The Third People's Hospital of Hubei Province from November 2018 to October 2019 were included as the control group. A total of 40 patients undergoing craniocerebral tumor surgery who were admitted to the Third People's Hospital of Hubei Province from November 2019 to November 2020 were included as the observation group. The control group was given routine nutritional support, and the observation group was given multi-level nutritional support based on NRS 2002. The nutritional status, serum nutritional indexes, prognosis and complications were compared between the two groups before the intervention and one month after the intervention.Results:After 1 month of intervention, the thickness of triceps skinfold and muscle circumference of unaffected upper arm in the two groups were greater than those before intervention, and those of the observation group were greater than the control group, and the differences were statistically significant ( P<0.05) . The levels of prealbumin, albumin and hemoglobin in the two groups were higher than those before the intervention, and the observation group were higher than the control group, and the differences were statistically significant ( P<0.05) . The Glasgow score and ability of daily living score of the two groups were higher than those before the intervention, and the observation group were higher than the control group, and the differences were statistically significant ( P<0.05) . After 1 month of intervention, there were no statistically significant differences in body mass index, constipation, diarrhea, pulmonary infection and malnutrition between the two groups ( P>0.05) . Conclusions:Multilevel nutritional support based on NRS 2002 can improve the nutritional status and prognosis of patients undergoing craniocerebral tumor surgery and the application effect is good.
10.Multi-omics Analyses Provide Insight into the Biosynthesis Pathways of Fucoxanthin in Isochrysis galbana

Isochrysis galbana is considered an ideal bait for functional foods and nutraceuticals of humans because of its high fucoxanthin(Fx)content.However,multi-omics analysis of the regula-tory networks for Fx biosynthesis in I.galbana has not been reported.In this study,we report a high-quality genome assembly of I.galbana LG007,which has a genome size of 92.73 Mb,with a contig N50 of 6.99 Mb and 14,900 protein-coding genes.Phylogenetic analysis confirmed the monophyly of Haptophyta,with I.galbana sister to Emiliania huxleyi and Chrysochromulina tobinii.Evolutionary analysis revealed an estimated divergence time between I.galbana and E.huxleyi of~133 million years ago.Gene family analysis indicated that lipid metabolism-related genes exhibited significant expansion,including IgPLMT,IgOAR1,and IgDEGS1.Metabolome analysis showed that the content of carotenoids in I.galbana cultured under green light for 7 days was higher than that under white light,and β-carotene was the main carotenoid,accounting for 79.09%of the total carotenoids.Comprehensive multi-omics analysis revealed that the content of β-carotene,antheraxanthin,zeaxanthin,and Fx was increased by green light induction,which was significantly correlated with the expression of IgMYB98,IgZDS,IgPDS,IgLHCX2,IgZEP,IgLCYb,and IgNSY.These findings contribute to the understanding of Fx biosynthesis and its regulation,pro-viding a valuable reference for food and pharmaceutical applications.
