Objective To investigate the technical capacity and service quality of occupational health technical service institutions (hereinafter referred to as "occupational health institutions") in Guangdong Province. Methods All occupational health institutions in Guangdong Province that had valid occupational health service qualifications and within the validity period were included for analysis. Data on basic information, employed personnel, and results of professional technical capacity assessments across occupational health institutions were obtained through the Guangdong Provincial Occupational Health Technical Quality Control Center. Results A total of 99 institutions with 2 732 technical staff were included in this study. Occupational health institutions in Guangdong Province were mainly distributed in the Pearl River Delta region, accounting for 87.9% (87/99) of the total. The number of public and private health institutions was 23 and 76, accounted for 23.2% and 76.8% respectively. In terms of technical personnel, the percentage of individuals worked in public or private health institutions was 24.1% and 75.9%, respectively. Personnel titles were predominantly intermediate level and no title, accounting for 38.7% and 26.4%, respectively. Individuals with a bachelor′s degree or above accounted for 67.4%. Engineering and other professionals accounted for 35.4% and 30.5%, respectively. Private institutions undertook 97.3% of testing and evaluation workload related to occupational hazard in the province. The number of occupational health institutes acquiring category Ⅰ and Ⅱ service license were 97 and 13. Among institutions participating in inter-laboratory comparisons, the overall pass rates for quantitative items were 95.5% in public and 70.3% in private institutions, while the pass rates for qualitative items were 100.0% and 94.5%, respectively. Conclusion Occupational health institutions in Guangdong Province face issues such as imbalanced regional distribution, uneven development, and insufficient technical competence and testing capacity of professional personnel. Health authorities at all levels should continue to strengthen supervision and quality control to solidify the technical foundation and comprehensively enhance service capacity and quality.

Objective:To investigate the effectiveness of a free toe lateral ventral flap combined with metacarpophalangeal (MCP) osteotomy and tendon insertion reconstruction in treating severe lateral deviation in patients with postoperative duplicated thumb deformity. Methods:A retrospective analysis was conducted on the clinical data of children with severe lateral deviation after surgery for duplicated thumb deformity, treated at the Department of Pediatric Orthopaedics of the Ninth People’s Hospital of Wuxi City from January 2018 to June 2023. The children were divided into 2 groups: the control group underwent "MCP osteotomy and tendon insertion reconstruction", which included metacarpal (or phalangeal) osteotomy with internal fixation, interphalangeal joint (IPJ) and metacarpophalangeal joint (MCPJ) arthroplasty, as well as reconstruction of the insertion points of extensor and flexor tendons, collateral ligaments, abductor pollicis tendons, and joint capsules. The treatment group, in addition to the surgical steps performed in the control group, underwent reconstruction of the asymmetric nail fold using a free lateral toe flap and digital volume augmentation. The occurrence of postoperative complications was observed and recorded, and the circumference of the digit, joint lateral deviation angle, joint range of motion, and the postoperative scar conditions of the reconstructed thumbs of the 2 groups were compared. The appearance and function of the reconstructed thumbs were evaluated by using the Japanese Society for Surgery of the Hand (JSSH) score (total score of 20), the aesthetic scoring system (ASS) (total score of 18), and the modified Tada score (total score of 8). In all three scoring systems, the higher the score, the better the postoperative outcomes. The measurement data of normal distribution were expressed as Mean±SD, and independent sample t-test was used to compare the two groups. The count data were expressed as frequency, and χ2 test was used to compare the two groups. Results:A total of 41 children were enrolled, with 25 males and 16 females, aged 9 to 18 years (mean 11.3 years, ) with a time since initial surgery ranging from 6 to 16 years (mean 10.8 years). There were 25 cases in the control group and 16 cases in the treatment group. All surgeries were successfully completed, with no postoperative complications such as infection, skin or flap necrosis, or bone nonunion occurred. All children were followed up for 10-39 months (mean of 22.1 months). Preoperative comparison of the 2 groups in terms of gender, age, side of deformity, lateral deviation angle of MCPJ, lateral deviation angle of IPJ, and circumference of the digit was comparable with no statistically significant differences. At the final follow-up, there were no statistically significant differences between the two groups in MCPJ lateral deviation angle, IPJ lateral deviation angle, MCPJ range of motion, IPJ range of motion, Vancouver scar scale score, and modified Tada score (all P>0.05). However, in the treatment group, the ratio of reconstructed-side to healthy-side phalangeal circumference (1.05±0.04 vs. 0.76±0.03), JSSH score [(17.1±0.8) points vs. (15.2±0.6) points] and ASS score [(16.6±0.7) points vs. (14.5±0.7) points] were higher than those of the control group, and the differences were statistically significant (all P<0.01). Conclusion:The combination of a free lateral toe flap with MCP osteotomy and tendon insertion reconstruction for treating severe lateral deviation after surgery for duplicated thumb deformity fully leverages the technical advantages of microsurgery. The appearance of the lateral nail fold and the digital circumference are close to normal and can restore good thumb extension and flexion function.

Objective:To investigate the impact of high-density lipoprotein (HDL) subfraction cholesterol, measured by the vertical auto profile (VAP) technique based on vertical density gradient ultracentrifugation, on the occurrence and progression of coronary heart disease. Methods:This retrospective case-control study consecutively enrolled 94 inpatients diagnosed with coronary artery disease (CAD) by percutaneous coronary angiography at Ningbo University Affiliated First Hospital between June 2023 and June 2024 (CAD group), and 48 outpatients from the cardiology department without carotid or coronary atherosclerosis(non-CAD group). The VAP technique was employed to measure HDL subfraction cholesterol levels (HDL 3-C and HDL 2-C) and their subtypes (HDL 2a-C, HDL 2b-C, HDL 2c-C; HDL 3a-C, HDL 3b-C, HDL 3c-C, HDL 3d-C). Logistic regression analysis was performed to assess the association between HDL subfraction composition and CAD. CAD patients were further stratified by the number of affected coronary vessels (left anterior descending artery, left circumflex artery, and right coronary artery): 44 with single-vessel disease, 22 with double-vessel disease, and 28 with triple-vessel disease for correlation analysis. All CAD patients underwent 6-month clinical and telephone follow-up to record major adverse cardiovascular events (MACE), including acute myocardial infarction, stroke, and repeat revascularization. Using the median HDL 3d-C level (0.064 mmol/L) as cutoff, CAD patients were divided into high-level ( n=48) and low-level ( n=46) subgroups for Kaplan-Meier survival analysis with log-rank testing. Results:Compared with non-CAD controls, CAD patients showed significantly higher HDL 3d-C [0.064 (0.041, 0.095) mmol/L vs 0.055 (0.038, 0.067) mmol/L] and HDL 3b-C [0.031 (0.001, 0.054) mmol/L vs 0.007 (0.004, 0.029) mmol/L], lower HDL 3c-C (0.220±0.080 mmol/L vs 0.254±0.062 mmol/L) and HDL 3a-C [0.282 (0.224, 0.351) mmol/L vs 0.334 (0.269, 0.433) mmol/L] (all P<0.05). Logistic regression revealed that HDL2b-C was a protective factor against atherosclerosis severity ( OR=0.914, 95% CI 0.896-0.987, P<0.001); HDL 3d-C served as both a CAD risk factor ( OR=2.303,95% CI 1.740-3.047, P<0.001) and disease progression indicator ( OR=1.224, 95% CI 1.123-1.335, P=0.025). MACE patients ( n=6) had elevated HDL3d-C versus non-MACE cases ( n=88) [0.120 (0.083, 0.173) mmol/L vs 0.061 (0.037, 0.092) mmol/L, P<0.05]. The high HDL 3d-C subgroup demonstrated significantly lower 6-month survival (χ2=4.777, P=0.029). Conclusion:Contrary to conventional understanding, our study reveals that HDL2b serves as a protective factor against coronary artery disease progression, whereas HDL 3d-C acts not only as a pathogenic factor for CAD but also as a critical determinant of CAD-related adverse events.

Dust and chemical substances are widely present occupational hazards in workplaces. Long-term exposure to dust and chemical substances can pose serious threats to workers′ health. Owing to their advantages in real-time detection, rapid response, and high accuracy, online monitoring technologies enable continuous measurement and analysis of the concentration and composition of dust and chemical substances in workplaces. These technologies provide timely and effective data support for the prevention and control of occupational diseases and have become an important protective tool in the field of occupational hazard. Current online monitoring technologies for workplace dust mainly include the tapered element oscillating microbalance method, light scattering method, β-ray method, triboelectric charging, video exposure monitoring, and ultrasonic methods. Online monitoring devices for workplace chemical substances are still in the early stages of development. However, this equipment has been partially applied in environmental monitoring, covering methods such as spectral analysis, electrochemical sensors, cataluminescence sensors, and intelligent sensing systems. In the future, the development of online dust monitoring technology should focus on overcoming technical bottlenecks to improve detection accuracy and exploring the synergistic effects of different technologies to compensate for the limitations of single methods. Meanwhile, online monitoring technologies for chemical substances should aim to develop integrated detection systems that combine high precision, real-time performance, low cost, and stability.

Objective To establish a method for simultaneous determination of four high-molecular-weight thiol derivatives (TDs) in workplace air by gas chromatography. Methods The four kinds of vapor-phase macromolecular TDs (1-pentanethiol, 1-hexanethiol, 1-benzyl mercaptan, and n-octanethiol) in the workplace air were collected using the GDH-1 air sampling tubes, desorbed with anhydrous ethanol, separated on a DB-FFAP capillary column, and determined by flame ionization detector. Results The quantitation range of the four TDs was 0.30-207.37 mg/L, with the correlation coefficients greater than 0.999 00. The minimum detection mass concentrations and minimum quantitation mass concentrations were 0.18-0.32 and 0.60-1.05 mg/m3, respectively (both calculated based on the 1.5 L sample and 3.0 mL desorption solvent). The mean desorption efficiencies ranged from 87.07% to 103.59%. The within-run and between-run relative standard deviations were 1.92%-8.22% and 1.89%-8.45%, respectively. The samples can be stored at room temperature or 4 ℃ for three days and up to 7 days at -18 ℃. Conclusion This method is suitable for the simultaneous determination of four vapor-phase TDs in workplace air.

Objective:To evaluate the efficacy and safety of regional citrate anticoagulation (RCA) during plasma exchange (PE) in pediatric patients. Methods:We conducted a retrospective analysis of 12 critically ill children admitted to the Pediatric Intensive Care Unit (PICU) of Jinan Children's Hospital, who underwent 28 PE sessions with RCA between December 2023 and August 2024. Clinical records were reviewed to assess bleeding events, extracorporeal circuit performance, and changes in arterial blood gas parameters, serum total calcium (Ca tot), and activated clotting time before and after treatment. Results:No patients exhibited signs of increased bleeding. In one case, the procedure was discontinued prematurely due to elevated venous pressure. A significant decrease in ionized calcium (Ca ion) was observed 0.5 hours post-treatment. At the end of PE, pH, HCO 3?, base excess (BE), lactate, PaCO 2, Ca tot, and Na + levels increased, while K + and Ca ion levels decreased, with all changes being statistically significant. Four hours post-treatment, pH, HCO 3?, BE, PaCO 2, and Na + remained elevated, whereas Ca ion, lactate, and K + returned to baseline. By 12–15 hours post-treatment, all parameters—including pH, HCO 3?, BE, PaCO 2, Na +, K +, Ca ion, and lactate—had normalized, showing no significant differences from pre-treatment levels. Conclusions:RCA provides effective extracorporeal anticoagulation during pediatric PE without increasing bleeding risk. However, metabolic complications—primarily metabolic alkalosis—are common. These disturbances typically resolve spontaneously and do not lead to severe adverse events. While no ideal anticoagulant for PE has yet been established, RCA remains a safe and effective option, particularly for pediatric patients at higher risk of bleeding.

The aim of this research was to investigate the mechanism of chelidonine on H2 O2-in-duced inflammatory injury in porcine intestinal epithelial cells(IPEC-J2)through transcriptome sequencing.IPEC-J2 cells in the logarithmic growth phase were divided into the blank group(K group),H2O2 group(S group)and chelidonine group(L group),with three replicates in each group.Total RNA was isolated from each group for the purpose of constructing a sequencing li-brary.The assembled data underwent functional annotation, differential gene analysis,as well as GO and KEGG enrichment analyses.qPCR was used to confirm the expression of key differentially expressed genes(DEGs),and ELISA was utilized to assess the effect of chelidonine on the permea-bility of IPEC-J2 cells. The results indicated that the sequencing data met the necessary criteria and demonstrated a strong correlation between samples. The GO functional annotation results suggest that the intervention effects of chelidonine involve biological processes such as oxidative stress re-sponse and G2/M phase transition regulation of the mitotic cycle, and are closely associated with molecular functions, such as transmembrane transport activity. The KEGG enrichment analysis indicates that following H2 O2 treatment,DEGs in IPEC-J2 cells are predominantly enriched in the p53 signaling pathway, the coagulation cascade, the FoxO signaling pathway, and various other sig-naling pathways.Following pretreatment with chelidonine, the DEGs exhibit significant enrichment in several signaling pathways related to inflammation, including the TNF signaling pathway,syn-aptic vesicle cycle, and IL-17 signaling pathway. The results of qPCR were consistent with the se-quencing results.Chelidonine has also been found to effectively inhibit LDH release, elevate GLN content, and decrease DOA content. In conclusion,it can be seen that chelidonine can reduce cell permeability and alleviate H2 O2-induced inflammatory injury in IPEC-J2 cells by modulating in-flammation-related pathways such as the TNF signaling pathway.
