1.Epidemiological characteristics of respiratory disease mortality in residents of Baoshan,Shanghai 2009‒2020

ObjectiveTo investigate the epidemiological characteristics of respiratory disease mortality in Baoshan residents during the period of 2009‒2020. MethodsRespiratory disease deaths of Baoshan residents from 2009‒2020 were collected. ICD-10 codes were used to classify the causes of death, and R-4.2.1 was applied for statistical analysis. The average annual percent change (AAPC) of standardized mortality rates of different respiratory diseases were analyzed by using Joinpoint 4.9.0.0. ResultsThe average annual mortality rate of respiratory diseases in Baoshan from 2009 to 2020 was 58.86/105, and the standardized mortality rate was 35.62/105, which was the 3rd leading cause of mortality. The mortality rate of respiratory diseases was higher in men than in women (χ2=46.70, P<0.001). COPD ranked first among respiratory diseases in Baoshan from 2009 to 2020, followed by pneumonia, asthma and pneumoconiosis in that order. The standardized mortality rate for COPD decreased from 38.66/105 in 2009 to 19.88/105 in 2020 (AAPC=-6.6%, 95%CI: -8.2% to -4.9%, P<0.001). The standardized mortality rate of asthma decreased from 2.86/105 in 2009 to 1.43/105 in 2020 (AAPC=-5.8%, 95%CI: -8.8% to -2.8%, P<0.01). The standardized mortality rate of pneumoconiosis decreased from 0.64/105 in 2009 to 0.12/105 in 2020 (AAPC=-7.4%, 95%CI: -13.0% to -1.5%, P<0.05). The standardized mortality rate for pneumonia decreased from 2.63/105 in 2009 to 0.70/105 in 2020 (AAPC=-6.2%, 95%CI: -12.2% to 0.2%, P=0.056), but not statistically significant. The annual average mortality rates of COPD, pneumonia and asthma were all highest in January. Crude mortality rates for COPD (χ2=2 669.01, P<0.001), pneumonia (χ2=217.82, P<0.001), asthma (χ2=100.09, P<0.001), pneumoconiosis (χ2=26.46, P<0.001) and all categories of respiratory diseases (χ2=2 995.84, P<0.001) increased with age showed an increasing trend. The crude mortality rates for COPD (χ2=101.69, P<0.001), pneumonia (χ2=7.39, P<0.01) and asthma (χ2=7.41, P<0.01) were higher in the central than in the northern part of Baoshan District, while the crude mortality rate for COPD (χ2=19.97, P<0.001) was higher in the central than in the southern part. ConclusionThe attention should be focused on COPD; increased detection in males and the elderly, especially in winter and spring; and a good balance between environmental and economic when planning the regional development.
2.A case of intramandibular epidermoid cyst and literature analysis.

Epidermoid cysts are generally benign neoplastic lesions, the etiology of which is unclear and is mainly related to epithelial cells left in the tissues during the embryonic period and traumatically implanted in the tissues. The most common intraosseous sites are the phalanges and the skull. Epidermoid cysts occurring in the jaws are clinically rare. In this paper, we report a case of epidermoid cyst occurring in the mandible with embedded teeth and discuss the etiology, clinical manifestations, diagnosis, and treatment of epidermoid cysts in the jaws in the context of the relevant literature.

新冠病毒感染疫情严重威胁着世界各国人民的生命健康。目前,对病毒感染的防治研究主要集中在抑制病毒与分子受体的结合上。AXL作为新发现的严重急性呼吸综合征冠状病毒2型(SARS-CoV-2)受体,在协助病毒感染人体呼吸系统中发挥着重要作用,是未来临床干预的潜在靶点。本研究对已发表的单细胞测序数据进行整理和分析,发现AXL在年轻人肺细胞中的表达水平明显高于老年人。人胚肺二倍体成纤维细胞(2BS)是衰老研究的公认细胞株。本文采用2BS细胞构建复制性细胞衰老模型,发现年轻细胞中AXL的蛋白水平明显高于衰老细胞,据此推测年轻人感染的风险可能更高,需要注意防护。我们发现一种羊栖菜褐藻多糖硫酸酯组分(SFW-3)可显著下调年轻2BS细胞中AXL的表达水平,表明SFW-3具有一定的抗SARS-CoV-2感染的研究价值,同时表明2BS细胞株也可作为潜在的SARS-CoV-2体外感染模型。

Femoral neck fracture (FNF) in the elderly patients is currently a major health challenge worldwide, with excessive consumption of medical resources, high incidence of complications as well as suboptimal outcome and prognosis. Hip joint arthroplasty (HJA) has been the mainstream treatment for FNF in the elderly, but the conventional surgical approaches and techniques are still confronted with a series of bottlenecks such as dislocation, limp and limb length discrepancy. In recent years, direct anterior approach (DAA) for HJA (DAA-HJA) has been a major new choice in the field of joint replacement, which achieves improved clinical effectiveness of HJA in the treatment of elderly FNF, due to the fact that DAA approach involves the neuromuscular interface and accords with the idea of soft tissue retention and enhanced recovery after surgery. However, there is still a lack of unified understanding of standard technique and procedure of DAA-HJA in the treatment of elderly FNF. Therefore, relevant experts from the Hip Joint Group of Chinese Orthopedics Association of Chinese Medical Association, Youth Arthrology Group of Orthopedic Committee of PLA, Orthopedic Committee of Chongqing Medical Association, Branch of Orthopedic Surgeons of Chongqing Medical Doctor Association and Sport Medicine Committee of Chongqing Medical Association were organized to formulate the " Chinese expert consensus on the technical standard of direct anterior hip arthroplasty for elderly femoral neck fracture ( version 2023)" based on evidence-based medicine. This consensus mainly proposed 13 recommendations covering indications, surgical plans, prosthesis selections, surgical techniques and processes, and postoperative management of DAA-HJA in elderly patients with FNF, aiming to promote standardized, systematic and patient-specific diagnosis and treatment to improve the functional prognosis of the patients.
5.Survey analysis of influential factors of medication compliance and adverse drug reactions of home -based patients with severe mental disorders in Jinshan district of Shanghai

OBJECTIVE To analyze the status quo ,influential factors of medication compliance and adverse drug reactions of home-based patients with severe mental disorders . METHODS Patients with severe mental disorders at home who were enrolled in the Shanghai mental health information management system in Jinshan district of Shanghai in Jan . 2022 were selected as the research objects . In combination with the relevant basic data registered in the information system and through face -to-face or telephone surveys by mental health doctors ,basic information ,medication compliance ,mental symptoms ,social function defects , drug use information and adverse drug reactions of patients were understood . Univariate and multivariate Logistic regression analysis of medication compliance were performed . RESULTS Totally 2 684 questionnaires were distributed and 2 606 valid questionnaires were recovered ,with an effective recovery rate of 97.10%. The overall medication compliance of patients with severe mental disorders at home was 68.11%. The results of multivariate Logistic regression analysis showed that 51-60 years old ,unmarried, divorced or widowed ,partial social function defects ,no social function defects , and use of third -generation antipsychotic drugs were the promotion factors of medication compliance , (P<0.05);medical insurance ,non participation in work ,total number of hospitalizations of 1-4 times,total number of hospitalizations ≥ 5 times,general mental symptoms ,and use of second -generation antipsychotic drugs were the obstacles to medication compliance (P<0.05). The incidence of adverse drug reactions in patients with severe mental disorders a t home was 10.59%,mainly caused by second -generation antipsychotic drugs,and no new and serious adverse reactions were reported . CONCLUSIONS The medication compliance of patients with severe mental disorders at home is affected by many factors . It is urgent to need pharmacists ’participation to promote the standardization and rationality of patients ’medication.
6.Study on the effect of AIDS preventive cascade services provided by social organizations for men who have sex with men in China

Objective:To analyze the effect and main gaps of each stage in the AIDS prevention cascade for men who have sex with men (MSM) provided in intervention projects supported by the China AIDS Fund for non-governmental organizations (CAFNGO) and provide suggestions to improve the quality of cascade services and project management.Methods:Data were collected through the CAFNGO management information system and field interviews to analyze the differences in the number of MSM receiving HIV testing and confirming tests, the newly reported patients, and the number of antiviral treatment (ART) referrals of newly established reported patients among different social organization service areas. A service chain chart was also drawn.Results:Between 2016 and 2020, 1 508 MSM intervention projects were funded by CAFNGO, including 1 183 234 MSM being mobilized to receive HIV testing. However, only 68.8% (1 183 234/1 719 139) of the testing capacity of social organizations was covered by these projects. As a result, 55 783 HIV-positive MSM were detected in preliminary screening, and only 86.6% (48 327/55 783) received confirming tests. The proportion of newly reported infections was 3.8% (45 347/1 183 234). The ratio of antiviral treatment (ART) referrals for newly reported patients between 2017 and 2020 was 89.8% (32 719/36 444). 75.8%(1 143/1 508) of total MSM intervention projects were implemented by community-based organizations (Non-registered civil affairs departments). In comparison, organizations registered in civil affairs departments took up 24.2% (365/1 508) of the total MSM intervention projects. No significant difference was noticed in the proportion of newly reported infected (3.8% and 3.8%) and the ratio of ART referrals (89.7% and 89.9%) between community-based organizations and registered organizations' projects. But these two proportions are significantly different between these two types of organizations in some areas in China.Conclusions:The AIDS prevention cascade established in CAFNGO has effectively promoted the early detection and treatment of infected MSM. However, CAFNGO needs more financial support to extend testing coverage for MSM. Meanwhile, confirmation testing for positives in preliminary screening and ART referrals needs to be improved for newly reported patients. In addition, various capacity building needs to be provided for different social organizations.
7.A logistic regression model for prediction of glioma grading based on radiomics.

OBJECTIVES:
Glioma is the most common intracranial primary tumor in central nervous system. Glioma grading possesses important guiding significance for the selection of clinical treatment and follow-up plan, and the assessment of prognosis. This study aims to explore the feasibility of logistic regression model based on radiomics to predict glioma grading.
METHODS:
Retrospective analysis was performed on 146 glioma patients with confirmed pathological diagnosis from January, 2012 to December, 2018. A total of 41 radiomics features were extracted from contrast-enhanced T
RESULTS:
A total of 5 imaging features selected by LASSO were used to establish a logistic regression model for predicting glioma grading. The model showed good discrimination with AUC value of 0.919. Hosmer-Lemeshow test showed no significant difference between the calibration curve and the ideal curve (
CONCLUSIONS
The logistic regression model using radiomics exhibits a relatively high accuracy for predicting glioma grading, which may serve as a complementary tool for preoperative prediction of giloma grading.

8.Epidemic features of coronavirus disease 2019 in Hunan Province.

OBJECTIVES:
To explore and analyze the epidemic features of coronavirus disease 2019 (COVID-19) in Hunan Province from January 21, 2020 to March 14, 2020, as well as to investigate the COVID-19 epidemics in each city of Hunan Province.
METHODS:
The epidemic data was obtained from the official website of Hunan Province's Health Commission. The data of each city of Hunan Province was analyzed separately. Spatial distribution of cumulative confirmed COVID-19 patients and the cumulative occurrence rate was drawn by ArcGIS software for each city in Hunan Province. Some regional indexes were also compared with that in the whole country.
RESULTS:
The first patient was diagnosed in January 21, sustained patient growth reached its plateau in around February 17. Up to March 14, the cumulative confirmed COVID-19 patients stopped at 1 018. The cumulative occurrence rate of COVID-19 patients was 0.48 per 0.1 million person. The number of cumulative severe patients was 150 and the number of cumulative dead patients was 4. The mortality rate (0.39%) and the cure rate (99.6%) in Hunan Province was significantly lower and higher respectively than the corresponding average rate in the whole country (0.90% and 96.2%, Hubei excluded). The first 3 cities in numbers of the confirmed patients were Changsha, Yueyang, and Shaoyang. While sorted by the cumulative occurrence rate, the first 3 cities in incidence were Changsha, Yueyang, and Zhuzhou.
CONCLUSIONS
The epidemic of COVID-19 spread out smoothly in Hunan Province. The cities in Hunan Province implement anti-disease strategies based on specific situations on their own and keep the epidemic in the range of controllable.

9.Investigation on the Situation of Prescribing Extended Prescription and Analysis of Patient ’s Compliance in Execution in Jinshan District of Shanghai from 2017 to 2018

OBJECTIVE: To provide theoretical basis and data support for community extended prescription management and pharmaceutical care. METHODS: Data of all patients using extended prescription from 2017 to 2018 were derived from the health information system database of Jinshan district of Shanghai and 11 streets towns under its jurisdiction. Retrospective survey was used to analyze the basic medical information of extended prescription patients and the compliance with extended prescription execution (insisting on dispensing drugs through extended prescription within 2 years), and the factors affecting compliance were analyzed. RESULTS: From 2017 to 2018, there were 4 835 patients with Jinshan district extended prescription, with prescribing rate (number of extended prescriptions/number of prescriptions) of 2.08%, mainly patients over 60 years old (4 038 cases, 83.51%) and patients with common chronic diseases [4 793 casetimes, 83.18% (4 793/5 763)]; there was statistical significance in the prescribing rate of extended prescription among different streets and towns (P<0.05). The overall compliance rate of extended prescription patients in Jinshan district was 81.62% (62.02%-88.84%). There was no significant difference in compliance rate of extended prescription execution between male and female patients (P>0.05). Age, type of disease and types of extended prescription drugs had influence on compliance rate of extended prescription patients. Compliance rate of extended prescription in patients under 40 years of age or hypertension and chronic airway disease or those who were given one kind of drug was lower than other patients in same group (P<0.05). CONCLUSIONS: Big data can be used for monitoring the compliance of extended prescription in the future. At the same time, pharmacists participating in family doctor team formulate relevant guidance and education for poor compliance group so as to improve the rationality and effectiveness of extended prescription for patients.
10.An artificial neural network model for glioma grading using image information.

To explore the feasibility and efficacy of artificial neural network for differentiating high-grade glioma and low-grade glioma using image information.
Methods: A total of 130 glioma patients with confirmed pathological diagnosis were selected retrospectively from 2012 to 2017. Forty one imaging features were extracted from each subjects based on 2-dimension magnetic resonance T1 weighted imaging with contrast-enhancement. An artificial neural network model was created and optimized according to the performance of feature selection. The training dataset was randomly selected half of the whole dataset, and the other half dataset was used to verify the performance of the neural network for glioma grading. The training-verification process was repeated for 100 times and the performance was averaged.
Results: A total of 5 imaging features were selected as the ultimate input features for the neural network. The mean accuracy of the neural network for glioma grading was 90.32%, with a mean sensitivity at 87.86% and a mean specificity at 92.49%. The area under the curve of receiver operating characteristic curve was 0.9486.
Conclusion: As a technique of artificial intelligence, neural network can reach a relatively high accuracy for the grading of glioma and provide a non-invasive and promising computer-aided diagnostic process for the pre-operative grading of glioma.
