1.Clinical Efficacy of Qi-regulating and Phlegm-removing Method(Liu Junzitang Combined with Linggang Wuwei Jiangxintang) in Treating AECOPD with Increased EOS

ObjectiveTo assess the efficacy and safety of the Qi-regulating and phlegm-removing method(Liu Junzitang Combined with Linggang Wuwei Jiangxintang) for treating acute exacerbations of chronic obstructive pulmonary disease (AECOPD) with increased eosinophils (EOS). MethodsSixty-eight AECOPD patients with increased EOS who were hospitalized in the Department of Pulmonary Diseases of Jinhua Traditional Chinese Medicine Hospital from April 2023 to April 2024 were recruited and randomly assigned to an experimental group (EG) or a control group (CG). Both groups received conventional Western medicine, with the EG additionally receiving Liujunzitang and Linggan Wuwei Jiangxintang. The therapeutic efficacy indicators were measured after the treatment. The main therapeutic efficacy indicators included partial pressure of oxygen (PaO2) and partial pressure of carbon dioxide (PaCO2). The secondary efficacy indicators included the TCM symptom scores, the COPD Assessment Test (CAT) score, the Modified Medical Research Council (mMRC) Dyspnea Scale score, and the length of hospital stay. The indicators were measured at baseline and on days 3 and 7 of intervention. The safety was evaluated based on the adverse events. ResultsBaseline characteristics were not statistically different between the two groups. Compared with CG, EG showed no significant difference in PaO2 (P=0.773), PaCO2 (P=0.632) and or CAT score (P=0.336) at on day 3 but better PaO2 (P=0.004), PaCO2 (P=0.008), and CAT score (P=0.013) were significantly better at on day 7. Compared with CGAfter treatment, EG had lower TCM syndrome scores of than CG EG on day 3 (P=0.005) and day 7 were significantly decreased (P0.001). There was no significant difference in mMRC score between the two groups on day 3 (P=0.514) and day 7 (P=0.176) as wasor the length of hospital stay (P=0.915). The generalized linear mixed model (GLMM) showed that compared with CG, EG had significant improvements over time in PaO2, PaCO2, TCM syndrome symptom scores, CAT score, and mMRC score. ConclusionRegulating qi Qi and removing phlegm combined with conventional Western medicine can significantly alleviateimprove the clinical symptoms and improve the lung function of AECOPD patients with increased EOS increased AECOPDwhich has and demonstrates good safety.

3.Correlation analysis of alternative splicing regulator ARL6IP4 expression with the pathological characteristics and clinical prognosis in colon cancer

Purpose To investigate correlation of ADP ribosylation-like factor 6 interacting protein 4(ARL6IP4)expression with the pathological characteristics and clinical prognosis in primary colon cancer.Methods The ARL6IP4 mRNA expression in tumor and adjacent normal tissues of 133 colon cancer patients was analyzed by RT-qPCR,and its relationship with tumor location,pathological TNM stage,and 3-year survival prognosis was assessed.Additionally,ARL6IP4 protein expression was analyzed by immunohistochemistry in 30 cases,of which 16 cases were analyzed by immunofluorescence.Results The colon cancer presented significantly higher mRNA and protein levels of ARL6IP4 than adjacent normal tissues(t=4.221,P=5.200 × 10-5;t=7.421,P=3.537 × 10-8).The relative ex-pression level of ARL6IP4 mRNA in colon cancer was positively correlated with pathological TNM stage,N stage and M stage(P<0.05),and negatively correlated with 3-year cumulative survival probability(P<0.01).Additionally,sig-moid colon cancer presented significantly higher ARL6IP4 expression than other colon cancers,and at the cellular lev-el,ARL6IP4 was predominantly expressed in the cell nucleus.Conclusion The ARL6IP4 expression in colon cancer is higher than that in adjacent normal tissues,which is closely related to tumor metastasis and clinical prognosis.

5.Research progress in diagnosis and treatment of salivary gland tumors

Salivary gland tumor is one of the most common tumors in oral and maxillofacial regions.The diagnosis and treatment of salivary gland tumors had been a clinical characteristic project in Peking University School and Hospital of Stomatology since long time ago.Here we introduced the research pro-gress in diagnosis and treatment of salivary gland tumors during the past 10 years.Among 7 190 cases of salivary gland tumors treated in this institution,4 654 cases(64.7%)were benign,and 2 536(35.3%)were malignant,with benign:malignant ratio of 1.84∶1.Parotid was the most common loca-tion,followed by minor salivary gland and submandibular gland,while sublingular gland tumor was sel-dom seen.The proportion of minor salivary gland tumor was relatively high.Among 1 874 cases with pri-mary malignant tumors,the cases with T3 and stage Ⅲ accounted for only 9.6%and 10.3%,respec-tively,which indicated that there was shortcoming in the T classification and clinical stage formulated by Union for International Cancer Control(UICC),and further revision was required.The 5,10,and 15 year survival rates of 1 637 cases with postoperative follow-up were 93.1%,87.2%and 79.3%,respec-tively,which were much higher than those we reported 30 years ago.The improvement of treatment re-sults was related to more widely used combined treatment with surgery and postoperative radiotherapy,and the increase in patients with early stage.Adenoid cystic carcinoma was the malignant tumor with high rate of distant metastasis.The 5 and 10 year survival rates of the patients with pulmonary metastasis were 76.2%and 51.8%,respectively,which indicated that the pulmonary metastatic carcinomas developed slowly.Recurrent rate of carcinoma ex pleomorphic adenoma was 46.7%after single treatment of sur-gery,while it decreased to 27.5%after combined theraphy with surgery and radiotherapy,indicating that postoperative radiotheraphy could reduce the recurrent rate effectively.The normal myoepithelial cells had the inhibiting role in the invasion and metastasis of carcinoma ex pleomorphic adenoma.The evaluation of integrity of myoepithelial cells surrounding the tumor mass is helpful to understand the invasiveness of the tumors.The new surgical modalities such as extracapsular resection and partial sialoadenectomy were used in treatment of benign tumors of parotid gland and submandibular gland with advantages of decreased tissue damage and preservation of glandular function.Application of digital surgical techniques such as mixed reality combined with surgical navigation and real-time three-dimensional holograms in the surgical treatment of parotid gland tumors showed the benefits of more safety and precision,and less tissue da-mage.
6.Analysis of related factors for vascular luminal dilatational remodeling after balloon angioplasty for intracranial atherosclerotic stenosis

Objective:To explore the factors associated with vascular luminal dilatational remodeling (VLDR) following balloon angioplasty for intracranial atherosclerotic stenosis (ICAS).Methods:A case-control study was conducted to analyze the data of symptomatic severe ICAS patients who received either paclitaxel-coated balloon angioplasty (PCBA) or plain balloon angioplasty (POBA) at our center from January 2019 to January 2022 and completed the six-month follow-up. The patients were divided into VLDR group and non-VLDR group according to whether VLDR occurred on follow-up digital subtraction angiography (DSA). The baseline data, preoperative and postoperative lesion characteristics (DSA), and perioperative related information were collected. The definition of VLDR was a decrease in luminal stenosis rate by more than 10% at the time of follow-up compared to the immediate postoperative period. Multivariate logistic regression was performed to analyze possible factors affecting VLDR such as balloon type, balloon length, and expansion time.Results:A total of 88 patients were included in this study, with 16 in the VLDR group and 72 in the non-VLDR group. The follow-up time for all included patients was 6.00 (5.00, 7.00) months. VLDR occurred in 18.2% (16/88) of cases, with a VLDR incidence of 30.4% (14/46) after PCBA and 4.8% (2/42) after POBA. Univariate logistic regression analysis revealed that treatment balloon type, balloon length, inflated time, immediate postoperative stenosis rate, follow-up time and Mori classification may affect the occurrence of VLDR. Multivariate logistic regression analysis showed that the use of paclitaxel-coated balloon (PCB) ( OR=9.82, 95% CI 1.99-48.49, P=0.005) and postoperative immediate stenosis rate ( OR=1.07, 95% CI 1.00-1.14, P=0.042) were independently associated with VLDR. Conclusion:The occurrence of VLDR following balloon angioplasty in ICAS was associated with the use of PCB and immediate postoperative stenosis rates, which will provide guidance for the clinical application of PCB.
7.A case report of severe acute kidney injury caused by near drowning and literature review

This article reports a case of acute kidney injury (AKI) following near drowning with reduced urine output as the main manifestation. The patient was a 37-year-old middle-aged man. His renal function deteriorated sharply after accidentally falling into the water, and renal pathology showed acute tubular injury. After hemodialysis treatment, urine output increased significantly, and renal function and proteinuria improved significantly. AKI following near drowning lacks typical clinical manifestations and is prone to delayed diagnosis and treatment. The patients with a history of near drowning should be followed up to determine whether they are complicated by AKI.
8.Research progress on pedicle screw placement safety in orthopedic treatment of type 1 neurofibromatosis scoliosis

Neurofibromatosis(NF)as an autosomal dominant disease usually involves multiple sys-tems throughout the body and is classified as type 1 or type 2 NF.Neurofibromatosis scoliosis(NFS)is main-ly type 1,and severe neurofibromatosis scoliosis type 1(NF1-S)is manifested by abnormalities in spinal anat-omy and morphology,such as rotation of the parietal vertebrae and abnormal morphology of the pedicles,which leads to the difficulty and maximal risk of pedicle screw placement in scoliosis orthopedic surgery.In or-der to improve the safety of surgery,this paper discusses the morphology and anatomical structure of the pedi-cles in the patients with NF1-S,and analyzes the accuracy and safety of different pedicle-assisted screw place-ment techniques to provide reference for clinical surgery and promote the improvement of treatment level.
9.Application of musculoskeletal ultrasound in the overseas medical service task of"Mission Harmony-2023"

Objective To study the application value of musculoskeletal ultrasonic examination and treatment in the overseas medical service task of"Mission Harmony-2023".Methods The data of 202 overseas patients and officers and soldiers on hospital ship who had indications and underwent musculoskeletal ultrasonic examination and ultrasound guided interventional treatment were analyzed.The measures to better carry out this work in the future was explored.Results There were 202 individuals who underwent musculoskeletal ultrasonic examination,and positive results were obtained in 131 individuals.Among overseas patients,the incidence of knee diseases was the highest(50.96%);among hospital ship officers and soldiers,the incidence of knee diseases was the highest(51.85%).Four patients underwent ultrasound guided musculoskeletal injection therapy,with an average treatment time of(18.60±5.19)min,preoperative visual analogue scale(VAS)pain score of(7.45±1.02),postoperative VAS pain score of(2.70±0.68),and postoperative observation time of 30 min.Conclusion Musculoskeletal ultrasound examination has strong practicality,not only can it effectively diagnose musculoskeletal system diseases,but also guide precise treatment,making it worthy of promotion and use in future"Mission Harmony".
10.Evaluation of augmented reality technology in the recognizing of oral and maxillofa-cial anatomy

Objective:To evaluate the outcome of Augmented reality technology in the recognizing of oral and maxillofacial anatomy.Methods:This study was conducted on the undergraduate students in Peking University School of Stomatology who were learning oral and maxillofacial anatomy.The image da-ta were selected according to the experiment content,and the important blood vessels and bone tissue structures,such as upper and lower jaws,neck arteries and veins were reconstructed in 3D(3-dimension-al)by digital software to generate experiment models,and the reconstructed models were encrypted and stored in the cloud.The QR(quick response)code corresponding to the 3D model was scanned by a net-worked mobile device to obtain augmented reality images to assist experimenters in teaching and subjects in recognizing.Augmented reality technology was applied in both the theoretical explanation and cadaveric dissection respectively.Subjects'feedback was collected in the form of a post-class question-naire to evaluate the effectiveness of augmented reality technology-assisted recognizing.Results:In the study,83 undergraduate students were included as subjects in this study.Augmented reality technology could be successfully applied in the recognizing of oral and maxillofacial anatomy.All the subjects could scan the QR code through a connected mobile device to get the 3D anatomy model from the cloud,and zoom in/out/rotate the model on the mobile.Augmented reality technology could provide personalized 3D model,based on learners'needs and abilities.The results of likert scale showed that augmented reality technology was highly recognized by the students(9.19 points),and got high scores in terms of forming a three-dimensional sense and stimulating the enthusiasm for learning(9.01 and 8.85 points respective-ly).Conclusion:Augmented reality technology can realize the three-dimensional visualization of impor-tant structures of oral and maxillofacial anatomy and stimulate students'enthusiasm for learning.Be-sides,it can assist students in building three-dimensional space imagination of the anatomy of oral and maxillofacial area.The application of augmented reality technology achieves favorable effect in the recog-nizing of oral and maxillofacial anatomy.
