1.The Regulatory Effects and Mechanisms of Piezo1 Channel on Chondrocytes and Bone Metabolic Dysregulation in Osteoarthritis

Osteoarthritis (OA), a highly prevalent degenerative joint disease worldwide, is defined by articular cartilage degradation, abnormal bone remodeling, and persistent chronic inflammation. It severely compromises patients’ quality of life, and currently, there is no radical cure. Abnormal mechanical stress is widely regarded as a core driver of OA pathogenesis, and the exploration of mechanical signal perception and transduction mechanisms has become crucial for deciphering OA’s pathophysiological processes. Piezo1, a key mechanosensitive cation channel belonging to the Piezo protein family, has recently gained significant attention due to its pivotal role in mediating cellular responses to mechanical stimuli in joint tissues. This review systematically examines Piezo1’s expression patterns, regulatory mechanisms, and pathological functions in OA, with a particular focus on its dual roles in modulating chondrocyte homeostasis and bone metabolism disorders, while also delving into the underlying molecular signaling pathways and potential therapeutic implications. Piezo1, consisting of approximately 2 500 amino acids and forming a unique trimeric propeller-like structure, is widely expressed in chondrocytes, osteocytes, mesenchymal stem cells, and synovial cells. It exhibits permeability to cations such as Ca2+, K+, and Na+, and directly responds to membrane tension changes induced by mechanical stimuli like fluid shear stress and mechanical overload. In OA patients and animal models, Piezo1 expression is significantly upregulated, especially in cartilage regions subjected to abnormal mechanical stress (e.g., human temporomandibular joint cartilage). This overexpression is closely associated with aggravated cartilage degeneration, increased chondrocyte apoptosis, accelerated cellular senescence, and intensified inflammatory responses. Mechanical overload and pro-inflammatory cytokines (e.g., IL-1β) are key inducers of Piezo1 upregulation: IL-1β activates the PI3K/AKT/mTOR signaling pathway to enhance Piezo1 expression, forming a pathogenic positive feedback loop that inhibits chondrocyte autophagy, promotes apoptosis, and further accelerates joint degeneration. Mechanistically, Piezo1 mediates OA progression through multiple interconnected pathways. When activated by mechanical stress, Piezo1 triggers excessive Ca2+ influx, leading to endoplasmic reticulum stress (ERS) and mitochondrial dysfunction, which directly induce chondrocyte apoptosis. This process involves the activation of downstream signaling cascades such as cGAS-STING and YAP-MMP13/ADAMTS5. YAP, a transcriptional regulator, upregulates the expression of matrix metalloproteinase 13 (MMP13) and aggrecanase (ADAMTS5), thereby accelerating cartilage matrix degradation. Additionally, Piezo1-driven Ca2+ overload promotes the accumulation of reactive oxygen species (ROS) and upregulates senescence markers (p16 and p21), accelerating chondrocyte senescence via the p38MAPK and NF-κB pathways. Senescent chondrocytes secrete senescence-associated secretory phenotype (SASP) factors (e.g., IL-6, IL-1β), further amplifying joint inflammation. In terms of bone metabolism, Piezo1 maintains joint homeostasis by promoting the differentiation of fibrocartilage stem cells into chondrocytes and balancing bone formation and resorption through regulating the FoxC1/YAP axis and RANKL/OPG ratio. Therapeutically, targeting Piezo1 shows promising potential. Preclinical studies have demonstrated that Piezo1 inhibitors (e.g., GsMTx4) can reduce joint damage and alleviate pain in OA mice. Simultaneously, siRNA-mediated co-silencing of Piezo1 and TRPV4 (another mechanosensitive channel) decreases intracellular Ca2+ concentration, inhibits chondrocyte apoptosis, and promotes cartilage repair. Conditional knockout of Piezo1 using Gdf5-Cre transgenic mice alleviates cartilage degeneration in post-traumatic OA models by downregulating MMP13 and ADAMTS5 expression. Despite existing challenges, such as off-target effects of inhibitors, inefficient local drug delivery, and interindividual genetic variability, strategies like developing selective Piezo1 antagonists, optimizing targeted nanocarriers, and combining Piezo1-targeted therapy with physical therapy provide viable avenues for clinical translation. The authors propose that Piezo1 serves as a critical therapeutic target for OA, and future research should focus on deciphering its context-dependent regulatory networks, developing tissue-specific intervention strategies, and validating their efficacy and safety in clinical trials to address the unmet medical needs of OA patients.
2.Identification of core genes of osteoarthritis by bioinformatics

BACKGROUND:At present,osteoarthritis has become a major disease affecting the quality of life of the elderly,and the therapeutic effect is poor,often focusing on preventing the disease process,and the pathogenesis of osteoarthritis is still not fully understood.Bioinformatics analysis was carried out to explore the main pathogenesis of osteoarthritis and related mechanisms of gene coding regulation. OBJECTIVE:To screen core differential genes with a major role in osteoarthritis by gene expression profiling. METHODS:Datasets were downloaded from the Gene Expression Omnibus(GEO):GSE114007,GSE117999,and GSE129147.Differential genes in the GSE114007 and GSE117999 data collections were screened using R software,performing differential genes to weighted gene co-expression network analysis.The module genes most relevant to osteoarthritis were selected to perform protein interaction analysis.Candidate core genes were selected using the cytocape software.The candidate core genes were subsequently subjected to least absolute shrinkage and selection operator regression and COX analysis to identify the core genes with a key role in osteoarthritis.The accuracy of the core genes was validated using an external dataset,GSE129147. RESULTS AND CONCLUSION:(1)A total of 477 differential genes were identified,265 differential genes associated with osteoarthritis were obtained by weighted gene co-expression network analysis,and 8 candidate core genes were identified.The least absolute shrinkage and selection operator regression analysis finally yielded a differential gene ASPM with core value that was externally validated.(2)It is concluded that abnormal gene ASPM expression screened by bioinformatics plays a key central role in osteoarthritis.
3.Surgical approach and efficacy evaluation for tympanosclerosis under endoscopic surgery

Objective:To evaluate the surgical management and clinical efficacy of endoscopic operation for tympanosclerosis (TS).Methods:A retrospective analysis was conducted on 228 patients with TS who underwent endoscopic surgery at Shaanxi Provincial People′s Hospital between January 2019 and December 2023. There were 79 males and 149 females, aged 18-68 years (median 50 years). Surgical management, perforation healing rate, pre-and post-operate hearing characteristics, and complications were analyzed. The air conduction threshold values at 500 Hz, 1 000 Hz, 2 000 Hz and 4 000 Hz, average air conduction pure tune audiometry (AC-PTA), and air-bone conduction (ABG) pre-and post-operation were compared. Statistical analysis was performed using SPSS 25.0.Results:All 132 cases of type Ⅰ tympanosclerosis underwent myringoplasty. Among 55 cases with type Ⅱ tympanosclerosis, 33 (60.0%) received typeⅠand 22 (40.0%) received type Ⅱ tympanoplasty. Of 16 cases with type Ⅲ tympanosclerosis, 10 (62.5%) underwent type Ⅰand 6 (37.5%) underwent type Ⅱtympanoplasty. Among 25 cases with type Ⅳ, 5 (20.0%) underwent type Ⅰ, 13 (52.0%) type Ⅱ, and 3 (12.0%) type Ⅲ tympanoplasty, while, 4 (16.0%) underwent tympanoplasty with autologous cartilage. The average follow-up period was 8.2 months (6 months to 3 years); The overall healing rate of the tympanic perforation was (97.8%)223/228. All cases exhibited improved air conduction hearing threshold at all frequencies, AC-PTA and ABG postoperatively. The differences between types Ⅰ, Ⅱ at 500 Hz, 1 000 Hz, 2 000 Hz, 4 000 Hz, AC-PTA and ABG were statistically significant (all P<0.001 for type Ⅰ, all P<0.05 for type Ⅱ). In type Ⅲ, improvements were significant for all tested parameters except at 4 000 Hz (all P<0.05) and no statistically significant difference were found in type Ⅳ. No severe complications such as profound sensorineural hearing loss or facial nerve paralysis were encountered. Conclusion:Totally, endoscopic transcanal surgery is an effective management for tympanosclerosis, providing favorable short-term hearing outcomes with an acceptable safety profile.
4.Optimization Study of Rat Models for Sequelae of Pelvic Inflammatory Disease

Objective:To establish a stable rat model of sequelae of pelvic inflammatory disease(SPID)with clinical characteristics,and to provide a reliable experimental model for the study of the pharmcological effect and mechanism of SPID.Methods:Twenty-four 7-week-old SD rats were divided into sham operation group,model-A(108 cfu/mL mixed bacterial solution,0.2 mL),model-B(109 cfu/mL mixed bacterial solution 0.2 mL),and model-C(108 cfu/mL E.coli 0.2 mL).The weight of the rat's uterine was weighed and the uterine index was calculated.The automatic hematology analyzer was used to detect the blood routine;hematoxylin-eosin staining(HE)and masson staining were used to detect uterine pathlogical changes in rats.Enzyme-linked immunosorbent assay(ELISA)was used to detect interleukin-1β(IL-1β),interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)in rat uterine tissue homogenates.Western blot was used to detect the expression of proteins related to NF-κB signaling pathway.Results:Compared with the sham operation group,the uterine index of model-A,model-B,and model-C were significantly increased(P<0.05,P<0.01).The levels of WBC and NE in the model-A increased significantly(P<0.01).The level of LY in model-B decreased significantly(P<0.01).The levels of IL-1β,TNF-α in model-A,model-B,and model-C were significantly increased(P<0.01).The levels of IL-6 in model-A and model-B were significantly increased(P<0.05,P<0.01).The collagen volume fraction of model-A and model-B were significantly increased(P<0.01).Mechanism study indicates that the expression levels of p-IKKβ/IKKβ,p-IκBα/IκBα and p-p65/p65 in model-A were significantly increased(P<0.01),and the expression levels of IκBα/β-actin were significantly decreased(P<0.01).The expression level of p-IKKβ/IKKβ in model-B was significantly increased(P<0.01).Conclusions:A stable rat model of SPID that conforms to clinical characteristics can be successfully constructed by combining 0.2 mL of mixed bacterial solution with a concentration of 108 cfu/mL and mechanical injury.This modeling method intervened in the expression of the NF-κB inflammatory signaling pathway.
5.Study on the effectiveness and safety of a novel intravascular shock wave balloon for pre-treatment of severe coronary artery calcification lesions

Objective To evaluate the efficacy and safety of a novel intravascular lithotripsy(IVL)balloon—Vesscrack shockwave balloon—for vascular preparation before stent implantation in patients with severe coronary artery calcification(CAC).Methods This was a prospective,single-arm,multicenter study conducted in China from June 2022 to October 2022.Patients with severe CAC were treated with the Vesscrack shockwave balloon for lesion preparation,followed by drug-eluting stent(DES)implantation.Of these,33 patients underwent optical coherence tomography(OCT).The primary endpoint was procedural success,defined as successful stent implantation with residual stenosis≤30%and the absence of in-hospital major adverse events,including cardiac death,target vessel-related myocardial infarction,or target lesion revascularization.Results A total of 170 patients[mean age:(65.9±7.9)years,116 males]were enrolled.After treatment with IVL and DES,the minimum lumen diameter increased significantly compared to baseline[(2.34±0.40)mm vs.(0.95±0.33)mm,P<0.001],the degree of stenosis was significantly reduced[(13.24±6.60)%vs.(65.18±10.59)%,P<0.001].Procedural success was achieved in 100%of cases,and device success was 98.8%.The 30-day patient-related cardiovascular clinical composite endpoint(POCE)rate was 0.0,with no target lesion failure,no confirmed or potential thrombotic events were observed.The shockwave energy generator demonstrated excellent stability and ease of use.Among the 33 patients assessed with OCT,after IVL intervention,the maximum calcified area of the lumen[(3.51±1.51)mm2 vs.(2.85±1.80)mm2,P<0.001],and the minimum lumen area within the target lesion[(3.08±1.04)mm2 vs.(2.02±0.75)mm2,P<0.001],and after DES intervention,the luminal area of the largest calcified site[(6.59±1.64)mm2 vs.(2.85±1.80)mm2,P<0.001]and the minimum luminal area within the target lesion[(6.19±1.45)mm2 vs.(2.02±0.75)mm2,P<0.001]were significantly increased,and the differences were statistically significant.Conclusions The Vesscrack shockwave balloon is effective and safe for vascular preparation in patients with severe CAC prior to stent implantation.It achieves significant calcified plaque modification,high procedural success rates,and minimal complications.
6.Lycium barbarum polysaccharide ameliorates ovarian granulosa cell aging in rats by activating CAMKK2/AMPK/MCU signaling pathway

Aim To explore the mechanism of Lycium barbarum glycopeptide(LbGP)improving aging in rat primary ovarian granulosa cells.Methods This study divided the cells into a normal group,a DOX group,and four different LbGP concentration treatment groups post-DOX intervention.Results Cell proliferation was assessed using CCK-8,EDU,and Ki67 assays,while aging markers and mitochondrial function-related fac-tors were detected using immunofluorescence and West-ern blotting.The results showed that,compared to the DOX group,LbGP treatment significantly increased cell viability(P<0.05)and promoted proliferation(P<0.05).Post LbGP treatment,the β-galactosidase-posi-tive area in cells was significantly reduced compared to the DOX group(P<0.05).Immunofluorescence re-sults indicated that,compared to the DOX group,levels of p21 and γH2AX significantly decreased(P<0.05),while pRB increased(P<0.05)after LbGP treatment.Western blot results showed that,compared to the DOX group,the aging phenotype proteins p21 and p53 significantly decreased(P<0.05),and pRB notably increased(P<0.05)in the LbGP treatment group.The release of cytC into the cytoplasm and the activated caspase-9 significantly decreased(P<0.05);levels of CAMKK2,pAMPK,and mitochondrial calcium homeostasis regulator MCU increased(P<0.05);nuclear energy metabolism-related proteins SirT1,PGC1α/β and ATP5A1 significantly increased(P<0.05);compared to the DOX group,ROS levels significantly decreased after LbGP treatment(P<0.05).Conclusions The results suggest that LbGP can ameliorate DOX-induced aging in rat primary ovar-ian granulosa cells,potentially through the upregulation of the CAMKKβ/AMPK signaling pathway,thereby im-proving mitochondrial calcium homeostasis and increas-ing the expression levels of cell energy metabolism-re-lated regulatory proteins.This provides an experimen-tal basis for LbGP's potential role in supporting the im-provement of ovarian function.
7.Analysis of dosimetric verification results of intensity-modulated radiotherapy for breast cancer based on EPID fraction images

Objective To investigate the stability and reproducibility of the treatment fractions during the intensity-modulated radiotherapy(IMRT)for breast cancer and the effect of respiratory motion on the dose irradiation of breast cancer radiotherapy by comparing the results of breast cancer dosimetric verification based on fractionated images by an electronic portal imaging device(EPID).Methods A total of 28 IMRT patients admitted to some hospital from January to June 2023 were grouped according to the pathological results and effects of respiratory motion on the accuracy of radiotherapy during clinical treatment,including 14 cases in a breast group and 14 cases in a non-breast group with 8 ones of head and neck tumors,5 ones of esophageal cancer and 1 case of cervical cancer.All the patients underwent a scan with cone beam computed tomography(CBCT)before the first radiotherapy,and image registration was carried out with a positioning CT.An EPID was used to acquire transmission dose images of 10 fractions of radiotherapy,and γ analysis was performed using the RIT 113 QA software to compare the images of the subsequent 9 fractions with those of the first fraction,with the images of the first fraction of radiotherapy as the baseline values.Absolute maximum dose normalization was implemented under the condition of 10%dose assessment threshold,and the γ-pass rates under the 3 criteria of 2%/2 mm,3%/2 mm and 3%/3 mm were counted separately.The fraction dose verification results of the 28 patients were divided into 3 treatment phases of 2-4 times(T1),5-7 times(T2)and 8-10 times(T3)to analyze the stability of dose irradiation during the radiotherapy.SPSS 22.0 software was used for statistical analysis.Results Under the condition of 10%dose assessment threshold,the breast and non-breast groups had the γ-pass rates being(95.80±2.65)%and(94.60±6.59)%under the 2%/2 mm criterion and(98.46±1.31)%and(97.50±3.30)%under the 3%/2 mm criterion respectively,and the differences were statistically significant(all P<0.05).Under the assessment criteria of 2%/2 mm,3%/2 mm and 3%/3 mm,the breast group had the γ-pass rates of fractions of treatment significantly lower than those of the non-breast group(all P<0.05),while the γ-pass rates showed no significant differences at T1,T2 and T3 treatment phases(all P>0.05).Conclusion EPID fraction images contribute to evaluating IMRT accuracy effectively.IMRT has high stability and reproducibility during the treatment cycle,while respiration may result in dose deviation during the fraction radiotherapy for breast cancer,and optical surface tracking technology or active breathing control technology is suggested to be involved in to relieve dose deviation.[Chinese Medical Equipment Journal,2025,46(6):54-58]

Objective To investigate the antimicrobial resistance of clinical isolates from elderly patients(≥65 years)in major medical institutions across China.Methods Bacterial strains were isolated from elderly patients in 52 hospitals participating in the CHINET Antimicrobial Resistance Surveillance Program during the period from 2015 to 2021.Antimicrobial susceptibility test was carried out by disk diffusion method and automated systems according to the same CHINET protocol.The data were interpreted in accordance with the breakpoints recommended by the Clinical and Laboratory Standards Institute(CLSI)in 2021.Results A total of 514 715 nonduplicate clinical isolates were collected from elderly patients in 52 hospitals from January 1,2015 to December 31,2021.The number of isolates accounted for 34.3%of the total number of clinical isolates from all patients.Overall,21.8%of the 514 715 strains were gram-positive bacteria,and 78.2%were gram-negative bacteria.Majority(90.9%)of the strains were isolated from inpatients.About 42.9%of the strains were isolated from respiratory specimens,and 22.9%were isolated from urine.More than half(60.7%)of the strains were isolated from male patients,and 39.3%isolated from females.About 51.1%of the strains were isolated from patients aged 65-<75 years.The prevalence of methicillin-resistant strains(MRSA)was 38.8%in 32 190 strains of Staphylococcus aureus.No vancomycin-or linezolid-resistant strains were found.The resistance rate of E.faecalis to most antibiotics was significantly lower than that of Enterococcus faecium,but a few vancomycin-resistant strains(0.2%,1.5%)and linezolid-resistant strains(3.4%,0.3%)were found in E.faecalis and E.faecium.The prevalence of penicillin-susceptible S.pneumoniae(PSSP),penicillin-intermediate S.pneumoniae(PISP),and penicillin-resistant S.pneumoniae(PRSP)was 94.3%,4.0%,and 1.7%in nonmeningitis S.pneumoniae isolates.The resistance rates of Klebsiella spp.(Klebsiella pneumoniae 93.2%)to imipenem and meropenem were 20.9%and 22.3%,respectively.Other Enterobacterales species were highly sensitive to carbapenem antibiotics.Only 1.7%-7.8%of other Enterobacterales strains were resistant to carbapenems.The resistance rates of Acinetobacter spp.(Acinetobacter baumannii 90.6%)to imipenem and meropenem were 68.4%and 70.6%respectively,while 28.5%and 24.3%of P.aeruginosa strains were resistant to imipenem and meropenem,respectively.Conclusions The number of clinical isolates from elderly patients is increasing year by year,especially in the 65-<75 age group.Respiratory tract isolates were more prevalent in male elderly patients,and urinary tract isolates were more prevalent in female elderly patients.Klebsiella isolates were increasingly resistant to multiple antimicrobial agents,especially carbapenems.Antimicrobial resistance surveillance is helpful for accurate empirical antimicrobial therapy in elderly patients.

Objective:To prepare the polyclonal antibody against pilus assembly protein(SpaD),a biofilm formation related protein of Corynebacterium striatum(C.striatum),and to evaluate its inhibitory effect on biofilm formation by strong biofilm-producing strains of C.striatum along with its potential application value.Methods:A total of 117 strains of C.striatum isolated from clinical specimens of hospitalized patients at Affiliated Hospital of Inner Mongolia Medical University from 2011 to 2021 were selected as the study subjects.The biofilm formation ability of each strain was detected by crystal violet staining assay.Real-time fluorescence quantitative PCR(RT-qPCR)method was used to examine the distribution of SpaD protein encoding gene spaD in strong biofilm-producing strains of C.striatum.The strong biofilm-producing strains of C.striatum were divided into control group,and proteinase K group,and cystol violet staning method was used to detect the inhibitory effect of proteinaskon the biofilm formation abilitys of the strong biofilm-producing strains of C.striatum.The recombinant SpaD protein was constructed using protein recombination technology and the was divided into control group and 5 and 10 mg·L-1 SpaD recombinant proterin groups,and cystol violet staning method was used to detect the inhibitory effect of SpaD recombinant protein.The rabbit anti-SpaD polyclonal antibodies were subsequently obtained through animal immunization,the experiment was divided into control group and 1∶400,1∶200,and 1∶100 SpaD polyclonal antibody groups,the inhibitory effect of SpaD polyclonal antibodies on biofilm and crystal violet staining method was used to detect abilities of strong biofilm-producing strains of C.striatum.Results:The Crystal violet staining results revealed that strong,moderate,and weak biofilm-producing strains accounted for 47.9%(56/117),29.0%(34/117),and 23.1%(27/117),respectively.The RT-qPCR results showed that all the strong biofilm-producing strains carried the spaD gene.Compared with control group,the biofilm formation abilities of 13 strong biofilm-producing C.striatum strains in both 5 and 10 mg·L-1 SpaD recombinant protein groups were significantly decreased(P<0.05).Compared with control group,the biofilm formation abilities in 61.5%(8/13)and 7.7%(1/13)of strong biofilm-producing C.striatum strains in 1∶100 and 1∶200 SpaD polyclonal antibody groups were decreased,respectively(P<0.05).Conclusions:The spaD gene is highly expressed in strong biofilm-producing clinical strains of C.striatum.Anti-SpaD polyclonal antibodies significantly inhibits biofilm formation in these clinical isolates,demonstrating a inhibitory effect as a manner of concentration-dependent.SpaD can be a promising novel target for therapeutic intervention against biofilm-producing C.striatum infections.

In recent years,significant breakthroughs have been achieved in the immunotherapy for Alzheimer's disease.In line with global advancements,two anti-amyloid-β monoclonal antibodies have been approved and successfully launched in China for clinical use.Lecanemab and Donanemab were officially used in June 2024 and April 2025 in China,respectively.In order to standardize the rational and safe application of anti-amyloid-β monoclonal antibodies for Alzheimer's disease in China,this article integrates recom-mendations from the clinical trials and real-world experience from the author's team and domestic peers to further update the recom-mendations for the clinical use of anti-amyloid-β monoclonal antibody based on the 2024 version.It includes indications for therapy,pre-treatment evaluation and preparation,administration protocols and safety measures during treatment,and post-treatment monitor-ing strategies.
