1.Construction and application of a continuous care program for patients with cardiac implantable electronic devices

Objective To develop a continuous care program for patients with cardiac implantable electronic devices(CIED)utilizing the Omaha system,and to assess its effectiveness.This study aims to provide a reference for the continuous care of patients with CIED.Methods Using the Omaha system as the theoretical framework,based on literature search and semi-structured interviews,a continuity of care program for CIED patients based on the Omaha system was constructed through 2 rounds of expert inquiry.Convenience sampling method was used to select CIED implant patients from the cardiovascular department of a tertiary hospital in Wuhan as the research subjects.61 patients admitted from April to June 2024 were assigned as an experimental group with a continuity of care program based on the Omaha system;61 patients admitted from December 2023 to March 2024 were designated as a control group and received routine nursing measures.The Omaha performance indicators and quality of life at 3 months after discharge were compared between 2 groups upon admission and 1 month after discharge.Results The final 2 groups each included 59 patients.A month after discharge,the Omaha efficacy index in the experimental group was higher than that in the control group,and 3 months after discharge,the quality of life in the experimental group was higher than that in the control group(P<0.001).Conclusion The CIED patient continuity of care program based on the Omaha system could improve patients'cognitive level,promote healthy behavior,enhance their self-efficacy,and improve their quality of life.
2.Interpretation of the "EULAR/PReS recommendations for the diagnosis and management of Still′s disease"

This article provides an interpretation of the EULAR/PReS recommendations for the diaganosis and management of Still′s disease focusing on the diagnostic criteria, therapeutic targets, and strategies for managing complications in systemic juvenile idiopathic arthritis (sJIA) and adult-onset Still′s disease (AOSD). The guidelines introduce, for the first time, the unified nomenclature of "Still′s disease" for both sJIA and AOSD, emphasizing the importance of early recognition and stepwise management. Particular attention is given to the role of IL-1 and IL-6 inhibitors in reducing glucocorticoid use. This article aims to assist clinicians in China in better understanding and applying these guidelines to enhance the diagnosis and treatment of Still′s disease.
3.Interpretation of the 2024 European guidelines on the diagnosis and management of pediatric eosinophilic esophagitis

Eosinophilic esophagitis (EoE) is a chronic inflammatory disease driven by immune mechanisms, characterized by abnormal eosinophilic infiltration of the esophageal epithelium. EoE′s pathogenesis is closely associated with food allergens, and clinical manifestations primarily include dysphagia, food impaction, and children patients may also have growth restriction. The European Society for Paediatric Gastroenterology, Hepatology, and Nutrition first published guidelines for the management of pediatric EoE in 2014. With increasing understanding of the disease mechanisms and treatment responses, the 2024 guidelines have comprehensively updated the diagnostic and therapeutic strategies.This paper provides a systematic interpretation of the 2024 guidelines, focusing on the revised diagnostic criteria, optimization of dietary therapy, and improved management strategies for esophageal strictures. Furthermore, a comparison of the 2014 and 2024 guidelines highlights significant advancements in EoE management over the past decade and their implications for clinical practice.

According to the International League Against Epilepsy (ILAE) standards, the Newborn Working Group of the ILAE put forward 6 necessary questions about the management of neonatal anti-seizure medication and gave evidence-based recommendations in 2023. The basic framework is systematic review+expert consensus. The clinical recommendations of ILAE guidelines 2023 and the similarities and differences between ILAE guidelines 2023 and ILAE guidelines 2011 were analyzed and interpreted in this paper, in order to provide reference for colleagues involved in neonatal convulsion management in China.
8.Shared decision making in acute coronary syndromes:a scoping review

Objective To review the application of shared decision making in acute coronary syndrome,so as to provide references for future research and application.Methods Based on the scoping review methodology,a systematic search was conducted in SinoMed,CNKI,Wanfang,PubMed,Embase,Cochrane Library,Web of Science,and CINAHL.The search time limit was from the establishment of the database to January 8,2023,and the included literature was summarized and analyzed.Results A total of 17 articles were included.The application of shared decision making in acute coronary syndromes includes diagnosis,treatment,primary and secondary prevention.5 patient decision aids were retrieved.The influencing factors include patient factors,medical staff factors and implementation process factors.The clinical application has good effectiveness and feasibility.Conclusion Shared decision making can help patients with acute coronary syndrome to increase their decision making knowledge,improve their decision-making experience and improve the quality of decision making.In the future,we should broaden the application scope of shared decision making in acute coronary syndromes,develop scientific and practical decision aids,take targeted measures,and explore the application of shared decision making in clinical diagnosis and treatment in China.
9.Clinical management and surgical techniques for neonates and infants with ebstein anomaly: an interpretation of the 2024 expert consensus from the American Association for Thoracic Surgery

Currently, there is a lack of localized guidelines or consensus on the management of Ebstein anomaly (EA) in China, particularly concerning the treatment of neonates and infants. The 2024 Expert Consensus issued by the American Association for Thoracic Surgery provides detailed guidance on the clinical management of EA, covering key aspects such as the timing of surgical interventions, selection of surgical techniques, and postoperative management. The interpretation of this consensus is of great significance for thoracic surgeons in China to understand the latest international academic developments and can serve as a valuable reference for clinical practice. The consensus outlines the latest surgical techniques adopted internationally, including the Starnes procedure, biventricular repair, and 1.5 ventricle repair, and provides an in-depth analysis of the optimal timing and strategies for interventions in different cases. This offers a theoretical foundation and technical reference for advancing the field in China, contributing to the standardization and regulation of EA treatment.
10.Diagnosis and treatment of eosinophilic gastrointestinal disorders beyond eosinophilic esophagitis in pediatric: interpretation of joint European and North American guidelines

To interpret the clinical practice guidelines for pediatric and eosinophilic gastrointestinal disorders beyond eosinophilic esophagitis (non-EoE EGID), jointly developed by the European Society for Paediatric Gastroenterology, Hepatology and Nutrition and the North American Society for Pediatric Gastroenterology, Hepatology and Nutrition. The guidelines provide new standards for the diagnosis and treatment of non-EoE EGID. They include detailed explanations on disease classification, diagnostic and therapeutic challenges, guideline development methodology, epidemiology, etiological mechanisms, symptoms, endoscopic and imaging examinations, other tests, differential diagnosis, and treatment strategies. The paper explores the importance of comprehensive assessment in children with non-EoE EGID. Improvement suggestions are proposed, such as developing symptom assessment tools, conducting quality of life impact studies, standardizing eosinophil density reporting, refining diagnostic criteria, and formulating specific treatment guidelines. These measures aim to enhance the diagnostic and therapeutic accuracy for non-EoE EGID and improve the clinical management of affected children.
