2.The correlation between serum phosphate disorder and the prognosis of sepsis in children

Objective:To explore the relationship between serum phosphate levels and poor prognosis in children with sepsis.Methods:A retrospective cohort study was conducted to the children with sepsis or septic shock admitted to Gansu Provincial Maternal and Child Health Hospital from 2017 to 2023.Based on the pretreatment serum phosphate values of the children,they were divided into hypophosphatemia group,normal blood phosphate group,and hyperphosphatemia group.The clinical data of characteristics,length of hospital stay and regression of septic children in different serum phosphate groups were recorded,and the prognosis of the children was followed up 3 months after discharge from the hospital.The relationship between serum phosphate levels and prognosis of sepsis in children was explored using univariate and multivariate Logistic regression analysis.Results:A total of 200 children aged 0-14 years diagnosed with sepsis were included,of whom 100 cases (50.0%) had hypophosphatemia,43 cases (21.5%) had hyperphosphatemia,and 57 cases (28.5%) had normophosphate status (normophosphate group).Compared with the normophosphate and hypophosphate groups,the proportion of critically ill patients in the pediatric critical illness score of the hyperphosphatemia group was significantly increased.The incidence of septic shock increased by 5.5% and 24.1%,respectively( P=0.032).The duration of mechanical ventilation was significantly longer than the other two groups ( P=0.013).Multivariate Logistic regression analysis showed that the 3-month post-discharge mortality rate ( RR=6.960,95% CI 1.580-30.667, P=0.010) and total mortality ( RR=6.880,95% CI 2.038-23.233, P=0.002) of septic children in the hyperphosphatemia group were 6.960 and 6.880 times higher than those in the normophosphate group,and the differences were statistically significant. Conclusion:Elevated serum phosphate levels may be an early warning factor for poor prognosis of sepsis in children,and PICU healthcare professionals should pay attention to this indicator and take timely diagnostic and therapeutic measures to improve the prognosis.

5.GPR17 modulates anxiety-like behaviors via basolateral amygdala to ventral hippocampal CA1 glutamatergic projection.

Anxiety disorders are one of the most epidemic and chronic psychiatric disorders. An incomplete understanding of anxiety pathophysiology has limited the development of highly effective drugs against these disorders. GPR17 has been shown to be involved in multiple sclerosis and some acute brain injury disorders. However, no study has investigated the role of GPR17 in psychiatric disorders. In a well-established chronic restraint stress (CRS) mouse model, using a combination of pharmacological and molecular biology techniques, viral tracing, in vitro electrophysiology recordings, in vivo fiber photometry, chemogenetic manipulations and behavioral tests, we demonstrated that CRS induced anxiety-like behaviors and increased the expression of GPR17 in basolateral amygdala (BLA) glutamatergic neurons. Inhibition of GPR17 by cangrelor or knockdown of GPR17 by adeno-associated virus in BLA glutamatergic neurons effectively improved anxiety-like behaviors. Overexpression of GPR17 in BLA glutamatergic neurons increased the susceptibility to anxiety-like behaviors. What's more, BLA glutamatergic neuronal activity was required for anxiolytic-like effects of GPR17 antagonist and GPR17 modulated anxiety-like behaviors via BLA to ventral hippocampal CA1 glutamatergic projection. Our study finds for the first and highlights the new role of GPR17 in regulating anxiety-like behaviors and it might be a novel potential target for therapy of anxiety disorders.
6.Reward effect of flubromazolam and its underlying neural circuit mechanism

Flubromazolam(Flub)is a novel psychoactive substance of benzodiazepines and the mechanism underlying its addiction still remains elusive.This study investigated the reward effect of Flub using conditioned place preference(CPP)mouse model.The neuronal activity was evaluated by c-Fos expression,and the neural circuit was tracked by virus tracing.This study also investigated the regulatory effect of neural circuits on Flub-induced reward effects through chemogenetic approach.The results showed that,at the dose of 3 mg/kg,Flub significantly increased CPP score and c-Fos expression in dopaminergic(DA)neurons of ventral tegmental area(VTA).Inhibition of VTA dopaminergic neuron activity dramatically decreased Flub-induced CPP score.Virus tracing verified GABAergic neuronal projection of medial rostrum tegmental nucleus(RMTg)to VTA dopaminergic neurons.Activation of RMTgGABA→VTADA circuit or blockade of benzodiazepine receptors(BZR)in RMTg significantly decreased Flub-induced CPP score.These results indicate that Flub produced reward effect via BZR-mediated RMTgGABA→VTADA circuit.
7.Clinical application of Fastpass Scorpion suture passer for arthroscopic Bankart repair.

OBJECTIVE:
To explore the effectiveness and advantages of using Fastpass Scorpion suture passer to stitch the inferior capsulolabral complex in arthroscopic Bankart repair compared with traditional arthroscopic suture shuttle.
METHODS:
The clinical data of 41 patients with Bankart lesion, who met the selection criteria and were admitted between August 2019 and October 2021, was retrospectively analyzed. Under arthroscopy, the inferior capsulolabral complex was stitched with Fastpass Scorpion suture passer in 27 patients (FS group) and with arthroscopic suture shuttle in 14 patients (ASS group). There was no significant difference between the two groups ( P>0.05) in gender, age, injured side, frequency of shoulder dislocation, time from first dislocation to operation, and preoperative Rowe score of shoulder. Taking successful suture and pull-tightening as the criteria for completion of repair, the number of patients that were repaired at 5∶00 to 6∶00 (<6:00) and 6∶00 to 7∶00 positions of the glenoid in the two groups was compared. The operation time, and the difference of Rowe shoulder score betwee pre- and post-operation, the occurrence of shoulder joint dislocation, the results of apprehension test, and the constituent ratio of recovery to the pre-injury movement level between the two groups at 1 year after operation.
RESULTS:
Both groups completed the repair at 5∶00 to 6∶00 (<6∶00), and the constituent ratio of patients completed at 6∶00 to 7∶00 was significantly greater in the FS group than in the ASS group ( P<0.05). The operation time was significantly shorter in the FS group than in the ASS group ( P<0.05). All incisions in the two groups healed by first intention. All patients were followed up 12-36 months (mean, 19.1 months). No anchor displacement or neurovascular injury occurred during follow-up. Rowe score of shoulder in the two groups significantly improved at 1 year after operation than preoperative scores ( P<0.05), and there was no significant difference in the difference of Rowe shoulder score between pre- and post-operation between the two groups ( P>0.05). At 1 year after operation, no re-dislocation occurred, and there was no significant difference in the apprehension test and the constituent ratio of recovery to the pre-injury movement level between the two groups ( P>0.05).
CONCLUSION
Compared with the arthroscopic suture shuttle, using Fastpass Scorpion suture passer to stitch the inferior capsulolabral complex in arthroscopic Bankart repair is more convenient, saves operation time, and has good effectiveness.

8.Clinical features and genetic analysis of a child with acute form of Tyrosinemia type I due to a novel variant of FAH gene.

OBJECTIVE:
To analyze the clinical phenotype and genetic basis for a child with acute form of tyrosinemia type I (TYRSN1).
METHODS:
A child with TYRSN1 who presented at the Gansu Provincial Maternal and Child Health Care Hospital in October 2020 was selected as the subject. The child was subjected to tandem mass spectrometry (MS-MS) and urine gas chromatography-mass spectrometry (GC-MS) for the detection of inherited metabolic disorders, in addition with whole exome sequencing (WES). Candidate variants were validated by Sanger sequencing.
RESULTS:
The child's clinical features included abdominal distension, hepatomegaly, anemia and tendency of bleeding. By mass spectrometry analysis, her serum and urine tyrosine and succinylacetone levels have both exceeded the normal ranges. WES and Sanger sequencing revealed that she has harbored c.1062+5G>A and c.943T>C (p.Cys315Arg) compound heterozygous variants of the FAH gene, which were inherited from her father and mother, respectively. Among these, the c.943T>C was unreported previously.
CONCLUSION
Considering her clinical phenotype and result of genetic testing, the child was diagnosed with TYRSN1 (acute type). The compound heterozygous variants of the FAH gene probably underlay the disease in this child. Above finding has further expanded the spectrum of FAH gene variants, and provided a basis for accurate treatment, genetic counseling and prenatal diagnosis for her family.

9.comparison of complications incidence of 8-and 12-core prostate biopsy guided with transrectal ultrasound

Objective To compare complications incidence rates of 8-core and 12-core prostate biopsies guided with transrectal ultrasound retrospectively,and to study intervention ways to decrease complications.Methods The data of 260 consecutive patients undergoing first-time transrectal ultrasound-guided biopsy were analyzed.132 patients underwent 8-core biopsy and 128 patients underwent 12-core biopsy.Results In 8-core group,there were 7 cases of infections,15 cases of gross hematuria,7 cases of rectal bleeding and 6 cases of acute urinary retention,and in 12-core group,5 cases of infections,19 cases of gross hematuria,9 cases of rectal bleeding and 7 cases of acute urinary retention in 12-core group.There are no statistical differences of complications incidence rate between two groups (P > 0.05).when prostate volume > 45 ml,detection rate of prostate cancer in 12-core group was significantly higher than that in 8-core group.Conclusions There are no statistical differences of complications incidence rate between 8-and 12-core prostate biopsy.Homologous measure must be taken to reduce complications.12-core prostate biopsy is recommended as prostate volume > 45 ml.
10.Normal reference ranges of inferior vena cava Doppler indices from gestation 16 to 40 weeks of fetus with color ultrasound

Objective To study the estimated value on normal reference ranges of inferior vena cava (IVC) Doppler indices from gestation 16 to 40 weeks of fetus with color ultrasound.Methods Seven hundred and forty-four single birth gravida with gestation 16 to 40 weeks were grouped according to the gestational weeks with 27-35 cases each.The IVC Doppler indices were measured by color ultrasound,including preload index,peak velocity index and pulsatile index.Results All IVC Doppler indices rapidly declined from gestation 16 to 24 weeks,and they were relatively constant or slightly decreased throughout the end of pregnancy.Conclusions To set up the normal reference ranges of IVC Doppler indices for gestation from 16 to 40 weeks means a lot for every pregnant.This normative data may be a useful noninvasive tool for evaluation of fetal cardiac function.
