Diabetic retinopathy(DR), as one of the most common and serious microvascular complications of diabetes mellitus, seriously threatens human health, and belongs to "Xiaoke eye diseases" in traditional Chinese medicine(TCM), which has been richly experienced by medical practitioners through the ages, but is mostly recorded in a piecemeal manner and has not been systematically researched. This disease is featured by long course and repeated attack, and is refractory, which belongs to the research category of "persistent illness entering collaterals". Systematic establishment of TCM collateral disease theory for guiding prevention and treatment of DR has important clinical value. On the basis of close correlation between tertiary collaterals at the terminal of collaterals and capillaries and microcirculation, the concept of "tertiary collaterals-microvascular" is proposed. It is pointed out that DR falls within the scope of "tertiary collaterals-microvascular" diseases, and presents four types of micro-pathological characteristics, including stasis, insufficiency, growth and bleeding of tertiary collaterals. It is concluded that "deficiency of both Qi and Yin" is the basic pathogenesis of DR, and "blood stasis and collateral obstruction" is the important pathogenesis and key factor. Thus, the treatment method of "dispersing blood stasis, dredging collateral, tonifying Qi and Yin, stopping hemorrhage and improving eyesight" is determined, and the formula of Tongluo Mingmu capsules is developed. The article tightly focuses on the pathological changes such as stasis, growth, insufficiency and bleeding of collaterals, addresses both symptoms and root causes, and plays a synergistic role of both dispersing stasis and stopping bleeding. In this way, it can realize the purpose of tonifying Qi and Yin to replenish the essence, dispersing stasis and dredging collaterals to meet the requirement, as well as stopping hemorrhage and improving eyesight to deal with changes. Fundamental researches demonstrate that Tongluo Mingmu capsules has synergy effects of protecting both retinal capillaries and retinal cells. Phase-Ⅲ clinical trial of new drug has proven definite clinical efficacy and good safety, which provides a new drug choice for enhancing clinical effect of DR, and further supports the scientific value of Luobing theory in preventing and treating DR and other clinically significant diseases.

The theoretical basis of premature aging originates from The Yellow Emperor's Inner Classic. The etiology of premature aging is complex, and the disease mechanism is based on deficiency. The treatment for premature aging is based on tonicity. The essence-Qi-spirit theory of Qiluo doctrine summarizes that "essence is the origin of life, Qi is the driving force of life, and spirit is the embodiment of life", which is the law of life. The theory puts forward the core disease mechanism of aging, which states that "deficiency of kidney essence is the root of aging, deficiency of primordial Qi is the key to aging, impairment of soma and spirit is the manifestation of aging". The theory also proposes the treatment of "tonifying kidney and supplementing essence, harmonizing Yin and Yang, warming and supporting primordial Qi, and nourishing soma and spirit" and the representative anti-aging drugs. The article unfolds from the perspective of the concepts of natural life span, premature senility before fifty, decline, and aging and also explains the origins and connotations of premature aging. The article explains the disease mechanism of premature aging under the guidance of the essence-Qi-spirit theory of Qiluo doctrine, which is "early deprivation of kidney essence, deficiency of primordial Qi, accumulation of deficiencies into impairment, and decline and impairment of soma and spirit", summarizes the progress of modern medical research on the treatment of premature aging and representative drugs, and finds that Bazi Bushen capsules have a precise therapeutic effect on the overall premature aging, systematic functional decline, and related diseases. The study provides theoretical basis and new ideas to solve the problems of premature aging and geriatric diseases.

Objective:To explore the feasibility of deep learning image reconstruction (DLIR) in reducing radiation dose and improving image quality in 100 kV portal vein CT angiography (CTPV), and compare the image quality with traditional 120 kV CTPV.Methods:Consecutive 100 patients who underwent upper abdominal CTPV examinations at the Third Affiliated Hospital of Sun Yat-sen University from June 2021 to December 2022 were prospectively enrolled in this study. They were divided into two groups: the standard dose group (S group) using 120 kV and the low dose group (L group) using 100 kV. In the S group, adaptive statistical iterative reconstruction-Veo (ASIR-V) was employed with weights of 40%, 60%, and 80% (S-AV40, S-AV60, and S-AV80, respectively), along with a medium-level DLIR (S-DM). The L group used two weights of 60% and 80% of ASIR-V (L-AV60 and L-AV80) and medium to high-intensity DLIR (L-DM and L-DH) for reconstruction. Objective evaluation measurements, including CT values and standard deviation (SD) of the main portal vein (MPV), right portal vein (RPV), left portal vein (LPV), hepatic parenchyma, and right vertical muscle, were independently measured by two radiologists. The signal-to-noise ratio (SNR) and contrast-to-noise ratio (CNR) were also calculated. The images of eight groups were blindly evaluated using a 5 points scale by three radiologists. The Volume CT dose index (CTDIvol) and dose length product (DLP) were recorded for both protocol groups, and the effective dose (ED) was calculated. Statistical analysis was performed using Single factor variance analysis for objective indicators and Kruskal-Wallis H test for subjective scores. Results:The effective dose in the L group was reduced by 51% compared to the S group. There were significant statistical differences in SD, SNR and CNR among eight groups for MPV, RPV, and LPV ( P<0.001 for all). The SD of L-DH showed no statistical difference compared to the S-AV80 group ( P>0.05), but it was significantly lower than the other six groups ( P<0.05 for all). Except for CNR in the MPV, which showed no statistically significant difference between the L-DH and S-AV80 groups ( P>0.05), both SNR and CNR were significantly higher in the L-DH group compared to the other groups ( P<0.001 for all). The L-DH (4.61±0.16) achieved the highest subjective image quality score, which was statistically higher than the scores in the other six groups ( P<0.001 for all) except for the S-DM group (4.31±0.19). There was good consistency among the radiologists regarding objective parameter measurements and subjective image quality scores ( ICC=0.584-0.960). Conclusion:Compared to standard-dose CTPV at 120 kV, the combination of 100 kV with the DLIR algorithm significantly reduced the radiation dose by 51% in CTPV while maintaining higher SNR, CNR and subjective scores. The high-level DLIR algorithm produced the best image quality for 100 kV CTPV.

Perimenopausal syndrome (MPS), a common endocrine system disease, is one of the diseases responding specifically to traditional Chinese medicine (TCM). The China Association of Chinese Medicine organized experts in endocrinology, gynecology, and interdisciplinary fields of both Western and Chinese medicine to discuss the advantages and challenges of diagnosing and treating MPS with Western medicine, TCM, and integrative medicine. Experts at the conference believe that MPS is initiated by estrogen decline and rooted in deficiency, with the pathogenesis being imbalance between Yin and Yang in the kidney. The hormone replacement therapy in Western medicine for menopause can rapidly alleviate related symptoms by quickly restoring the estrogen level and timely detect and delay complications of menopause, whereas such a therapy has certain risks, necessitating close monitoring of adverse reactions. Moreover, the various contraindications and precautions limit the clinical application of the hormone replacement therapy. TCM has advantages in synergistically alleviating symptoms such as hot flashes, sweating, sleep disorders, and emotional abnormalities of MPS without causing obvious adverse reactions. However, its efficacy is slower than the hormone replacement therapy, and the TCM evidence for preventing and treating complications of menopause remains unclear. Three suggestions were proposed for the future development of both Western and TCM for ameliorating MPS. First, an integrated diagnosis and treatment system for MPS with both Western and Chinese medicine should be established. Second, high-quality evidence-based interventions for MPS should be developed with TCM alone or in combination with Western medicine. Third, efforts should be made to promote the new TCM drug development and the interdisciplinary cooperation for treating MPS.

ObjectiveTo study the effect of Bazi Bushen capsules on delaying the aging process of naturally aging mice and its mechanism. MethodThe mice were randomly divided into four groups according to their body weight, namely, aging group, low-dose Bazi Bushen capsules group (1 g·kg-1), high-dose Bazi Bushen capsules group (2 g·kg-1), and rapamycin group (0.002 g·kg-1). The debilitating signs were detected by behavioral tests and the weakness index was measured. The percentages of spleen T and B lymphocytes, effector T cells (TE), memory T cells (TM), naive T cells (TN), helper T cells (Th), cytotoxic T cells (Tc) ,Th1 cells, Th2 cells, and regulatory T cells (Treg) were determined by flow cytometry. Cell proliferation and the cell counting kit-8 (CCK-8) assay was used to detect the proliferation of lymphocytes in mice. The hematoxylin-eosin (HE) staining was used to observe the histopathological changes in the mouse spleen. The expression of cyclin-dependent kinase inhibitor 2A (p16) and cyclin-dependent kinase inhibitor 1A (p21) was detected by immunohistochemistry (IHC). The mRNA expression of senescence-related proteins p16 and p21 was determined by real-time fluorescence quantitative polymerase chain reaction (Real-time PCR), and the inflammatory cytokines, including tumor necrosis factor-α (TNF-α), interferon-γ (IFN-γ), interleukin (IL)-1β, IL-2, IL-4, IL-10, and IL-12 p70, in peripheral blood of mice were detected by Luminex. ResultAs compared with the aging group, mice in the Bazi Bushen capsules and rapamycin groups showed significantly improved debilitating signs and reduced weakness index scores (P<0.05, P<0.01), increased proportions of T cells, TN cells, Tc cells, Th2 cells, and Treg cells in the spleen, decreased proportions of TE cells, TM cells, Th cells, Th1 cells (P<0.05, P<0.01), and increased proliferation of splenic lymphocytes (P<0.05, P<0.01). In the Bazi Bushen capsules and rapamycin groups, clear structure of the red and white marrow marginal zone was observed in the spleen of mice, the area of the white marrow was increased, and the area of the red marrow was correspondingly decreased. The protein and mRNA expression of aging-related proteins p16 and p21 in the spleen was decreased (P<0.01), the levels of serum pro-inflammatory cytokines IL-1β, IL-2, IL-12 p70, IFN-γ, and TNF-α levels were decreased, and the levels of IL-4 and IL-10 were increased (P<0.05, P<0.01) in the Bazi Bushen capsules and rapamycin groups as compared with the aging group. ConclusionBazi Bushen capsules have the effect of regulating the debilitating signs of natural aging mice, regulating the immune homeostasis and inflammation level of the body, and reducing cell aging.
