Background:
High-mobility group box protein 1 (HMGB1) is a chromatin-binding protein involved in arthritis, ischemia, sepsis, atherosclerosis, neurodegenerative disorders, meningitis, and cancer. HMGB1 exhibits dual roles in cancer, acting as either a tumor suppressor or oncoprotein depending on context.
Objective:
This research aimed to elucidate HMGB1’s functional significance in cutaneous squamous cell carcinoma (cSCC).
Methods:
We overexpressed HMGB1 in cSCC cell lines using recombinant adenovirus and examined its effects on cell proliferation, colony formation, and cell migration.
Results:
Immunohistochemical analysis revealed elevated HMGB1 expression levels in cSCC tissue relative to normal epidermis. To assess the influence of HMGB1, we employed recombinant adenoviruses expressing HMGB1 to transduce SCC cell lines (SCC12 and SCC13). Enhanced HMGB1 expression significantly promoted cellular proliferation and colony formation capacity.Notably, HMGB1 overexpression elevated the levels of proliferation regulators, including P63, SOX2, CDK4 and CDK6. Furthermore, HMGB1 overexpression substantially enhanced tumor invasiveness, accompanied by upregulation of epithelial-mesenchymal transition (EMT) biomarkers. Mechanistically, overexpression of HMGB1 enhanced transforming growth factor-β signaling by increasing phosphorylation of SMAD2/3, the key mediators of EMT.
Conclusion
These data imply that HMGB1 acts as a tumor-promoting factor in cSCC.

Diabetic cataract, a prevalent ocular complication of diabetes mellitus, arises from a complex interplay of pathological mechanisms, with oxidative stress and glycation stress playing central roles. Autophagy, a critical cellular self-protection mechanism, sustains intracellular homeostasis by selectively degrading damaged organelles and misfolded proteins, thereby counteracting the detrimental effects of oxidative and glycation stress under hyperglycemic conditions. Emerging evidence indicates a synergistic interaction between glycation stress and oxidative stress, which may exacerbate autophagic dysfunction and accelerate the onset and progression of diabetic cataract. However, the precise molecular mechanisms underlying this relationship remain incompletely understood. This review systematically examines the regulatory role of autophagy inthe pathogenesis of diabetic cataract, with a particular focus on how autophagic impairment influences disease progression under the combined effects of glycation and oxidative stress. By elucidating these mechanisms, the paper aims to provide novel insights into molecular diagnostic approaches and targeted therapeutic strategies for diabetic cataract.

This paper summarizes Professor AI Rudi's experience in the diagnosis and treatment of skin pruritus based on the "unity of restoring form, regulating qi, and harmonizing spirit", employing internal herbal medicine combined with external treatments. It is believed that the core pathogenesis of pruritus is the "imbalance of form, qi, and spirit", with disturbed spirit as the onset, disordered qi as the key pathogenic factor, and physical changes as the manifestation of the disease. The treatment principle follows "restoring form-regulating qi-harmonizing spirit", with a combination of internal and external therapies, and differentiation based on deficiency and excess. For excess conditions caused by pathogenic disturbances to the heart spirit, treatment is based on different patterns of wind-heat, damp-heat, and blood-heat, using Sangye (Morus alba)-Sangbaipi (Morus alba cortex)-Longchi (Draconis os) to disperse wind and clear heat, calm the spirit; Difuzi (Kochia scoparia)-Qinghao (Artemisia annua)-Tanxiang (Santalum album) to clear damp-heat and aromatically open the spirit; Mudanpi (Paeonia suffruticosa)-Chuanxiong (Ligusticum chuanxiong)-Shuiniujiao (Bubalus bubalis cornua) to cool the blood, activate circulation, and calm the spirit. For deficiency conditions caused by insufficient nourishment of the heart spirit, treatment is based on patterns of qi deficiency or blood deficiency, using Huangqi (Astragalus membranaceus)-Fuping (Lemna minor)-Wuweizi (Schisandra chinensis) to tonify the qi and stabilize the exterior; Heshouwu (Polygonum multiflorum)-Jili (Tribulus terrestris)-Shouwuteng (Polygonum multiflorum vine) to nourish the blood, moisten dryness, and calm the spirit. External treatments integrate traditional Chinese medicine therapies such as medicinal baths, gua sha, and ear acupuncture, with custom herbal wash formulas for restoring form, jojoba oil gua sha for regulating qi, and ear seed therapy using Wangbuliuxing (Vaccaria segetalis) for harmonizing the spirit, achieving a holistic treatment effect for form, qi, and spirit.

Background:
Maturity-onset diabetes of the young (MODY) due to variants of hepatocyte nuclear factor 1-beta (HNF1β) (MODY5) has not been well studied in the Chinese population. This study aimed to estimate its prevalence and evaluate the application of a clinical screening method (Faguer score) in Chinese early-onset diabetes (EOD) patients.
Methods:
Among 679 EOD patients clinically diagnosed with type 2 diabetes mellitus (age at diagnosis ≤40 years), the exons of HNF1β were sequenced. Functional impact of rare variants was evaluated using a dual-luciferase reporter system. Faguer scores ≥8 prompted multiplex ligation-dependent probe amplification (MLPA) for large deletions. Pathogenicity of HNF1β variants was assessed following the American College of Medical Genetics and Genomics (ACMG) guidelines.
Results:
Two rare HNF1β missense mutations (E105K and G454R) were identified by sequencing in five patients, showing functional impact in vitro. Another patient was found to have a whole-gene deletion by MLPA in 22 patients with the Faguer score above 8. Following ACMG guidelines, six patients carrying pathogenic or likely pathogenic variant were diagnosed with MODY5. The estimated prevalence of MODY5 in Chinese EOD patients was approximately 0.9% or higher.
Conclusion
MODY5 is not uncommon in China. The Faguer score is helpful in deciding whether to perform MLPA analysis on patients with negative sequencing results.

Background:
s/Aims: Distal pancreatectomy with splenectomy (DPS) is a common surgical procedure for pancreatic body cancer. However, spleen-preserving distal pancreatectomy (SPDP) utilizing the Warshaw technique (WT) in malignancies is generally not favored due to concerns about inadequate resection. This study aims to assess the feasibility and oncologic outcomes of employing SPDP with WT in pancreatic body cancer.
Methods:
We conducted a retrospective analysis comparing 21 SPDP patients with 63 DPS patients matched by propensity score from January 2018 to November 2022. Clinical outcomes and follow-up data were analyzed using R.
Results:
Both groups exhibited similar demographic, intraoperative, and pathological characteristics, with the exception of a reduced number of total lymph nodes (p = 0.006) in the SPDP group. There were no significant differences in the rates of postoperative complications, recurrence, or metastasis. Local recurrence predominantly occurred in the central region as opposed to the spleen region. There were no cases of isolated recurrences in the splenic region. Median overall survival and recurrence-free survival times were 51.5 months for SPDP vs 30.5 months for DPS and 18.7 months vs 16.8 months, respectively (p > 0.05). The incidence of partial splenic infarction and left-side portal hypertension in the SPDP group was 28.6% (6/21) and 9.5% (2/21), respectively, without necessitating splenic abscess puncture, splenectomy, or causing bleeding from perigastric varices.
Conclusions
SPDP did not negatively impact local recurrence or survival rates in selected pancreatic body cancer patients. Further studies are necessary for validation.

C-Reactive protein(CRP)is an important acute-phase response protein, which is widely used in the assessment of inflammation and cardiovascular disease risk, and acts as a pathogenic factor directly involved in the disease process of certain conditions. Therefore, developing immunosuppressants targeting CRP or investigating its pathogenic mechanisms is of significant importance. Most B-cell epitopes are conformational epitopes, and studying conformational epitopes is typically challenging. To date,no methods have been reported for mapping the conformational epitopes of CRP in solution. In this study,a rapid strategy was developed for studying conformational epitopes by combining hydrogen/deuterium exchange mass spectrometry(HDX-MS)with multiparametric prediction of B-cell epitopes and protein secondary structure analysis. This approach was successfully applied to the binding sites and allosteric targets of the 115 kDa full pentameric CRP and the clinically used monoclonal antibody(mAb)5A8.The results showed that the amino acid residues 84-103,138-146,and 165-173 together form the potential conformational epitopes for mAb 5A8 on CRP,while the amino acid residues 21-32 and 175-178 were identified as potential allosteric targets. The discovery of the mAb 5A8 binding sites and allosteric targets was crucial for improving clinical diagnostic capabilities.Experimental results demonstrated that this workflow allowed rapid conformational epitope mapping of CRP under near-physiological conditions, with advantages such as high speed, high sensitivity, and high throughput.

Drug impurity control is essential for ensuring pharmaceutical quality. In this study,an unknown impurity at relative retention time(RRT)of 0.45 in Torasemide injection was systematically investigated using high performance liquid chromatography-ion trap time-of-flight mass spectrometry(LC-IT-TOF-MS/MS),forced degradation studies, and nuclear magnetic resonance(NMR)spectroscopy, aiming to elucidate its chemical structure, propose a formation mechanism, and establish a control strategy.By analysis of the ultraviolet absorption spectrum and high-resolution MS1 and MS2 data of the impurity, its possible structure was postulated.Subsequently, the target impurity was prepared via forced degradation experiment, and its structure was confirmed by NMR spectroscopy.A detailed analysis of its formation mechanism revealed that Torasemide first hydrolyzed in solution to form Torasemide sulfonamide, and then underwent condensation, dehydration, and cyclization reactions with trace amounts of formaldehyde in the excipient polyethylene glycol(PEG)400 to generate the impurity. It was experimentally proven that the target impurity was related to the residual oxygen content in Torasemide injection.Ultimately,by optimizing the prescription process, the target impurity level was controlled below the individual impurity limit of the final product(≤0.3%).This study provided valuable insights for enhancing the quality control of Torasemide injection.

Thyroid hormone(TH)plays an important role in human development and is involved in gene and protein expression in almost all tissues, especially in the development of the central nervous system.TH requires a TH transporter to enter the cell, and three families of TH transporter proteins are known, namely monocarboxylate transporters(MCTs),organic anion transporting polypeptides(OATPs)and L-type amino acid transporter(LAT).MCT8 has been found to be a specific TH transporter, and OATP1C1 also plays an important role.Deficiency of TH transporters may lead to different degrees of dysfunction in the nervous system and endocrine system.Currently, more studies have been conducted on MCT8 deficiency, which presents with characteristic psychomotor retardation and TH abnormalities, and there are no specific treatment options. In this paper,we summarize the research progress on clinical phenotype, pathogenic mechanism, and treatment of thyroid hormone transporter defects related diseases to provide reference for clinical research.

This paper summarizes Professor AI Rudi's clinical experience in treating skin abscesses using the method of "three parts and six methods", which emphasize a combined internal and external therapeutic approach. He identifies retained pathogenic heat in the skin as the key etiological factor and proposes treatment principles tailored to the anatomical location of the lesions. For abscesses in the upper part of the body (head, face and neck), wind-heat and blood-heat are considered dominant, and the treatment focuses on dispersing the exterior, clearing heat, cooling the blood, and reducing swelling, with custom formulations such as self-fomulated Shufeng Xiaodu Decoction (疏风消毒饮) and Jiedu Xiaozhong Decoction (解毒消肿汤). For those in the middle part of the body (chest, abdomen, back and upper limbs), constrained heat and deficiency-heat predominate, and the treatment aims to relieve internal heat stagnation, reduce swelling, and expel toxins, using formulations like self-fomulated Qinggan Jiedu Decoction (清肝解毒汤) and Xiaozhong Erchen Decoction (消肿二陈汤). For abscesses in the lower part of the body in the (buttocks, perineum and lower limbs), damp-heat and stagnant heat are the main patterns, and the strategy is to clear damp-heat and resolve blood stasis, with formulations such as self-fomulated Jiedu Simiao Powder (解毒四妙散) and Qingjie Sanyu Decoction (清解散瘀汤). External therapies are equally emphasized: for unruptured lesions, self-fomulated Wumiao Ointment (五妙膏) is used to detoxify and reduce swelling, while for ruptured lesions, Danhuang Oil (蛋黄油) is applied to promote wound healing and relieve pain.
