1.Clinical efficacy of modified Yiqi Huayu pelvic inflammation decoction combined with retention enema of traditional Chinese medicine and moxibustion at Shenque acupoint in the treatment of chronic pelvic inflammatory disease

Objective:To explore the clinical efficacy of modified Yiqi Huayu pelvic inflammation decoction combined with retention enema of traditional Chinese medicine (TCM) and moxibustion at Shenque acupoint in the treatment of patients with chronic pelvic inflammatory disease (CPID).Methods:A total of 120 CPID patients admitted to the Qian′an City Traditional Chinese Medicine Hospital from January 2023 to June 2024 were prospectively enrolled and randomly divided into the control group (60 cases, treated with modified Yiqi Huayu pelvic inflammation decoction) and the observation group (60 cases, treated with modified Yiqi Huayu pelvic inflammation decoction plus TCM retention enema and moxibustion at Shenque acupoint) using a random number table. Each course of treatment was 3 weeks, with drug withdrawal for 1 week during menstruation, and continuous treatment for 2 courses. The clinical efficacy of the two groups was observed. The TCM symptom scores, blood microcirculation indicators [whole blood low shear viscosity (LBV), whole blood high shear viscosity (HBV), platelet aggregation rate (PAR), hematocrit (HCT)], and serum inflammatory factors [tumor necrosis factor-α (TNF-α), interleukin-2 (IL-2), high mobility group box 1 (HMGB1), granulocyte-macrophage colony-stimulating factor (GM-CSF)] before and after treatment were compared. The patients were followed up for 3 months after treatment to compare the incidence of adverse reactions and recurrence between the two groups.Results:The total clinical effective rate of the observation group (95.00%, 57/60) was higher than that of the control group (83.33%, 50/60) ( P<0.05). After treatment, the individual scores of main and secondary symptoms and the total score of TCM syndrome in the observation group were lower than those in the control group (all P<0.05). After treatment, the levels of LBV, HBV, PAR, HCT, TNF-α, IL-2, HMGB1, and GM-CSF in the observation group were lower than those in the control group (all P<0.05). There was no statistically significant difference in the total incidence of adverse reactions between the two groups ( P>0.05). During the 3-month follow-up after treatment, the recurrence rate of the observation group was 3.33%(2/60), which was lower than 15.00%(9/60) of the control group ( P<0.05). Conclusions:Modified Yiqi Huayu pelvic inflammation decoction combined with TCM retention enema and moxibustion at Shenque acupoint has a definite efficacy in the treatment of CPID. It can effectively relieve clinical symptoms, improve blood microcirculation and inflammatory factor levels of patients, with a low short-term recurrence rate and no significant increase in the incidence of adverse reactions.
2.Quality and Safety Evaluation of Antibacterial Agents in Aciclovir Eye Drops Based on National Drug Sampling and Testing

Objective To establish a high-throughput HPLC method for the simultaneous determination of 11 common antibacterial agents in acyclovir eye drops and to evaluate the quality and safety of the antibacterial agents in 42 batches of national drug inspection samples.Methods Gradient elution was performed on a Kromasil 100-5-C18(4.6 mm×250 mm,5 μm)column with acetonitrile-5 mmol·L-1 ammonium acetate aqueous solution(containing 1%triethylamine,pH adjusted to 4.5 by acetic acid)as the mobile phase.The detection wavelength was 262 nm.Results Good linear relationships were obtained for 11 antibacterial agents(r≥0.999 9).The average recovery range was 98.2%-101.8%,and the RSD was 0.7%-2.7%(n=9).As revealed by the systematic analysis of 42 batches of national drug inspection samples,some batches of samples were detected with out-of-prescription antibacterial agents 4-hydroxybenzoic acid and ethylparaben,and the results were 0.02-49 μg·mL-1.This indicated that ethylparaben degradation and colinear production pollution were the two major risk sources.Conclusions The method is accurate,sensitive,and specific and can be used for the qualitative and quantitative detection of antibacterial agents in acyclovir eye drops.Besides,some products have degradation of antibacterial agents and incomplete cleaning during co-linear production.It is still essential to further reinforce product quality control.

5.Impact of smart physical education assignment on physical health of male college students

Objective:
To explore the impact of smart physical education assignment on physical health of male university students, so as to provide theoretical support and practical references for physical health improvement of male university students and implementing smart sports assignments.
Methods:
From September 2023 to January 2024, 317 sophomore male students from six Taekwondo elective classes at Hunan Institute of Engineering were selected and were randomly divided into an intervention group (n=157) and a control group (n=160). The intervention group was given sports assignments twice a week through smart means with an intervention duration of 15 weeks, each time for 25-35 minutes, in addition to the teaching according to the public course syllabus, while the control group was taught according to the public course syllabus. The physical and health indicators of both groups were tested before and after intervention,then the differences in various physical health indicators between two groups of students before and after intervention were compared through ttest and Mann-Whitney U test.
Results:
After the intervention, the vital capacity, 50 m run, sitandreach, 1 000 m run, and pullup scores of the intervention group significantly improved compared to those before intervention. The scores improved from (3 918.27±737.34)mL, 7.88(7.53,8.45)s, 9.80(2.70,15.75) cm, 4.30(4.12,4.50) min and 3.00(0.00,7.50) times to (4 574.19±800.61) mL, 7.65(7.37,8.12)s, 17.20(11.80,21.55)cm, 4.13(3.58,4.31)min and 5.00(1.00,10.00) times,respectively (t/Z=-7.60, 2.61, -8.39, 5.62, -2.72, P<0.05). Before intervention, there was no statistically significant difference in physical health indicators between the intervention group and the control group (P>0.05).After intervention,the scores of the intervention group on the vital capacity,50 m run,sitandreach,1 000 m run and pullup, were significantly higher than those of the control group [(4 310.97±808.90)mL, 7.75(7.40,8.30)s, 14.10(8.42,17.87)cm, 4.29(4.08,4.45)min and 4.00(1.00,7.00) times] (t/Z=2.91, -4.55, -4.75, -4.15, 2.58, P<0.05).
Conclusions
Having 25-35 min smart physical education assignment twice a week can effectively improve physical health level of male college students. It is recommended to assign appropriate amount of smart sports homework to improve physical health level of college students, while ensuring the amount and intensity of physical activity in public physical education courses.
6.Study on the Analgesic Material Basis of Zhejiang Vinegar Corydalis Rhizoma Based on Chemometrics

ABSTRACT
OBJECTIVE To explore the correlation between HPLC fingerprint and analgesic efficacy of Zhejiang vinegar Corydalis Rhizoma, and to screen out the material basis of analgesic effect of Zhejiang vinegar Corydalis Rhizoma.METHODS Established HPLC fingerprint of Zhejiang vinegar Corydalis Rhizoma and dysmenorrhea model in rats. The weight loss rate, writhing response, serum malondialdehyde(MDA), estradiol(E2), prostaglandin E2(PGE2) and prostaglandin F2α(PGF2α) levels were used as the evaluation indexes of analgesic efficacy. Combined with chemometrics, the main active components of analgesic effect of Zhejiang vinegar Corydalis Rhizoma and the important components with great contribution to the content were screened.RESULTS The fingerprints of different batches of Zhejiang vinegar Corydalis Rhizoma were established, and nine components were identified; compared with the blank group, the weight loss rate, writhing reaction, MDA, E2 and PGF2α levels in the model group were significantly increased. Compared with the model group, the above indexs in the administration group was decreased. Compared with the blank group, the PGE2 level in the model group was significantly decreased. Compared with the model group, the PGE2 level in the administration group was significantly increased. The vinegar Corydalis Rhizoma produced in Zhejiang had a good analgesic effect, and the main active components of its analgesic effect were protopine, palmatine hydrochloride, and dehydrocorydaline. The important components with greater contribution to the content were tetrahydropalmatine, protopine, palmatine hydrochloride and stylopine.CONCLUSION The efficacy of Zhejiang vinegar Corydalis Rhizoma is the result of the combined action of multiple components, and each component has a strong correlation with the pharmacodynamic indexes. This study provides a reference for the screening of analgesic material basis of Zhejiang vinegar Corydalis Rhizoma.
7.Slicing Angle Recognition of Fritillariae Thunbergii Bulbus Based on Improved YOLOv7-tiny Algorithm

ObjectiveTo realize the automatic recognition of the slicing angles of Fritillariae Thunbergii Bulbus (FTB) based on the improved YOLOv7-tiny algorithm. MethodFirstly, a diverse dataset of FTB images, totaling 16 000 pictures, with various angles was constructed. Furthermore, improvements were made to YOLOv7-tiny by replacing standard convolutions with ghost convolution (GhostConv), incorporating the coordinate attention (CA) mechanism as a preferred addition, substituting some activation functions with HardSwish function for decreasing the floating point operations. Additionally, a penalty term for angle recognition error was integrated into the loss function, and modifications were made to the non-maximum suppression (NMS) strategy to address cases where multiple detection results were associated with the same target. In order to verify the effectiveness of different improvement points on the optimization of the algorithm model, ablation experiments were carried out on all the improvement points, and the effectiveness of the improvement points was proved by comparing the prediction results before and after the addition of a certain improvement point on the basis of the original model or the model with the addition of an improvement point that has been verified to be effective, in order to evaluate the improvement of the indexes. ResultThe number of parameters required for the improved slicing angle recognition algorithm of FTB was about 55.4% of the original algorithm, and the amount of computation was about 59.4% of the original algorithm. The mAP@0.5[mean average precision at an intersection over union(IoU) of 0.5] increased by 12.2%, the mean absolute error(MAE) of the recognized angle was 5.02°, representing a reduction of 4.58° compared to the original algorithm. In the experimental environment of this paper, the average recognition time per image was as low as 8.7 ms, significantly faster than the average human reaction time. ConclusionThis study, by utilizing the improved YOLOv7-tiny algorithm, achieves effective slicing angle recognition of FTB with high accuracy and more lightweight, which provides a novel approach for stable and precise automated slicing of FTB, thereby providing valuable insights into the automation of processing other traditional Chinese medicines.
8.Radiotherapy enhances efficacy of PD-1 inhibitors in advanced hepatocellular carcinoma: A propensity-matched real-world study

Background::To address the need for immunotherapy in patients with advanced primary hepatocellular carcinoma (HCC), combination with radiotherapy (RT) has emerged as a promising strategy. In preclinical studies, irradiated tumors released tumor antigens to synergistically increase the antitumor effect of immunotherapy. Hence, we investigated whether RT enhances the efficacy of anti-programmed death receptor-1 (PD-1) inhibitors in advanced HCC in real-world practice.Methods::Between August 2018 and June 2021, 172 patients with advanced primary HCC were enrolled in the tertiary center (Zhongshan Hospital of Fudan University); 95 were treated with a combination of RT and the inhibitor of PD-1 (RT-PD1 cohort), and 77 were administered anti-PD-1 therapy (PD1 cohort). The first cycle of PD-1 inhibitors was administered within 60 days or concurrently with RT. Propensity score matching for bias reduction was used to evaluate the clinical outcomes.Results::Among 71 propensity-matched pairs, median progression-free survival was 5.7 months in the RT-PD1 cohort vs. 2.9 months in the PD1 cohort ( P <0.001). Median overall survival was 20.9 months in the RT-PD1 cohort vs. 11.2 months in the PD1 cohort ( P = 0.018). Compared with patients in the PD1 cohort, patients in the RT-PD1 cohort had significantly higher objective response rates (40.8%, 29/71 vs. 19.7%, 14/71, P = 0.006) and disease control rates (62.0%, 44/71 vs. 31.0%, 22/71, P <0.001). The incidences of toxic effects were not significantly different between the two cohorts. Conclusions::RT plus anti-PD-1 therapy is well tolerated. RT enhances the efficacy of anti-PD-1 therapy in patients with advanced primary HCC by improving survival outcomes without increased toxic effects.
9.Risk assessment of hepatocellular carcinoma in patients with hepatitis B-related cirrhosis and hypertension:a propensity score matching-based retrospective cohort study

Objective To analyze the factors affecting the risk of hepatocellular carcinoma(HCC)in patients with chronic hepatitis B-associated cirrhosis(CHB-Cir)complicated by essential hypertension(EH)and explore the impact of EH on HCC risk in patients with CHB-Cir.Methods This study was conducted among the patients with CHB-Cir with or without EH received antiviral therapy in the Infectious Disease Department,Third Affiliated Hospital of Xinxiang Medical University from January,2017 to January,2024.The cases with insufficient follow-up time or missing data were excluded.The patients were subjected to propensity score matching in a 1:1 ratio to form an EH group and a non-EH group.The Kaplan-Meier method was used to compare the cumulative incidence of HCC between the two groups,and the Cox proportional hazards regression model was used to analyze the risk of HCC and the factors affecting HCC risk.Results A total of 390 CHB-Cir patients(274 male and 116 female patients)were enrolled in this study,including 195 with EH and 195 without EH.In these patients,EH was significantly correlated with the occurrence of HCC(HR=1.69,P=0.002).Multivariate analysis suggested that the male gender(HR=1.73,P=0.005),a family history of liver cancer(HR=2.23,P<0.001),elevated alpha-fetoprotein(HR=2.83,P=0.001),elevated glutathione reductase(HR=1.53,P=0.046),reduced high-density lipoprotein(HR=1.46,P=0.027),and elevated low-density lipoprotein(HR=2.29,P=0.003)were all significantly correlated with HCC occurrence,while elevated triglycerides(HR=0.37,P<0.001)was a protective factor against HCC.In the EH group,treatment with non-RASIs drugs(HR=2.77,P=0.021)and no treatment/diuretic treatment(HR=7.18,P<0.001)were significantly correlated with HCC occurrence.Conclusion Hypertension increases the risk of HCC in patients with CHB-Cir,suggesting the importance of controlling hypertension in these patients.
