1.Echocardiographic features of critically ill patients with concurrent infections and their predictive value for 28-day mortality and cardiac injury

Objective To explore the echocardiographic features of critically ill patients with infection, and to evaluate the predictive value of echocardiographic parameters for 28-day mortality and myocardial injury. Methods Using a single-center prospective cohort design, 120 critically ill patients with infection admitted to the Intensive Care Unit, Jinshan Hospital of Fudan University from January 2024 to January 2025 were enrolled consecutively. According to the severity of infection, patients were divided into the sepsis group (n=75) and the non-sepsis group (n=45). Cox proportional hazards models and modified Poisson regression were used to analyze risk factors of 28-day all-cause mortality and myocardial injury. Kaplan-Meier survival curve and log-rank test were used to assess prognosis differences among patients with different ultrasound parameters. ROC curve and area under the curve (AUC) were applied to evaluate the predictive performance of ultrasound parameters, and restricted cubic spline (RCS) model was used to explore nonlinear relationships. Results The E/e′ ratio was significantly higher in the sepsis group than in the non-sepsis group (P<0.001), while left ventricular ejection fraction (LVEF) and left ventricular fractional shortening (LVFS) were lower in the sepsis group (P<0.05). The Cox proportional hazards model found no association between ultrasound parameters and 28-day all-cause mortality. Modified Poisson regression showed that, after adjusting for confounding factors, elevated E/e′ was an independent risk factor of myocardial injury (RR=1.12, 95% CI 1.07–1.17, P<0.001). Survival analysis demonstrated that patients with E/e′ >10.9 had lower 28-day survival rates (P=0.02), and the AUC for E/e′ predicting myocardial injury was 0.835. RCS analysis suggested a nonlinear relationship between E/e′ and myocardial injury risk (P<0.001). Conclusions E/e′, LVEF, and LVFS have no significant predictive value for 28-day mortality risk in critically ill patients with infections. Elevated E/e′ is an independent risk factor of myocardial injury in such patients, with a nonlinear relationship.
2.Changes and significance of neutrophil-to-lymphocyte ratio, monocyte-to-lymphocyte ratio, and procalcitonin, C-reactive protein, and interleukin-6 levels in neonates with septicemia

Objective:To investigate the changes and significance of neutrophil-to-lymphocyte ratio, monocyte-to-lymphocyte ratio, and procalcitonin, C-reactive protein, and interleukin-6 levels in neonates with septicemia.Methods:This study used a retrospective analysis mehod. A total of 69 neonates with septicemia who received treatment at the Neonatal Ward, Children's Hospital, The First People's Hospital of Chu Zhou were included in the study group. A total of 43 neonates without septicemia were included in the control group. Neutrophil-to-lymphocyte ratio (NLR), monocyte-to-lymphocyte ratio (MLR), and procalcitonin (PCT), C-reactive protein (CRP), and interleukin-6 (IL-6) levels were compared between the two groups. The changes in NLR, MLR, PCT, and CRP levels in neonates with septicemia were observed after treatment, compared with their pre-treatment levels. The receiver operating characteristic curve was used to analyze the value of laboratory indicators NLR, MLR, and PCT, CRP, and IL-6 levels in the diagnosis of septicemia in neonates.Results:The NLR, MLR, and PCT, CRP, and IL-6 levels in the study group were (4.79 ± 1.87), (0.49 ± 0.21), (8.41 ± 3.21) μg/L, (11.97 ± 4.23) mg/L, and (892.13 ± 361.43) ng/L, respectively. These values were significantly higher than those in the control group [(0.89 ± 0.23), (0.13 ± 0.06), (0.51 ± 0.26) μg/L, (2.16 ± 1.26) mg/L, (111.28 ± 51.69) ng/L, t = 10.11, 9.76, 23.06, 36.72, 15.74, all P < 0.05]. After treatment, NLR, MLR, and PCT and CRP levels in the study group were (3.04 ± 1.42), (0.29 ± 0.13), (1.26 ± 0.52) μg/L, and (3.28 ± 1.87) mg/L, respectively. These values were significantly lower than those measured before treatment in the same group [(4.79 ± 2.47), (0.49 ± 0.19), (8.41 ± 3.41) μg/L, (11.97 ± 5.26) mg/L, t = 6.50, 6.33, 33.05, 39.04, all P < 0.05]. The areas under the curve for NLR, MLR, and PCT, CRP, and IL-6 levels in diagnosing septicemia in neonates were 0.785, 0.765, 0.665, 0.763, and 0.753, respectively, with NLR showing the highest area under the curve. Conclusions:NLR and MLR are valuable for diagnosing and evaluating the prognosis of neonatal septicemia. Combined detection of PCT, CRP, and IL-6 levels can enhance the sensitivity and specificity of septicemia diagnosis.

Objectives:To investigate the clinical value of cardiac magnetic resonance imaging(CMR)feature-tracking strain analysis in risk stratification of diabetic heart failure with preserved ejection fraction(HFpEF).Methods:In this retrospective study,a total of 215 patients with diabetic HFpEF who underwent CMR at Chinese Academy of Medical Sciences Fuwai Hospital from January 2012 to December 2018 were included.Myocardial strain parameters were calculated using CMR feature-tracking technology.Patients were followed up by medical records or telephone calls.Composite endpoint event,all-cause death or heart failure hospitalization during follow-up were recorded.Patients were divided into event group and event-free group.Univariable and multivariable Cox proportional hazard regression analyses were performed to determine the risk factors for the outcomes in diabetic HFpEF.The effects of hypertension and obesity on the prognosis of diabetic HFpEF patients and whether they affect the prognostic value of CMR feature-tracking strain analysis were also analyzed.Results:During a follow-up of(7.1±1.8)years,93(43.3%)patients had endpoint events(event group),including 28 all-cause deaths and 65 heart failure hospitalization.Compared with the event-free group(n=122),patients in the event group had significantly lower left ventricular ejection fraction,higher prevalence and extent of late gadolinium enhancement,and significantly reduced global longitudinal strain(GLS),global circumferential strain,global radial strain,and global systolic longitudinal strain rate(all P<0.05).The absolute GLS value was significantly lower in event group than in event-free group,regardless of the presence of hypertension and obesity.Multivariate Cox regression analysis showed that estimated glomerular filtration rate(HR=0.983,95%CI:0.972-0.993,P=0.001),left atrial volume index(HR=1.015,95%CI:1.005-1.026,P=0.004),and GLS(HR=1.142,95%CI:1.060-1.231,P<0.001)were independent risk factors for adverse cardiovascular events in diabetic HFpEF patients.However,adjusted N-terminal pro-brain natriuretic peptide was not an independent prognostic factor.The cut-offvalue of GLS to predict outcome was-14.09%from ROC curve analysis.The Kaplan-Meier curve showed that in patients with and without hypertension and obesity,patients with the GLS>-14.09%had lower event-free survival compared to patients with GLS≤-14.09%(all P<0.05),and the ability of GLS to predict adverse outcomes was not affected by hypertension and obesity.Conclusions:GLS obtained by CMR feature-tracking strain analysis is an independent predictor of adverse outcomes in diabetic HFpEF,and its ability to predict adverse outcomes is independent of hypertension and obesity.

8.Tricuspid Valve Geometry of Idiopathic Pulmonary Hypertension:a Three-dimensional Transthoracic Echocardiography Study

Objectives:To evaluate the tricuspid valve(TV)geometric remodeling in patients with idiopathic pulmonary arterial hypertension(IPAH)by three-dimensional transthoracic echocardiography. Methods:Two-dimensional and three-dimensional transthoracic echocardiography were performed in 30 IPAH patients and 15 healthy controls,and the geometry parameters of TV were obtained by four-dimensional auto tricuspid valve quantitative(4D Auto-TVQ)in the right ventricular-focused apical view.Pulmonary arterial hypertension was determined by right heart catheterization within 48 hours of echocardiography. Results:The 4-chamber diameter,tricuspid annular(TA)perimeter,TA area,maximal tenting height,coaptation point height and tenting volume were larger in IPAH patients than those in healthy controls(all P<0.05),2-chamber diameter was similar between two groups.In IPAH group,maximal tenting height and coaptation point height were moderately correlated with right ventricular end-diastolic volume(r=0.710,r=0.515,both P<0.05),while TA perimeter,4-chamber diameter and TA area were moderately correlated with right atrial end-systolic volume(r=0.712,r=0.558,r=0.545,all P<0.05). Conclusions:IPAH patients have larger maximal tenting height,coaptation point height and tenting volume,TA enlargement is mainly visible in 4-chamber diameter.TV tenting height is associated with right ventricular volume,but TA size is associated with right atrial volume in IPAH patients.
