1.Research on the application of relaxation training combined with exercise intervention in colorectal cancer patients undergoing chemotherapy

Objective Analysis of the effects of relaxation training combined with exercise intervention in patients with colorectal cancer undergoing chemotherapy,aiming to provide reference for clinical nursing practice.Methods Using a convenience sampling method,80 colorectal cancer patients undergoing chemotherapy in the oncology ward of a tertiary A hospital in Changzhou,Jiangsu Province from November 2022 to November 2023 were selected as study subjects.Patients were divided into an experimental group and a control group using a random number table method,with 40 patients in each group.The experimental group received relaxation training combined with exercise interventionin addition to routine care provided to the control group.The control group received routine care.Differences in 6-minute walking distance,anxiety,depression and quality of life scores before and after six chemotherapy cycles were compared between the 2 groups.Results Finally,70 patients completed the intervention,with 35 patients in each group.After the intervention,there were differences in 6-minute walking distance,anxiety scores,depression scores,and overall health status scores between the 2 groups were all statistically significant(P<0.001).Conclusion Relaxation training combined with exercise intervention can maintain exercise endurance in colorectal cancer patients undergoing chemotherapy and alleviate anxiety and depression to some extent,helping to improve patients' quality of life.

3.Four cases of successful live birth after transferred with extremely low-grade cleavage-stage embryos on day 3 of IVF-ET and literature review

Objective:To explore the possible of clinical application value after transferred with day 3 extremely low-grade cleavage-stage embryos during in vitro fertilization and embryo transfer. Methods:The clinical outcomes of four cases with extremely low-grade cleavage-stage embryos on day 3 undergoing IVF-ET were retrospectively analyzed and the related literatures were reviewed.Results:Four cases successfully got pregnant after 12 days of transplantation of extremely low-grade embryos detected by human chorionic gonadotropin testing, and all of them were intrauterine singleton pregnancies. Finally these four patients had successful live births, and three of them were full-term delivery, one of them was delivery prematurely. All the babies were in the good health.Conclusion:There is still a degree of successful pregnancy and live birth rate after transferred with day 3 extremely low-grade cleavage-stage embryos. If the quality of all the embryos was extremely low-grade on day 3, we can provide new attempts and choices for the patients who were often giving up embryos transfer.

5.Cirrhotic portal hypertension reversal mechanism and its therapy

Portal hypertension is one of the most serious complications in patients with liver cirrhosis, and its prevention and treatment are essential to improve patient outcomes. The main pathophysiological basis of cirrhotic portal hypertension is increased intrahepatic vascular resistance and/or increased portal blood flow. In recent years, studies have suggested that liver sinusoid endothelial cells dysfunction, hepatic microvascular thrombosis, pathological angiogenesis, and gut-liver axis imbalance play critical roles in the development of portal hypertension. With respect to this, targeted therapy drugs have made significant advances. This article discusses the cirrhotic portal hypertension reversal mechanism and the current status of its treatment.
6.Construction and application of information system of nursing quality management based on PDCA vision field

Objective To design and apply the nursing quality management system of our hospital in order to strengthen nursing informationization construction, and improve management efficiency of nursing quality. Methods Based on the original design of nursing management system, the quality management system which take nursing as the main part was researched and developed by us and applied to the clinical. Our nursing management department combined PDCA cycle with our nursing quality management, and developed nursing unit quality evaluation criteria, each ward were checked by evaluating group of nursing quality management, the analysis and corrective measures should be asked to put forward. Results From July to December in 2016, nursing accidents, pressure ulcer, tumbling, pipe slippage, medication, errors and all adverse events incidence compared with 2015 year dropped 60.00%(0.03/0.05), 43.33% (0.13/0.30), 50.00% (0.04/0.08), 40.00% (0.04/0.10), 33.33% (0.01/0.03),43.85%(0.25/0.57), and all adverse events in 2016 compared to the period of 2015 had statistically difference (χ2=7.266, P<0.05), and the problem rectification rate was significantly higher than before(χ2=-3.212--2.637,P<0.05). Conclusion The construction and application of nursing quality management system realized the informationization and resource sharing of nursing quality management, it will help the quality of management to upgrade continuously and improve the efficiency of nursing quality management.
7.Analysis of GCDH gene mutations in 3 patients from Fujian area with glutaric academia type I.

OBJECTIVETo explore clinical features and mutation types in patients from Fujian area with glutaric academia type I(GA I).
METHODSSerum acylcarnitine and urine organic acid of 3 patients were determined with tandem mass spectrometry and gas chromatographic mass spectrometry. The patients also underwent magnetic resonance imaging analysis for the cranial region. Genomic DNA was extracted from peripheral blood samples, and the 12 exons and flanking regions of the GCDH gene were amplified with PCR and subjected to direct DNA sequencing. One hundred healthy newborns were used as controls.
RESULTSMutations of the GCDH gene were identified in all of the 3 patients. Two patients have carried compound heterozygous mutations including c.1244-2A>C and c.1147C>T(p.R383C), c.406G>T(p.G136C) and c.1169G>A(p.G390E), respectively. One has carried homozygous c.1244-2A>C mutation. The same mutations were not detected among the 100 healthy newborns. Only one patient received early intervention and did not develop the disease. The other two had irreversible damagesto their intelligence.
CONCLUSIONc.1169G>A(p.G390E) is likely pathogenic mutations for GA I patients from Fujianarea. Early screening of neonatal metabolic diseases is crucial for such patients.
8.FTO polymorphisms associated with metabolic components in Chinese obese children

Objective To investigate whether FTO gene SNP polymorphisms were associated with metabolic traits in Chinese children. Methods A total of 595 children aged between 6 and 17 years were enrolled. The five common SNPs in the FTO gene(rs1421085,rs17817449,rs8050136,rs3751812 and rs9939609)were genotyped. Metabolic compo-nents and insulin were measured. Results (1)The obese patients showed the components of the metabolic traits were significant difference compared to control group.(2)The allele frequency and the genotype distribution of the five SNPs were significant differences between two groups. Minor allele carriers of the five SNPs were associated with increased risks of childhood obesity compared with the wild-type genotype. (3)After adjustment of age, sex and BMI-z score,all of the five SNPs showed a trend towards higher SBP. In addition, rs3751812 and rs8050136 showed significant as-sociation with LDL-C and HOMA-IR levels. Conclusion The five common SNPs in the FTO gene contributes to the e-tiology of obesity and hypertension. The FTO gene polymorphisms (rs3751812 and rs8050136)are found to be associ-ated with increased insulin resistance and plasma LDL levels.
9.Comparison of clinical application of two definitions of metabolic syndrome in children and adolescents.

OBJECTIVETo compare and evaluate clinical applications of two definitions of metabolic syndrome in children and adolescents, which was developed by Pediatric Academy of Chinese Medical Association in 2012 (Chinese definition) and by International Diabetes Federation in 2007 (IDF definition), respectively.
METHODS593 obese children and adolescents aged 10 ≊16 y from July 2006 to December 2012 were enrolled in the study. The diagnostic concordance of two definitions for metabolic syndrome and individual components was estimated, and their sensitivity and specificity for detecting insulin resistance and early macrovascular complications were compared.
RESULTSThe concordance between two definitions for diagnosing metabolic syndrome was good (kappa=0.626); as for detecting the individual components, the Kappa concordance index were 1.000, 0.803, 0.780, 0.734 and 0.594 for hypertriglyceridemia, hyperglycemia, cholesterol abnormality and hypertension, respectively. The incidence of insulin resistance and early macrovascular complications, detected by the two definitions, were both increased with increasing number of abnormal components. The sensitivity and specificity for detecting insulin resistance in children with metabolic syndrome were 54.5% and 65.7% by Chinese definition, and 36.1% and 83.1% by IDF definition; while the sensitivity and specificity for detecting early macrovascular complications were 58.3% and 55.8% by Chinese definition, and 37.3% and 70.8% by IDF definition. After adjusting for age and sex, compared to the obese children and adolescents without metabolic syndrome, the odds ratios of insulin resistance and early macrovascular complications were 2.166 (P<0.001) and 1.771(P=0.008) for children with metabolic syndrome diagnosed by Chinese definition, and the odds ratio of insulin resistance and early macrovascular complications were 2.618 (P<0.001) and 1.357 (P=0.190) by IDF definition.
CONCLUSIONThe concordance between Chinese and IDF definitions for diagnosing metabolic syndrome in Chinese obese children and adolescents is good. Compared to IDF definition, Chinese definition is more sensitive for hypertension, hyperglycemia and hypercholesterolemia, thus it can more effectively detect insulin resistance and early macrovascular complication.

10.The role of dyslipidemia in metabolic syndrome in children and adolescents

The metabolic syndrome is composed of various metabolic disorders,which is associated with cardiovascular disease.Dyslipidaemia in the metabolic syndrome in children and adolescents,as defined by American Academy of Pediatrics and Chinese Society of Pediatrics,is characterised by elevated triglycerides and non-high-density lipoprotein cholesterol as well as low high-density lipoprotein cholesterol concentrations.Good evidence confirms that dyslipidaemia in children plays a vital role in the development of adulthood disease,such as atherosclerosis and hypertension,also closely related to metabolic syndrome,insulin resistance and hyperglycemia.Here we reviewed the the role of dyslipidemia in metabolic syndrome in children and adolescents.
