1.Multicenter retrospective analysis of the efficacy of neoadjuvant combined with adjuvant therapy in intrahepatic cholangiocarcinoma

Background and Aims:Intrahepatic cholangiocarcinoma(ICC)is a highly malignant liver tumor,with an increasing incidence worldwide,particularly in Asia.Although radical surgical resection is currently the only potentially curative treatment,the high recurrence rate and low postoperative overall survival(OS)rate of ICC remain major clinical challenges.Adjuvant therapy(AT)and neoadjuvant therapy(NAT)are important strategies to reduce postoperative recurrence and prolong OS.Several studies have shown certain efficacy of these treatments.However,the specific efficacy and safety of combined NAT and AT in ICC treatment require further validation.This study was conducted to evaluate the value of combining NAT and AT in improving the therapeutic outcomes of ICC patients through a multicenter retrospective analysis,so as to provide scientific evidence for optimizing treatment strategies.Methods:The clinicopathologic data of 576 patients with ICC who underwent radical resection and were pathologically confirmed from 13 hospitals in China between December 2011 and December 2017 were retrospectively collected.Patients were grouped based on their treatment modality:NAT+AT group,AT group,and non-NAT/AT group.The three patient groups were matched pairwise in a 1∶1 ratio using propensity score matching(PSM)to balance baseline data.The Kaplan-Meier method was used to analyze OS and disease-free survival(DFS),and subgroup analyses were conducted according to the 8th edition of the AJCC TNM staging system.Results:A total of 395 ICC patients were included in the final analysis,with 42 patients(10.6%)in the NAT+AT group,62 patients(15.7%)in the AT group,and 291 patients(73.7%)in the non-NAT/AT group.Before PSM,significant differences were observed between groups in terms of CA19-9,liver function Child-Pugh classification,intraoperative blood loss,surgical margin,differentiation grade,vascular invasion,ECOG score,and lymph node dissection ratio(all P<0.05).After PSM,there were no significant differences in baseline characteristics between the groups(all P>0.05).After matching,the median OS and DFS in the NAT+AT group were significantly better than in the AT and non-NAT/AT groups(both P<0.05),while there were no significant differences in OS and DFS between the AT and non-NAT/AT groups(both P>0.05).Subgroup analysis showed that in TNM stage I patients,DFS in the NAT+AT group was significantly better than in the non-NAT/AT group(P<0.05),but OS was not significantly different(P>0.05).In TNM stage Ⅱ and Ⅲ patients,both OS and DFS in the NAT+AT and AT groups were significantly better than in the non-NAT/AT group(both P<0.05),and DFS in the NAT+AT group was significantly better than in the AT group in TNM stage Ⅲ patients(P<0.05).Conclusion:NAT combined with AT provides better survival benefits for patients with locally advanced ICC,but its benefit for early-stage ICC patients is limited.However,the retrospective design and sample size limitations of this study may affect the stability of the results,and future large-sample,multicenter,prospective studies are needed for further validation.
2.Risk factors analysis and risk prediction model of anxiety and depression in patients with prostate cancer after castration

Objective:To analyze the risk factors of anxiety and depression in prostate cancer patients after castration, and establish a risk prediction model.Methods:A retrospective analysis was conducted on the data of 60 prostate cancer patients treated in Xi′an People′s Hospital from January 2019 to January 2022. The patients were divided into a training group ( n=42) and a validation group ( n=18) at a ratio of 7∶3. The patients received surgical castration and medical castration. One month after castration, the Self-Rating Anxiety Scale (SAS) and Self-Rating Depression Scale (SDS) were used to evaluate the anxiety symptoms and depression levels of the patients, respectively. Univariate and multivariate logistic regression analyses were used to identify the risk factors for negative emotions in prostate cancer patients after castration, and a risk prediction model was established. Results:In the training group, 19 patients had a SAS score ≥50, and 21 patients had an SDS score ≥50. Based on these scores, the training group was divided into a negative emotion group ( n=19) and an emotional stability group ( n=23). Multivariate logistic regression analysis showed that marital status, castration scheme, and postoperative Visual Analogue Scale (VAS) score were independent influencing factors for negative emotions in prostate cancer patients after castration ( OR=3.589, 3.364, 5.912, all P<0.05). In both the training group and the validation group, the risk scores of patients with negative emotions were significantly higher than those with emotional stability. In the training group, the area under the curve (AUC) of the risk prediction model for predicting negative emotions was 0.747, with a specificity of 71.02% and a sensitivity of 66.11%; in the validation group, the AUC, specificity, and sensitivity were 0.761, 66.59%, and 76.21%, respectively. The Hosmer-Lemeshow test showed that χ 2 was 4.285 6, P value was 0.830, and the c-index was 0.773(0.692-0.854). The calibration curve showed that the predicted curve was basically consistent with the actual curve, indicating that the prediction model had good discriminative ability and accuracy. Decision curve analysis showed that the model had high clinical significance. Conclusions:Marital status, castration scheme, and postoperative VAS score are important factors affecting anxiety and depression in prostate cancer patients after castration, and the regression model can successfully predict the risk of negative emotions.
3.Preliminary application of domestic single-port serpentine arm robotic surgical system in children's pyeloplasty

Objective:To investigate the safety and feasibility of the domestic single-port serpentine-arm robotic surgical system for pyeloplasty in children with congenital ureteropelvic junction obstruction(UPJO).Methods:Data of UPJO patients who underwent pyeloplasty using a domestic single-port ser-pentine-arm robotic surgical system(Beijing Surgerii Robotics Co.,Ltd.)in Beijing Children's Hospital from November 2023 to February 2024 were retrospectively collected.The patients who were not receiving surgical treatment for the first time,had hydronephrosis caused by other reasons(such as ureterovesical junction obstruction,posterior urethral valve,urinary tract stones,vesicoureteral reflux,ureterocele,etc.),had other urinary tract malformations(such as duplicated kidneys,congenital renal dysplasia,etc.),had severe atrophy of the affected kidney,severe urinary tract infection or severe renal insufficien-cy were excluded.All the surgeries were performed through the umbilicus and abdominal cavity,and the operation time,number of intraoperative incisions,incision size,intraoperative blood loss,and peri-operative complications were recorded.Statistical analysis was performed to compare changes in the an-teroposterior pelvic diameter(APD)and renal cortical thickness before surgery and 6 months postopera-tively.Results:A total of 10 patients were included(8 males and 2 females),with an average age of(10.20±3.12)years.Nine patients were on the left side and one patient was on the right side.The average height was(142.0±17.8)cm and the average weight was(37.6±17.9)kg.All the patients underwent surgery using the domestic single-port robotic surgery system,and no patient was converted to open pyeloplasty.The total operation time was(237±96)min,and the operation time on the operating table was(162.0±69.3)min.The intraoperative blood loss was 5.00(2.25,5.00)mL.No compli-cations,such as bleeding,urine extravasation,fever,and poor wound healing occurred during the perioperative period.Compared with the preoperative measurements,the APD was significantly shortened postoperatively(P=0.005),and the renal cortical thickness significantly increased(P=0.011).Con-clusion:The domestic single-port serpentine arm robotic surgical system is safe and feasible for UPJO pyeloplasty in children,with good surgical results,and can be promoted and applied in most domestic medical centers.
4.Research on objective evaluation of pilot ground simulation ejection training based on pressure detection

Objective In response to the practical issues of pilot ground simulation ejection training relying on subjective evaluation,which requires high professionalism and lacks objectivity and systematicity,an objective evaluation system and method for ejection training based on pressure detection has been designed.Methods According to the rules of ejection training about posture,maneuver and time,and utilizing technologies such as pressure detection,infrared detection,and time-triggered detection,this paper designed separate modules for ejection posture detection,ejection timing detection,and comprehensive analysis and evaluation.These modules are closely integrated with the ejection training process,achieving objective evaluation of the ejection training.Results This system and method can provide a scientific and real-time objective evaluation of the posture,movements,and time in ejection training without affecting the normal organization and process.Conclusion The objective evaluation system can provide a new approach that is highly objective and easy to operate for comprehensively evaluating and enhancing the effectiveness of ejection training,as well as for scientifically conducting ejection training and assessment.
5.Shear Wave Elastography in Evaluating the Therapeutic Effect of Acupuncture in Diabetic Peripheral Neuropathy

Purpose To explore the value of shear wave elastography(SWE)in evaluating the therapeutic effect of acupuncture in patients with diabetic peripheral neuropathy(DPN).Materials and Methods Sixty patients with type 2 DPN treated at the Department of Endocrinology and Preventive Treatment Center in the First Affiliated Hospital of Nanchang University from February 13,2022 to June 3 2023 were selected prospectively,and were divided by random number table into group A(30 cases)who received hypoglycemic plus mecobalamin tablets,and group B(30 cases)who received acupuncture in addition to the treatment given to group A.Routine ultrasound and SWE were conducted to measure the diameter,cross-sectional area and Young's modulus(E)of the median nerve and tibial nerve before and after treatment.Electrophysiological examination was also performed to obtain motor nerve conduction velocity and sensory nerve conduction velocity of the median nerve and tibial nerve before and after treatment.Additionally,a control group comprising 30 healthy adults was included as the control group,and the above ultrasonic parameters were obtained.The changes in the above parameters before and after treatment were compared among the groups,and the effectiveness of acupuncture was assessed based on electromyography results.Furthermore,a receiver operating characteristic curve was constructed for the difference in E(ΔE)of the median nerve and tibial nerve.Results There were no significant differences in diameter,cross-sectional area and E of the median nerve and tibial nerve between group A and group B before treatment(P>0.05),but they were higher than those in control group(P<0.05,P<0.001).After treatment,the E values of the median nerve and tibial nerve in group B were lower than those in group A(P=0.036,0.025),and the E value in both group A and B remained significantly higher than those in control group(all P<0.001).There were no significant differences in diameter and cross-sectional area of the median nerve and tibial nerve among group A,B and control group after treatment(F=0.824,0.176,0.106,0.982,P>0.05).The E values of the median nerve and tibial nerve in group A and group B were significantly lower after treatment compared with those before treatment(group A:t=5.422,4.429,both P<0.001;group B:t=6.110,7.081,both P<0.001).The area under the curve,sensitivity,specificity and optimal cutoff value for ΔE of the median nerve and tibial nerve were 0.920,95%,90%,4.600 kPa;and 0.820,85%,70%,6.250 kPa,respectively.However,there was no significant difference in diagnostic efficiency(Z=1.489,P=0.136).Conclusion SWE can provide information regarding the elastic modulus of peripheral nerves and evaluate nerve hardness,it offers a non-invasive and effective method for evaluating the therapeutic effect in DPN patients.

7.Dilation effect of luteolin in rat thoracic aortic rings based on Kv7 ion channels

Objective To investigate the vasodilatory effect of luteolin on isolated denuded-endothelium rat thoracic aorta(DRTA)vascular rings,and the mechanistic role of the Kv7 ion channel.Methods The tension of DRTA vascular rings was measured using an ex vivo tissue perfusion muscle-tone detection system.DRTA vascular rings were pre-contracted with 60 mmol/L KCl or 0.3 μmol/L U46619,and the effect of luteolin on vascular-ring relaxation was observed at 1,3,10,30,100 and 300 μmol/L.The effects of 4-AP,XE-991,and ML213 on luteolin-induced vasodilation were also observed.The effect of luteolin on KCNQ1-KCNQ5 expression in the thoracic aorta was detected by real-time fluorescence quantitative polymerase chain reaction.Expression levels of Kv7.1 and Kv7.4 proteins in the DRTA were detected by Western blot.Results(1)The luteolin-induced maximum vasodilation rates in DRTA pre-contracted with 60 mmol/L KCl and 0.3 μmol/L U46619 were(97.67±8.51)%and(98.42±9.76)%,respectively.The vasodilation effect was concentration-dependent(P<0.05).(2)4-AP(3 mmol/L)significantly reduced the vasodilatory effect of luteolin on DRTA vascular rings at 10,30,and 100 μmol/L(P<0.05),and XE-991(3 μmol/L)significantly reduced the effect of luteolin at 30 and 100 μmol/L(P<0.05),while ML213(1 μmol/L)significantly enhanced the vasodilatory effect of luteolin at 3,10,and 30 μmol/L(P<0.05).(3)The relative gene expression levels of each subtype of Kv7 channel in normal DRTA were KCNQ1>KCNQ5>KCNQ4>KCNQ3>KCNQ2,with KCNQ1 being the most highly expressed.(4)Luteolin significantly enhanced the expression levels of KCNQ1 at 3,10,30,and 100 μmol/L,KCNQ2 at 1,3,10,30,and 100 μmol/L(P<0.05),KCNQ3 at 3,10,30,and 100 μmol/L,and KCNQ4 at 10,30,and 100 μmol/L(P<0.05),but did not significantly enhance the expression of KCNQ5 at 1,3,10,30,or 100 μmol/L(P>0.05).(5)Luteolin significantly increased the expression of Kv7.1 protein in DRTA at 3,10,30,and 100 μmol/L(P<0.05)and the expression of Kv7.4 at 10,30,and 100 μmol/L(P<0.05).Conclusions Luteolin-induced dilation of DRTA vascular rings may be related to the enhanced gene expression of KCNQ1-4 and increased expression of Kv7.1 and Kv7.4 channel proteins.

9.Two case reports of pediatric urinary non-Hodgkin lymphoma

Pediatric urological non-Hodgkin lymphoma is relatively rare.When a single renal or ureteral tumor with multiple lymph node or bone metastases is present, it is very easy to be misdiagnosed as urinary tumor with metastasis, resulting in unnecessary radical nephrectomy. This paper reports two cases of non-Hodgkin lymphoma presenting with single urinary nodules. Case 1 was diagnosed with a 4.5cm tumor in the right kidney due to fever and fatigue. CT and PET-CT showed multiple bone destruction and lymph node metastasis. Renal biopsy was performed and ALK-positive anaplastic large cell lymphoma was diagnosed. Chemotherapy was effective for 6 months. Case 2 was diagnosed with left hydronephrosis due to intermittent low back pain for 2 weeks. Ultrasound examination revealed ureteral dilation with left hydronephrosis and low echo occupying of the lower ureter. The patient had sudden blurred vision before surgery. MRI examination revealed space occupying in the right optic nerve frame, and PET-CT found multiple bone, lymph nodes and nerve invasion. A lymph node biopsy was performed and Burkitt lymphoma was diagnosed. After 6 months of chemotherapy and radiotherapy, the tumor shrank significantly.
