Objective To gain insights into the biological characteristics of different populations of Culex tritaeniorhynchus within and around the Yellow Sea wetland from the perspective of the occurrence of resistance, we investigated the levels of resistance to deltamethrin and kdr gene mutation in the wetland and its peripheral areas. Methods Specimens were collected from Cx. tritaeniorhynchus populations at two monitoring sites in the Rare Bird National Nature Reserve and Tiaozi Ni Wetland Scenic Area, and also from two populations in Yancheng City and the Liuhe District of Nanjing, and the resistance of these mosquitoes to deltamethrin was determined using the CDC biotest bottle method. For each concentration of deltamethrin assessed, a random subset of exposed specimens was selected for amplification of the kdr gene fragment, followed by Sanger sequencing to identify and analyze resistance-associated mutations. Results The LC50 levels of deltamethrin among mosquitoes from the four populations in Luhe, Yancheng, the Rare Bird National Nature Reserve and the Tiaozi Ni Wetland Scenic Area were 2.048 5, 7.798 2, 3.473 3, and 17.695 5 mg/mL, respectively, with corresponding concentrations of deltamethrin ranging from 0.005 to 5.000,0.050 to 50.000,0.050 to 25.000 and 0.050 to 50.000 mg/mL, respectively. Furthermore, the ranges of the KT50 values were 11.76-107.43, 67.05-216.30,29.77-107.43 and 28.40-329.51 min; the 1-h knockdown rates were 34.58%-99.15%, 9.52%-43.80%, 55.09%-73.01%, and 10.09%-68.07%; and the 24-h mortality rates were 12.15%-67.52%,9.52%-79.56%,13.17%-82.21%, and 11.01%-78.99%, respectively. With respect to kdr gene mutation, we assayed a total of 63,70,59, and 57 mosquitoes for the four populations, for which we detected L1014F mutation frequencies of 14.29%, 35.00%, 20.34%, and 31.58%, respectively, with a majority of these mutations being heterozygous for resistance. In addition, five adult mosquitoes were identified has having synonymous mutations at site 1011[i. e. , AAT(asparagine)mutation to AAC(asparagine)]. Conclusions Our findings revealed the clear resistance of Cx. tritaeniorhynchus to deltamethrin in the Yancheng region of the Yellow Sea wetland, and the resistance phenotype and kdr frequency of Cx. tritaeniorhynchus in the wetland environment were comparable to those of Cx. tritaeniorhynchus in the wetland environment, thereby indicating that the resistance of different populations of Cx. tritaeniorhynchus was homogeneous under the pressure of different insecticide selection within and around the wetland. However, the underlying mechanisms need to be further studied.

Objective:To investigate the prevalence and genotype distribution of human papillomavirus (HPV) infection in the anorectal region among human immunodeficiency virus (HIV)-positive men who have sex with men (MSM) in Shenzhen, and explore the differences between HIV-positive and HIV-negative MSM populations, providing scientific evidence for HPV screening, vaccination, and related disease prevention. Methods:A total of 100 MSM recruited from the Department of Dermatovenerology of the Third People′s Hospital of Shenzhen between 2023 and 2024 were included. Questionnaire collected sociodemographic and clinical characteristics. Anorectal exfoliated cells were analyzed for HPV genotyping, and blood samples were tested for HIV antibodies and T lymphocyte subsets. Chi-square test was used to assess associations between qualitative variables. Results:Among 100 MSM, 58 were HIV-positive and 42 HIV-negative. The overall HPV infection rate was 93.10% (54/58) in HIV-positive MSM, with high-risk HPV at 79.31% (46/58) and low-risk HPV at 75.86% (44/58). The predominant genotypes were HPV6, 11, 16, 52, 18, 59, and 68. In HIV-negative MSM, HPV infection rate was 95.24% (40/42), with high-risk HPV at 57.14% (24/42) and low-risk HPV at 92.50% (37/40), dominated by HPV6, 11, 16, 51 and 52. HIV-positive MSM showed significantly higher infection rates of high-risk HPV16/18 ( P=0.032), HPV58 ( P=0.020), HPV59 ( P=0.031), and HPV68 ( P=0.007) compared to HIV-negative MSM. The maximum number of concurrent HPV infections was 12 in HIV-positive MSM versus 4 in HIV-negative MSM. Multivariate analysis revealed that HIV-positive MSM with CD4/CD8 ratio≤0.9 had significantly higher HPV positivity ( P<0.05). Conclusions:HIV-positive MSM exhibit elevated rates of high-risk and multiple HPV infections, closely associated with immune dysfunction. Strengthened HPV screening, vaccination, and immune status management are critical for preventing HPV-related malignancies in the population.

For middle-aged and elderly patients with conditions such as spinal fractures and degenerative spinal diseases, spinal internal fixation is a core surgical procedure for reconstructing spinal stability, heavily relying on the biomechanical stability provided by pedicle screw systems. Whereas, these patients are often complicated by osteoporosis that can significantly compromise the stability of the bone-pedicle screw interface, leading to a marked increase in pedicle screw loosening and surgical failure rates. The bone cement-augmented pedicle screw technique, which involves injecting bone cement into the vertebral body or screw trajectory to optimize the mechanical properties of the bone-pedicle screw composite, has been proven to significantly enhance fixation strength and effectively prevent screw-related failures, thereby reducing the incidence of internal fixation failure in high-risk populations undergoing spinal fusion. However, the widespread clinical application of this technique has faced challenges such as inaccurate clinical decision-making (indication and contraindication selection), non-standardized operative practices, and insufficient awareness of complication prevention, resulting in considerable variability in clinical outcomes and even severe complications. To address this, Prof. Luo Fei from First Affiliated Hospital of Army Medical University initiated the project and the Chinese Association Orthopaedic Surgeons organized relevant experts to develop the Evidence-based clinical practice guideline for bone cement-augmented pedicle screw technique ( version 2025), based on current evidence. The guidelines put forward 8 recommendations regarding the clinical value, scope of application, and operational standards of the technique, aiming to provide evidence-based medical support and technical standardization for clinical decision-making.

9.Home-Based Palliative Care: Benefits, Challenges, Opportunities and Future Directions in a Super-Aged Society
Journal of Hospice and Palliative Care 2025;28(3):81-88
Home-based palliative care (HBPC) has emerged as a critical model to address the complex needs of individuals with life-limiting illnesses, particularly in super-aged societies. This review explored the key components of HBPC and elaborated on its benefits, challenges, opportunities, and future directions, drawing on international evidence and practical frameworks. HBPC is reported to enhance the quality of life, reduce unnecessary hospitalizations, and increase the likelihood of patients dying in their preferred settings. Successful care delivery requires integrated multidisciplinary teams, effective symptom management, holistic care, and timely support from caregivers. However, barriers such as resource constraints, caregiver burden, and a lack of psychosocial support remain significant. Future directions and opportunities include integrating hospital-at-home models into routine HBPC services, strengthening the education of healthcare professionals and the public, and fostering compassionate communities to promote social engagement in end-of-life care. Advancing HBPC within diverse sociocultural contexts can bridge the gap between patient preferences and actual care delivery, offering a sustainable pathway to improve continuous palliative care provision for older adults globally.

Objective:
Oral anticoagulant (OAC) use increases the risk of death in conditions like hemorrhagic stroke, trauma, and traumatic brain injury. Early identification of OAC use is critical for timely interventions to mitigate hemorrhage risk and improve survival. We aimed to identify emergency medical services (EMS) care characteristics associated with patients using an OAC.
Methods:
We analyzed prehospital data (2018–2020) from the ESO Data Collaborative, focusing on adult (≥18 years) 911 EMS calls. The administered OACs were warfarin, dabigatran, rivaroxaban, and apixaban. We compared EMS call characteristics, patient demographics, response times, and interventions between OAC and non-OAC users. We used univariate logistic regression to identify independent predictors of OAC use.
Results:
Of 16,244,550 adult 911 EMS events, 906,575 involved OAC users (56 of 1,000 calls). Those using OAC were older (73.6 years vs. 56.9 years) and more often from nursing homes or long-term care facilities (17.0% vs. 9.2%) but less likely to have trauma (14.7% vs. 18.1%) or cardiac arrest (1.2% vs. 1.4%). The most common EMS primary clinical impressions for OAC users were chest pain (7.4%), altered mental status (7.3%), injury (6.5%), abdominal pain (4.3%), and brain injury (2.8%).
Conclusion
OAC users accounted for 1 in 18 adult EMS encounters. Specific patient and call characteristics were associated with OAC use. These findings should be incorporated into EMS training to facilitate recognition and appropriate management of OAC-related emergencies.
