1.Study on quality control of Jinbei oral liquid based on fingerprint,chemical pattern recognition and multi-index content determination

OBJECTIVE To establish a quality control method for Jinbei oral liquid based on multi-wavelength switching high performance liquid chromatography (HPLC) fingerprint, chemical pattern recognition and multi-index content determination. METHODS A total of 15 batches of Jinbei oral liquid were used as test samples. The Similarity Evaluation System for Chromatographic Fingerprint of Traditional Chinese Medicine ( Version 2012 ) was adopted to establish multi-wavelength switching HPLC fingerprints, followed by chromatographic peak identification and similarity evaluation. Cluster analysis, principal component analysis and orthogonal partial least squares-discriminant analysis were applied to conduct chemical pattern recognition on the 15 batches of samples. The multi-wavelength switching HPLC method was used to simultaneously determine the contents of neochlorogenic acid, chlorogenic acid, cryptochlorogenic acid, forsythoside A, baicalin, salvianoli c acid B and wogonoside in samples. RESULTS A total of 25 common peaks were calibrated in the fingerprints of 15 batches of Jinbei oral liquid, among which 7 common peaks were unambiguously identified. The similarity of all samples was higher than 0.960. Chemical pattern recognition results showed that samples S1-S8 were clustered into group 1, and samples S9-S15 were clustered into group 2. Baicalin, wogonoside, neochlorogenic acid, chlorogenic acid and salvianolic acid B were identified as the differential quality markers. The average contents of neochlorogenic acid, chlorogenic acid, cryptochlorogenic acid, forsythoside A, baicalin, salvianolic acid B and wogonoside in 15 batches of Jinbei oral liquid were 0.213 7, 0.085 3, 0.104 9, 0.287 3, 0.420 1, 0.062 1, 0.176 0 mg/mL, respectively. CONCLUSIONS The established quality control method for Jinbei oral liquid combining multi-wavelength switching HPLC fingerprint, chemical pattern recognition and multi-index content determination is stable and reliable, which can provide a reference for the formulation of quality standards of this preparation.

3.Survey of residents'willingness to use"internet+nursing service"in a community in Xiamen city

Objective To investigate residents'willingness to use"internet+nursing services"of a community in Xiamen City and provide reference for the code of practice to promote the service.Methods A total of 350 community residents in Xiamen City were investigated by convenience sampling from January to December 2023.The questionnaire was self-designed with regard to their willingness to use"internet+nursing services".Results 336 questionnaires were effectively collected.The total score of willingness to use"internet+nursing services"of community residents in Xiamen City was(68.80±8.73)points,but only 19.6%of residents knew about"internet+nursing services".There were statistically significant differences in willingness to use among community residents with different ages,medical payment type,whether they knew"internet+nursing services"or not,and the number of service items needed(P<0.05).Among them,age,whether they knew"internet+nursing services"or not and the number of service items needed were the main influencing factors of willingness to use(P<0.05).Conclusion Community residents in Xiamen City show above the middle level of willingness to use"internet+nursing services".However,the popularity of"internet+nursing services"has a large room for improvement.The publicity to the residents should be strengthened,and the care needs of residents with different demographic characteristics should be considered.The supporting application functions of such platforms should be designed to meet the health needs of community residents,especially elderly patients with great demand for services.
4.Effect of PD-1/PD-L1 inhibitors on lipopolysaccharide-induced inflammation in mouse microglia

Objective:To investigate the effect of PD-1/PD-L1 inhibitor BMS-1 on LPS-induced inflammation in mouse microg-lia cells(BV-2 cells).Methods:Bv-2 cells were divided into Control group,LPS group and LPS+BMS-1 group.Bv-2 cells in Control group were cultured in DMEM medium for 78 hours,cells in LPS group were stimulated with 100 ng/ml LPS for 6 hours after 72 hours of normal culture,Bv-2 cells in LPS+BMS-1 group were treated with 50 nmol/ml BMS-1 for 72 hours and then stimulated with 100 ng/ml LPS for 6 hours.Expressions of PD-1 and iNOS mRNA in each group were detected by RT-qPCR,and expressions of PD-1 and iNOS protein in microglia were detected by Western blot.Flow cytometry was used to detect cell apoptosis in each group.Levels of inflamma-tory cytokines IL-1β,IL-6,TNF-α and IL-10 were detected by ELISA.Results:RT-qPCR and Western blot results showed that com-pared with Control group,LPS group had significantly increased expression of PD-1 and iNOS(P<0.05).Compared with LPS group,LPS+BMS-1 group had significantly decreased expression of PD-1(P<0.05)and significantly increased expression of iNOS(P<0.05).Flow cytometry showed that compared with Control group,LPS group had a significantly increased in apoptosis of microglia(P<0.000 1).Compared with LPS group,LPS+BMS-1 group had a significantly increased in apoptosis of microglia(P<0.000 1).ELISA results showed that compared with Control group,LPS group had no significantly increased in pro-inflammatory factors IL-1β and IL-6(P>0.05),while significantly increased in TNF-α(P<0.000 1)and anti-inflammatory factor IL-10(P<0.000 1).Pro-inflammatory cyto-kine IL-1β in LPS+BMS-1 group was significantly higher than that in LPS group(P=0.000 1),IL-6 and TNF-α were also significantly higher than those in LPS group(P<0.000 1),while anti-inflammatory cytokine IL-10 in LPS+BMS-1 group was significantly lower than that in LPS group(P<0.000 1).Conclusion:BMS-1 can promote LPS-induced inflammatory response or impede the recovery of inflammation,and increase apoptosis of microglia.PD-1/PD-L1 pathway may be a potential therapeutic target for neuroinflammation.

7.A study on rehabilitation exercise delaying the progression of chronic obstructive pulmonary disease by regulating NADPH oxidase in lung

Objective:To investigate the expression of NADPH oxidase(Nox)in chronic obstructive pulmonary disease(COPD).Method:The rats were randomly divided into control group,COPD group,COPD medium exercise group and COPD intensive exercise group,with 10 rats in each group.The expression of NADPH oxidase,hypoxia induc-ible factor-1 alpha(HIF1α),inflammatory factors tumor necrosis——factor alpha(TNFα),interleukin-6(IL-6),matrix metalloproteinase 2(MMP-2)and matrix metalloproteinase 9(MMP-9)were detected by immunohistochemistry.Result:Compared with the control group,the expression of NADPH oxidase subtypes Nox1,Nox2,Nox5,HIF1α,TNFα,IL-6,MMP-2 and MMP-9 in COPD group increased significantly.Medium exercise reduced the expression of Nox2,Nox5,HIF1α and MMP-9 in COPD animal model,while intensive exercise reduced the expression of Nox1,Nox2,Nox5,HIF1α,TNFα,IL-6 and MMP-9.Conclusion:Rehabilitation exercise down regulated the expression of NADPH oxidases in lung,reduced the expression of downstream inflammatory factors and matrix metalloproteinase.The protective function of rehabili-tation exercise might achieve by inhibiting the expression of hypoxia inducible factor.
8.A Comparative Study of Digital 3D Technology Assisted Versus Traditional One-hole Split Endoscopic Surgery for Lumbar Spinal Stenosis With Degenerative Scoliosis

Objective To compare the clinical efficacy of digital 3D technology assisted and traditional one-hole split endoscopic surgery for lumbar spinal stenosis with degenerative scoliosis(LSS-DS).Methods Clinical data of 42 patients with LSS-DS who underwent one-hole split endoscopic surgery from January 2022 to December 2023 were retrospectively analyzed.Among them,20 cases were treated with digital 3D technology assisted one-hole split endoscopic surgery(3D group),and 22 cases were treated with traditional one-hole split endoscopic surgery(traditional group).The operation time,intraoperative blood loss,lateral articular surface preservation rate,Visual Analogue Scale(VAS)of lower extremity pain,Oswestry Disability Index(ODI)and modified MacNab criteria at the last follow-up were compared between the two groups.Results Both groups of patients were operated successfully.The operation time of the 3D group and the traditional group was(73.3±24.0)min and(65.7±23.8)min,respectively,with no significant difference(t=1.020,P=0.314).The intraoperative blood loss was(65.3±14.7)ml and(66.7±17.1)ml,respectively,with no significant difference(t=-0.298,P=0.767).The retention rate of lateral articular surface in the 3D group[(88.2±6.3)%]was significantly higher than that in the traditional group[(82.7±4.6)%,t=3.201,P=0.003].One patient in the traditional group had dural tear and was cured after conservative treatment.There were 3 cases of calf muscle venous thrombosis in the 3D group and 4 cases in the traditional group.All the 7 patients were given anticoagulant therapy and appropriately delayed time of ambulation.In the3D group,20 cases were followed up for6-13 months,with a mean of(8.2±2.0)months;in the traditional group,22 cases were followed up for 6-12 months,with a mean of(7.7±1.7)months.According to the modified MacNab criteria,the excellent and good rate was 95.0%(19/20)in the 3D group and 90.1%(20/22)in the traditional group at the last follow-up,and there was no significant difference between the two groups(Z=-0.311,P=0.756).The VAS score and ODI of the two groups were significantly improved at 3 months and 6 months after operation(all P=0.000),but there was no significant difference between the two groups at each time point(P>0.05).Conclusion The short-term efficacy of digital 3D technology assisted one-hole split endoscopic surgery in the treatment of LSS-DS is definite,and the symptoms of patients are significantly improved.
9.Correlation between AST/ALT ratio and metabolic syndrome in hypertensive patients

Objective To explore the association between aspartate aminotransferase(AST)/alanine aminotransferase(ALT)ratio and metabolic syndrome(MS)in elderly hypertensive patients,and to provide reference for early detection and prevention of MS in elderly hypertensive patients.Methods A questionnaire survey and physical examination were conducted among 616 elderly hypertensive patients at community health service centers.Participants were divided into two groups based on MS status:MS group(n=334)and non-MS group(n=282).According to AST/ALT levels,participants were divided into four groups:q1 group(AST/ALT ≤0.88,n=156),q2 group(0.88<AST/ALT ≤ 1.10,n=155),q3 group(1.10<AST/ALT ≤ 1.37,n=154),and q4 group(AST/ALT>1.37,n=151).Blood biochemical parameters including triglycerides(TG),high-density lipoprotein cholesterol(HDL-C),AST,ALT,and blood glucose were measured.The impact of AST/ALT levels on MS was analyzed using a Logistic regression model,while the risk prediction for MS occurrence was evaluated through receiver operating characteristic(ROC)curves.Results MS group showed higher body mass index(BMI),TG,ALT levels,abnormal glucose levels,female proportion,and abdominal obesity rate compared to non-MS group.HDL-C and AST/ALT values of MS group were lower than those in non-MS patients(P<0.05).Logistic regression analysis revealed that after adjusting for BMI,smoking,alcohol consumption,physical activity,education level,marital status,TG,HDL-C,and glucose levels,both q3 and q4 groups demonstrated reduced MS risk compared to group q1 group(P<0.05).ROC curve analysis indicated that the area under the curve for AST/ALT in MS was 0.638(P<0.05).Conclusion The level of AST/ALT was negatively correlated with MS in elderly hypertensive patients,and AST/ALT has certain predictive value for the risk of MS in elderly hypertensive patients.
10.The effect of transcutaneous acupoint electrical stimulation on nausea and vomiting after painless gastroscopy and colonoscopy

Objective Through a non-inferiority trial design,to verify whether the efficacy of transcuta-neous electrical acupoint stimulation(TEAS)in intervening with nausea and vomiting after painless gastros-copy and colonoscopy was not inferior to that of ondansetron,and evaluate its effect on patient recovery.Meth-ods A total of 112 patients who experienced nausea and vomiting after painless gastroscopy and colonoscopy in the gastroscopy and colonoscopy center of the hospital from March 2022 to March 2023 were selected as the research subjects,and divided into the TEAS group(n=56)and the control group(n=56)according to the random number table method.The TEAS group received TEAS intervention,while the control group received ondansetron hydrochloride injection intervention.The non-inferiority margin was set at 10.0%.The interven-tion effective rate of nausea and vomiting after examination,as well as heart rate,blood pressure,length of stay,satisfaction,and visual analogue score(VAS)were compared between the two groups.Results The in-tervention effective rate of the TEAS group was close to that of the control group[82.4%(46/56)vs.80.4%(45/56)],risk difference(RD)=2.0%(95%CI:-9.8%to 13.8%),the lower limit of 95%CI(-9.8%)was higher than the preset cut-off value(-10.0%),and non-inferiority was established(P=0.032).Com-pared with before the examination,the heart rates of the two groups were slower,and the levels of systolic blood pressure(SEP)and diastolic blood pressure(DBP)were lower during the examination,and the differ-ences were statistically significant(P<0.05).There was no statistically significant difference in the two groups of heart rate,SBP,DBP,length of stay[(69.9±14.2)min vs.(75.3±22.8)min],satisfaction score(9.8±0.6 vs.9.2±2.0),VAS(1.5±0.5 vs.2.1±0.5,P>0.05).Conclusion The efficacy of TEAS in in-tervening with nausea and vomiting after painless gastroscopy and colonoscopy is not inferior to that of ondan-setron.
