This paper summarizes Professor LIU Shangyi's clinical experience in treating acute mastitis based on the "membrane collaterals" theory. It is considered that constraint and stagnation of the membrane collaterrals is the primary pathogenic factor, with damp, heat, phlegm, and stasis being the secondary manifestations. If untreated, the condition can progress to heat toxin injuring the membrane. In treatment, the emphasis is diffusing and unblocking membrane collaterals, unblocking breast and relieving constraint. The clinical course of the disease can be divided into three stages including the early stage with mild constraint heat, the middle stage with escalating toxic heat, and the later stage with a decline of pathogens but deficiency of healthy qi. In correspondence, self-made Tongru Formula (通乳方) for diffusing membrane and venting constraint, self-made Toumo Jiedu Formula (透膜解毒方) for unblocking membrane and relieving toxin, and Tuoli Xiaodu Powder (托里消毒散) with modifications for restoring membrane and rectifying healthy qi should be used, respectively.

Aim To investigate the key targets and mechanisms of diabetic gastroparesis(DGP)by in-tegrating network pharmacology and molecular docking technology with animal experiments, and to specifically focus on exploring the effects of hedysarum polybotrys polysaccharide(HPS)on DGP through animal experi-mentation to validate its potential as a treatment for di-abetic gastroparesis. Methods The chemical constit-uents of HPS were analyzed, and the active chemical components of Radix Astragali were identified using the TCMSP database. The Swisstarget database was utilized to screen for HPS active ingredient targets, while DGP-related targets were identified from disease databases such as TTD,GeneCards,Drugbank, and DisGeNET.The STRING database was used to construct the PPI network, and Cytoscape 3.10.1 software was employed for network topology analysis and selection of key tar-gets.Subsequently,a compound-target-pathway net-work diagram was constructed.Key targets underwent GO function(biological function, molecular function, and cellular function)and KEGG pathway enrichment analysis using the Metascape database.Molecular doc-king was performed using Pymol 2.5 and AutoDock software.DGP rat model was established to observe the histopathological changes in small intestine after eight weeks of HPS intervention through HE staining.Addi-tionally,Western blot was conducted to detect the ex-pression of AGEs,RAGE,and NF-κB in eggs. The re-sults revealed a total of 302 key targets. Results A total of 302 key targets which were further analyzed for gene GO function and KEGG pathway enrichment.CUL3,YWHAZ,and NTRK1 were predicted as the key targets with critical pathways including the AGE-RAGE signaling pathway in diabetic complications, viral carci-nogenesis,hepatitis B,and alcoholism signaling path-way among others.Furthermore,in vivo experiments confirmed that HPS could improve small intestine histo-pathology in DGP rats, resulting in significant protective effects on this organ. It also reduced the expression of AGEs,RAGE,and NF-κB protein, hence achieving its purpose of treating DGP.Conclusion HPS has the characteristics of multi-component,multi-target and multi-pathway action, which may affect the regulatory role of AGE-RAGE signaling pathway on DGP,and provide new ideas for the subsequent clinical improve-ment of DGP.

Aim To investigate the effects of SAHA on the expression of P-gp and the pharmacokinetic pa-rameters of its substrate phenytoin sodium in rats under hypoxic environments. Methods Wistar rats were randomly divided into the normioxic group, the hypoxic model group, and the low-,medium-and high-dose vorinostat(SAHA)groups.Liver tissues were col-lected,and the expression levels of P-gp and HDAC5 were detected by Real-time PCR and Western blot. The morphological changes of liver tissues were ob-served by HE staining.Following intragastric adminis-tration of 50 mg·kg-1 phenytoin sodium to each group, blood samples were collected, and the plasma concentration of phenytoin sodium was determined u-sing UFLC-MS/MS to calculate pharmacokinetic pa-rameters.Results Compared with the normoxic group, the expression of HDAC5 in the liver tissues of hypoxia model rats increased, while the expression of P-gp decreased. After SAHA treatment,HDAC5 expression decreased, and P-gp expression increased.Among the SAHA groups, the medium-dose group showed the most significant effect, and HE staining re-sults indicated that this concentration did not cause damage to rat liver tissues.Compared with the normox-ic group, the AUC,Cmax, and T1/2 of phenytoin sodium in hypoxia model rats were significantly raised. After administration of the medium dose of SAHA,the AUC,Cmax,MRT,and T1/2 were significantly reduced, while CLZ/r was significantly increased. Conclusions Un-der hypoxic environments, the expression of P-gp in rat liver tissue is significantly downregulated, leading to increased systemic exposure of phenytoin, reduced clearance, and consequently elevated blood concentra-tions,raising the risk of central nervous system toxici-ty.In contrast,SAHA suppresses HDAC5 expression, thereby activating P-gp transcription and enhancing its efflux function. This results in decreased systemic ex-posure and improved clearance of phenytoin,signifi-cantly reducing drug accumulation in body and ulti-mately lowering the risk of adverse effects.

Aim To explore the key components and mechanisms of Lizhong decoction in treating rats with cold-damp diarrhea based on network pharmacology, molecular docking technology and animal experiments. Methods By literature review and database collec-tion,the components of Lizhong decoction, therapeutic targets, and the mapping with diarrhea disease targets were conducted to construct an intersection target pro-tein-protein interaction network for screening core tar-gets,and GO and KEGG pathway enrichment analysis was performed to build an"active component-target-pathway"network,followed by molecular docking vali-dation.Forty-eight rats were randomly divided into the normal control group(K),model group(DG),Lizhong decoction group(LZDG),and Pulsatilla decoction group(BTDG).Subsequently,a rat cold-damp diar-rhea model was established using Senna combined with low-temperature high-humidity environment, and the rats were intervened with Lizhong decoction and Pul-satilla decoction.HE staining was used to detect path-ological changes in intestinal tissue,ELISA was em-ployed to measure the levels of peripheral blood IL-6,IL-10,IL-1 β,and TNF-α,and western blot was used to determine the expression of colon tight junction pro-teins.Results Network pharmacology initially identi-fied 125 compounds in Lizhong decoction,5 186 drug target components,438 disease targets, and 60"drug-disease"shared targets.GO and KEGG enrichment a-nalysis showed that signaling pathways such as IL-17 and TNF were highly enriched.Molecular docking in-dicated that the core components of the drug had good binding activity with corresponding key targets.Liz-hong decoction could effectively improve the clinical symptoms of rats with cold-damp diarrhea, and com-pared with the DG group, the diarrhea rate, diarrhea in-dex,and other related indicators also gradually de-creased to normal levels.Compared with the DG group, the LZDG group showed reduced inflammation levels and a recovery in energy metabolism levels. Conclusion It can regulate targets such as MMP9 and IL-17 signaling pathways through multi-components like Calycosin and formononetin to exert its therapeutic effect on cold-damp diarrhea.
