1.Relationship between decline of exercise ability and mitochondrial damage in skeletal muscle of mice with high-altitude de-acclimatization

AIM:This study aims to investigate the mechanisms underlying skeletal muscle mitochondrial damage associated with decline in exercise function during high-altitude de-adaptation,using a mouse model.METHODS:Twen-ty-four healthy male C57BL/6J mice were randomly assigned to two groups:a high-altitude de-adaptation group and a plain control group.The model group was exposed to a low-pressure,low-oxygen chamber simulating an altitude of 7 000 meters for two weeks,followed by eight days of rearing in a plain environment.The control group was maintained in a plain envi-ronment for the same duration.Grip strength and pole-climbing tests were conducted on the 1st,3rd,and 5th days post-re-turn to assess muscle strength and motor coordination.Treadmill exercises were performed on the 4th and 8th days to eval-uate exercise endurance.After the treadmill exercise on the 8th day,serum,liver,and skeletal muscle tissues were col-lected.Levels of lactic acid(LA),glucose(GLU),creatine kinase(CK),lactate dehydrogenase(LDH),alanine trans-aminase(ALT),and aspartate aminotransferase(AST)in serum,as well as glycogen levels in the liver and muscle,were analyzed.Additionally,the expression of proteins related to mitochondrial biogenesis,fission,fusion,and oxidative phos-phorylation in muscle tissues was assessed using Western blot.RESULTS:(1)The model group exhibited significant re-ductions in grip strength,increased pole-climbing T-turn and total times,and decreased total time and distance in the ex-haustion running test.(2)Serum levels of LA,CK,LDH,ALT,and AST were elevated,while GLU levels decreased,and glycogen levels in both the liver and muscle were reduced in the model group following the treadmill exercise.(3)Ab-normal indicators in the model group did not return to normal by the end of the de-adaptation period.(4)Western blot analysis revealed decreased expression of mitochondrial oxidative phosphorylation proteins(ATP6V1A and Mt-CO2)and mitochondrial biogenesis proteins(PGC-1α and FGF21),increased levels of mitochondrial fusion proteins(OPA1 and MFN1),and no significant changes in fission protein expression(FIS1 and DRP1)in muscle tissue from the model group.CONCLUSION:Exercise capacity in mice during the high-altitude de-adaptation period significantly declined,particu-larly in terms of muscle strength,motor coordination,and endurance.This decline is closely associated with abnormal pro-tein expression related to skeletal muscle mitochondrial energy metabolism and production.
2.Syndrome Differentiation and Treatment of Alopecia Areata Based on the Turbid Toxin Theory from the Perspectives of the Liver,Spleen,and Kidney

Alopecia areata (AA) is a common hair loss disorder, and the core pathogenesis is the internal gene-ration of turbid toxin caused by qi movement disorder in the liver, spleen, and kidney. Turbid toxin serves as both a pivotal etiological trigger and a pathological driver of disease exacerbation. Clinically, AA can be classified into four principal patterns, including liver constraint with spleen deficiency, internal accumulation of damp-heat, liver-kidney depletion, and qi-blood depletion. Therapeutic strategies prioritize clearing and resolving turbid toxin while regulating the qi movement of the liver, spleen, and kidney. Accordingly, different formulas were applied. Self-formulated Shugan Jianpi Huazhuo Formula (疏肝健脾化浊方) is suggested to drain dampness, resolve turbidity, and unblock qi movement. Self-formulated Sanjiao Fenxiao Jiedu Formula (三焦分消解毒方) can be used to clear heat, drain dampness and resolve toxin. Self-formulated Zishen Yanggan Toudu Decoction (滋肾养肝透毒汤) can clear and vent latent toxins, while Self-formulated Guiqi Shengfa Didu Formula (归芪生发涤毒方) is employed to tonify qi and blood, purge toxins, and regenerate vitality. By differentiating and treating AA based on the functional patterns of the liver, spleen, and kidney, this approach expands the application scope of the turbid toxin theory and provides valuable insights for treatment of AA.
3.Establishment and validation of a nomogram prediction model for cognitive impairment in patients with hypertension based on the CHARLS database

Objective:To establish a predictive model for the risk of cognitive impairment in patients with hypertension.Methods:Data were obtained from the China Health and Retirement Longitudinal Study(CHARLS),and 17 indicators were analyzed,including demographic features,behavioral factors,and health status.The study cohort was randomly divided into a training set(n=2 918)and an internal validation set(n=1 249)at a ratio of 7∶3,and 1 457 patients with hypertension who were treated in Chongqing Traditional Chinese Medicine Hospital from January to December 2024 were included as an external validation set.The least absolute shrinkage and selection operator regression analysis was used to identify predictive variables,and 10-fold cross-validation was used to determine the optimal model.A logistic regression model was used to investigate the risk factors for cognitive impairment in patients with hypertension,and then a nomogram prediction model was established.The calibration curve was used to assess the accuracy of the model,and the area under the ROC curve(AUC)and the decision curve analysis were used to assess the predictive performance of the model.Results:A total of 4167 hypertensive patients aged≥45 years were included from the CHARLS database,among whom 668 had cognitive impairment.The multivariate logistic regression analysis showed that age(odds ratio[OR]=1.408,95%CI=1.040-1.056),sex(OR=0.570,95%CI=0.492-0.660),body mass index(OR=0.931,95%CI=0.914-0.948),educational level(OR=0.235,95%CI=0.200-0.277),place of residence(OR=1.674,95%CI=1.447-1.936),physi-cal activity(OR=0.459,95%CI=0.373-0.562),depression(OR=1.386,95%CI=1.198-1.604),and total cholesterol level(OR=0.997,95%CI=0.995-0.999)were predictive variables.The ROC curve analysis showed that the nomogram model had good performance,with an AUC of 0.814(95%CI=0.802-0.826)in the training set,0.817(95%CI=0.788-0.846)in the internal validation set,and 0.725(95%CI=0.699-0.752)in the external validation set.Conclusion:The nomogram model developed in this study can effectively predict the risk of cognitive impairment in Chinese patients with hyperten-sion and has a good application value.

5.Sleep architecture in drug naive patients with schizophrenia: A meta-analysis

Objective:To assess the characteristics change of sleep architecture in drug naive patients with schizophrenia,compared with healthy control.Methods:The key words including schizophrenia and sleep architecture (or sleep structure or sleep disturbance or polysomnogram and so on) were used to search literatures in MEDLINE,Embase,Springer,PsychINFO,google scholar,Wanfang data,published from 1980 to 2015.Fifteen studies that compared sleep architecture in drug naive patients with schizophrenia and healthy control were included.Literature quality evaluation was performed with the Newcastle-Ottawa Scale.The meta-analysis was performed by using Stata13.0 software.Results:Compared to healthy control,the total sleep time decreased (P < 0.01),the sleep latency increased (P < 0.01),the sleep efficiency decreased (P < 0.01),and the rapid-eye-movemem (REM) sleep latency increased (P < 0.01) significantly in drug naive patients with schizophrenia.The proportion of stage1 was increased,and the proportions of stage4 and slow wave sleep stage were decreased,the differences between case and control were statistically significant.Conclusion:In the control of drug effects,patients with schizophrenia may have poorer sleep quality of be poorer than healthy controls,such as the decreased total sleep time,specifically slow wave sleep,prolonged sleep latency and decreased sleep efficiency.
6.Clinical analysis of 9 cases of type 2 diabetes mellitus complicated with invasive pulmonary aspergillosis

Objective To investigate the clinical characteristics and prognosis in the patients with type 2 diabetes mellitus and invasive pulmonary aspergillosis(IPA) for better management of the disease .Methods Clinical data of 9 cases of type 2 diabetes associated with IPA treated in Qingdao Municipal Hospital from January 2008 to December 2013 were analyzed retrospectively . Results The diagnosis of IPA was proven in 5 and probable in 4 of the 9 patients .The main clinical manifestations were fever , cough and expectoration .The findings of CT scan mainly showed pulmonary nodules along the bronchovascular bundle and cavity signs .Bronchoscopy showed congestion ,edema ,and erosion of bronchial mucosa covered with yellow‐white or brown pus ,partially or completely blocking the lumen .Antifungal treatment was effective for 4 patients .The other five patients died . Conclusions Type 2 diabetes mellitus is a risk factor for developing invasive pulmonary aspergillosis .Early diagnosis and proper treatment are critical for improved prognosis .
7.Comparison of Establishing Rat Model of Chronic Obstructive Pulmonary Disease by Inhalation of PM2.5 or ;Intratracheal Instillation of PM2.5 Suspension

Objective:To compare the effect for establishing rat model of chronic obstructive pulmonary disease(COPD)be-tween two methods,inhalation of PM2.5 and intratracheal instillation of PM2.5 suspension.Methods:A total of 30 adult rats were randomly divided into control group(group A),group fumigated with PM2.5(group B),and group instilled intratracheal-ly with PM2.5 suspension(group C).Then the lung function of rats,the counts of inflammatory cells in bronchoalveolar lavage fluid(BALF),and the pathological sections of lung tissues,were observed on different phases.Results:Compared with that in group A,the body mass in group B or group C decreased.And the tidal volume(VT)and the peak expiratory flow(PEF)de-creased.Furthermore,total cell count,lymphocyte count(L),neutrophil count(N),lymphocyte proportion(L%),and neutro-phil proportion(N%)in BALF increased.All the differences showed statistical significance(all P <0.05).Morphological de-tection showed enlarged alveolar space,disruption of alveolar septa,fusion of alveoli,and formation of emphysema in group B. And it also showed disordered arrangement of airways epithelial cells,partial airway epithelial hyperplasia,inflammatory cell infiltration and proliferation of smooth muscle in group B.Furthermore,mean linear intercept (MLI)and mean alveolar num-ber (MAN)in group B increased,while compared with that in group A.The pathological changes in group C were similar to those in group B.However,the degrees of pathological changes in group C were more severe,and most of them were acute in-juries.Conclusions:Rat model of COPD established by inhalation of PM2.5 was more reasonable than that established by intra-tracheal instillation of PM2.5 suspension.
