1.Study of Correlation between TCM Inspection and Coronary Heart Disease Symptoms and TCM Syndrome Types

Objective To explore the correlation between TCM inspection and coronary heart disease(CHD)symptoms and TCM syndrome types.Methods Totally 336 patients with CHD or non-CHD were included in Guangdong Provincial Hospital of Traditional Chinese Medicine.A self-developed scoring standard for TCM inspection was used to collect information on TCM inspection and related symptoms and signs,and the correlation between TCM inspection and age,symptoms and TCM syndrome types was explored.Results The positive rates of Frank's sign,skin fold in nasal root(SFNR),poker face(PF),sublingual meridians and TCM comprehensive inspection increased with age(P<0.05);However,there was no significant difference in the distribution of positive and negative groups in different age groups,including hand inspection,pulse diagnosis,vertical fold between eyebrows(VFE),greasy tongue coating and dark tongue texture(P>0.05).The subjects in positive group of hand inspection,Frank's sign,PF,pulse diagnosis,dark tongue texture and TCM comprehensive inspection were more prone to suffer from chest pain than those in the negative group(P<0.05);as the score increased,the proportion of chest pain increased.The Frank's sign positive group is more prone to have palpitations than the negative group(P<0.05);the PF and pulse diagnosis positive group were more prone to have dizziness than the negative group(P<0.05);the main TCM syndrome types in the positive group of TCM inspection were yang qi deficiency combined with phlegm dampness and stasis obstruction.Conclusion The subjects in positive group of TCM inspection were more prone to suffer from chest pain,palpitations and dizziness than those in the negative group.The main TCM syndrome types in the positive group were yang qi deficiency combined with phlegm dampness and stasis obstruction.

How distinct transcriptional programs are enacted to generate cellular heterogeneity and plasticity,and enable complex fate decisions are important open questions.One key regulator is the cell's epigenome state that drives distinct transcriptional programs by regulating chromatin acces-sibility.Genome-wide chromatin accessibility measurements can impart insights into regulatory sequences (in)accessible to DNA-binding proteins at a single-cell resolution.This review outlines molecular methods and bioinformatic tools for capturing cell-to-cell chromatin variation using single-cell assay for transposase-accessible chromatin using sequencing (scATAC-seq) in a scalable fashion.It also covers joint profiling of chromatin with transcriptome/proteome measurements,computational strategies to integrate multi-omic measurements,and predictive bioinformatic tools to infer chromatin accessibility from single-cell transcriptomic datasets.Methodological refinements that increase power for cell discovery through robust chromatin coverage and integrate measure-ments from multiple modalities will further expand our understanding of gene regulation during homeostasis and disease.
4.Tissue expansion in treatment of facial congenital giant nevus

Objective To investigate the effect of tissue expansion in facial congenital giant nevus.Methods From October 2014 to October 2016,7 cases of facial congenital giant nevus patients were treated with expanded skin flaps,including single expansion and secondary expansion.Partial excision and skin tissue expansion were used to drastically remove giant pigmented nevi on body or extremities of infants.There were some points for attention during the surgery:the size and location of the partial excision and expander should be designed reasonably;no tumor-manipulation and non-tension principle should be taken great notice,and expanders should be linked up with one another when several expanders were used at the same time,so as to reduce the possibility of forming the envelop,which was favorable for the second surgery of transfer of a skin flap.Results All cases of giant pigmented nevi excision were on the face,which were completely removed from the patients.The areas of the nevi were from 7 cm × 5 cm to 15 cm × 13 cm.All patients were followed up for 3-12 months (mean 6 months).All flaps survived well.Expander exposure was happened in 2 cases with no bad results.One case of scar received late repair due to surgical scars and all got satisfactory results.Conclusions Tissue expansion is an ideal method in the treatment of facial congenital giant nevus.
5.Determination test of the leakage rate and over-expansion property of the expander

Objective To measure the leakage rate in the process of expansion in vivo and in normal saline and the over-expansion property of the expander.Methods We chose 17 cases that were treated with the skin expansion.We recorded total injected saline volume intentionally in these cases before the second surgery,and recorded the remainder of the volume of saline postoperatively.We injected normal saline into 8 expanders,respectively,exceeding 50% of the volume-rating;and subsequently put these expanders into plastic bucket filled with saline.30 days after,we measured the remaining volume of the normal saline.We injected 100 ml normal saline into 4 expanders,respectively,of which nominal volume was 100 ml,and then left these expanders in saline and measured the remainder volume of the saline 30 days later.In the second time,we injected 150 ml and repeated the abovementioned process.It was repeated 7 times until the injection volume reached 400 ml.We processed the data and depicted a curve of effective expansion.Results The results showed that the leakage rate reached (29.0+12.5) % in vivo.Experiments in vitro confirmed that 85.0% (8.0/9.4) of the saline leaked through the injection port and 14.8% (1.4/9.4) leaked through the membrane of the expander.In addition,over-expansion performance index test confirmed that the over-expansion property of an expander was about twice the nominal volume.Conclusions The expander is not completely sealed structure.Normal saline can leak through the injection part and the membrane of the expander.The over-expansion property of an expander is limited.When the volume injected into the expander exceeds a certain value,the effective expansion performance is not increased with it.
6.Clinical evaluation of guiding apparatus assisted individual posterior cervical pedicle screw fixation

ObjectiveTo evaluate the clinical effects of guiding apparatus assisted individual posterior cervical pedicle screw fixation technique.MethodsThe study enrolled 24 patients treated with posterior cervical decompressive single open-door laminoplasty and transpedicular screw-rod fixation from January 2008 to December 2010.The point of screw penetration and screw path direction were confirmed by measuring the transverse nail angle (TNA) and sagittal nail angle (SNA) of nail channel on the pre-operative CT plain scan of cervical pedicle and sagittal two-dimensional imaging of transpedicular axis.According to the results of CT measurement,individual cervical pedicle screw was implanted with the assistance of self-made guiding apparatus and then fixed after decompression.Transverse screw angle ( TSA ) and sagittal screw angle ( SSA ) were determined on the CT scan of cervical pedicle and sagittal two-dimensional imaging of transpedicular axis one week postoperatively in order to analyze the accuracy of placement of pedicle screws.Periodical anteroposterior and lateral X-ray radiographs of cervical vertebra were taken postoperatively to detect the stabilization of internal fixation.ResultsA total of 223 pedicle screws were inserted successfully into the C3 ~ C7,of which 220 crews were inserted accurately but three had slight inclination according to the postoperative CT,with placement accuracy of 98.7%.The comparison between inclination angle of inserted screws and that of preoperative transpedicular axis showed insignificant statistical difference ( P > 0.05 ).All the patients were followed up for 6-34 months ( mean,18.5 months),which showed no neurovascular complications related to screws perforation out of pedicle cortex or no screw loosening,prolapse or breakage.ConclusionPosterior cervical pedicle screw insertionperformed according to the individual CT measurement is easy and safe and has a high accuracy rate under the assistance of self-made guiding apparatus.
7.Role of three vessels and trachea view in ultrasonic diagnosis of fetal heart disease and great artery abnormality

Objective To evaluate the role of three vessels and trachea view (3VT) in diagnosis of fetal heart disease and great artery abnormality.Methods 39 pregnant women undergoing fetal ultrasound scan and diagnosed with great artery abnormality were enrolled in this study.Then the image characteristics and significance of 3VT in these 39 cases were analyzed and evaluated.Results In these 39 cases proved by autopsy,persistent truncus arteriosus(7 cases),transposition of the great arteries(7 cases),coarctation of the aorta(5 cases),tetralogy of Fallot (4 cases),pulmonary stenosis (3 cases),hypoplastic left ventricle syndrome(2 cases),persistent left superior vena cava (2 cases),pulmonary valve stenosis with single ventricle(2 cases),Ebstein anomaly with pulmonary stenosis (1 case),interruption of the aortic arch( 1 case),hypoplastic right ventricle(1 case),dextroaortic arch (1 case),and ductus arteriosus stenosis( 1 case) were detected.All of them had anomalies performance on 3VT.While in the 2 cases of double outlet right ventricle,only one eoncurrenting with pulmonary artery stenosis showed reduced inner diameter of pulmonary artery on 3VT.According to the performance of 3VT in great artery abnormality,it could be divided into 4 categories:abnormal location,abnormal ratio,abnormal amount and abnormal blood direction of great artery.Conclusions 3VT is a useful supplement to the 4-chamber view,and has significant value in diagnosis of fetal heart and great arteries abnormities.
8.Expression of κ1 opioid receptor and ultrastructure changes in heart atrium from the patients with persistent atria fibrillation

Objective To study the effects of persistent atrial fibrillation (AF) on the expression of k1 opioid receptor and change of ultrastructure in heart atrium.Methods The mRNA and protein expressions of κ1 receptor in atrial tissue were detected with RT-PCR and immunochemistry,respectively,in 24 patients with AF and 24 patients with sinus rhythm (SR).Mitochondria were analyzed in the atrial tissue by electron microscopy in 4 patients with AF and 4cases with SR.Results The κ1 receptor mRNA in patients with AF was lower than in patients with SR (262±20 vs.196± 11,P<0.05),and amount of protein expression was also deduced (1261±90vs.2325± 131,P<0.05).Mitochondria size was decreased in persistent AF compared with SR[(1.0±0.2) μm vs.(0.8±0.2) μm,P<0.05].Conclusions The decrease of k1 receptor expression suggests reduction of atrial protective capacity in persistent AF,combining with remodeling of ultrastructure.
9.The correlation between the type of hypospadias and external genital system malformations

Objective To investigate the correlation of the type of hypospadias and external genital system malformations. Methods From Jan 2000 to Oct 2009, the data of 632 patients diagnosed with hypospadias was analyzed retrospectively. The proportion of all types of hypospadias and the frequency of external genital system malformations were statistically analyzed. Results Among all cases, 12.6% exhibited mild hypospadias, 22. 8% moderate, 43. 2% severe and 21.4% had extremely severe hypospadias. One hundred and eighty-four cases with external genital system malformations were classified into penoscrotal transposition, cryptorchidism, hydrocele of tunica vaginalis,concealed penis and pseudovagina. The probability was about 24.8% (157 cases) of 632 patients with hypospadias. There was significant difference and a positive correlation among the type of hypospadias with total malformations, penoscrotal transposition, cryptorchidism. Conclusions In this patient cohort, moderate and severe hypospadias accounted for the majority of hypospadias. Penoscrotal transposition was the most frequent external genital system malformation in hypospadias, followed by cryptorchidism and hydrocele of tunics vaginalis. More severe hypospadias is accompanied with a higher probability of the external genital system malformations, penoscrotal transposition and cryptorchidism.
10.Treatment of dens fracture with cannulated screw fixation under locational guidance

Objective To evaluate the clinical effect of the cannulated screw fixation in treatment of the dens fracture under locational guidance. Methods The study involved 27 patients treated with the cannulated screw fixation under locational guidance from January 2005 to January 2009.There were 19 patients with type II fracture and eight with type light m fracture.The lateral and open mouth position X-ray examination of the cervical ventebrae was done to observe the fracture healing. Results The operation lasted for a range of 40 minutes to 1.3 hour (average 1.0 hour),which showed no any complications.The patients were followed up for average 6.5 months(3-12 months),which showed sound fracture healing in 26 patients but nonhealing in one. Conclusion The cannulated screw fixation for treatment of the dens fracture under locational guidance is characterized by easy operation,minor trauma and satisfactory outcome.
