Objective To investigate the acoustic characteristics of speech in Parkinson's disease(PD)pa-tients compared to healthy individuals using smartphone-recorded speech,laying the foundation for future intelligent cloud-based PD detection.Methods A total of 81 PD patients and 63 healthy controls were randomly selected to re-cord their speech using Huawei,Xiao mi,and Apple smartphones through the"Active Health"screening app from the National Key R&D Program.Using the Praat library in Python,parameters such as mean fundamental frequen-cy(Mean F0),minimum fundamental frequency(Min F0),maximum fundamental frequency(Max F0),formants(F1,F2,F3,F4),shimmer,noise-to-harmonics ratio(NHR),and jitter were extracted and compared.Results In PD patients,jitter and shimmer were significantly higher in the/ba/,/bi/,/bu/,and/bo/vowels compared to the control group,while Mean F0 and Max F0 was significantly lower in/bi/and/bo/.Min F0 was significantly higher in/bi/and/bu/for PD patients.No significant differences were found in F1,F2,F3,or F4 between PD pa-tients and controls in/ba/,/bi/,/bo/and/bu/.Conclusion Quantitative analysis of smartphone-recorded speech can detect speech changes in PD patients,providing a solid foundation for the development of a cloud-based PD de-tection system.

3.Global esophageal cancer epidemiology in 2022 and predictions for 2050: A comprehensive analysis and projections based on GLOBOCAN data.

BACKGROUND:
The burden of esophageal cancer varies across different regions of the world. The aim of this study is to analyze the current burden of esophageal cancer in 185 countries in 2022 and to project the trends up to the year 2050.
METHODS:
We extracted data on primary esophageal cancer cases and deaths from the GLOBOCAN 2022 database, which includes data from 185 countries. Age-standardized incidence rates (ASIR) and mortality rates (ASMR) per 100,000 person-years were calculated by stratifying by Human Development Index (HDI) levels and regions. Considering changes in population size and age structure, we assumed that the risks of incidence and mortality remain constant at the levels of 2022 to forecast the number of new cases and deaths from esophageal cancer globally by 2050.
RESULTS:
In 2022, an estimated 511,054 people were diagnosed with esophageal cancer globally, and 445,391 died from the disease. The global ASIR and ASMR for esophageal cancer were 5.00 and 4.30 per 100,000, respectively. The highest rates were observed in East Africa (7.60 for incidence, 7.20 for mortality per 100,000), East Asia (7.60 for incidence, 5.90 for mortality per 100,000), Southern Africa (6.30 for incidence, 5.90 for mortality per 100,000), and South Central Asia (5.80 for incidence, 5.50 for mortality per 100,000). Among the 185 countries worldwide, esophageal cancer was among the top five causes of cancer incidence in 18 countries and among the top five causes of cancer mortality in 25 countries. In 2022, China had 224,012 new cases and 187,467 deaths from esophageal cancer, accounting for approximately 43.8% and 42.1% of the global total, respectively, which is higher than the proportion of China's population to the global population (17.9%). ASIR was 8.30 per 100,000, and ASMR was 6.70 per 100,000. The highest burden of esophageal cancer was in high HDI countries, with new cases and deaths accounting for 51.3% and 50.0% of the global total, respectively. The ASIR and ASMR were highest in the high HDI group (6.10 and 5.10 per 100,000, respectively), also exceeding the global averages. There was a trend of decreasing mortality to incidence ratio with increasing HDI, but no correlation was observed between HDI and ASIR or ASMR. In all regions worldwide, the incidence and mortality rates were higher in males than in females (with a male-to-female ASR ratio ranging from 1.10 to 28.7). Compared to 2022, it is projected that by 2050, the number of new esophageal cancer cases will increase by approximately 80.5%, and deaths will increase by 85.4% due to population growth and aging.
CONCLUSIONS
The burden of esophageal cancer remains heavy. Adopting a healthy lifestyle, including reducing tobacco and alcohol intake, avoiding moldy foods, and increasing intake of fresh fruits and vegetables, can help reduce the risk of stomach and esophageal cancer. In addition, the development and implementation of evidence-based and effective public health policies are critical to reducing the global disease burden of esophageal cancer.

4.Nodal marginal zone lymphoma with elevated monoclonal IgM: report of 1 case and review of literature

Objective:To investigate the clinicopathological features, diagnosis, differential diagnosis and treatment of nodal marginal zone lymphoma (NMZL) with elevated monoclonal IgM.Methods:The clinical data of one NMZL patient with elevated monoclonal IgM treated at Yancheng No.1 People's Hospital in July 2020 were retrospectively analyzed, and the related literature was analyzed.Results:The patient was a 57-year-old female and the main clinical manifestations were fatigue and bone pain in left rib. Serum immunofixation electrophoresis showed IgM-κ type M proteinemia, bone marrow cytology showed a few plasmacytoid lymphocytes, bone marrow biopsy and immunohistochemistry showed B-cell non-Hodgkin lymphoma, bone marrow genetic testing showed MYD88 L265p and CXCR4 were both negative, postoperative pathology result of retroperitoneal lymph node biopsy was marginal zone lymphoma (mature small B type, prone to NMZL),and immunohistochemistry results: CD3, CD5, CD138, κ, λ, CD10, Cyclin D1 were negative, CD20, Pax-5, CD23 (FDC), bcl-2 were positive; Ki-67 positive index < 5%. The final diagnosis was NMZL with elevated monoclonal IgM. Partial remission was achieved after 8 cycles of reduced-dose CHOP regimen; thalidomide was used in the maintenance treatment, the disease condition was stable until August in 2021 and the follow-up was continuing.Conclusions:NMZL with elevated monoclonal IgM is relatively rare. Its diagnosis should be differentiated from Waldenstr?m macroglobulinemia and other inert B-cell lymphomas. Currently, there is no standard treatment and following the principle of individualized treatment can improve the prognosis of patients.
5.The Clinical Value of Artery Umbilical Cord Blood Gas in Predicting Neonate Condition: A Prospective Cohort Study

Objective::To evaluate the predictive ability of neonate condition through the traditional parameters and artery umbilical cord blood gas (aUCBG).Methods::A prospective cohort study was conducted in obstetrics and gynecology department between October 2017 and August 2018 at Tongji Hospital in Wuhan, China, and 360 aUCBG samples were collected. The average age of pregnant women was (29.50±4.42) years, range from 19 to 48 years old. The gestational age range from 28 +4 weeks to 41 +3 weeks at admission. Logistic regression and area under the curve (AUC) from Receiver operating characteristic curves were used to identify risk factors, such as, premature rupture of membranes (PROM), high blood pressure, premature delivery (PD), low 1-minute Apgar scores (Apgar 1), low 5-minute Apgar scores (Apgar 5), pH, base excess, bicarbonate, neonatal blood sugar (NBS), and so on, to predict neonatal condition and evaluate the predictive ability of traditional and aUCBG parameters. Results::In all cases, PROM, PD, Apgar 1, Apgar 5, pH, base excess, bicarbonate, total carbon dioxide, and neonatal blood sugar were risk factors and were associated with poor condition of neonate. Apgar 1 were an independent risk factor. Combined traditional and aUCBG parameters had higher AUC of 0.895 (95% confidence interval ( CI): 0.830-0.960, P<0.001). In cesarean section subgroup, high blood pressure, PD, and Apgar 1 were risk factors and were associated with poor condition of neonate. Apgar 1 and low pH were the independent risk factors. Combined traditional and aUCBG parameters had highest AUC of 0.940 (95% CI: 0.886-0.993, P<0.001). In vaginal delivery subgroup, maternal age above 35 years, PROM, PD, Apgar 1, Apgar 5, and male newborn were risk factors and were associated with poor condition of neonate. Maternal age above 35 years was an independent risk factor. Combined traditional and aUCBG parameters had highest AUC of 0.897 (95% CI: 0.828-0.965, P<0.001). For pregnant women without comorbidities and complications of pregnancy, aUCBG may not be necessary. Conclusion::In high-risk pregnancies, especially lower Apgar scores, PD, and maternal age above 35-year old, aUCBG is recommended. Traditional parameters combined with aUCBG might increase the predicting ability of neonate condition.

Objective:
To observe the effect of medical instant hemostasis gauze combined with filament speed instant gauze on the drainage and flap healing after modified radical mastectomy.
Methods:
From August 2015 to August 2016, a total of 80 patients with modified radical mastectomy for breast cancer admitted to Huanxing Tumor Hospital, Chaoyang District, Beijing were selected.According to the random number table method, 80 patients who were ready for modified radical mastectomy for breast cancer were randomly divided into study group (40 cases) and control group (40 cases). Two kinds of hemostatic materials (medical hemolytic hemostatic gauze combined with fibril quick hemostatic gauze) were applied to the surgical wounds in the study group during the operation, while no medical hemostatic materials were used in the control group during the operation, and the other treatment was the same as that in the study group.Total drainage volume and drainage tube removal time were compared between the two groups 1 to 5 days after operation.

8.Application of nursing quality evaluating indicators in gastrointestinal surgery department

Objective To evaluate the application of nursing quality indicators in gastrointestinal surgery department. Methods The indicator system was established by use of the Delphi method, applied in nursing quality control in gastrointestinal surgery department, and was used to analyze and improve nursing quality continuously according to the routine data collection. Results After the application of quality indicators for 6 months, the complications′ observation and prevention awareness rate of the patients increased from 75.00% to 92.00%(x2=4.200, P<0.05), and most of the process indicators had improved. Conclusions The nursing quality indicators provide basis for nursing quality management,and provide standards and guidance for clinical works.
9.Effects of Losartan on the regulation of the expression of myocardial AT_(1a)mRNA,AT_(1b) mRNA from renovascular hypertensive rats

CONCLUSION Losartan has antihypertensive effects and can reverse myocardial hypertrophy induced by renovascular hypertension. Losartan regulates the mRNAs of the AT 1 receptor subtypes differently, which suggests it possibly has selectivity for AT 1a or AT 1b .
