Objective To investigate the temporal changes in pathological damage and macrophage polarization in liver and spleen tissues of mice infected with Angiostrongylus cantonensis, and to preliminarily unravel the peripheral immune responses during the early stage of A. cantonensis infection. Methods Forty female BALB/c mice at ages of 6 to 8 weeks were randomly divided into four groups, including the control group and 7-, 14-, and 21-day infection groups, with 10 mice in each group. Each mouse in the infection groups was inoculated with 30 third-stage (L3) larvae of A. cantonensis by oral gavage, and five mice were randomly selected from each infection group on days 7, 14, and 21 post-infection, while mice in the control group were given the same volume of physiological saline and five mice were randomly selected from the control group on the day of oral gavage. Mouse liver and spleen tissues were sampled. The histopathological changes of mouse liver and spleen tissues were observed using hematoxylin and eosin (HE) staining, and the percentage of positive staining area and the co-localization positive rates of the macrophage surface antigens F4/80, CD86, and CD206 were quantified in mouse liver and spleen tissues using immunohistochemical and immunofluorescence staining. In addition, five mice were collected from each infection group on days 7, 14, and 21 post-infection, and five mice were collected from the control group on the day of oral gavage. Mouse liver and spleen tissues were sampled for detection of macrophage markers CD86 and CD206 and macrophage phenotyping using flow cytometry, and the expression of M1 macrophage markers, including inducible nitric oxide synthase (Nos2), tumor necrosis factor-α (TNF-α), interleukin-1β (IL-1β) and M2 markers, including arginase 1 (Arg1), mannose receptor C-type 1 (Mrc1) and chitinase-like protein 3 (Chil3) was quantified in mouse liver and spleen tissues using real-time quantitative PCR (RT-qPCR) assay. Results Proliferative lesions of the hepatocyte were observed in mouse liver tissues and the follicular structures of the mouse spleen white pulp were disrupted 21 days post-infection with A. cantonensis. Immunohistochemical staining showed that there were significant differences in the percentages of F4/80, CD86 and CD206 positive staining areas in the liver and spleen tissues among the four groups of mice (F = 242.40, 197.14, 183.19, 157.65, 242.35 and 146.24; all P values < 0.001), and the percentages of positive staining in the liver and spleen tissues of mice in the 14-day infection group [(4.45 ± 0.51)%, (3.74 ± 0.67)%, (8.32 ± 0.72)%, (16.56 ± 1.14)%, (11.62 ± 0.52)%, and (8.29 ± 0.72)%, respectively] and the 21-day infection group [(3.70 ± 0.11)%, (3.22 ± 0.43)%, (11.53 ± 1.03)%, (12.59 ± 1.05)%, (9.02 ± 0.83)%, and (11.67 ± 1.10)%, respectively] were higher than in the control group [(0.35 ± 0.16)%, (0.40 ± 0.02)%, (0.93 ± 0.05)%, (2.78 ± 0.26)%, (2.33 ± 0.20)%, and (1.85 ± 0.20)%, respectively] (all P values < 0.05). Immunofluorescence staining showed significant differences in the positive rates of F4/80 co-localization with CD86 and CD206 in mouse liver and spleen tissues among the four groups (F = 24.42, 25.28, 54.51 and 130.55; all P values < 0.001). Flow cytometry detected significant differences in the proportions of CD86+ and CD206+ macrophages in mouse liver and spleen tissues among the four groups (F = 67.98, 18.41, 29.77, 172.80; all P values < 0.001), and the proportions of CD206+ macrophages in the liver and spleen of the 21-day infection group were significantly higher than those in the control group [(9.25 ± 2.55)% vs (3.83 ± 0.72)%, and (4.22 ± 0.56)% vs (0.47 ± 0.18)%, respectively] (both P values < 0.05). In addition, RT-qPCR assay quantified significant differences in the relative mRNA expression of M1 macrophage markers (IL-1β, TNF-α and Nos2) and M2 macrophage markers (Arg1, Chil3 and Mrc1) in mouse liver and spleen tissues among the four groups (F = 41.30, 31.82, 199.33, 19.96, 62.01, 119.76, 23.67, 95.90, 72.27, 82.59, 123.41 and 29.75; all P values < 0.05). Conclusions A. cantonensis infection may cause progressive pathological damage in mouse liver and spleen tissues, accompanied by dynamic temporal changes in macrophage polarization. M1 macrophage polarization predominates at the early stage of A. cantonensis infection and shifts towards M2 polarization at the later stages, suggesting that M2 polarization may participate in immune regulation at late stages of A. cantonensis infection by suppressing excessive inflammatory responses and promoting tissue repair.

Objective To explore the feasibility of magnetic compression technique (MCR) in the treatment of severe single mid-ureteral stricture in a rabbit model. Methods After 10 female New Zealand rabbits were selected, the distal end of the right ureter was ligated with silk thread. Two weeks after the operation, when the right ureter was dilated, the mid-ureter was ligated again with silk thread to create a severe single mid-ureteral stricture animal model. Longitudinal incisions were made at the upper and lower ends of the ligation site, and cylindrical NdFeB magnets with central holes (sub-magnets and main magnets) were placed to anastomose and recanalize the mid-ureteral stricture. The magnets were adjusted to the appropriate position to automatically align and attract each other. The longitudinal incision sites of the ureter were sutured with 6-0 Prolene thread, and the abdominal cavity was closed layer by layer. The operation time was recorded. X-ray films were taken every two days to confirm the position of the magnets and record the time of magnet detachment. Three weeks after the operation, the anastomotic specimens were obtained, and the formation of the anastomosis was evaluated with gross observation and histological staining. Results Two weeks after ligation, all experimental rabbits showed obvious dilation of the right ureter. The mid-ureteral ligation and placement of the male and female magnets were successfully completed in all 10 experimental rabbits, with an operation time of (24.00±5.15)minutes. The male and female magnets attracted each other well. X-ray examination showed that the magnets detached and entered the distal ureter (14.0±1.9) days after magnetic compression. The recanalization of the severe single mid-ureteral stricture was successfully achieved. The gross specimens and histological examination of the anastomosis showed that the anastomosis remained patent and the mucosal healing was good. All experimental rabbits survived well after the operation without serious complications. Conclusion MCT provides a new idea and novel method for the treatment of mid-ureteral stricture. This study confirmed the feasibility of this technique in treating severe single mid-ureteral stricture in a rabbit model. The operation is relatively simple and has potential for clinical application.

OBJECTIVE To explore the mechanism of the active ingredient compatibility(quercetin, quercitrin and kaempferol at a mass ratio of 2∶9∶3)of Dimocarpus longan Lour. leaves(abbreviated as CDL) on ameliorating glucose and lipid metabolism disorders in type 2 diabetes mellitus (T2DM) rats. METHODS SD rats were randomly divided into blank control group, model group, metformin hydrochloride group (100 mg/kg), and CDL high-, medium- and low-dose groups (280, 140, 75 mg/kg), with 10 rats in each group. Rats in the blank control group were fed with standard chow, while rats in the other groups were given high-sugar and high-fat diet combined with intraperitoneal injection of streptozotocin to establish the T2DM rat model. After successful modeling, rats in each administration group were given corresponding drug solution, and rats in the blank control group and model group were intragastrically administered with equal volume of pure water, once a day, for consecutive 4 weeks. Fasting blood glucose (FBG) was detected at fixed time every week. The curves of oral glucose tolerance test (OGTT) and intraperitoneal insulin tolerance test (IPITT) were plotted, and the area under curve (AUC) was calculated. The pancreatic islet function indexes [fasting insulin (FINS), homeostasis model assessment of insulin resistance (HOMA-IR), insulin sensitivity index (ISI)],blood lipid indexes [total cholesterol (TC), triglyceride (TG), low-density lipoprotein cholesterol (LDL-C), high-density lipoprotein cholesterol (HDL-C)] and hepatic glycogen content were determined. The pathological morphological changes of liver and pancreatic tissues were observed. The protein and mRNA expression levels of molecules related to phosphatidylinositol 3-kinase (PI3K)/protein kinase B (Akt) signaling pathway in liver tissues were detected. RESULTS Compared with the blank control group, the FBG, AUC of IPITT curve, AUC of OGTT curve, HOMA-IR, the levels of FINS, TC, TG and LDL-C, as well as the protein and mRNA expression of phosphatase and tensin homolog, forkhead box protein O1 and glycogen synthase kinase-3β in liver tissues were significantly increased in the model group ( P <0.05). ISI, the levels of HDL-C and hepatic glycogen content, along with the protein and mRNA expression of PI3K, insulin receptor substrate-1, Akt and protein expression of phosphorylated Akt in liver tissues were markedly decreased ( P <0.05). In model group rats, the arrangement of hepatocytes was irregular, the overall structure of pancreatic lobules was disordered, and a large number of inflammatory cell infiltration was observed. Compared with the model group, most of the above quantitative indexes were significantly reversed in the CDL high-dose group ( P <0.05), and the pathological lesions of liver and pancreas were obviously alleviated. CONCLUSIONS CDL can regulate glucose and lipid metabolism disorders, elevate insulin sensitivity and relieve insulin resistance in T2DM rats. Its mechanism may be related to the activation of the PI3K/Akt signaling pathway.

Objective To explore the correlation and lag effect between meteorological factors and the incidence of scarlet fever in Xicheng District, Beijing, and to provide a theoretical basis for the surveillance, early warning, and scientific prevention and control of scarlet fever. Methods Based on the daily scarlet fever incidence data and concurrent meteorological data in Xicheng District, Beijing from 2010 to 2019, the distributed lag non-linear model (DLNM) was used to analyze the impact of meteorological factors on the incidence of scarlet fever. Results The risk of scarlet fever was the highest when the daily average temperature was 34.2℃ with a lag of 0 days (RR=1.175, 95% CI:1.006-1.372). The risk was the second highest when the daily average temperature was 2.2℃ with a lag of 12 days (RR=1.123, 95% CI:1.044 -1.209). The cumulative relative risk of scarlet fever was statistically significant when the daily average temperature ranged from 27.2℃ to 34.2℃, with the highest cumulative relative risk at 34.2℃ (RR=1.906, 95%CI:1.215-2.989). When the daily average relative humidity was 19.5% with a lag of 6 days, the risk of scarlet fever incidence was the highest (RR=1.022, 95% CI: 1.005-1.040). The cumulative relative risk was statistically significant when the daily average relative humidity ranged from 24.7% to 40.2%, with the highest cumulative relative risk at 27.3% (RR=1.170, 95% CI: 1.020-1.343). The risk of scarlet fever was the highest when the daily average vapor pressure was 1.2 hPa with a lag of 5 days (RR=1.029, 95%CI:1.008-1.050). The cumulative relative risk of scarlet fever was statistically significant when the daily average vapor pressure was 1.2-7.4 hPa, and the highest cumulative risk was when the daily average vapor pressure was 1.2 hPa (RR=1.362, 95% CI:1.022-1.815). Conclusion There is a nonlinear relationship between meteorological factors and the incidence of scarlet fever in Xicheng District, Beijing, with a certain lag effect. Daily average temperature (27.2-34.2℃), daily average relative humidity (28.6~36.3%) and daily average vapor pressure (1.2-7.4 hPa) increase the risk of scarlet fever. These factors can be used as indicators for the prevention, control, surveillance, and early warning of scarlet fever.

ObjectiveTo explore the mechanisms through which Si Junzitang (SJZD) ameliorates Alzheimer's disease (AD) induced by scopolamine (SCOP) in mice and the PC12 cell model induced by H2O2 based on the Kelch-like ECH-associated protein 1 (Keap1)/nuclear factor erythroid 2-related factor 2 (Nrf2)/heme oxygenase-1 (HO-1) signaling pathway. MethodsIn the animal experiments, an AD model was established in mice by intraperitoneal injection of SCOP (3 mg·kg-1). Morris water maze and open field tests (OFT) were conducted to assess learning and memory abilities. Hematoxylin-eosin (HE) staining and Nissl staining were performed to observe pathological changes in neurons. Immunofluorescence was used to detect amyloid β-protein1-42 (Aβ1-42) expression, and immunohistochemistry was employed to detect phosphorylated (p)-Tau expression. Transmission electron microscopy (TEM) was employed to observe ultrastructural changes in hippocampal neurons and synapses. Biochemical methods were used to measure the levels of acetylcholine (ACh), acetylcholinesterase (AChE), superoxide dismutase (SOD), malondialdehyde (MDA), catalase (CAT), and lactate dehydrogenase (LDH). Real-time PCR and Western blot were employed to measure the mRNA and protein levels of molecules in the Keap1/Nrf2/HO-1 pathway in the hippocampus. In the cell experiments, a PC12 cell model of oxidative damage model was established with H2O2. Cell count kit-8 (CCK-8) assays and flow cytometry were adopted to measure cell viability and apoptosis rates, and Western blot was employed to quantify the expression levels of proteins in the Keap1/Nrf2/HO-1 pathway. ResultsThe animal experiments showed that compared with the model group, the SJZD and donepezil groups showed shortened escape latency (P<0.01), increased time in the target quadrant and platform crossings, and increased movement distance and duration in the central area of the open field (P<0.05, P<0.01). HE and Nissl staining showed more organized neurons and increased Nissl bodies in the drug intervention groups (P<0.05, P<0.01), and the Aβ1-42 and p-Tau expression levels were downregulated (P<0.05, P<0.01). TEM revealed reduced ultrastructural damage in hippocampal neurons and synapses in the drug intervention groups. In addition, the drug intervention groups showed declined levels of MDA, LDH, and AChE (P<0.05, P<0.01), elevated levels of SOD, CAT, and ACh (P<0.05, P<0.01), reduced Keap1 expression and increased Nrf2, HO-1, and NQO1 expression in the hippocampus (P<0.05, P<0.01). The cell experiments showed that compared with the model group, the SJZD-containing serum increased the cell viability (P<0.05, P<0.01), and decreased total apoptosis rates (P<0.05, P<0.01). The drug intervention groups showed upregulated protein levels of Nrf2 and HO-1 and downregulated protein level of Keap1 (P<0.05, P<0.01). ConclusionSJZD demonstrates protective effects against SCOP-induced AD in mice and H2O2-induced damage in PC12 cells through antioxidant mechanisms mediated by the Keap1/Nrf2/HO-1 pathway.

ObjectiveTo investigate the mechanisms of Tongmai Yangxin pills (TMYX) and Tongxinluo capsules (TXL) in treating no-reflow (NR) after myocardial ischemia and reperfusion following the concept of treating the same disease with different methods, based on integrative pharmacology and experimental validation. MethodsEighty 8-week-old SPF-grade SD rats were randomly assigned into four groups (n=20): sham operation, NR, TMYX (4 g·kg-1), and TXL (2 mg·kg-1). A rat model of myocardial ischemia-reperfusion no-reflow was established by in-situ ligation of the left anterior descending coronary artery. Gastric gavage was first performed 4 h after the operation, and samples were collected on day 7. Thioflavin S staining was used to observe the NR area in rat myocardium. Echocardiography was performed to examine the cardiac function. Hematoxylin-eosin (HE) staining was conducted to observe the pathological changes of the myocardial tissue. An automatic biochemical analyzer was adopted to measure myocardial enzyme activity. Then, integrated pharmacology was applied for Kyoto Encyclopedia of Genes and Genomes (KEGG) enrichment analysis. Finally, Western blot was employed to quantify the protein levels of key targets in the myocardial tissue, including soluble guanylyl cyclase (sGC), cyclic guanosine monophosphate (cGMP)/dependent protein kinase (PKG), phosphorylated phosphatidylinositol 3-kinase (p-PI3K), phosphorylated protein kinase B (p-Akt), and hypoxia-inducible factor-1α (HIF-1α). ResultsCompared with the sham operation group, the NR group showed increased NR area of myocardium, decreased left ventricular ejection fraction (EF), left ventricular fractional shortening (FS), left ventricular outflow tract peak velocity (LVOT Peak), and left ventricular stroke volume (LVSV) (P<0.01), fractured and disordered myocardial fibers as well as inflammatory cell infiltration in the myocardial tissue, enhanced activities of creatine kinase (CK), creatine kinase isoenzyme (CK-MB), and lactate dehydrogenase (LDH) in the myocardial tissue (P<0.01), and downregulated protein levels of sGC, PKG, phosphorylated (p)-PI3K (Tyr458), and p-Akt (Tyr315) in the myocardial tissue (P<0.05, P<0.01). The expression level of HIF-1α protein showed a downward trend. Compared with the NR group, the TMYX group and TXL group exhibited a decreasing trend in myocardial NR area, EF, FS, and LVOT Peak significantly increased (P<0.05, P<0.01), LVSV showed an upward trend, ameliorated myocardial pathological morphology and inflammatory infiltration, reductions in CK, CK-MB, and LDH activities (P<0.05, P<0.01), and upregulated protein levels of sGC, PKG, p-PI3K (Tyr458) in myocardial tissue (P<0.05, P<0.01), the protein expressions of p-Akt (Tyr315) and HIF-1α showed an upward trend. A comparison of the therapeutic effects between the two compound prescriptions showed that TMYX tended to exert a better effect in restoring cardiac function and protecting cardiac structure in NR rats, whereas TXL was more effective in reducing myocardial NR area and lowering myocardial enzyme activities in NR rats. Integrative pharmacology analysis combined with experimental verification demonstrated that both TMYX and TXL could alleviate NR through the following mechanisms: activating the cyclic cGMP/PKG signaling pathway to regulate vascular tone, activating the PI3K/Akt signaling pathway to dilate blood vessels, and activating the HIF-1 signaling pathway to inhibit oxidative stress. However, TMYX had an advantage in activating the cGMP/PKG pathway, while TXL was superior in activating the PI3K/Akt pathway. The two compound prescriptions exerted comparable effects on the HIF-1 signaling pathway, which might serve as their common therapeutic pathway. ConclusionBoth TMYX and TXL could alleviate NR damage. TMYX exerts its protective effect against NR mainly by activating the cGMP/PKG signaling pathway, while TXL exerts its effect mainly through the PI3K/Akt pathway. The HIF-1α signaling pathway may be a common pathway for the two compound prescriptions to exert their protective effects against NR. This study reveals the similarities and differences between TMYX and TXL in the treatment effect and mechanism for NR, providing an experimental basis and a theoretical basis for better clinical application of the two compound prescriptions.

Journal of Public Health and Preventive Medicine 2026;37(5):111-115
Objective To analyze the predictive value of the combined application of triglyceride-glucose index (TyG) and triglyceride-glucose-body mass index (TyG-BMI) for the severity of coronary artery lesions in postmenopausal patients with coronary heart disease (CHD). Methods Menopausal women who were diagnosed by coronary angiography and received treatment in the cardiovascular department were selected as the research subjects. According to the Gensini score, the patients were divided into a moderate group (n=151) and a severe group (n=203). The clinical indexes of the two groups were compared and ROC analysis was used to predict the efficacy. Results The BMI, age, hypertension rate, WBC, PLT, NEUT, TC, TG, LCI, TyG, and TyG‑BMI in the severe group were significantly higher than those in the moderate group, while ALB was significantly lower than that in the moderate group (P<0.05). Multivariate logistic regression showed that age, hypertension, WBC, TyG, and TyG‑BMI were independent risk factors for severe coronary artery disease in postmenopausal women (P<0.05). ROC curve analysis showed that the AUC of TyG was 0.745, the AUC of TyG‑BMI was 0.797, and the AUC of TyG combined with TyG‑BMI was 0.803, all showing good predictive performance. Conclusion TyG and TyG‑BMI are effective indicators for predicting the severity of coronary artery disease in postmenopausal CHD patients. The combination of the two can further improve predictive efficacy, providing a simple and economical reference for early clinical risk stratification.

ObjectiveTo investigate the feasibility and efficacy of modified video-endoscopic inguinal lymphadenectomy (M-VEIL), guided by the layer surgery concept, in reducing postoperative complications in patients with penile cancer. MethodsA retrospective analysis was conducted on 27 patients with penile cancer who underwent bilateral laparoscopic inguinal lymphadenectomy at the Affiliated Hospital of Qingdao University between March 2018 and March 2022. All patients had undergone prior penile surgery. M-VEIL followed a standardized four-step procedure guided by fascial planes to achieve compartment-oriented resection, with deep lymphatic trunks ligated by silk sutures and endoscopic compression suture of the skin to the muscle layer using 4-0 absorbable threads. ResultsPreoperative physical examinations and imaging findings revealed bilateral inguinal lymphadenopathy, and no distant metastases were detected in any patient. Among the 27 patients, 12 underwent traditional video-endoscopic inguinal lymphadenectomy (T-VEIL; mean age 61.08±12.75) years and 15 underwent M-VEIL (mean age 54.47±14.64) years. All 27 operations were successfully completed without conversion to open surgery. No significant differences were observed between the two groups in operative time, intraoperative blood loss, postoperative hospital stay, number of lymph nodes retrieved, or lymph node positivity rate (all P >0.05). Drain removal time was significantly shorter in the M-VEIL group (15.93±6.89 days vs. 23.33±10.42 days; P<0.05). Lymphatic leakage (2 vs. 8 patients) and surgical site infection (1 vs. 6 patients) occurred less frequently in the M-VEIL group (both P <0.05). No significant differences were found in lymphocele, flap necrosis, lymphedema, or lower limb edema between the two groups (all P >0.05). During postoperative follow-up for 1-3 years, 2 patients developed pelvic lymph node metastases, while the remaining 25 showed no evidence of local or pelvic recurrence. ConclusionCompared with T-VEIL, M-VEIL appears to reduce postoperative complications and shorten drain removal time in patients undergoing inguinal lymphadenectomy for penile cancer, while providing favorable clinical outcomes. Preliminary evidence suggests that M-VEIL is a safe and feasible modified procedure.

Objective:To investigate the protective effect of prunetin on the neurons in the rats with cerebral ischemia reperfusion injury(CIRI),and to clarify its possible mechanisms.Methods:Thirty-six SD rats were randomly divided into sham operation group, model group, low dose of prunetin group(3.5 mg·kg-1),medium dose of prunetin group(7.0 mg·kg-1),high dose of prunetin group(14.0 mg·kg-1),and positive drug edaravone(Eda)group(n=6).Zealonga method was used to evaluate the neurological function damage of the rats in various groups;open field experiment was used to evaluate the autonomous motor function;Triphenyltetrazolium chlorde(TTC)staining was used to evaluate the areas of cerebral infarction of the rats in various groups;HE staining and Nissl staining were used to observe the pathomorphology of brain tissue of the rats in various groups.Additionally,twenty-one SD rats were randomly divided into sham operation group, model group, prunetin group,c-Jun N-terminal kinase(JNK)inhibitor group,p38 inhibitor group,JNK inhibitor+prunetin group, and p38 inhibitor+prunetin group(n=3).TUNEL staining was used to detect the positive rates of apoptosis of neurons of the rats in various groups;Western blotting method was used to detect the expression levels of apoptosis-related proteins and JNK/p38 signaling pathway-related proteins in brain tissue of cerebral infarction side of the rats in various groups. Results:Compared with sham operation group, the neurological deficit score of rats in model group was significantly increased(P<0.001),the total motor distance was shortened(P<0.001),and the ratio of cerebral infarction area was increased(P<0.001).In sham group, the neuronal structure in the rat brain tissue was clear and well-organized,with an abundance of Nissl bodies and no apparent pathological changes observed.Compared with model group, the neurological deficit scores of the rats in medium and high doses of prunetin groups were decreased(P<0.05),total motor distances of rats were increased(P<0.05),and the cerebral infarction areas of rats were decreased(P<0.05);the neurons showed disarrayed arrangement, cytoplasmic condensation, nuclear consolidation, and lysing and deletion of Nissl bodies were decreased.Compared with sham operation group, the positive rate of apoptosis of neurons in model group was significantly increased(P<0.001),the expression level of B-cell lymphoma-2(Bcl-2),Bcl-2-associated X protein(Bax)and cleaved Caspase-3 proteins in brain tissue of the rats were significantly increased(P<0.05 or P<0.01).Compared with model group, the positive rats of apoptosis of neurons of the rats in prunetin group were decreased(P<0.05),the expression level of Bcl-2 protein in brain tissue of the rats was increased(P<0.001),and the expression levels of Bax and cleaved Caspase-3 proteins were significantly decreased(P<0.05).Compared with inhibitor groups, the positive rates of apoptosis of neurons in inhibitor+prunetin groups were decreased(P<0.01),and the expression levels of p-JNK and p-p38 proteins in brain tissue of the rats as well as the ratios of p-JNK/JNK and p-p38/p38 were decreased(P<0.05).Conclusion:Prunetin has the effect of reducing the neurological function damage, decreasing the area of cerebral infarction, reducing the pathological damage, and inhibiting neuronal apoptosis in the rats, and its mechanism may be related to inhibiting neuronal apoptosis through regulating the JNK/p38 signaling pathway.
