1.Construction of a self-management scheme for osteoporosis related to endocrine therapy in patients with early-stage breast cancer

Objective:To construct a self-management scheme for osteoporosis related to endocrine therapy in patients with early-stage breast cancer, so as to provide a reference for clinical nursing intervention.Methods:In the chronic disease self-management domain theory as the instruction, using literature research, the semi-structured interview to build the first draft of the plan, through the expert meeting law to plan to revise the first draft, and form a self-management scheme for osteoporosis related to endocrine therapy in patients with early-stage breast cancer.Results:A total of 10 experts were included, aged 37-47 (42.70 ± 3.43) years. The recovery rate of the expert questionnaire was 10/10, the expert authority coefficient was 0.93, and the coefficient of variation of each item ranged from 0.06 to 0.19. The final formed self-management scheme for osteoporosis related to endocrine therapy in patients with early-stage breast cancer includes three intervention modules: self-prevention, self-monitoring, and self-coping. It encompasses six intervention themes: enhancing knowledge reserves, non-pharmacological prevention, pharmacological prevention, risk assessment and self-reporting of symptoms, non-pharmacological intervention, and pharmacological intervention.Conclusions:The construction of the self-management scheme for osteoporosis related to endocrine therapy in patients with early-stage breast cancer is of great significance. The content of the scheme is scientific, reliable and targeted, which can provide guidance for the self-management of osteoporosis in patients with early breast cancer during endocrine therapy.

3.First aid cardiopulmonary resuscitation at the exit of capsule and strategy coping with the "Shenzhou 11"spacecraft

Objective To summarize the medical security experience in first-aid and resuscitation for astronauts at the exit of capsule after the spacecraft returns to the main landing site in the process of human spaceflight in China,and thus to provide a powerful security measures for Chinese aerospace medicine.Methods The medical support experiences were summarized in human spaceflight from "Shenzhou V" to "Shenzhou X",relevant reports on emergency rescue and resuscitation were consulted in in-orbit process and after emergency return and landing for domestic and foreign astronauts,astronauts' physiological changes in cardiopulmonary resuscitation were analyzed during emergency return,and then,corresponding strategies were proposed and tested in practice (actual combat) by combining with the flight characteristics of the spacecraft "Shenzhou XI".Results On the basis of the original emergency treatment,the countermeasures for the cardiopulmonary resuscitation were proposed after the spacecraft returned to the main landing site in human spaceflight,the emergency equipment was adjusted,the emergency procedures were optimized,and anti-fog glidescopes were added,laryngeal masks were introduced to perform supraglottic ventilation as the quickest and most effective airway opening measure on site.In addition,ultrasound examination was applied in practice as an important treatment and assessment method for basic life support and advanced life support.All these could ensure the rescuing ability on cardiopulmonary resuscitation during their stay in space for the medium-term and after their return to the main landing site.Conclusions During the return of the astronauts of the spacecraft "Shenzhou XI" to the main landing site,the first aid and support program had been improved specifically and the process had been optimized to ensure the successful completion of medical security mission of China's human spaceflight.
4.Preliminary study on therapeutic effect of Baimai Ointment on stroke and blood circulation mechanism

Objective To observe the preventive and therapeutic effect of Baimai Ointment on stoke,and explore the principle of promoting blood circulation and removing blood stasis.Method By dermal administration,the crude drug contents of Baimai Ointment given to rats were 0.075,0.150,and 0.300 g/kg respectively.Focal cerebral ischemic model was induced by the middle cerebral artery occlusion (MCAO),and the preventive and therapeutic effect of Baimai Ointment was evaluated.The degree of hematoma absorption in rats was measured to observe the effect on promoting blood circulation and removing blood stasis of Baimai Ointmnent.The acute blood stasis model and carotid thrombosis model were established in rats to study the effects on blood viscosity and thrombosis time.Results Baimai Ointment had no significant prevent effect on stroke in rats.It effectively improved behavior coordination in stroke model rats.It also significantly decreased the area of hematoma on rats,speeded up the repairment of local demage.After administration of Baimai Ointment,blood viscosity of blood stasis model rats was obviously decreased,while thrombosis time was effectively prolonged on carotid thrombosis model.Conclusion Baimai Ointment can significantly improve the behavior coordination in stroke model rats,plays a significant role in treatment,but no significant preventive effect,a preliminary study shows that the possible therapeutic principles may be promoting blood circulation and removing blood stasis.
5.Experience in first-aid and rescue for the ‘SHENJIU' astronauts in the landing site

Objective To discuss the experience of medical treatment and rescue of the ‘SHENZHOU' astronauts in the landing place in the past 10 years,and to establish more effective mobile ICU in medical helicopters to ensure ‘SHENJIU' astronauts safety.Methods The data collected from foreign nations and our country was reviewed.Formerly experience in first-aid and rescue astronauts of our team was summerized.The important reasons of accidental injuries of astronaut during aerospace flight were listed.More effective and reasonable prophylactic measurements and clinical treatments of the accidental injuries of astronauts were brought forward.Results we established three effective mobile ICU that could ensure the safety of the astronauts.The carriers of the ICU were helicopters,and damage control surgery can be performed there.Fine armature made us more effective.Conclusions Reasonable and effective prophylactic measurements and clinical treatments were the important aspect of the successful manned aerospace flight.The first-aid system in helicopters could realize the destination of swift response and first-aid.
6.MR imaging diagnosis of shoulder joint injury

Objective To analyze the imaging characteristics and the diagnostic value of conventional MRI and indirect MR arthrography in shoulder joint injury.Methods MR imaging in90 patients with shoulder joint disease were retrospectively analyzed by two musculoskeletal radiologist based on the results of arthroseopic examination,including 57 cases of rotator cuff disease and 33 cases of recurrent shoulder instability.The result of the indirect MR arthrography and conventional MR was statistically compared by Fisher exact probability test.Results (1)In the group of rotator cuff disease,37 patients underwent indirect MRA,demonstrating 10 cases of partial-thickness rotator cuff tear and 1 case of missed diagnosis;demonstrating 17 cases of full-thickness rotator cuff tear and 2 cases of misdiagnosis.For partialthickness rotator cuff tear,the sensitivity,specificity and accuracy of indirect MR arthrography were 90.9%(10/11),92.3%(24/26),and 91.9%(34/37),respectively;for full-thickness rotator cuff tear,the sensitivity,specificity and accuracy of indirect MR arthrography were 89.5%(17/19),94.4%(17/18),and 91.9%(34/37),respectively.Indirect MR arthrography in the diagnosis of partial and full thickness rotator cuff was significantly better than conventional MR(P<0.05).(2)In the group of recurrent shoulder instability,labium tear was found in 26 cases,Hill-sachs lesion in 27,reverse Hill-sachs in 2,bony Bankart lesion in 7,and capsular tear in 18 cases,respectively.Twenty patients underwent indirect MRA,demonstrating 14 cases of labrum tear,1 case of missed diagnosis,and 5 cases of normal labrum.For labrum tear,the sensitivity,specificity and accuracy of indirect MR arthrography were 93.3%(14/15),100.0%(5/5),and 95.0%(19/20),respectively.Indirect MR arthrography in the diagnosis of labrum tear was significantly better than conventional MR(P<0.05).Conclusion MRI,especially indirect MR arthrography,is the most helpful method for the diagnoses of shoulder joint injury.
7.Comparative Study of MR Imaging in Diagnosis of Shoulder Instability

Objective To evaluate the diagnostic value of indirect MR arthrography(MRA) and conventional MR for shoulder instability.Methods 24 cases of shoulder instability were retrospectively analyzed.13 patients underwent conventional MR,11 patients underwent indirect MRA.Based on the results of arthroscopic examination,the efficacy of indirect MRA images and conventional MR images in diagnosis of the labrum,the capsule, and the bone structure injury.Results Indirect MRA in diagnosis of labrum tear was signifiantly better than conventional MR(P0.05).Conclusion Indirect MRA is more helpful to diagnose the labrum tear than conventional MR.Further study should be taken for capsule and ligament injury.
