1.Regulatory effect of histone lactylation modification in hepatic fibrosis

Hepatic fibrosis is a reversible pathological process in various chronic liver diseases and is closely associated with the development and progression of severe liver diseases such as liver cirrhosis and hepatocellular carcinoma, and it has emerged as a significant global health challenge. In recent years, studies have shown that histone lactylation, a newly discovered epigenetic modification, actively participates in regulating the progression of hepatic fibrosis. This article systematically reviews the core regulatory effect of histone lactylation modification in the interaction between inflammatory microenvironment and hepatic fibrosis, in order to clarify the cascade regulatory mechanism of “inflammation-hepatic fibrosis” and provide new insights for early diagnosis, targeted intervention, and prevention of malignant transformation in hepatic fibrosis.

Objective To analyze the epidemiologic burden of colorectal cancer in Xishan District, Kunming City, Yunnan Province from 2018 to 2020. Methods Indicators of epidemiologic burden were calculated, including incidence rate, mortality rate, age-specific incidence/mortality rates, potential years of life lost (PYLL), and disability-adjusted life years (DALY) based on the National Disease Control and Prevention Center’s "Cancer Information Registration and Reporting System" and "Cause of Death Registration System". Objective To construct glucose oxidase(GOx)–loaded nanogels (GONGs), optimize their formulation, and evaluate their capacity to inhibit the Warburg effect in glioma cells. Methods A responsive polymer (HAM) was synthesized and used to self-assemble GONGs, which were then characterized. Encapsulation efficiency and drug loading were determined using fluorescence spectrophotometry. Biocompatibility was tested by measuring cytotoxicity and hemolytic activity. Western blotting was used to evaluate the effects of GONGs on the expression of proteins associated with the Warburg phenotype and oxidative damage in glioma cells. Results GONGs prepared at a drug–to–polymer ratio of 1∶10 exhibited a particle size of 140.3 nm and a zeta potential of −27.2 mV. Compared with free GOx, GONGs markedly reduced cytotoxicity, increased the IC50 in hUVEC cells from 2.150 nmol/L to 74.86 nmol/L, and significantly decreased hemolysis. At a GOx concentration of 2 nmol/L, GONGs effectively downregulated glycolysis-related proteins, such as HK2 and LDHA, and inhibited glutamine metabolism in glioma cells. Conclusion GONGs exhibited high GOx loading capacity, significantly reduced GOx-induced cytotoxicity, inhibited the Warburg effect in glioma cells and induced oxidative damage.

Liver fibrosis is a critical pathological process in the progression of chronic liver diseases and is mainly driven by the activation of hepatic stellate cells, and it is characterized by excessive deposition of extracellular matrix. Programmed cell death is widely observed in liver fibrosis and plays diverse roles in different types of cells. It is not only a mechanism for maintaining cellular homeostasis through metabolism, but it can also regulate the development and progression of diseases. Ferroptosis has recently emerged as a research focus due to its close association with lipid metabolism. This article systematically reviews the related concepts of lipid metabolism, ferroptosis, and liver fibrosis, analyzes the bridging role and dual regulatory function of ferroptosis in lipid metabolism and liver fibrosis, and further highlights the clinical application value of targeting ferroptosis in hepatic stellate cells for the treatment of liver fibrosis, in order to provide new perspectives for the mechanistic studies on liver fibrosis and the optimization of related clinical intervention strategies.
5.Evaluation of the tele-intelligent rehabilitation system based on a wearable device in early rehabilitation of patients after arthroscopic anterior cruciate ligament reconstruction

Objective To explore the effectiveness of the tele-intelligent rehabilitation system based on a wearable device in early rehabilitation of patients after arthroscopic anterior cruciate ligament re-construction(ACLR).Methods A total of 70 patients(48 male,22 female,18~29 years old)undergo-ing arthroscopic ACLR were randomly divided into a tele-rehabilitation group(n=36)and a convention-al rehabilitation group(n=34).The tele-rehabilitation group underwent rehabilitation training using the tele-intelligent rehabilitation system based on a wearable device,while the conventional rehabilitation group conducted a home-based self-rehabilitation exercise according to the conventional education con-tent after surgery.Before surgery and 12 weeks postoperatively,both groups were assessed using the International Knee Documentation Committee(IKDC),lower extremity functional scale(LEFS),knee active range of motion(AROM),numeric pain rating scale(NPRS)and 12-item short-form health survey(SF-12).Results Before surgery,there was no significant difference between the two groups in the IKDC score,LEFS,knee mobility,walking NPRS,as well as physical component summary(PCS)and mental component summary(MCS)scores in the SF-12 scale(P>0.05).However,12 weeks postoperatively,the IKDC score,LEFS score,and knee mobility in the tele-rehabilitation group were 73.4±9.8,70.2±5.1,and 127.9°±5.8°,respectively,significantly better than those of the conventional rehabilitation group(64.3±13.7,63.6±9.6,and 122.7°±10.6°,respectively)(P<0.05 for all).Moreover,the walking NPRS scores in the tele-rehabilitation group were significantly low-er than the conventional rehabilitation group(P=0.01),while the PCS and the MCS scores of the for-mer were significantly superior to the latter(P<0.01).In addition,the minimum clinically important difference compliance rate of IKDC and LEFS score and the patient acceptable symptom state compli-ance rate of NPRS score in tele-rehabilitation group were significantly better than the latter group(P<0.05).Conclusion The remote intelligent rehabilitation system based on a wearable device is superior to the conventional rehabilitation in promoting early knee function recovery,joint mobility,relieving pain,and bettering the life quality of patients after ACLR.
6.Study on the mechanism of LncRNA DLEU2 affecting the ability of migration and proliferation of oral squamous cell carcinoma cells by regulating the miR-186-5p/IGF2BP3 axis

Objective:To explore the effects of LncRNA DLEU2 on the migration and proliferation of oral squamous cell carcino-ma(OSCC)cells by regulating the miR-186-5p/insulin-like growth factor 2 mRNA binding protein 3(IGF2BP3)axis.Methods:QRT-PCR and Western blot were applied to detect the mRNA and protein levels of LncRNA DLEU2,miR-186-5p,and IGF2BP3 in OSCC cell line SCC-25 and human normal oral keratinocyte line NOK,respectively.SCC-25 cells were transfected with si-DLEU2,miR-186-5p inhibitor and negative control respectively.The relationship between LncRNA DLEU2,miR-186-5p and IGF2BP3 was verified by double luciferase reporter gene assay.qRT-PCR was applied to detect the mRNA expression of LncRNA DLEU2 and miR-186-5p in SCC-25 cells.CCK-8 test was applied to detect the proliferation of SCC-25 cells.Flow cytometry was applied to detect the apoptosis rate of SCC-25 cells.Transwell assay was used to detect cell invasion and migration.Western blot was used to detect EMT-associated and IGF2BP3 protein levels.Results:Silencing LncRNA DLEU2 decreased the proliferative ac-tivity,migration and invasion of SCC-25 cells and decreased the protein levels of N-cadherin and Vimentin,increased the apopto-sis rate,miR-186-5p expression and E-cadherin protein levels.Down-regulation of miR-186-5p weakened the inhibitory effect of silenced LncRNA DLEU2 on the malignant phenotype of SCC-25 cells.LncRNA DLEU2 negatively regulated the miR-186-5p/IGF2BP3 axis.Conclusion:Silencing LncRNA DLEU2 may down-regulate the expression of IGF2BP3 by up-regulating miR-186-5p,inhibites the proliferation,migration,and invasion of SCC-25 cells,and promotes the apoptosis of the cells.
7.Interventional Treatment and Prognostic Analysis Initial Diagnosed of Pregnan-cy-Related Uterine Arteriovenous Malformations

Objective:To evaluate the safety,efficacy,and prognosis of interventional treatment for pregnan-cy-related uterine arteriovenous malformations(UAVM).Methods:A retrospective analysis was conducted of the clinical data from 8 patients with pregnancy-associated UAVM who underwent interventional therapy after initial di-agnosis by ultrasound at Peking Union Medical College Hospital between February 1,2016,and January 4,2024.Results:All 8 patients underwent ultrasound examination,and color Doppler imaging revealed abundant blood flow signals within the lesions,presenting a high-velocity and low-resistance blood flow spectrum.The median age of patients was 29 years,with termination of pregnancy occurring at 7-38 weeks of gestation.Clinical manifesta-tions included persistent irregular vaginal bleeding in 2 cases(25.0%),"on-off"hemorrhage in 2 cases(25.0%),acute massive vaginal bleeding(≥400 ml per episode)in 5 cases(62.5%),abdominal pain in 3 ca-ses(37.5%),secondary anemia in 4 cases(50.0%),and persistent elevation of serum β-hCG in 2 cases(25.0%).All 8 patients underwent uterine artery angiography.Postoperatively,5 cases were diagnosed with preg-nancy-related UAVM,with 1 case also complicated by a right ovarian arteriovenous fistula.The remaining 3 cases revealed only abnormal vascular clusters without early venous opacification.Bilateral uterine artery embolization(UAE)was performed in 4 cases,unilateral UAE(all right-sided)in 2 cases,and no UAE was performed in 2 ca-ses.Hysteroscopic lesion resection or curettage was performed in 3 cases,with 2 cases receiving adjuvant gona-dotropin-releasing hormone agonist(GnRH-a)therapy postoperatively.All 5 patients ultimately diagnosed with UAVM achieved hemostasis within 24 hours after UAE,with mild post-embolization syndrome observed in 1 case.During follow-up,only the patient with the concomitant ovarian arteriovenous fistula experienced recurrent sudden vaginal bleeding and subsequently underwent laparoscopic uterine lesion resection following GnRH-a therapy.Conclusions:UAE is the treatment of choice for pregnancy-related UAVM patients with complex or severe bleed-ing initially diagnosed by ultrasound.However,management should be tailored based on imaging and clinical fea-tures to balance therapeutic efficacy with fertility preservation.
8.Global Burden of Liver Cirrhosis and the Attributable Risk Factors:An Analysis and Forecast Based on GBD 2021

Background:Liver cirrhosis is characterized by chronic inflammation,progressive fibrosis,and eventual liver dysfunction,and poses a major global health challenge.Aims:To assess the global burden of liver cirrhosis and its risk factors from 1990 to 2021.Methods:Using data extracted from the Global Burden of Diseases,Injuries,and Risk Factors Study(GBD)2021,the incidence,mortality,disability-adjusted life years(DALYs),and their age-standardized rates of liver cirrhosis were analyzed.Furthermore,a stratified analysis was conducted by sex,age,region,and etiology,with projections of the trends in the next 15 years.Results:Compared to 1990,the global incidence number of liver cirrhosis in 2021 was increased by 58.2%,the death number and DALYs rose by 39.5%and 27.9%,respectively.While the global age-standardized incidence rate(ASIR)showed a slight increase,the age-standardized death rate(ASDR)and DALY rate continued to decline.Both ASIR and ASDR exhibited negative correlations with the sociodemographic index(SDI).All age-standardized rates were higher in males than in females.Since 1990,the incidence rate increased in younger populations,while the mortality and DALY rates declined in most age groups.Non-alcoholic fatty liver disease(NAFLD)emerged as the leading cause of incidence,whereas chronic hepatitis B and C remained the primary contributors to deaths and DALYs.The incidence of NAFLD was prominent in high and high-middle SDI regions,while chronic hepatitis B was concentrated in low SDI regions.Projections to 2036 indicated a continuing rise in ASIR,and declines in ASDR and DALY rate.The incidence of chronic hepatitis B was projected to decrease markedly,whereas that of NAFLD was expected to continue increasing.Conclusions:Between 1990 and 2021,the global incidence of liver cirrhosis showed a modest increase;in contrast,both mortality and DALY rates demonstrated a steady decline.Burden of liver cirrhosis poses notable regional disparities.NAFLD dominates incidence in high-income regions,while viral hepatitis remains predominant in low-income areas,highlighting the need for region-specific prevention strategies.
9.Analysis of the therapeutic effect of ultra-micro 8.0 mm single-port thoracoscopic nerve resection through areola incision for the treatment of primary palmar hyperhidrosis

Objective To explore the clinical efficacy of ultra-micro 8.0 mm single-port thoracoscopic nerve resection through areola incision in the treatment of primary palmar hyperhidrosis(PPH).Methods 84 patients with PPH from January 2018 to January 2022,were divided into the observed group[(inframammary approach,IMA)group,n=42]and control group[axillary approach(AA)group,n=42].The observed group was treated with ultra-micro 8.0 mm IMA single-port endoscopic thoracic sympathectomy(ETS),the control group used the traditional AA single-port ETS.The visual analogue scale(VAS)score,serum C-reactive protein(CRP),cortisol(Cor),interleukin-6(IL-6)levels,the postoperative cosmetic effect and compensatory hyperhidrosis of the two groups were compared.Results The VAS scores of 2,12 and 24 h after surgery in the observation group were significantly lower than those of the control group,the differences were statistically significant(P<0.05);At 12 and 24 h postoperation,the levels of CPR,Cor,and IL-6 levels in both groups of patients were significantly higher than preoperation,but those in the observation group were significantly lower than those of the control group,the differences were statistically significant(P<0.05);The total satisfaction rate of postoperative incision in the observation group was significantly higher than that of the control group,the difference was statistically significant(P<0.05);There was no statistically significant difference of the incidence of compensatory hyperhidrosis between the two groups(P>0.05).Conclusion The clinical efficacy of ultra-micro 8.0 mm IMA single-port ETS through areola incision in the treatment of PPH is good.Compared with the traditional axillary single-port thoracoscopic method,it has the advantages of small trauma,less bleeding,less patient pain,high safety and high patient satisfaction.It is worthy of clinical promotion and application.
10.A retrospective study on an innovative modular surgical technique in laparoscopic total gastrectomy combined with spleen-preserving hilar lymphadenectomy

Objective:This study aimed to propose an innovative modular surgical technique and explore its safety and application value in laparoscopic total gastrectomy combined with spleen-preserving hilar lymphadenectomy for advanced proximal gastric cancer invading the greater curvature.Methods:A retrospective collection was conducted on 34 patients with proximal gastric cancer invading the greater curvature who underwent laparoscopic total gastrectomy combined with spleen-preserving hilar lymphadenectomy in the same center from October 2020 to December 2022. The technical key points, precautions and crucial steps of the modular surgical technique were summarized, and the Clinical indicators were analyzed.Results:All 34 patients successfully completed the operation under laparoscopy without conversion to open surgery. The average operation duration was 151.9±4.1 minutes, and the duration of splenic hilar lymphadenectomy was 12.9±1.5 minutes. The median intraoperative blood loss was 50(20, 50) ml, and the blood loss during splenic hilar lymphadenectomy was 5 (2, 5) ml. The median number of harvested lymph nodes was 32.0 (23.5,39.5), and the number of submitted No.10 lymph nodes was 3 (2, 4). The metastasis rate of No.10 lymph nodes was 20.6% (7/34). No patient had intraoperative complications. During the postoperative hospital stay, one patient had incision infection (Clavien-Dindo I), and one patient had pulmonary infection (Clavien-Dindo II). The time for the first postoperative feeding was 3 (2, 5) days, the time for the first postoperative flatus was 2 (2,3) days, the time for the first postoperative defecation was 3 (3, 4) days, the total postoperative drainage volume was 1047.5 (607.5,1397.5) mL, the time for postoperative drainage tube removal was 7 (6, 9) days, and the length of postoperative hospital stay was 7.0 (6.0, 9.5) days.Conclusions:The application of the innovative modular surgical technique in laparoscopic total gastrectomy combined with spleen-preserving hilar lymphadenectomy can simplify surgical process and enable safe, precise and comprehensive dissection of splenic hilar lymph nodes.
