1.Comparative study on the impact of experiential communication and GLTC communication in outpatient settings on patients'emotions

Objective:From the perspective of patients'emotions,to compare and study the effectiveness between experiential communication and GLTC communication of outpatient doctors,understand the main communication details that affect patients'emotions,and provide a reference for improving patient emotions and enhancing doctor-patient communication skills in the future.Methods:From July 2021 to January 2022,a pilot study of GLTC communication program in outpatient settings was conducted in a total of 24 departments from 4 tertiary comprehensive hospitals in Jiangsu Province.Descriptive analysis,independent sample t-test,multiple linear regression and other analyses were conducted on the relevant data.Results:After experiential communication in outpatient settings,patients showed improvements in dimensions of emotion such as anxiety,fatigue,confusion,and depression,but there was a decrease in patient vitality(P<0.05).After GLTC communication in outpatient settings,patients'emotions in all six dimensions improved(P<0.05).Meanwhile,the GLTC group had better emotions than the experiential group after communication(P<0.05).After training,the completion rate of relevant communication details of doctors in the GLTC group was mostly significantly higher than that in the experiential within the same unit time(P<0.05).Multiple linear regression showed that smiling,comforting language,and kind gaze were the main factors affecting patients'overall emotions.Conclusion:Experiential communication in outpatient settings has certain communication effects,but there is still a lot of room for improvement compared to GLTC communication in outpatient settings.GLTC communication in outpatient settings can improve patients'emotions,and doctors'smiles,comforting language,and other communication details need to be emphasized in outpatient settings.How to improve the execution of GLTC communication in outpatient settings plans is an important direction for future research.
2.Intervention Mechanism of Biejiajian Wan on Primary Liver Cancer by Regulating lncRNA SNHG5/miRNA-26a-5p/GSK-3β Signal Axis

ObjectiveTo investigate the mechanism of Biejiajian Wan in the intervention of primary liver cancer based on long non-coding RNA SNHG5 (lncRNA SNHG5)/micro RNA-26a-5p (miRNA-26a-5p)/glycogen synthase kinase-3β (GSK-3β) signal axis. MethodDouble luciferase reporting assay was used to verify the targeted interaction between lncRNA SNHG5 and miRNA-26a-5p, miRNA-26a-5p, and GSK-3β in HepG2 cells. Nude-mouse transplanted tumor model of human HepG2 were established and randomly divided into model group, Biejiajian Wan low-dose group (0.5 g·kg-1), medium-dose group (1.0 g·kg-1), and high-dose group (2.0 g·kg-1), and sorafenib group (100 mg·kg-1), with 10 mice in each group. The mice were given intragastric administration of normal saline or drug for 28 days, and the tumor volume was measured at different time. Hematoxylin-eosin (HE) staining was used to observe the histological changes of tumors. The nucleic acid levels of lncRNA SNHG5, miRNA-26a-5p, GSK-3β, and β-catenin mPNA in tumor tissue were detected by real-time quantitative polymerase chain reaction (Real-time PCR). The protein expression levels of GSK-3β and β-catenin in tumor tissue were detected by western blot. ResultCompared with the SNHG5-WT (wild type) + miRNA NC (negative control) group, the relative luciferase activities of the SNHG5-WT + miRNA-26a-5p mimic group were decreased (P<0.05). Compared with the GSK-3β-WT + miRNA NC group, the relative luciferase activity of the GSK-3β-WT + miRNA-26a-5p mimic group was decreased (P<0.05). Compared with the model group, the tumor volume of Biejiajian Wan low-dose, medium-dose, and high-dose groups was significantly decreased (P<0.05, P<0.01). Compared with the model group, the cells in the tumor tissue of nude mice in each dose group of Biejiajian Wan were sparsely arranged with necrocytosis, which showed concentration-dependent changes. Compared with the model group, the expression levels of lncRNA SNHG5, GSK-3β, and β-catenin were decreased (P<0.05, P<0.01), while the expression of miRNA-26a-5p was increased in each dose group of Biejiajian Wan (P<0.05, P<0.01). Compared with the model group, the protein expression levels of GSK-3β and β-catenin were decreased in each dose group of Biejiajian Wan (P<0.05, P<0.01). ConclusionBiejiajian Wan may affect the necrosis of liver cancer cells through lncRNA SNHG5/miRNA-26a-5p/GSK-3β signal axis and thus play an anti-tumor role. This research will provide more theoretical basis for the clinical application of Biejiajian Wan.
3.Research progress of biomarkers in the prediction of retinopathy of prematu-rity

Retinopathy of prematurity(ROP)is a retinal vascular proliferative disease that occurs in preterm low-birth-weight infants and is the leading cause of blindness in children worldwide.Currently,with the intensive study of ROP,it has been found that retinal neovascularization can spontaneously regress at any time in the early stages of the disease.Therefore,early screening,diagnosis and intervention are particularly important in children with ROP.However,the exist-ing screening methods have problems such as long screening times and poor cooperation of infants and children,which may lead to missed diagnoses,thus more reliable prediction methods are needed.This article briefly describes the screening and prediction methods for ROP at home and abroad in recent years,and systematically analyzes the predictive value of different types of biomarkers for ROP,with a view to providing a basis for establishing a reasonable prediction model for ROP.
4.Management and Control of Perioperative Hypertension

Uncontrolled hypertension in the perioperative period may affect the hemodynamic stability of patients during surgery and thus affect the prognosis of patients.This increases the risk of other complications and death.For hypertensive patients undergoing surgery,it is necessary to weigh the pros and cons,and choose appropriate antihypertensive drugs to reduce the incidence of perioperative adverse events.There is no unified conclusion on perioperative blood pressure management in China.This paper systematically reviewed the perioperative blood pressure control objectives and drug use programs,including non-cardiac surgery,cardiac surgery,pregnancy,and pheochromocytoma.It provided a reference for the perioperative management of hypertensive patients.
5.Characteristics of individuals positive for SARS-CoV-2 nucleic acid in a centralized isolation site for people entering China

Objective:
To analyze the characteristics of individuals positive for SARS-CoV-2 nucleic acid in a centralized isolation site for people entering China in Huzhou City of Zhejiang Province from December 18, 2021 to January 12, 2022, so as to provide insights into the prevention and control of overseas imported COVID-19.
Methods:
The basic characteristics, nucleic acid detection and epidemiological investigations were collected from individuals positive for SARS-CoV-2 nucleic acid in a centralized isolation site for people entering China from December 18, 2021 to January 12, 2022, and the temporal distribution, population distribution, source of importation, and virus typing were descriptively analyzed.
Results :
From December 18, 2021 to January 12, 2022, a total of 2 974 individuals in 19 flights were recorded in this centralized isolation site, and 33 cases were tested positive for SARS-CoV-2 nucleic acid, including 21 confirmed cases with common type, 9 confirmed cases with mild type, and 3 cases with asymptomatic infections. There were 11 cases with Omicron infections ( 33.33% ), 5 cases with Delta infections ( 15.15% ), and 17 cases with infection of unidentified types ( 51.52% ). The median interval ( interquartile range ) from the time of entry to the time of a positive test was 4.0 ( 7.0 ) days among all positive cases, 0 ( 4.0 ) day among cases with Omicron infections and 4.5 ( 8.5 ) days among cases with infections of Delta and unidentified types. The positive cases had a mean age of ( 36.97±8.58 ) years, and included 27 men (81.82%). There were 30 cases ( 90.91% ) receiving two and more doses of COVID-19 vaccines, and 7 cases ( 21.21% ) with a previous history of SARS-CoV-2 infections. There were 19 cases ( 57.58% ) from African countries, and 7 of 11 cases with Omicron infections were imported from African countries.
Conclusion
Omicron infection was predominant among individuals positive for SARS-CoV-2 nucleic acid in this centralized isolation site for people entering China from December 18, 2021 to January 12, 2022, with no severe cases detected, and most positive cases were imported from African countries.
6.Advances in immunotherapy for human epidermal growth factor receptor 2-positive breast cancer

Human epidermal growth factor receptor 2 (HER2)-targeted therapy has greatly improved the prognosis of HER2-positive breast cancer. HER2-targeted therapy combined with chemotherapy dominated by trastuzumab+ pertuzumab is important in the neoadjuvant therapy, postoperative adjuvant therapy and late-stage standard treatment for HER2-positive breast cancer. Antibody-drug conjugates (ADCs) and tyrosine kinase inhibitors (TKIs) have further improved the efficacy of therapy. However, advanced breast cancer will eventually get a recurrence or drug resistance. HER2-positive breast cancer is characterized by moderate immunogenicity with the presence of large tumor-infiltrating lymphocytes (TILs), which provides a theoretical basis for immunotherapy. The application of HER2-targeted cancer vaccines and immune checkpoint inhibitors is promising and would offer more treatment options for the patients.
7.The relationship between body mass index and response time of cardioinhibitory type vasovagal syncope in children

Objective:To investigate the relationship between body mass index (BMI) and response time of cardioinhibitory type vasovagal syncope (VVS-CI) in children.Methods:The clinical data of 56 children with syncope or pre-syncope were retrospectively analyzed and they visited specialist clinic for syncope and were diagnosed as VVS-CI in the Second Xiangya Hospital, Central South University from December 2012 to September 2019.Based on height and weight, BMI was calculated, and divided into low BMI group (35 cases) and normal BMI group (21 cases). Between the 2 groups, baseline heart rate, head-up tilt test (HUTT) positive response heart rate, baseline head-up tilt test (BHUT) positive response time, and sublingual nitroglycerin-provocated HUTT (SNHUT) positive response time were compared.The correlation between BMI and positive response time was analyzed.SPSS 22.0 software was applied for statistical analysis.Results:There were no significant differences in age, sex, duration of disease and number of syncope between the 2 groups (all P>0.05). No significant differences were found in baseline heart rate and positive response heart rate between the 2 groups [(78.5±15.3) times/min vs.(72.8±8.7) times/min, t=1.223, P=0.230; (44.0±13.9) times/min vs.(47.0±10.0) times/min, t=-0.664, P=0.511]. Compared with normal BMI group, BHUT positive patients/SNHUT positive patients were higher in low BMI group (27/8 cases vs.9/12 cases, χ2=4.839, P=0.027), and the positive response time of BHUT was shorter [(13.1±4.6) min vs.(23.7±9.5) min, t=-2.691, P=0.023]. There were no significant differences in SNHUT positive response time between the 2 groups ( P>0.05). Low BMI was correlated with BHUT positive response time ( r=0.750, P=0.005). Normal BMI was not associated with BHUT positive response time ( r=0.316, P=0.217). There was no correlation between low BMI and normal BMI and SNHUT positive response time ( r=0.177, P=0.431; r=0.021, P=0.940). Conclusions:Low BMI is positively correlated with BHUT positive response time of children with VVS-CI.The time it takes for syncope occurrence was shorter in children with low BMI than that in normal BMI.
8.Differences of heart rate variability in cardioinhibitory type vasovagal syncope children with different body mass index

Objective:To investigate the difference of heart rate variability in cardioinhibitory type vasovagal syncope(VVS-CI) children with different body mass index(BMI).Methods:Clinical data of thirty-four children with syncope or pre-syncope were retrospectively analyzed, who visited specialist clinic for syncope and diagnosed as VVS-CI at the Second Xiangya Hospital of Central South University from January 2012 to December 2019.BMI was calculated based on height and weight, and divided into lean group(BMI≤18.4 kg/m 2, n=19) and normal group(BMI 18.5-23.9 kg/m 2, n=15). Heart rate variability(HRV) of 24 h dynamic electrocardiogram was analyzed using linear analysis method.Time domain index included SDNN, SDANN, rMSSD and pNN50.Frequency domain index included total power(TP), ultra low frequency power(ULF), very low frequency power(VLF), low frequency power(LF), high frequency power(HF) and LF/HF. Results:There was no significant difference in SDNN, SDANN and rMSSD between lean and normal group( P>0.05), but pNN50 increased in lean group( P<0.05). No significant differences were found in TP, ULF, LF, HF and LF/HF between two groups( P>0.05), while VLF was lower in lean group than that in normal group( P<0.05). There was no statistical difference in time domain index and frequency domain index between different gender between lean and normal group( P>0.05). SDNN, SDANN and LF were higher in<12 years old than those in≥12 years old in lean group( P<0.05). There was no statistical difference in rMSSD, pNN50, TP, ULF, VLF, HF and LF/HF( P>0.05). ULF increased and LF decreased in<12 years old compared to ≥12 years old in normal group( P<0.05). No statistical differences were found in SDNN, SDANN, rMSSD, pNN50, TP, VLF, HF and LF/HF( P>0.05). Conclusion:The autonomic nervous regulation function of VVS-CI children with low BMI and normal BMI is different, resulting in HRV difference.There were also differences in HRV between<12 years old and ≥12 years old with the same BMI.
9.Effect of clinical pharmacist intervention on compliance of patients with cerebral infarction

Objective To explore the factors resulting in poor medication compliance in patients with cerebral infarction and study the effect of medication education by pharmacist on the compliance of patients .Methods 128 patients with cerebral infarction recurrence from January to June of 2016 were selected .Patient′s medication history and knowledge were evaluated . The compliance was compared before medication education ,3 days and 30 days after medication education .Results Only 36.7% of the patients took drugs every day before hospitalization .The patient′s medication knowledge was limited .The poor compliance was related to the absence of medication education .The compliance was improved significantly after medication edu-cation .Conclusion Pharmacists play an important role in patient′s care .Pharmacists should take responsibility in medication education to improve medication compliance and reduce the recurrence of cerebral infarction .
10.Experimental study of integrin αvβ3 micro positron emission tomography (microPET)/CT imaging with 68Ga-DOTA-RGD2 in diagnosis of breast cancer osteolytic bone metastases

Objective To explore the value of integrin αvβ3 micro positron emission tomography (microPET)/CT imaging with 68Ga-DOTA-RGD2 in diagnosis of breast cancer osteolytic bone metastases.Methods MDA-MB-231 was injected in left ventricle of hairless mice at the density of 1.0×10 7/mL.Animal model with parathyroid hormone(PTH)-induced osteolysis in the calvarium was established.Animals were randomly divided into RGD group (n=10) and FDG group (n=10), and were detected by 68Ga-DOTA-RGD2microPET/CT and 18F-NaFmicroPET/CT respectively.Animals were sacrificed and bone lesions were collected for pathological examination.Results Bone radiotracer uptake ratio of osteolytie lesion to normal calvrium (O/N) was (4.18±0.570) by 68Ga-DOTA-RGD2, which was significantly higher than (1.24±0.28) in group FDG by 18F-NaFmicroPET/CT imaging (P<0.05).Conclusion 68Ga-DOTA-RGD2 integrin receptor αvβ3 imaging can effectively detect breast cancer bone metastases and locate osteolytic lesions.
