1.Impact of 5G remote robotic cholecystectomy on postoperative rehabilitation and quality of life in patients with gallstones

Objective:To evaluate the effect of 5th generation mobile communication technology(5G) remote robot-assisted cholecystectomy on postoperative recovery and quality of life in patients with gallstones.Methods:Clinical data of patients who underwent 5G remote robotic cholecystectomy(20 cases) vs traditional robotic cholecystectomy(20 cases) at Gansu Provincial People's Hospital from May to September 2023 were retrospectively analyzed. Intraoperative and postoperative related indexes were compared between the two groups, and changes in pain analog scores before, 12 hours, and 24 hours after surgery, and global quality of life (GIQLI) scores before and 3 months after surgery were evaluated.Results:There was no significant difference between the two groups in terms of intraoperative and postoperative conditions (intraoperative blood loss, operative time, time to exhaustion, and length of hospital stay). Patients' pain analog scores at 12 and 24 hours postoperatively were lower than preoperatively, but the differences were not statistically significant( P>0.05). In addition, there was no significant difference in recovery indicators between the two groups compared to the control group ( P>0.05). Conclusion:The postoperative recovery metrics of 5G tele-robotic-assisted cholecystectomy were consistent with the results of conventional robotic surgery.

3.Robotic-assisted"3+1"Surgical Mode Strategy for 6 Cases of Median Arcuate Ligament Syndrome

Objective To investigate the feasibility of robotic-assisted"3+1"surgical mode strategy for the treatment of median arcuate ligament syndrome(MALS).Methods Six patients diagnosed as MALS were enrolled between June 2019 and October 2023.The utilization of robotic-assisted"3+1"surgical mode(a 4-hole operation with 3 robot arms and 1 assistance hole)was carried out.Through a posterior splenic artery approach,the compression of the midline arcuate ligament on the abdominal trunk was relieved,which was an abdominal trunk vascularization.Results All the 6 patients underwent robotic-assisted"3+1"surgical mode successfully.The surgical duration(including the time for connecting and disconnecting the machine)was 38-52 min,with a mean of 43.2 min.The intraoperative bleeding volume was 5-25 ml,with a mean of 15 ml.The postoperative hospital stay was 6-9 d,with a mean of 7.5 d.All the 6 patients were followed up for 1-45 months after surgery(median,2.5 months).One patient remained unchanged in body weight,while 5 patients gained 1.0-2.0 kg in body weight,with a mean of 1.5 kg.The symptom of postprandial epigastric pain completely resolved in 5 patients,while the symptom was reduced but not completely alleviated in one patient.Conclusion Treatment of MALS with the robotic-assisted"3+1"surgical model involving abdominal trunk vascularization through a posterior splenic artery approach is safe and feasible.
4.The experience on trocar layout method for robotic "3+2" mode gastrointestinal surgery

Objective:Based on experience of robotic gastrointestinal surgery at the Department of General Surgery, Clinical Medicine Center of Gansu Provincial Hospital, this study explored the principles and methods of trocar layout for robotic "3+2" mode gastrointestinal surgery, suitable for beginners.Methods:From Apr 2017 to Oct 2022, the robotic gastrointestinal surgery team of Gansu Provincial Hospital completed 998 cases of robotic "3+2" mode gastrointestinal surgery, including 600 cases of gastric cancer, 100 cases of rectal cancer, 98 cases of descending colon and sigmoid colon cancer, 20 cases of transverse colon cancer, and 180 cases of right colon cancer. Through the continuous optimization and improvement of the problems encountered during the operation, combined with the operator's experience, and taking into account various aspects, we developed the robotic "3+2" mode trocar layout for gastrointestinal surgery.Results:Four principles of trocar layout were developed, namely, the principle of lens placement around the navel, the principle of symmetry in the main operation, the principle of 8-10cm distance between trocar holes, and the principle of symmetry in the auxiliary hole lens. Three trocar layout methods and principles applicable to robotic gastric surgery, and four applicable to robotic colorectal surgery were developed.Conclusion:The trocar layout method of robotic "3+2" mode gastrointestinal surgery is established based on a large number of robotic gastrointestinal surgery experiences. This method is simple and easy to learn, with strong repeatability and operability.
5.KIF2C: a novel link between Wnt/β-catenin and mTORC1 signaling in the pathogenesis of hepatocellular carcinoma.

Hepatocellular carcinoma (HCC) is the most common primary liver malignancy and is the fourth-leading cause of cancer-related deaths worldwide. HCC is refractory to many standard cancer treatments and the prognosis is often poor, highlighting a pressing need to identify biomarkers of aggressiveness and potential targets for future treatments. Kinesin family member 2C (KIF2C) is reported to be highly expressed in several human tumors. Nevertheless, the molecular mechanisms underlying the role of KIF2C in tumor development and progression have not been investigated. In this study, we found that KIF2C expression was significantly upregulated in HCC, and that KIF2C up-regulation was associated with a poor prognosis. Utilizing both gain and loss of function assays, we showed that KIF2C promoted HCC cell proliferation, migration, invasion, and metastasis both in vitro and in vivo. Mechanistically, we identified TBC1D7 as a binding partner of KIF2C, and this interaction disrupts the formation of the TSC complex, resulting in the enhancement of mammalian target of rapamycin complex1 (mTORC1) signal transduction. Additionally, we found that KIF2C is a direct target of the Wnt/β-catenin pathway, and acts as a key factor in mediating the crosstalk between Wnt/β-catenin and mTORC1 signaling. Thus, the results of our study establish a link between Wnt/β-catenin and mTORC1 signaling, which highlights the potential of KIF2C as a therapeutic target for the treatment of HCC.

6.Study on Improvement of Quality Standard of Equisetum hyemale

OBJECTIVE:To provide reference for improving the quality sta ndard of Equisetum hyemale . METHODS :Totally 10 batches of E. hyemale from different sites were collected as samples. TLC method was used to qualitatively identify kaempferol- 3-O-β-sophoroside. The contents of heavy metal ,aflatoxin,impurity,moisture,total ash ,acid-insoluble ash ,water-soluble extract and ethanol-soluble extract were determined according to supplementary provisions of Chinese Pharmacopoeia (2015 edition). HPLC method was used to determine the content of kaempferol- 3-O-β-sophoroside in sample. HPLC fingerprint of water-soluble extract from E. hyemale was also established. RESULTS :TLC identification showed that in the chromatogram of the test sample , fluorescent spots with the same color were displayed on the corresponding positions of the chromatogram of substance control of kaempferol-3-O-β-sophoroside,and without interference from blank control. Among 10 batches of samples ,the contents of impurities were 0.19%-2.32%;the water contents were 10.12%-11.87%;the total ash contents were 6.67%-10.11%;the acid-insoluble ash contents were 1.34%-2.12%;the water-soluble extract contents were 9.17%-13.99%;the ethanol-soluble extract contents were 7.49%-13.68%,respectively. It is preliminarily proposed that the impurity content shall not exceed 3.00%;the total ash content shall not exceed 10.00%;the acid-insoluble ash content shall not exceed 2.50%;the water-soluble extract content shall not be less than 9.00% ;the ethanol-soluble extract content shall not be less than 5.00%. Arsenic(0.064-0.225 mg/kg) 010815) was detected in 9 batches of samples ;cadmium(0.106-0.132 E-mail:cruise0303@163.com mg/kg)was detected in 6 batches of samples ;lead(0.221- 1.896 mg/kg)was detected in all samples ,but no mercury or rebecca aflatoxin was detected. The results of HPLC method met the relevant requirements of Chinese Pharmacopoeia . The content of kaempferol- 3-O-β-D-sophoroside in 10 batches of samples was 627.12-5 384.53 mg/kg,and the similarity of HPLC fingerprints of 10 batches of samples was more than 0.900. CONCLUSIONS : A new qualitative and quantitative analysis method for kaempferol- 3-O-β-D-sophoroside was established ;the heavy metals , aflatoxins,impurities and other items in E. hyemale were detected ;the limits of impurity ,ash and extract were determined. The established method is simple ,accurate and reproducible ,and can be used for quality control of E. hyemale .

Lung cancer is currently the malignant tumor with the highest morbidity and mortality in the world, and the main type is non-small cell lung cancer. Immune checkpoint inhibitor is a landmark discovery in the history of cancer treatment, which rewrites the history of cancer treatment, and improves the medical treatment of advanced tumors by a big step forward. The article summarizes the research progress of therapeutic drugs against anti-programmed cell death protein and programmed cell death protein ligand antibodies in the clinical diagnosis and treatment of non-small cell lung cancer. The principle of drug action, the differences in the diagnosis and treatment of non-small cell lung cancer in different clinical stages, and future research directions are discussed to provide the usage guidelines of immune checkpoint inhibitors for clinical oncologists.

9.Study on the Effects and Its Mechanism of Calcium Phosphate Bone Cement Loading Total Flavonoids of Davallia mariesii on Osteoblast Differentiation in Induced Membrane of Bone Defect Model Rats

OBJECTIVE: To investigate the effects and its mechanism of calcium phosphate bone cement (CPC) loading total flavonoids of Davallia mariesii on osteogenic differentiation of induced membrane in rats. METHODS: Drug-loading CPC and drug-loading polymethyl methacrylate (PMMA) cement were prepared with the contents of Qianggu capsules (total flavonoids of D. mariesii as active ingredient) using CPC and PMMA cement as carrier. Totally 64 male SD rats were randomly divided into drug-loading CPC group, drug-loading PMMA cement group, no-drug CPC group, no-drug PMMA cement group, with 16 rats in each group. The femur of rats was separated and osteotomized to prepare bone defect model, and then the corresponding bone cement was implanted. Four weeks after modeling, the induced membranes of rats were cut and protected. Bone cement was taken out and autogenous cancellous bone was implanted. At the 4th week after modeling, X-ray photographs were taken on the hind limb bones of rats. At the 4th week after modeling and 6th week after bone grafting, induced membranes and new bone were taken from the bone defect area of rats respectively. HE staining was used to observe the morphology of induced membrane, and the width of bone rabecular and the number of osteoblasts of new bone tissue were measured. Immunohistochemistry was used to detect the protein expression of BMP-2 and VEGF in induced membrane. Western blotting assay was used to detect the protein expression of Smad1, Smad4 and Smad7 in new bone. RESULTS: Compared with other 3 groups, the degradation of bone cement in drug-loading CPC group was more obvious in the bone defect areas, which showed that the formation of induced membrane was observed and the bone defect areas were smaller; capillary endothelial cells were abundant and orderly arranged in the induced membranes, and the width of bone trabeculae and the number of osteoblasts in the new bone tissue increased significantly (P<0.05); the protein expression of BMP-2 and VEGF in the induced membrane, the protein expression of Smad1, Smad4 and Smad7 in new bone were increased significantly (P<0.05). CONCLUSIONS: CPC loading total flavonoids of D. mariesii promotes the formation of induced membrane osteoblast in bone defect model rats, which may be associated with regulating osteoblast differentiation by activating BMP-2/Smad pathway; at the same time, it can promote bone healing by promoting the differentiation of vascular endothelial cells, accelerating the formation of capillary network and increasing the expression of vascular endothelial cells.
10. The safety and effectiveness of the Da Vinci robot "3+ 2" mode in radical gastrectomy

Objective:
To explore the safety, feasibility and advantages of Da Vinci robot "3+ 2" mode in radical gastrectomy.
Methods:
Clinical data of 30 patients received radical gastrectomy at our department under the "3+ 2" mode of Da Vinci robot from Jan. 2018 to Oct. 2018 were retrospectively analyzed. 30 patients who received classical robot mode in the same period were randomly selected as control group.
Results:
Compared with the control group, the observation group had a shorter operative time (174±16)min
